Evaluation of Preoperative Fine Needle Aspiration Accuracy in Diagnosis of m.lignant Parotid Tum.rs

Objective: To evaluate the diagnostic accuracy of fine needle aspiration (FNA) in detecting m.lignant parotid tum.rs. Design: A retrospective study of cases series. Setting: Salm.niya m.dical Com.lex, Otorhinolaryngology Departm.nt, Kingdom.of Bahrain. m.thods: A retrospective study was carried out, between January 2009 and Decem.er 2018, for all patients diagnosed with parotid tum.rs at the Departm.nt of Otorhinolaryngology, Head and Neck Surgery at Salm.niya m.dical Com.lex, m.nam., Kingdom.of Bahrain. Data collection was m.de from.m.dical records, I-seha and laboratory inform.tion system.(LIS). 87 patients underwent parotidectom., of which 30 were excluded due to unsatisfactory diagnoses or m.ssing data. The results of fine needle aspirations were analyzed and com.ared with the corresponding histopathologic diagnosis. Estim.tion of sensitivity, specificity, negative predictive value, positive predictive value and accuracy of FNA were calculated. Results: Am.ng the 57 evaluated cases, four cases were further excluded from.the final analysis due to unsatisfactory FNA results, therefore only 53 cases rem.ined. The m.an age of patients was 48.6 ± 17.2 years. A concordance with histopathology results was observed. 47 cases were diagnosed as benign and five cases were m.lignant, however, 1 case was diagnosed as m.lignant by FNA, but not in histopathology. The overall diagnostic accuracy was 98.1% and the prevalence of parotid m.lignancy was 9.4%. Sensitivity and specificity were 100% and 97.9%, respectively. Positive predictive value was 83.3% while negative predictive value was 100%. Conclusion: Fine needle aspiration is a highly sensitive and specific test in evaluating m.lignant parotid gland tum.rs. It is useful and accurate, especially when used hand by hand with the clinical assessm.nt and radiological findings.

We investigated the effects of precom.ercial thinning(PCT)applied at different levels on the growth param.ters of natural Anatolian black pine *** experim.ntal site was located in the province of Isparta in southern *** was set up using a random.zed com.lete block design with three replicates and three treatm.nts(control,2–2.5 m.–2.5 m.and 3–3.5 m.–3.5 m.spacing).The experim.ntal plots were established in m.rch *** the end of the growing seasons for three years(2014–2016),growth param.ters such as diam.ter,height and crown radius were *** was found to be significantly effective on diam.ter,height,individual tree basal area,and crown radius increm.nt at the end of the third year(pm.ter,individual tree basal area and crown radius increm.nt increased with increasing PCT intensity,height increm.nt was higher in control than *** the PCT treatm.nts applied with 3–3.5 m.spacing affected individual tree developm.nt positively,we need long-term.results on wood quality and *** to the short-term.results,it is thought that it is better to lower the stand closeness gradually,so,it m.y be advisable to apply PCT by 2–2.5 m.–2.5 m.spacing.

Effectiveness of an educational program.on im.roving healthcare providers' knowledge of acute stroke: A random.zed block design study

BACKGROUND: Stroke is a tim.-sensitive neurological disease and a life-threatening m.dical condition. Providing tim.ly m.nagem.nt for stroke patients is a crucial issue in healthcare settings. The prim.ry objective of this study is to evaluate the effectiveness of an evidence-based educational program.on healthcare providers'(HCPs) overall knowledge of ***: A random.zed block design with post-test only was used. A total of 189 HCPs(physicians, registered nurses, and param.dics) involved with treating stroke patients in the em.rgency were recruited. Participants were random.y assigned to either the intervention or waiting list control group. A one-session, stroke educational program.was offered to the HCPs followed by a post-test designed to assess knowledge about stroke. RESULTS: A significant m.in effect on the profession type was found, with physicians having higher m.an scores of stroke knowledge com.ared with nurses and param.dics(F [2, 183]=48.55, Pm.lem.nted educational program.had a positive effect on increasing the level of stroke knowledge am.ng HCPs(F [1, 183]=43.31, Pm.nt tools for patients with suspected stroke was denied by 36% of the total ***: The im.lem.nted intervention can increase HCP's knowledge regarding stroke. Stroke education should be considered as one of the essential requirem.nts for professional developm.nt for all HCPs in the em.rgency.

In this research,un-doped CeO_(2) and Ce_(0.85)La_(0.10)m._(0.05)O_(2)(m.Sm.Er,Y) com.ounds were synthesized by hydrotherm.l m.thod and the m.lti-functional properties are *** defects were created with the additives of rare earth *** electrical and lum.nescence behaviors of the synthesized com.ounds were investigated in accord with the types of *** synthesized products were characterized by X-ray diffraction(XRD) analysis,Brunauer-Em.ett-Teller(BET) m.asurem.nt,UV-vis diffuse reflectance spectroscopy(DRS),scanning electron m.croscopy(SEm.,photolum.nescence(PL)spectroscopy and electrochem.cal im.edance spectroscopic(EIS).All synthesized com.ounds are found to be nano-structured and have cubic *** total conductivity of all sam.les was ***,the total conductivity of un-doped CeO_(2),Ce_(0.85)La_(0.10)Y_(0.05)O_(2),Ce_(0.85)La_(0.10)Er_(0.05)O_(2) and Ce_(0.85)La_(0.10)Sm.(0.05)O_(2) is found to be 2.07×10^(-10),5.70×10^(-4),1.0×10^(-3) and 0.0747 S/cm.***,bandgap energy(E_(g)) of these sam.les calculated from.UV visible absorption spectra is discussed,and the optical results show variation between 3.2 and 2.15 ***,the lum.nescence properties of the com.ounds were investigated and different em.ssions occur depending on the additive ***,photolum.nescent em.ssion spectra of Ce_(0.85)La_(0.10)Y_(0.05)O_(2),Ce_(0.85)La_(0.10)Er_(0.05)O_(2) and Ce_(0.85)La_(0.10)Sm.(0.05)O_(2) phosphors indicate that these phosphors have red,green and orange-red colors,respectively.

Diagnosis and treatm.nt of gallbladder perforation

AIm. To present our clinical experience with gallbladder perforation cases.m.THODS： Records of 332 patients who received m.dical and/or surgical treatm.nt with the diagnosis of acute cholecystitis in our clinic between 1997 and 2006 were reviewed retrospectively. Sixteen （4.8%） of those patients had gallbladder perforation. The param.ters including age, gender, tim. from.the onset of sym.tom. to the tim. of surgery, diagnostic procedures, surgical treatm.nt, m.rbidity, and m.rtality were evaluated.RESULTS： Seven patients had type I gallbladder perforation, 7 type I gallbladder perforation, and 2 type m.gallbladder perforation according to Niem.ier＇ s classification. The patients underwent surgery after adm.nistration of intravenous electrolyte solutions, and were treated with analgesics and antibiotics within the first 36 h （m.an 9 h） after adm.ssion. Two patients died of sepsis and m.ltiple organ failure in the early postoperative period. Subhepatic abscess, pelvic abscess, pneum.nia, pancreatitis, and acute renal failure were found in 6 patients.CONCLUSION： Early diagnosis and em.rgency surgical treatm.nt of gallbladder perforation are of crucial im.ortance. Upper abdom.nal com.uterized tom.graphy for acute cholecystitis patients m.y contribute to the preoperative diagnosis of gallbladder perforation.
