The effects of an exercise program on health-related quality of life in postpartum mothers: A randomized controlled trial

Background and Objective: There is controversy on the psychological effects of postpartum exercise. The study aimed to evaluate the effectiveness of a postpartum exercise program on health-related quality of life and psychological well-being. Methods: We conducted a randomized controlled trial in Tokyo, Japan. The intervention groups participated in ball-exercise classes (weekly exercise of 90 minutes for four weeks) at three months postpartum. The exercise class included the following: 1) greeting and warm-up;2) aerobic exercise involving bouncing on an exercise ball 55 or 65 cm in diameter;3) rest and self-introduction;4) stretching and cooling down. The primary outcome measure was health-related Quality of Life (QOL) assessed using the MOS Short-Form 36-Item Health Survey (SF-36v2). The secondary outcome measures were Rosenberg Self-Esteem Scale (RSES) and Edinburgh Postnatal Depression Scale (EPDS) scores. The intervention group was compared to the control group at four months postpartum. Results: Of the 120 women screened, 110 women met the study criteria. Nine could not be included and the remaining 101 were allocated randomly into intervention and control groups (50 and 51 participants respectively). Analysis of covariance adjusting for baseline values indicated that the SF36 subscales of physical functioning (p = 0.018) and vitality (p = 0.016) significantly improved in the intervention group compared to the control group, although there were no significant differences between the groups in the SF36 Physical Component Summary (PCS) and Mental Component Summary (MCS) scores. The RSES increased in the intervention group (p = 0.020) compared to the control group. No significant group differences were observed in EPDS scores. Conclusions: The postpartum exercise class program provided to healthy postpartum women appears to have contributed to promoting health-related QOL and self-esteem.

Neurostatistical imaging for diagnosing dementia:translational approach from laboratory neuroscience to clinical routine

Statistical analysis in neuroimaging （referred to as “neurostatistical imaging”） is important in clinical neurology. Here, neurostatistical imaging and its superiority for diagnosing dementia are reviewed. In neurodegenerative dementia, the proportional distribution of brain perfusion, metabolism, or atrophy is important for understanding the symptoms and status of patients and for identifying regions of pathological damage. Although absolute quantitative changes are important in vascular disease, they are less important than relative values in neurodegenerative dementia. Even under resting conditions in healthy individuals, the distribution of brain perfusion and metabolism is asymmetrical and differs among areas. To detect small changes, statistical analysis such as the Z-score - the number of standard deviations by which a patient＇s voxel value differs from the normal mean value - comparing normal controls is useful and also facilitates clinical assessment. Our recent finding of a longitudinal one-year reduction of glucose metabolism around the olfactory tract in Alzheimer＇s disease using the recently-developed DARTEL normalization procedure is also presented. Furthermore, a newly-developed procedure to assess brain atrophy with CT-based voxel-based morphometry is illustrated. The promising possibilities of CT in neurostatistical imaging are also presented.

Activation of Trifluoromethylthio Moiety by Appending Iodonium Ylide under Copper Catalysis for Electrophilic Trifluoromethylation Reaction

A novel iodonium-ylide compound 2 that appends a trifluoromethylthio （SCF3） group is disclosed as a new, shelf-stable electrophilic trifluoromethylation reagent. Unlike known shelf-stable electrophilic trifluoromethylation reagents, 2 has a stable SCF3 group which is activated by appending iodonium ylide under copper catalysis via sul- fonium ylide to generate a cationic trifluoromethyl （CF3） species. Reagent 2 was found to be an efficient electro- philic trifluoromethylation reagent for a wide range of silyl enol ethers 3 under copper catalysis. Cyclic and acyclic a-trifiuoromethyl ketones 4 were obtained by reagent 2 in moderate to good yields. On the other hand, a difluoro- methylthio analogue 5 did not affect intermolecular transfer difluoromethylation to substrates. Instead, intramolecu- lar 1,4-migration proceeded similar to the Stevens rearrangement to provide 6 in 21% yield, independent of the presence of nucleophiles 3.

Relationship of Stress Coping Behaviors and Changes in Resilience during an Integrated Nursing Practicum

This study aims to clarify what kinds of resilience and coping strategies students employ to successfully complete their clinical practicum and provide suggestions for instruction that will allow students to carry out effective learning activities. The study subjects were 86 students enrolled at Nursing University A for the 2019 school year and planning to take the integrated nursing practicum. The survey was conducted using an anonymous, self-administered questionnaire. The Wilcoxon signed-rank test was used to determine the amount of change in resilience factors from the surveys before and after the practicum. Subsequently, multiple regression analysis was carried out with stress coping behaviors that were significantly associated with resilience factors in univariate analysis as independent variables and the amount of change in resilience factors before and after the practicum as dependent variables. A total of 56 responses were received (response rate 65.8%) from the 85 survey subjects. Innate resilience factors increased significantly from 41 pre-practicums to 44 post-practicums. Acquired resilience factors also increased significantly from 32 pre-practicums to 35 post-practicums (p −0.456, p < 0.01) were significantly associated with innate resilience factors. Examining the relationship between the amount of change in student resilience and stress coping behaviors revealed that the stress coping behavior of “changing point of view” was associated with innate resilience. This finding suggests that resilience may be increased by changing one’s perspective on an event by taking on a new way of thinking. Dear Editor,Microdissection testicular sperm extraction(micro-TESE)is an optimal sperm extraction method for men with nonobstructive azoospermia(NOA).1 According to a recent systematic review by Corona et al.,2 the sperm retrieval rate by micro-TESE and live birth rate per intracytoplasmic sperm injection(ICSI)cycle among NOA patients were 46%and 24%,*** cases where sperm can be collected from NOA patients,the live birth rate remains *** the potential outcomes of ICSI can be predicted by sperm motility assessment,such as pregnancy not anticipated because of immotile sperm,unnecessary ovarian stimulation or oocyte retrieval from female partners could be avoided.
