A systematic review and meta-analysis of clinical trials implementing aromatase inhibitors to treat male infertility

Aromatase activity has commonly been associated with male infertility characterized by testicular dysfunction with low serum testosterone and/or testosterone to estradiol *** this subset of patients,and particularly in those with hypogonadism,elevated levels of circulating estradiol may establish a negative feedback on the hypothalamic–pituitary–testicular axis by suppressing follicle-stimulating hormone(FSH)and luteinizing hormone(LH)production and impaired *** manipulation via different agents such as selective estrogen modulators or aromatase inhibitors to increase endogenous testosterone production and improve spermatogenesis in the setting of infertility is an off-label option for *** carried out a systematic review and meta-analysis of the literature of the past 30 years in order to evaluate the benefits of the use of aromatase inhibitors in the medical management of infertile/hypoandrogenic ***,eight original articles were included and critically *** steroidal(Testolactone)or nonsteroidal(Anastrozole and Letrozole)aromatase inhibitors were found to statistically improve all the evaluated hormonal and seminal outcomes with a safe tolerability *** the evidence is promising,future prospective randomized placebo-controlled multicenter trials are necessary to better define the efficacy of these medications.

基于Si CMOS技术的前端读出ASIC主要是根据3D Si PIN阵列热中子探测器的输出信号特性设计的。所设计的读出ASIC的主要电路模块包括电荷灵敏放大器(CSA)、模拟开关设计、具有三级电荷灵敏自动转换的自动增益控制模块(AGC)、相关双采样(CDS)和基准电流源电路。仿真结果表明,前端电路的输入动态范围为10 fC~8.0 pC。根据热中子探测器输出信号特性设计的ASIC的3个增益系数分别为1.9 V/pC、0.39 V/pC和94 mV/pC。所设计的ASIC的积分非线性小于1%。单通道静态功耗约为5.36 mW。零输入探测器电容时的等效噪声电荷为241.6e;。计数率可达1 MHz。

PET in uterine malignancies

Positron Emission Tomography (PET) or integrated PET/Computed Tomography (PET/CT) with 18F-Fluoro-Deoxy-Glucose (18F-FDG) is a functional imaging modality, useful in the characterization of undetermined morphological findings, and in the staging/re-staging of a large number of malignancies. Although its use in uterine malignancies has been poorly investigated, in recent years the employment of this technique has constantly increased. In this review, we evaluate the role of PET (/CT) with 18FFDG in uterine malignancies (cervical and endometrial cancers as well as uterine sarcomas), underlying its advantages and discussing its limitations. Metabolic and anatomic information given by PET/CT with 18F-FDG could be useful in the evaluation of local and distant disease involvement at the staging, in the detection of disease recurrence, and in the evaluation of the response after chemotherapy and/or radio-therapy.

Inter-Conversion of Carbohydrate Reserves from Pollen maturation to Rehydration in a Chili Pepper

Carbohydrate metabolism is critical for male fertility. Carbohydrate reserves (soluble sugars and starch) were quantified in three advanced stages of pollen development, from the final maturation to rehydration, in a chili pepper cultivar (Capsicum annuum L. cv. Calypso Red) with partially dehydrated pollen, to define possible carbohydrate inter-conversions. At the same time, the activity of soluble enzymes involved in sucrolysis and amylolysis were quantified to reveal potential metabolic pathways. The carbohydrates found (sucrose, glucose, fructose, and starch) apparently inter-convert. There would be a close relation between enzymatic activities and substrates, and the carbohydrate reserves could be key factors on the regulation of enzymatic activities. All the enzymes tested were active, although the relevance of each one changed along pollen maturation and rehydration, defining different profiles of enzymatic activities for each stage. Some usually neglected enzymes (e.g. soluble neutral invertase) have shown an important role in the stages analyzed, suggesting alternative processes to evaluate in the studies of male fertility control.

How to approach the pediatric flatfoot

The most difficult aspect regarding treatment of the pediatric flatfoot is understanding who needs surgery,when it is necessary, and what procedure to be done.A thorough history, clinical examination, and imaging should be performed to guide the surgeon through an often complex treatment path. Surgical technique can be divided in three categories: Soft tissue, bony,and arthroereisis. This paper will describe the jointpreserving techniques and their application to treat the pediatric flatfoot deformity.

The aim of this paper is to discuss the risk of recurrence in patients with differentiated thyroid cancer and emphasize the importance of risk-group stratification,early recurrence identification and application of new imaging modalities,what is the ***,follow-up of patients with thyroid carcinoma should be carried out by specialized teams throughout ***,interdisciplinary case discussions in tumor conferences may improve the use of multimodal therapy especially in patients with poorly differentiated thyroid *** baseline follow-up,if there is a suspicion of thyroid carcinoma,early PET-CT should be used for early detection and appropriate ***,due to the good localization possibility,the PET-CT enables a focused surgical procedure with avoidance of an unnecessary tumor search and thereby a reduction of the risk of injury of neighboring structures which is a concern with reoperative neck surgery.

Hallux rigidus: How do I approach it?

Hallux rigidus is a degenerative disease of the first metatarsalphalangeal(MTP) joint and affects 2.5% of people over age 50. Dorsal osteophytes and narrowed joint space leads to debilitating pain and limited range of motion. Altered gait mechanics often ensued as 119% of the body force transmit through the 1^(st) MTP joint during gait cycle. Precise etiology remains under debate with trauma being often cited in the literature. Hallux valgus interphalangeus, female gender, inflammatory and metabolic conditions have all been identified as associative factors. Clinical symptoms, physical exam and radiographic evidence are important in assessing and grading the disease. Non-operative managements including nonsteroidal antiinflammatory drugs, intraarticular injections, shoe modification, activity modification and physical therapy, should always be attempted for all hallux rigidus patients. The goal of surgery is to relieve pain, maintain stability of the first MTP joint, and improve function and quality of life. Operative treatments can be divided into joint-sparing vs joint-sacrificing. Cheilectomy and moberg osteotomy are examples of joint-sparing techniques that have demonstrated great success in early stages of hallux rigidus. Arthrodesis is a joint-sacrificing procedure that has been the gold standard for advanced hallux rigidus. Other newer procedures such as implant arthroplasty, interpositional arthroplasty and arthroscopy, have demonstrated promising early patient outcomes. However, future studies are still needed to validate its long-term efficacy and safety. The choice of procedure should be based on the condition of the joint, patient's goal and expectations, and surgeon's experience with the technique.

Voltage Control in Smart Grids: An Approach Based on Sensitivity Theory

Due to the development of Distributed Generation (DG), which is installed in Medium-Voltage Distribution Networks (MVDNs) such as generators based on renewable energy (e.g., wind energy or solar energy), voltage control is currently a very important issue. The voltage is now regulated at the MV busbars acting on the On-Load Tap Changer of the HV/MV transformer. This method does not guarantee the correct voltage value in the network nodes when the distributed generators deliver their power. In this paper an approach based on Sensitivity Theory is shown, in order to control the node voltages regulating the reactive power exchanged between the network and the dispersed generators. The automatic distributed voltage regulation is a particular topic of the Smart Grids.

In the second half of 2014 the manchester Institute of Biotechnology, based in manchester （UK）, hosted the first SupraBiology congress, an event attended by representa- tives of different academic institutions and industry based in both the UK and China. The congress was aimed to serve as a platform to discuss and promote potential collaborations between the UK and China on the subject of Systems Biology and High Performance Computing. The event, sponsored by the ＂BBSRC China Partnering Awards＂ and ISBE, was organised as a sequence of talks addressing the different aspects of Systems Biology that can benefit from High Performance Computing. A general discussion session followed where the scientific, techni- cal, and logistic aspects of the prospected UK-China collaborations were examined.

An unexpected urinary bladder xanthoma:Case report

In spite of the fact that xanthomas of the skin,tendons and mucosae are relatively common conditions,urinary bladder xanthoma is a rare benign lesion,only occasionally reported in *** usually appears as an exophytic mass into the bladder cavity,which often worries the clinicians,because it is considered as a neoplasm that need both an excision and *** herein describe the 32nd case,accidentally identified in a 63-year-old man with a right urinary bladder mass on follow-up abdominal ultrasound scan for a previous open *** also referred mild obstructive lower urinary tract ***,a cystoscopy with biopsies was performed and the subsequent histopathological examination was consistent with the diagnosis of urinary bladder ***,no additional therapeutics interventions were needed,and the patient currently presents nor symptoms,recurrences,or other *** its rarity,it is important to correctly identify urinary bladder xanthoma,differentiating it from other pathological entities which present similar histopathological characteristics,but which sometimes could be associated with a patients’dismal ***,an accurate diagnosis of urinary bladder xanthoma can avoid both additional treatments and follow up,but also allows to recognize some potential detrimental pathologies which could be associated with it,such as either an altered lipid metabolism or urothelial *** and prompt recognition and treatment of these last can notably improve patient’s outcome.
