Evaluation of the Safety and Efficacy of Extracts from the Leaves of Five Plants Used for the Treatment of Arterial Hypertension in Benin

Cardiovascular diseases (CVD) are nowadays real health problems in the world. High blood pressure is one of the most important risk factors for CVD and is affecting more and more people in Benin. The objective of our work is to evaluate the safety and the efficacy of the leaves of five plants used for the treatment of hypertension in Benin. Acute toxicity was evaluated on wistar rats which orally administered a single dose of 2000 mg/kg of body weight of hydro-ethanolic extract of the leaves of Phyllanthus amarus Schumach. & Thonn., Persea americana MILL., Ipomoaea fistulosa Mart. ex Choisy., Heliotropium indicum L., Schrankia leptocarpa DC., and were monitored over a period of 14 days. Subacute toxicity was evaluated on rats which received a daily dose of 200 mg/kg of body weight of the plant leaf extract over a period of 28 days. Plant efficacy was assessed by measuring potassium in plant leaves. Administration of the single dose of the extract did not cause any deaths in rats;the weight of the rats varied depending on the extracts administered. Concerning the subacute treatment, the levels of aspartate amino transaminases (AST) and Alanine amino transaminases (AST) did not vary significantly after the 28 days of treatment with the different extracts. On the other hand, a significant increase in serum creatinine was observed in rats treated with extracts of Phyllanthus amarus. The leaves of Heliotropium indicum, Ipomoaea fistulosa and Phyllanthus amarus contain the highest levels of potassium. Among the five plants studied, only the leaves of Phyllanthus amarus seem to induce renal toxicity. Extracts from the leaves of Heliotropium indicum, Ipomoaea fistulosa might be the most effective in inducing hypotensive activity and do not show toxicity.

Introduction: Birth spacing to prevent closely-spaced pregnancies, which are a source of maternal and neonatal morbidity and mortality, is not always respected by our women, as recommended by the World Health Organization (WHO). The objective was to study the factors associated with closely-spaced pregnancies among women followed at the Departmental Hospital Center du Zou et du Colline (DHC/Z-C) in 2022. Study Method: This was a descriptive, analytical cross-sectional study with prospective data collection from 20 May to 30 June 2022. Results: At the end of the study, 83 of the 117 women surveyed had close pregnancies, a frequency of 70.94%. They had an average age of 27.0 ± 6.3 years and were of Fon ethnicity and related in 91.57% of cases. The majority were married (96.60%). The factors significantly associated with these closely spaced pregnancies were age between 25 and 30 years, household occupation, age at first birth, age at marriage, women’s perception of contraceptive method (CM), women’s knowledge of the existence of a center providing family planning services and non-use of MC. Conclusion: Close pregnancies are still very common in the commune of Abomey. It is therefore important to increase the use of contraceptive methods in this part of the country.

Bacteriological Profile of Infections Encountered in a Pneumology Service in a Moderate-Income Country [Pneumology Department of CHU Cocody (Côte d’Ivoire)]

Introduction: Infections represent a real public health problem aggravating the morbidity and mortality of hospitalized patients. Methods: This was a retrospective study with descriptive purposes over a period of 05 years, in the Pneumology Department of the University Hospital of Cocody. Results: The average age in our population was 42 years. We observed a male predo- minance of 64.5%, with a sex ratio of 1.8. Medical history was dominated by HIV infection (23.5%), followed by tuberculosis (15.6%). Concerning lifestyle, smoking was found in 38% of cases. Symptoms progressed chronically in 80% of cases. An infectious syndrome was found in 75% of cases. Microbial culture was positive in 42% of cases. Pseudomonas aeruginosa and Klebsiella pneumoniae were encountered in 26.4% of cases each, followed by Echerichia coli in 10.4% of cases. Klebsiella pneumoniae in the first three years topped the list, but gradually. Pseudomonas aeruginosa maintained its leadership over the last three years. Pseudomonas aeruginosa isolates expressed a resistance rate of 9.8% to ceftazidime and 8.1% to imipenem;to aztreonam (36%), ticarcillin (33.3%) and levofloxacin. These strains were susceptible to fosfomycin (100%), mero- penem (96.6%) and amikacin (96%). For isolated strains of Entero- bacteria- ceae, resistance was observed about ticarcillin (83.3%) and amoxicillin clavula- nic acid (71.2%). Streptococcaceae showed resistance to tetracycline (69.2%) and erythromycin (50%). Over the years there has been an increase in re- sistance to amoxicillin-clavulanic acid ceftriaxone. The death rate was 14%. Conclusion: The bacterial profile of infections is dominated by germs respon- sible for nosocomial infection with significant mortality.

Surface Hardness as an Indicator of Soil Strength of Agricultural Soils

Soil strength is an important quality of agricultural soils prone to traffic. Surface hardness (SH) measured by the Clegg Impact Tester (CIT) was evaluated as an indicator for assessing soil strength. Proctor tests were performed on a diverse range of soils to examine the relationships between bulk density (BD), penetration resistance (PR), SH and water content. All three indices showed typical response curves with increasing water content, with notable differences among the soils. Maximum dry bulk density (MDBD), peak penetration resistance (PPR) and peak surface hardness (PSH) showed values of 1.98 Mg m-3, 8.2 MPa and 248 Cmax for Piarco, River Estate and Piarco respectively. Corresponding critical moisture contents (CMC) were much greater for MDBD compared to PPR and PSH. SH showed a significant positive correlation with PR, but not BD. Further divulgence into the relationships between SH and other soil properties as well as crop response will facilitate greater use of the CIT.

Spina bifida, or spinal dysraphism, is a malformative pathology related to an anomaly in the development of the nervous system, occurring during embryogenesis. The neural tube does not close properly around the 28th day of life and affects the development of the spinal column and spinal cord. Spina bifida is characterised by damage to the nervous system and will generate handicaps and damage of varying degrees: neurological motor, sensory, cognitive, genito-phincter (bladder and anorectal) deficits with consequences for the quality of life of these people. The literature describes the association between spinal dysraphism and genital prolapse. However, genital prolapse is an exceptional and rare entity in newborns. We report the observations of two newborns: the first case of a newborn born at term, at 7 days of age, who presented a prolapse of the uterine cervix in association with myelomeningocele, without any neuromuscular repercussions, and the second case of a newborn at 10 days of age, presenting with a lumbosacral spina bifida and a uterine prolapse. They benefited from conservative medical treatment characterised by manual reduction of the prolapse in both cases with a favourable evolution. In the case of spina bifida, a cure of myelomeningocele was performed surgically with simple postoperative course.

Knowledge of the clinical and biological profile of patients infected with HIV and hepatitis B and/or C is essential in order to identify and implement effective management strategies. Methods: This was a retrospective descriptive study from January 01, 2016 to June 01, 2021. Adult patients aged at least 18 years infected with HIV type 1 and/or 2, na?ve to ARV treatment. Univariate analyses were assessed using Pearson’s Chi2 test. The Student Newman test was used for comparison between groups using R software version 4.0.2. Objective: To draw up the epidemiological, clinical, paraclinical and evolutionary profiles of HIV-treated-patients in relation to HIV/HBV and HIV/HCV co-infections in order to allow the identification and the implementation of effective management strategies. Results: Of the 379 patients included 280 (73.88%) were women. At treatment initiation, the mean age was 40.14 ± 11.84 years. The majority of patients consulted at WHO stage III (51.45%). Clinical suspicion was the most frequent screening circumstance (51.71%). The pathologies frequently reported at the first consultation were diarrhea (28%) and shingles (16%). Body mass index was normal in 50.5% of patients. HIV1 infection was the majority (91.03%). A total of 270 had a CD4 count at treatment initiation. The mean CD4 cell count was 304.17 ± 242.06 cells/μL, and 116 (42.59%) of them had a CD4 ≤ 200 cells/μL. Viral load at treatment initiation was documented in 62 patients (16.35%) and 70.97% of them had a detectable viral load (greater than 1000 copies/mL). The clinical and biological evolution was relatively good in patients after therapeutic initiation. HIV-HBV co-infection was 24.11% and HIV-HCV co-infection was 2.26%. The mortality rate was 3.69%. Conclusion: These results reflect a significant delay in HIV infection diagnosis. Furthermore, hepatitis B and/or C is co-infections that increasingly affect people living with HIV.
