Threatened miscarriage is the commonest complication of early pregnancy and affects about 20% of pregnancies. It presents with vaginal bleeding with or without abdominal cramps. Increasing age of women, smoking, obesity or polycystic ovary syndrome(PCOS) and a previous history of miscarriage are risk factors for threatened miscarriage. The pathophysiology has been associated with changes in levels of cytokines or maternal immune dysfunction. Clinical history and examination, maternal serum biochemistry and ultrasound findings are important to determine the treatment options and provide valuable information for the prognosis. Bed rest is the commonest advice, but there is little evidence of its value. Other options include progesterone, human chorionic gonadotropin(HCG) and muscle relaxants. The complementary and alternative medicine(CAM) therapies such as acupuncture and Chinese herbs have also been tried. There is some evidence from clinical studies indicating that CAM therapies may reduce the rate of miscarriage, but the quality of studies is poor. Thus, further double-blind,randomized-controlled trials are necessary to confirm its effectiveness, especially acupuncture and Chinese herbs.

Geotechnical and geochemical assessment of natural sands being used as fine aggregates for construction purposes in Ahanta West and its Environs was carried out, as well as addressing the environmental problems associated with the exploitation of these sand through the sand winning activities. The results from geotechnical and geochemical tests were bench marked with British, American and Ottawa Standards for testing materials. Representative samples were taken from five different sand winning sites namely, Pompuni, Agona Banso, Fretsi, Tumentu stream and Fretsi river. The geotechnical tests carried out were specific gravity, grain size analysis, moisture content, silt content, permeability test using the constant head permeameter method. The photometer (Hydro Test HT 1000) was used to determine silica, sulphate and chloride concentrations in the sand samples. The oxides for the geochemical analysis were determined using the X-ray fluorescence (XRF) method. The results from the study areas showed that except for Fretsi, the specific gravity, grain size analysis, moisture content, silt content and permeability tests were within the acceptable range for natural sands. Sand samples from Pumponi and Agona Banso had the required Cu and Cc values of being well graded. Sulphate and chloride concentrations were within standards for all the samples;however, silica was low for all the samples. Fe2O3, Al2O3, MnO and Na2O did not meet the Ottawa sand standard. Natural sand is not recommended to be used for construction purposes however, these sands may be treated or blended if it is to be used for construction purposes. Background： Direct ZN （Ziehl-Neelsen） sputum smear microscopy for diagnosis of TB （tuberculosis） has low sensitivity, especially in TB/HIV co-infected patients. Sputum concentration by bleach （NaOCI） with sedimentation has been used to increase the sensitivity of sputum smear microscopy in many settings but with varying results. Objective： To determine whether bleach plus centrifugation significantly improves the detection of AFB （acid-fast bacilli） in ZN smear-negative sputum specimens. Methods： Three hundred and seventy sputum specimens were collected from new TB suspects attending a Nairobi referral district hospital and processed for direct microscopy using ZN technique and culture on Lowenstein Jensen Media. All smear-negative specimens were treated with 3.5% bleach and left to stand for 30 min before centrifugation. The bleach treated smears were processed and examined using ZN technique. Results： Of the 370 specimens, 200 （54%） were positive culture. The number of sputum samples that were smear-positive by direct ZN was 138 （37.2%）, with a sensitivity of 66%. After treatment of direct ZN smear-negative specimens with 3.5% bleach and centrifugation, the total number of AFB smear-positive samples increased to 171 with an increase in sensitivity of 66% to 81.1% （15.1%）. Conclusion： In this study, bleach with centrifugation significantly increased the yield of sputum smear microscopy. Further evaluation of these techniques in routine programmes is required especially in settings where the burden of TB/HIV is high.

Effects of the substitution of cow’s milk with soymilk on the micronutrients,microbial,and sensory qualities of yoghurt

The consumption of imported dairy products is high especially in developing countries including Nigeria where little or no milk is produced locally which makes the products expensive,out of reach to common consumers and also inconvenient for lactose intolerance and ***,as plant-based milk was substituted for cow’s milk in parts and in whole for the production of yoghurt and the effect of substitution was evaluated on the minerals,vitamins,microbial,and sensory properties of the products using the established *** differences(p<0.05)exits among the yoghurt samples in the micronutrient *** soymilk substitution produced yoghurts with trace/macro mineral contents’range of 0.14–28.50 mg/100 g and vitamin contents(ranged 0.02–0.43 mg/100 g)that were of comparable range of values with the yoghurt made with 100%cow’s milk(0.21–29.60 mg/100 g minerals and 0.05–0.58 mg/100 g vitamins).Generally,no significant difference(p<0.05)was observed in most of the sensory attributes evaluated(range 8.00–8.01)in the entire samples(except in the 100%soymilk yoghurt).The microbial analysis proved the safety of the entire products for *** showed the positive effect of substituting soymilk for cow’s milk at all the levels used,thus,the lactose intolerance and vegans can find alternative to dairy yoghurt in the 100%soymilk substitution and enjoy the functional health benefits of ***,the total dependence on importation of dairy milk and cost of production of composited milk yoghurt could be reduced to improve the nation’s economy and make the product economically accessible to common man.
