Role of double-balloon enteroscopy in malignant small bowel tumors

AIM: To assess the double-balloon enteroscopy(DBE) role in malignant small bowel tumors(MSBT).METHODS: This is a retrospective descriptive study performed in a single center. All consecutive patients who underwent a DBE with final diagnosis of a malignant neoplasm from 2004 to 2014 in our referral center were included. Patient demographic and clinical pathological characteristics were recorded and reviewed. MSBT diagnosis was achieved either by DBE directed biopsy with multiple tissue sampling, endoscopic findings or histological analysis of surgical specimen. We have analyzed double-balloon enteroscopy impact in outcome and clinical course of these patients. RESULTS: Of 627 patients, 28(4.5%)(mean age = 60 ± 17.3 years) underwent 30 procedures(25 anterograde, 5 retrograde) and were diagnosed of a malignant tumor. Patients presented with obscure gastrointestinal bleeding(n = 19, 67.9%), occlusion syndrome(n = 7, 25%) and diarrhea(n = 1, 3.6%). They were diagnosed by DBE biopsy(n = 18, 64.3%), histological analysis of surgical specimen(n = 7, 25%) and unequivocal endoscopic findings(n = 2, 7.1%). Gastrointestinal stromal tumor(n = 8, 28.6%), adenocarcinoma(n = 7, 25%), lymphoma(n = 4, 14.3%), neuroendocrine tumor(n = 4, 14.3%), metastatic(n = 3, 10.7%) and Kaposi sarcoma(n = 1, 3.6%) were identified. DBE modified outcome in 7 cases(25%), delaying or avoiding emergency surgery(n = 3), modifying surgery approach(n = 2) and indicating emergency SB partial resection instead of elective approach(n = 2).COnCLUSIOn: DBE may be critical in the management of MSBT providing additional information that may be decisive in the clinical course of these patients.

The impact of cultural assumptions on simulated energy, comfort, and investment returns of design decisions in two desert climates

In Latin America, default assumptions and borrowed templates and methodologies are often used in energy modeling, resulting in models that might not represent their cultural context and leading to policies awkwardly fit to local practices. Policy-driving low-income housing studies in Mexico, for example, activated both heating and cooling in energy models even though less than 5% of the homes in the country have heating systems. This paper illustrates the importance of modeling local sociocultural habits and practices, and how this can affect design outcomes. Here, we modeled low-income housing representative of typical residences in two desert climates-Hermosillo, Mexico, and Copiapo, Chile-using EnergyPlus. Settings representing local practices in each region were tested against default values, including occupancy settings, regional construction systems, and importantly, HVAC settings related to partial conditioning. Their impacts were measured via variation in energy use, comfort conditions, and the payback period of design upgrades. Results demonstrated how certain assumptions can have a high “design significance”, a term we propose for inputs that completely change optimal design decisions, as well as the importance of considering thermal comfort in such decisions. Including partial conditioning, for example, resulted in at least double the payback period and discomfort degrees for design upgrades in 16 of 24 instances.

Hepatitis delta virus(HDV) seems to strongly suppress hepatitis B virus(HBV)replication, although little is known about the mechanism of this interaction. Both these viruses show a dynamic distribution of mutants, resulting in viral quasispecies. next-generation sequencing is a viable approach for analyzing the composition of these mutant spectra. As the regulatory hepatitis B X protein(HBx) is essential for HBV replication, determination of HBV X gene(HBX)quasispecies complexity in HBV/HDV infection compared to HBV monoinfection may provide information on the interactions between these two *** To compare HBV quasispecies complexity in the HBX 5' region between chronic hepatitis delta(CHD) and chronic HBV mono-infected *** Twenty-four untreated patients were included: 7/24(29.2%) with HBeAgnegative chronic HBV infection(CI, previously termed inactive carriers), 8/24(33.3%) with HBeAg-negative chronic hepatitis B(CHB) and 9/24(37.5%) with CHD. A serum sample from each patient was first tested for HBV DnA *** HBX 5' region [nucleotides(nt) 1255-1611] was then PCR-amplified for subsequent next-generation sequencing(MiSeq, Illumina, United States). HBV quasispecies complexity in the region analyzed was evaluated using incidencebased indices(number of haplotypes and number of mutations), abundancebased indices(Hill numbers of order 1 and 2), and functional indices(mutation frequency and nucleotide diversity). Sweet syndrome and differentiation syndrome in a patient with acute promyelocytic leukemia

The differentiation syndrome is an inflammatory reaction with increased capillary permeability that occurs in up to 25% of patients with acute promyelocytic leukemia treated with all-trans retinoic acid. A 50-year-old man with acute promyelocytic leukemia underwent chemotherapy with idarubicin and all-trans retinoic acid. On day +21 the patient developed pruritic prepatelar papules as well as several 10 mm subcutaneous nodules in both thighs accompanied by persistent fever. On the day +25 the patient presented with bilateral pulmonary crackles, infiltrates in the right lower lobe and severe hypotension which required dopamine infusion. Biopsy of one of the thighs nodules was performed. A Sweet syndrome associated to a differentiation syndrome was suspected. All-trans retinoic acid therapy was discontinued and dexamethasone was administered. In 48 h the patient showed remission of the fever and the infiltrates and the skin lesions acquired a residual aspect. It is debatable whether these two syndromes are distinct entities with common mechanisms or whether they are poles of the same spectrum. Dermatologists and hematologists must be aware of these two syndromes and its pathophysiologic association.

Effective parameter study for the facile and controlled growth of silver molybdate nano/micro rods

Controlled growth of nano/micro structures by controlling the effective parameters is the basic requirement for the application point of view in various areas. Here we report the facile growth of silver molybdate nanolmicro rods by mixing the solution of silver nitrate and ammonium molybdate at ambient condition followed by hydrothermal treatment at various temperatures for 12 h. To achieve the goal for the synthesis of long, high yield and homogeneous nanorods various effective parameters have been studied to set the most effective conditions for the growth. Among possible effective parameters first the temperature of the furnace was set by warring the temperature and then at the set temperature the concentration of reactants （nH4）6Mo7O24 and silver nitrate are varied respect to each other. The pH and temperature values were monitored during the mixing of the reactants. Structurallmicrostructural characterization revealed the optimum condition of 150℃ of the furnace and the concentration of （nH4）sMo7O24 and silver nitrate as described in various tables.

The wear and rolling contact fatigue (RCF) testing approaches for wheels and rails have been reviewedand evaluated in this study. The study points out the advantages and limitations of the existing approaches. Thebroad analysis revealed that scaled laboratory-based wear testing is widely applied. However, it is necessary topredetermine the input parameters and observing parameters for scaled wear testing for three reasons: first, toemulate the real-world scenarios as closely as possible;second, to postprocess the results received from the scaledtesting and transfer them into real practice at full scale;third, to present the results in a legible/appropriate ***, most of the important parameters required for wear testing have been discussed with fundamental andsystematic explanations provided. Additionally, the transition of the parameters from the real-world into the testdomain is explained. This study also elaborates on the challenges of the RCF and wear testing processes andconcludes by providing major considerations toward successful testing.

Sequential ultrasound-assisted extraction of pectin and phenolic compounds for the valorisation of ‘Granny Smith’ apple peel

This study aims to evaluate the impact of the sequential extraction system for phenolic compounds and pectin from waste ‘Granny Smith’ apple peel using ultrasound. The effects of solvent, formic acid concentration (CFA), and the number of extraction steps on the content of individual and total phenolic compounds (TPC), total flavonoids, and antioxidant capacity, were determined. Then, pectin was obtained from apple peel before (BE) and after (AE) phenolic compounds extraction, using conventional (TR) or ultrasound-assisted procedures (US). The two-steps 80% Acetone (0% CFA) extraction system had the highest TPC (3.47 g GAE/Kg). Procyanidin B2 (0.03-0.77 g/kg) was the major phenolic compound extracted from ‘Granny Smith’ apple peel. AE pectin extraction yield (6.38% for US and 4.92% for TR) was higher than BE. Pectin obtained had 57-60% DE, 9.3-10.3% Methoxyl content, and 436-460 equivalent weight. Wasted apple peel is a great low-cost source of phenolic compounds and pectin. Furthermore, it is possible to achieve the highest yields of both compounds through appropriate extraction sequences (AE: phenolic compound extraction followed by pectin extraction) and alternative technologies like ultrasound-assisted extraction.

Physiological performance in lizards may be affected by climate across latitudinal or altitudinal *** the coastal dune barriers in central-eastern Argentina,the annual maximum environmental temperature decreases up to 2℃ from low to high latitudes,while the mean relative humidity of the air decreases from 50%to 25%.Liolaemus multimaculatus,a lizard in the family Liolaemidae,is restricted to these coastal *** investigated the locomotor performance of the species at 6 different sites distributed throughout its range in these dune *** inquired whether locomotor performance metrics were sensitive to the thermal regime attributable to *** thermal performance breadth increased from 7%to 82%with latitude,due to a decrease in its critical thermal minimum of up to 5℃ at higher *** from high latitude sites showed a thermal optimum,that is,the body temperature at which maximum speed is achieved,up to 4℃lower than that of lizards from the low *** relatively low temperatures,the maximum running speed of high-latitude individuals was faster than that of low-latitude *** parameters of locomotor performance were labile,decreasing as a function of *** results show populations of *** adjust thermal physiology to cope with local climatic *** suggests that thermal sensitivity responds to the magnitude of latitudinal fluctuations in environmental temperature.
