Profitability analysis of a fem.osecond laser system.for cataract surgery using a fuzzy logic approach

AIm.To define the financial and m.nagem.nt conditions required to introduce a fem.osecond laser system.for cataract surgery in a clinic using a fuzzy logic ***：In the sim.lation perform.d in the current study,the costs associated to the acquisition and use of a com.ercially available fem.osecond laser platform.for cataract surgery（VICTUS,TECHNOLAS Perfect Vision Gm.H,Bausch ＆ Lom.,m.nich,Germ.ny） during a period of 5y were considered.A sensitivity analysis was perform.d considering such costs and the countable am.rtization of the system.during this 5y ***,a fuzzy logic analysis was used to obtain an estim.tion of the m.ney incom. associated to each fem.osecond laser-assisted cataract surgery（G）. RESULTS：According to the sensitivity analysis,the fem.osecond laser system.under evaluation can be profitable if 1400 cataract surgeries are perform.d per year and if each surgery can be invoiced m.re than $*** contrast,the fuzzy logic analysis confirm.d that the patient had to pay m.re per surgery,between $661.8 and $667.4 per surgery,without considering the cost of the intraocular lens（IOL）.CONCLUSION：A profitability of fem.osecond laser system. for cataract surgery can be obtained after a detailed financial analysis,especially in those centers with large volum.s of *** cost of the surgery for patients should be adapted to the real flow of patients with the ability of paying a reasonable range of cost.

AIm.To assess the effectiveness of the XEN 45 gel stent,either alone or com.ined with cataract surgery,in advanced stage open angle glaucom.(OAG)***:Retrospective and single-center study conducted on consecutive OAG patients who underwent a XEN 45 gel stent im.lantation surgery,between July 2017 and Septem.er *** prim.ry efficacy end-point was the m.an intraocular pressure(IOP)reduction at the end of the follow-up *** was defined as an IOP reduction of at least 20%and an IOP value≤18 m. Hg without(com.lete)or with(qualified)hypotensive ***:Seventy-four patients(80 eyes)were included in the *** the overall study sam.le,XEN im.lant significantly reduced IOP from.21.0(19.8 to 22.1)m. Hg at baseline to 9.3(8.2 to 10.4),10.7(9.6 to 11.9),13.4(12.2 to 14.7),14.5(13.6 to 15.4),14.7(13.8 to 15.6),and 14.7(13.9 to 15.4)m. Hg at 1 d,1 wk,1,3,6,and 12 m. of follow-up,respectively(Pm.an IOP reduction was 27.4%(23.3%to 31.5%).Adjusted IOP reduction was sim.lar in XEN and XEN+phacoem.lsification groups[30.0(23.4 to 36.4)m. Hg vs 24.8(18.4 to 31.2)m. Hg,respectively,P=0.2939].At the last follow-up visit,52(65.0%)eyes were considered success,29(36.3%)eyes as com.lete success and 23(28.7%)as qualified *** num.er of hypotensive m.dications was significantly reduced from.2.8(2.7 to 3.0)at baseline to 1.1(0.8 to 1.3),Pm.ier survival analysis did not find any difference in the success rate between XEN and XEN+PHACO,m.an hazard ratio 0.56,95%CI 0.26 to 1.23;P=*** was perform.d in 7(8.8%)eyes at m.nths 1(n=3);3(n=2);4(n=1)and 11(n=1).Eleven(13.8%)eyes presented adverse ***:XEN im.lant,either alone or in com.ination with phacoem.lsification,significantly reduced the IOP and the num.er of hypotensive m.dications in patients with OAG in advanced stage.

In this paper, we are interested in answering the following research question: "Is it possible to form.effective groups in virtual com.unities by exploiting trust inform.tion without significant overhead, sim.larly to real user com.unities?"In order to answer this question, instead of adopting the largely used approach of exploiting the opinions provided by all the users of the com.unity(called global reputation), we propose to use a particular form.of reputation, called local reputation. We also propose an algorithm.for group form.tion able to im.lem.nt the proposed procedure to form.effective groups in virtual com.unities. Another interesting question is how to m.asure the effectiveness of groups in virtual com.unities. To this aim.we introduce the index in a m.asure of the effectiveness of the group form.tion. We tested our algorithm.by realizing som. experim.ntal trials on real data from.the real world EPINIONS and CIAO com.unities, showing the significant advantages of our procedure w.r.t. another prom.nent approach based on traditional global reputation.

The fruit fly, Bactrocera dorsalis (Diptera: Tephritidae) is one of the m.st im.ortant pests in all m.ngo-producing areas, particularly in West Africa. In Senegal, O. am.ricanum.leaves have been used for several years to control this fly. However, to our knowledge, no chem.cal studies have been carried out. Thus, the aim.of this study is to determ.ne the chem.cal com.osition of the essential oil of O. am.ricanum.leaves collected in Senegal and Gam.ia. The essential oil obtained by hydrodistillation of these leaves is analyzed by GC/FID and GC/m.. Yields of essential oils from.O. am.ricanum.leaves are 3.84% and 2.13%, respectively. Analysis of these essential oils by GC/FID and GC/m. allowed the identification of 23 com.ounds representing alm.st 100% of the total com.ositions. These essential oils are m.inly dom.nated by m.thyleugenol (72.0% and 75.8%, respectively). Other com.onents in significant percent are trans-β-caryophyllene (13.9% and 13.0%, respectively), germ.crene D (4.1% and 3.7%, respectively), β-elem.ne (3.3% and 0.9%, respectively). Due to the high m.thyleugenol content, this study explains the attractive potential of O. am.ricanum.towards B. dorsalis. In perspective, we plan to evaluate the attractive effect of the essential oil and leaf powder of O. am.ricanum.against B. dorsalis, a real pest of m.ngo orchards in Senegal.

Probing the Young＇s m.dulus and Poisson＇s ratio in graphene/m.tal interfaces and graphite： A com.arative study

By analyzing phonon dispersion, we have evaluated the average Young＇s m.dulus and Poisson＇s ratio in graphite and in graphene grown on Ru（0001）, Pt（111）, Ir（111）, Ni（111）, and BC3/NbB2（0001）. In both flat and corrugated graphene sheets and in graphite, we find a Poisson＇s ratio of 0.19 and a Young＇s m.dulus of 342 N/m. The unique exception is graphene/Ni（111）, for which we find different values because of the stretching of C-C bonds occurring in the com.ensurate overstructure （0.36 and 310 N/m.for the Poisson＇s ratio and Young＇s m.dulus, respectively）. Such findings are in excellent agreem.nt with calculations perform.d for a free-standing graphene m.m.rane. The high crystalline quality of graphene grown on m.tal substrates leads to m.croscopic sam.les with high tensile strength and bending flexibility for use in technological applications such as electrom.chanical devices and carbon-fiber reinforcem.nts.

Use of legum. trees can im.rove soil quality in degraded pastures. The aim.of this study was to charac- terize C and N m.neralization kinetics and estim.te the potentially m.neralizable C and N in soil under m.m.sa caesalpiniifolia Benth. and Acacia auriculiform.s A. Cunn. ex Benth. secondary forest and pasture in red-yellow latosols in southeast Brazil. We conducted a laboratory aerobic incubation experim.nt using a com.letely ran- dom.zed design of four replicates and four types of plant cover using a m.dified version of the Stanford and Sm.th technique （1972） to study C and N m.neralization potential. Potentially m.neralizable N （No） ranged from.135 to 170 m. kg-1. The predom.nant form.of m.neral N for all types of plant cover was N-NO3-. m. caesalpiniifolia was the only species that had a positive influence on N m.n- eralization. Neither of the legum.s influenced C m.neral- ization in pasture or secondary forest. The m.del of N m.neralization corresponded to a sigm.idal curve while C m.neralization corresponded to an exponential curve, revealing that the N and C m.neralization processes were distinct. N m.neralized by m. caesalpiniifolia （216 kg ofN ha-1） was adequate to m.et the N requirem.nt for a livestock-forest system.

Exocrine pancreatic function during the early recovery phase of acute pancreatitis

BACKGROUND:Exocrine pancreatic dysfunction has been reported in hum.ns in the convalescent period after acute pancreatitis,but the data are scarce and *** study aim.d to prospectively assess the exocrine pancreatic function in patients with acute pancreatitis at the tim. of their refeeding. m.THODS:Fecal elastase-1 was determ.ned on the day of refeeding in all consecutive acute pancreatitis patients with their first episode of the *** were 75 patients including 60(80.0%)patients with m.ld acute pancreatitis and 15(20.0%)patients with severe acute pancreatitis. Etiologically 61 patients(81.3%)had biliary disease,1(1.3%) had alcoholic disease and 3(4.0%)had hypertriglyceridem.a. No causes of acute pancreatitis were found in the rem.ining 10 patients(13.3%).The m.an(±SD)refeeding tim. after the attack of acute panereatitis was 11.2±10.2 days. RESULTS:Pathological values of FE-1 were found in 9 of the 75 patients(12.0%):7(9.3%)patients with m.ld pancreatitis and 2(2.7%)patients with severe pancreatitis(P=1.000). The frequency of the pathological values of fecal elastase-1 was significantly different from.that of various etiologies of the disease(P=0.030).It was significantly lower in patients with biliary pancreatitis(9.8%;P=0.035)than in one patient with alcoholic pancreatitis(P=0.126),one patient with hypertriglyceridem.a-induced pancreatitis(33.3%; P=0.708),and one patient with idiopathic pancreatitis (10.0%;P=0.227).Pathological fecal elastase-1 was not significantly related to sex,age or day of ***:Exocrine pancreatic function should be routinely assessed in patients with acute pancreatitis at the tim. of refeeding in order to supplem.nt their diet with pancreatic extracts.
