BACKGROUND:Primary sclerosing cholangitis(PSC)is a chronic cholestatic liver disease characterized by destruction and fibrosis of the bile *** study aimed to demonstrate the hepatic and extrahepatic characteristic findings and prognostic outcomes of Turkish patients with PSC. METHODS:The medical records of 35 consecutive patients with PSC from January 1988 to June 2007 were recorded *** the time of diagnosis,clinical features and laboratory data were collected. RESULTS:The mean age of the 35 patients was 41.69 years (range 15-80 years)at the time of diagnosis;14(40%)were female,and 21(60%)were *** mean duration of follow-up was 58.86 months(1-180 months).Twenty(57.1%) of the patients with PSC were asymptomatic and 22(62.9%) had inflammatory bowel *** the time of diagnosis, 20(57.1%)of the patients had both intra-and extra-hepatic ***-one(60%)of the patients,who had undergone ERCP for stent placement,had dominant bile duct stenosis. Cholangiocarcinoma was found in 2(5.7%)of the patients and cirrhosis was detected in 7(20%);5(14.3%)underwent liver *** median follow-up time after liver transplantation was 23 months and all are still *** (17.1%)patients died. CONCLUSIONS:PSC has a clinical course varied from advanced liver disease requiring liver transplantation within a short time to being asymptomatic for decades. The prognosis of Turkish patients with PSC is also disappointing as described in other studies.

In 1923, Masson1 described an unusual papillary endothelial proliferation that he named as 'hemangioendotheliome vegetant intravasculaire'. In 1976, Clearkin and Enzinger2 coined the term intravascular papillary endothelial hyperplasia (IPEH) that is now in use. This term refers to the presence of endothelium lined papillary projections within a vascular space. Characteristically, the papillary structures are composed of a single layer of swollen or plump endothelial cells without cellular pleomorphism, mitotic activity or necrosis.

Liquid polymers(LP)have become an important structural material used in the construction industry in the last *** paper investigates the viability of using commercially available LPs as a coating material to improve the flexural strength of fiber-modified concrete *** scope included preparing rectangular prism concrete beams with a concrete mixture including fly ash and fiber and coating them with four different liquid polymers at a uniform thickness following the curing process while one set of samples was maintained under the same conditions as a control group without *** addition,cylindrical samples were prepared to determine the compressive strength of the concrete *** the curing process in an unconfined open-air laboratory environment for another 28 days,concrete samples were tested to determine the flexural strength and deflection characteristics under center point loading *** results revealed that all four coating types enhanced both the flexural strength and the average maximum deflection of the beams compared to the control *** the enhancement in the flexural strength changed approximately between 5%and 36%depending on the coating type,the improvements in average maximum deflections varied between 3.7%and 28.4%.

Capecitabine is an orally available chemotherapeutic agent that is converted to 5-fluorouracil （5-FU） after absorbtion. Capecitabine and its active metabolite, 5-FU, have cardiotoxic effects with reported instances of acute coronary syndromes caused due to coronary vasospasm. However, these agents exert toxic effects on cardiovascular system and beyond vasospasm provacation. We report a 46-year-old patient diagnosed as acute inferior infarction who is treated with capecitabine for 3 months due to metastatic breast carcinoma, in whom thrombotic coronary occlusion was observed in angiography. This case demonstrates that apart from vasospasm, coronary thrombosis could be observed after capecitabine treatment, with a possible direct effect of this drug.

Effect of transcatheter aortic valve implantation on QT dispersion in patients with aortic stenosis

Background QT dispersion （QTd） is a predictor of ventricular arrhythmia. Ventricular arrhythmia is an important factor influencing morbidity and mortality in patients with aortic stenosis. Surgical aortic valve replacement reduced the QTd in this patients group. However, the effect of transcatheter aortic valve implantation （TAVI） on QTd in patients with aortic stenosis is unknown. The aim of this study was to investigate the effect of TAVI on QTd in patients with aortic stenosis. Methods Patients with severe aortic stenosis, who were not candi-dates for surgical aortic valve replacement due to contraindications or high surgical risk, were included in the study. All patients underwent electrocardiographic and echocardiographic evaluation before, and at the 6th month after TAVI, computed QTd and left ventricular mass index （LVMI）. Results A total 30 patients were admitted to the study （mean age 83.2 ± 1.0 years, female 21 and male 9, mean valve area 0.7 ± 3 mm2）. Edwards SAPIEN heart valves, 23 mm （21 patients） and 26 mm （9 patients）, by the transfemoral approach were used in the TAVI procedures. All TAVI procedures were successful. Both QTd and LVMI at the 6th month after TAVI were significantly reduced com-pared with baseline values of QTd and LVMI before TAVI （73.8 ± 4 ms vs. 68 ± 2 ms, P=0.001 and 198 ± 51 g/m2 vs. 184 ± 40 g/m2, P=0.04, respectively）. There was a significant correlation between QTd and LVMI （r=0.646, P〈0.001）. Conclusions QTd, which malign ventricular arrhythmia marker, and LVMI were significantly reduced after TAVI procedure. TAVI may decrease the possibility of ventricu-lar arrhythmia in patients with aortic stenosis.

目的:比较单用贝伐单抗与贝伐单抗联合曲安奈德治疗糖尿病黄斑水肿疗效。方法:回顾性比较随机研究。对21例42眼黄斑水肿的糖尿病患者进行了评估。单眼内注射1. 25 mg贝伐单抗(IVB组),在同组的眼玻璃体内注射1. 25 mg贝伐单抗和1 mg曲安奈德(IVTA-IVB组)。使用光学相干断层扫描(OCT)、ETDRS视力和眼压测量黄斑中心厚度(CMT)。结果:平均随访时间为4. 7±1. 5mo。在IVB组和IVTAIVB组中,注射前平均CMT分别为494. 7±114. 4μm,546. 8±165. 6μm;第一个月分别为430. 4±133. 2μm,363. 7±105. 3μm;第三个月分别为484. 8±167. 4μm,407. 3±108. 7μm;六个月后分别为550. 4±191. 5μm,516. 8±158μm。第一个月和第三个月存在显著差异(P <0. 05)。在IVB组和IVTA-IVB组中,注射前平均ETDRS视力数分别为57. 1±13. 5,48. 9±13. 9;第一个月分别为2. 2±14,58. 8±12. 1;第三个月分别为59±13. 7,59. 3±13. 6;第六个月分别为55. 6±14. 9,55. 5±8. 7。第三个月和第六个月存在显著差异(P<0. 05)。IOP无差异。第一次注射后IVTA-IVB组在第三个月时视力最佳并持续6个月,而IVB组在第一个月时视力最佳但仅持续了3个月。结论:6个月后发现,IVTA-IVB组较IVB组疗效显著,且无激素依赖性并发症。
