双气囊小肠镜与推进式肠镜检查的对比

Background: Double-balloon enteroscopy (DBE) is a novel procedure for the diagnosis of sm.ll-bowel pathology. The aim.of this retrospective study was to com.are the perform.nce and the diagnostic value of antegrade DBE with those of push enteroscopy (PE). m.thods: We reviewed endoscopic and histologic findings in 118 patients exam.ned by PE or antegrade DBE during a period 1980 to 2004. The m.xim.l length of insertion under plain radiograph was com.ared between patients exam.ned by PE and those exam.ned by antegrade DBE. Diagnostic yield was com.ared am.ng patients stratified by indication for enteroscopy and the duodenal pathology. Results: Ninety-one patients were exam.ned by PE and 27 patients by antegrade DBE. Length of insertion from.the ligam.nt of Treitz was significantly greater in antegrade DBE (m.dian, 92 cm. range, 40-144 cm. than in PE (m.dian, 22 cm. range, 0-98 cm. p m.atory or m.scellaneous diseases or polyposis, the diagnostic yield was not different between PE and antegrade DBE (64% vs. 82% , p = 0.13). However, it was higher in antegrade DBE (79% ) than in PE (31% , p = 0.012) in nonbleeding patients without duodenal pathology. In bleeding patients, the diagnostic yield was 40% in antegrade DBE and 36% in PE (p = 0.61). Conclusions: Antegrade DBE is superior to PE in exploration of the sm.ll intestine and in diagnostic yield for sm.ll-intestinal pathology.

Background and Objective: Diagnostic accuracy of probe EUS depends on the constant m.intenance of lum.nal m.dium.for acoustic coupling. This study com.ared the accuracy of probe EUS by a jelly-filled m.thod (EUS-J) and by a water-filled balloon m.thod (EUS-W) for the assessm.nt of invasion depth in superficial esophageal cancer. Design and Setting: A prospective, single-center study. Patients: Forty superficial esophageal cancers in 38 patients. Interventions: Patients were alternately assigned to EUS-J or EUS-W. The depth of invasion was classified into epithelium.r lam.na propria m.cosae (D1),m.scularis m.cosae or superficial layer of the subm.cosa (D2), and deep portion of the subm.cosa (D3). Depiction rate, interobserver variation between 2 observers, and accuracy for the determ.nation of invasion depth were com.ared between EUS-J and EUS-W. m.in Outcom. m.asurem.nts and Results: Eighteen cancers were exam.ned by EUS-J, and 22 cancers were exam.ned by EUS-W. The actual depth of invasion was D1 in 21 cancers, D2 in 9 cancers, and D3 in 10 cancers. Depiction rate (94.4% vs 77.2% , P = .14) and interobserver agreem.nt of EUS determ.nation (82.3% vs 58.8% , P = .13) was higher in EUS-J than in EUS-W. The overall accuracy for the diagnosis of invasion depth was 77.8% in EUS-J and 59.1% in EUS-W (P = .18). The sensitivity for the diagnosis of D1 cancer was significantly higher in EUS-J than in EUS-W (100% vs 50% , P = .03), while the specificity was not different between the 2 procedures (81.8% vs 87.5% ). Conclusions: EUS-J is superior to EUS-W for the assessm.nt of invasion depth in superficial esophageal cancer, especially for intram.cosal cancer.

Objective. The aim.of this study was to elucidate the predictive value of intra-operative enteroscopy (IOE) and the effect of enteral nutrition (EN) with regard to the postoperative recurrence of Crohn disease (CD). m.terial and m.thods. Forty patients requiring surgery for severe intestinal com.lications of CD were exam.ned by IOE, and the severity of the rem.ant sm.ll intestine was determ.ned. Patients were subclassified into either an EN group (> 1,200 kcal/day) or a non-EN group (m.unt of daily EN intake after surgery. Contributions of IOE findings and EN to postoperative recurrence were analysed retrospectively. Results. IOE identified intestinal lesions in 39 patients and active intestinal lesions in 24 patients. The cum.lative rate of postoperative recurrence was significantly higher in patients with cobblestone appearance confirm.d by IOE (p = 0.006). However, other active intestinal lesions were not related to postoperative recurrence. EN reduced the cum.lative rate of postoperative recurrence (p = 0.017), especially in patients with penetrating type (p = 0.005), in patients who did not have colitis (p = 0.051) and in patients who did not have active intestinal lesions confirm.d by IOE (p = 0.02). Conclusions. EN is a prophylactic that prevents the postoperative recurrence of sm.ll intestinal CD. Patients with the penetrating type of CD, and those who do not have active lesions in the sm.ll intestine according to IOE, are candidates for EN after surgery.

In order to establish a rapid m.thod for regional slope stability analysis under rainfall,m.tric suction and seepage force were taken into account after obtaining explicit solution of infiltration ***,sim.lified analysis m.del under 3D condition was put forward based on identification and division of slope units,as well as m.dification of sliding direction of each *** result shows that explicit solution of infiltration depth is of good precision;for the given m.del,safety factors without taking seepage force into account are 1.82-2.94 tim.s higher;the stagnation point of slope angle is located approxim.tely in the range of(45°,50°);the safety factor changes insignificantly when wetting front is deeper than 2 m.when m.tric suction changes in the specified range,the m.xim.m.variations of safety factor are less than 0.5,which proves that m.tric suction plays an insignificant role in m.intaining slope stability com.ared to the slope angle and infiltration *** with geographic inform.tion system.a practical application of regional slope stability assessm.nt verifies the applicability of the proposed m.thod.

BACKGROUND Endoscopic subm.cosal dissection(ESD)is widely accepted for early gastric cancer(EGC)without lym.h node m.tastasis,although ESD is challenging,even for sm.ll lesions,in the greater curvature(GC)of the upper(U)and m.ddle(m.thirds of the *** forceps-assisted endoscopic resection(GF-ER)is a type of endoscopic m.cosal resection that is perform.d via a double-channel *** To investigate the safety and efficacy of GF-ER vs ESD in the GC of the stom.ch’s U and m.*** We retrospectively reviewed the m.dical records of 506 patients who underwent ER of 522 EGC lesions in the stom.ch’s U and m.regions in three institutions between January 2016 and m.y *** lesions from.eight patients who underwent GF-ER for EGC(the GF-ER group)were com.ared to 63 lesions from.63 patients who underwent ESD(the ESD group).We also perform.d a subgroup analysis of sm.ll lesions(≤10 m.)in 6 patients(7 lesions)from.the GF-ER group and 20 patients(20 lesions)from.the ESD *** There were no statistically significant differences between the GF-ER and ESD groups in the en bloc resection rates(100%vs 100%)and the R0 resection rates(100%vs 98.4%).The m.dian procedure tim. in the GF-ER group was shorter than that in the ESD group(4.0 m.n vs 55.0 m.n,Pm.,the m.dian procedure tim. in the GF-ER group was still shorter than that in the ESD group(4.0 m.n vs 35.0 m.n,Pm.y be an effective therapeutic option for sm.ll lesions in the GC of the stom.ch’s U and m.regions.

Im.unosenescence is m.rked by accelerated degradation of host im.une responses leading to the onset of opportunistic infections, where senescent T cells show rem.rkably higher ontogenic defects as com.ared to healthy T cells. The m.chanistic association between T-cell im.unosenescence and hum.n im.unodeficiency virus(HIV) disease progression, and functional T-cell responses in HIV-tuberculosis(HIV-TB) co-infection rem.ins to be elaborately discussed. Here, we discussed the association of im.unosenescence and chronic im.une activation in HIV-TB co-infection and reviewed the role played by m.diators of im.une deterioration in HIV-TB coinfection necessitating the im.ortance of designing therapeutic strategies against HIV disease progression and pathogenesis.

BACKGROUND A right-sided round ligam.nt(RSRL)is a rare,congenital anom.ly of the intrahepatic portal vein,with a reported frequency of 0.2%-1.2%.For patients with perihilar cholangiocarcinom. associated with an RSRL,an accurate understanding of the vascular and biliary anatom. is *** SUm.ARY We report a 70-year-old m.le with perihilar cholangiocarcinom. associated with an *** percutaneous transhepatic em.olization of the left and anterior portal branches,we conducted a left trisectionectom. of the liver with extrahepatic bile duct resection and *** postoperative course was uneventful,and R0 resection was *** the liver volum. of each section was com.ared between 7 patients with an RSRL and 20 patients with norm.l portal vein anatom.,the posterior section in RSRL patients was significantly larger than that in patients with norm.l portal vein anatom.(m.dian:457 m. vs 306 m.,P=0.031).In patients with perihilar cholangiocarcinom. associated with an RSRL,left trisectionectom. has several surgical advantages:(1)The posterior branch of the portal vein often ram.fies independently,and the division of the portal vein is easily conducted;(2)A relatively large am.unt of rem.ant liver can be retained;and(3)The anatom. of the posterior branch of the Glissonian pedicle is sim.lar to that in patients with norm.l *** In patients with an RSRL and perihilar cholangiocarcinom. that does not involve the posterior section,left trisectionectom. m.y be a favorable choice.

BACKGROUND We recently reported on a hereditary enteropathy associated with a gene encoding a prostaglandin transporter and referred to as chronic enteropathy associated with SLCO2 A1 gene(CEAS). Crohn's disease(CD) is a m.jor differential diagnosis of CEAS, because these diseases share som. clinical features. Therefore, there is a need to develop a convenient screening test to distinguish CEAS from.*** To exam.ne whether prostaglandin E m.jor urinary m.tabolites(PGE-m.m. can serve as a biom.rker to distinguish CEAS from.*** This was a transactional study of 20 patients with CEAS and 98 patients with *** was diagnosed by the confirm.tion of hom.zygous or com.ound heterozygous m.tation of SLCO2 A1. We m.asured the concentration of PGEm.m.in spot urine by radioim.unoassay, and the concentration was com.ared between the two groups of patients. We also determ.ned the optim.l cut-off value of PGE-m.m.to distinguish CEAS from.CD by receiver operating characteristic(ROC) curve *** Twenty Japanese patients with CEAS and 98 patients with CD were ***-m.m.concentration in patients with CEAS was significantly higher than that in patients with CD(m.dian 102.7 vs 27.9 μg/g × Cre, P m.m.contributed to 7.3 increase in the likelihood for the diagnosis of CEAS [95% confidence interval(CI) 3.2-16.7]. A logistic regression analysis revealed that the association was significant even after adjusting confounding factors(adjusted odds ratio 29.6, 95%CI 4.7-185.7). ROC curve analysis revealed the optim.l PGE-m.m.cut-off value for the distinction of CEAS from.CD to be 48.9 μg/g × Cre with 95.0% sensitivity and 79.6% *** PGE-m.m.m.asurem.nt is a convenient, non-invasive and useful test for the distinction of CEAS from.CD.
