Serum Uric Acid Level Has Stronger Correlations with Metabolic Syndrome-Related Markers in Women than in Men in a Japanese Health Check-Up Population

Background A serum uric acid (UA) level of 7.0 mg/dL has been used as the criterion for hyperuricemia in Japan regardless of gender, despite higher serum UA levels in men than in women. Serum UA has been identified as a predictive biomarker for metabolic syndrome (MetS);however, the gender differences in the association between UA levels and MetS-related conditions in a Japanese population have not been completely assessed. Objective To examine gender and age differences in the associations between serum UA levels and other biomarkers within a health-screened Japanese population and to evaluate the usefulness of serum UA as a predictor of MetS between the two genders. Methods A cross-sectional study of healthy individuals in Japan (16,391 men;16,656 women) was conducted. Associations between UA and several biomarkers were analyzed for each gender type and for age- and serum UA level-stratified groups. Logistic regression was used to analyze the association of age and serum UA levels with MetS-related conditions. Receiver operating characteristic (ROC) curve analysis was performed to identify the UA cut-off value for predicting the risk of the MetS-related conditions. Results Serum UA levels in women had stronger correlations with MetS-related biomarkers than in men. After adjusting for age, the odds ratios for a 1-mg/dL serum UA increase for diabetes mellitus and dyslipidemia in women were 1.13 (95% confidence interval, 1.04 - 1.23) and 1.30 (1.25 - 1.34), respectively. In ROC analysis, women had significantly higher area under the curve (AUC) values for MetS prediction than men. Conclusion An elevated serum UA level has a higher predictive ability for the risk of MetS-related conditions in Japanese women than in men. The enzymatic oxidation of (+)-catechin (Cat) with copper-containing oxidase, tyrosinase, was applied to hair colouring in the study. The Cat is oxidised to be o-benzoquinone form, the catechinone, which works as the hair dyestuff. Four kinds of hair colouring techniques using Cat were examined and each of their dyeability was compared in order to improve it. The first dyeing method is dyeing hair by the redissolved catechinone, which is preliminarily obtained by the oxidation of Cat (Redissolution Dyeing Method). The second one is dyeing hair in the dye solution just after the dye being formed by the enzymatic reaction (Sequential Dyeing Method). The third one is dyeing hair in solution containing Cat and the dyeing process is accompanied by the oxidation of Cat (Simultaneous Oxidation Dyeing Method). The fourth one is two-step dyeing that the hair immersed in Cat solution at the first step and then it is oxidised enzymatically to colour at the second step (Post-Oxidation Dyeing Method). The hair is dyed pale yellow by Redissolution Method, deeper yellowish brown by Sequential and Simultaneous Oxidation Dyeing Method and slightly reddish brown by Post-Oxidation Method with Cat. The order of their dyeability is the Simultaneous Oxidation ≈ Post-Oxidation > Sequential > Redissolution Dyeing Method at 30°C. The dyeability of Redissolution Method increases with an increase in dyeing temperature (T) between 20°C - 60°C. In Simultaneous Oxidation Method, the T increment also provides higher dyeability, and the colour of dyed hair becomes duller over 50°C. The Post-Oxidation Method shows higher dyeability at the lower oxidation temperature (20 °C - 40°C).

Successful treatment of multiple lung metastases of hepatocellular carcinoma by combined chemotherapy with docetaxel, cisplatin and tegafur/uracil

We report the successful treatment of multiple lung metastases after hepatic resection for hepatocellular carcinoma (HCC) with combined docetaxel, cisplatin (CDDP), and enteric-coated tegafur/uracil (UFT-E). A 68-year-old man was diagnosed with multiple lung metastases of HCC 7 mo after partial hepatectomy for HCC. Oral UFT-E was given daily and docetaxel and CDDP were given intra-arterially (administered just before the bronchial arteries) every 2 wk via a subcutaneous injection port. One month after starting chemotherapy, levels of tumor marker, protein induced by vitamin K absence(PIVKA--), decreased rapidly,and after a further month, chest X-ray and computed tomography revealed the complete disappearance of multiple liver metastases. Two years after the combined chemotherapy, HCC recurred in the liver and was treated but no pulmonary recurrence occurred. In the absence of a standardized highly effective therapy, this combined chemotherapy with docetaxel, CDDP and UFT-E may be an attractive option for multiple lung metastases of HCC.

Comparison of a new aspiration needle device and the Quick-Core biopsy needle for transjugular liver biopsy

AIM: To evaluate sample adequacy, safety, and needle passes of a new biopsy needle device compared to the Quick-Core biopsy needle for transjugular liver biopsy in patients affected by liver disease. METHODS: Thirty consecutive liver-disease patients who had major coagulation abnormalities and/or relevant ascites underwent transjugular liver biopsy using either a new needle device (18 patients) or the Quick-Core biopsy needle (12 patients). The length of the specimens was measured before fixation. A pathologist reviewed the histological slides for sample adequacy and pathologic diagnoses. The two methods’ specimen adequacy and complication rates were assessed. RESULTS: Liver biopsies were technically successful in all 30 (100%) patients, with diagnostic histological core specimens obtained in 30 of 30 (100%) patients, for an overall success rate of 100%. With the new device, 18 specimens were obtained, with an average of 1.1 passes per patient. Using the Quick-Core biopsy needle, 12 specimens were obtained, with an average of 1.8 passes per patient. Specimen length was significantly longer with the new needle device than with the Quick- Core biopsy needle (P < 0.05). The biopsy tissue was not fragmented in any of the specimens with the new aspiration needle device, but tissue was fragmented in 3 of 12 (25.0%) specimens obtained using the Quick-Core biopsy needle. Complications included cardiac arrhythmia in 3 (10.0%) patients, and transient abdominal pain in 4 (13.3%) patients. There were no cases of subcapsular hematoma, hemoperitoneum, or sepsis, and there was no death secondary to the procedure. In particular, no early or delayed major procedure-related complications were observed in any ***: Transjugular liver biopsy is a safe and effective procedure, and there was significant difference in the adequacy of the specimens obtained using the new needle device compared to the Quick- Core biopsy needle. Assessment of disease progression in patients with transfusion-associated chronic hepatitis C using transient elastography

METHODS： Between December 2006 and June 2008, a total of 524 transfusion-associated HCV-RNA positive patients with or without HCC were enrolled, Liver stiffness was obtained noninvasively by using Fibroscan （Echosens, Paris, France）, The date of blood transfusion was obtained by interview, Duration of infection was derived from the interval between the date of bloodtransfusion and the date of liver stiffness measurement （LSM）. Patients were stratified into four groups based on the duration of infection （17-29 years; 30-39 years; 40-49 years; and 50-70 years）. The difference in liver stiffness between patients with and without HCC was assessed in each group. Multiple linear regression analysis was used to determine the factors associated with liver stiffness.RESULTS： A total of 524 patients underwent LSM. Eight patients were excluded because of unsuccessful measurements. Thus 516 patients were included in the current analysis （225 with HCC and 291 without）. The patients were 244 men and 272 women, with a mean age of 67.8 ±9.5 years. The median liver stiffness was 14.3 kPa （25.8 in HCC group and 7.6 in non HCC group）. The patients who developed HCC in short duration of infection were male dominant, having lower platelet count, with a history of heavier alcohol consumption, showing higher liver stiffness, and receiving blood transfusion at an old age. Liver stiffness was positively correlated with duration of infection in patients without HCC （r = 0.132, P = 0.024） but not in patients with HCC （r = -0.103, P = 0.123）. Liver stiffness was significantly higher in patients with HCC than in those without in each duration group （P 〈 0.0001）. The factors significantly associated with high liver stiffness in multiple regression were age at blood transfusion （P 〈 0.0001）, duration of infection （P = 0.0015）, and heavy alcohol consumption （P = 0.043）CONCLUSION： Although liver stiffness gradually increases over time, HCC develops in patients with high stiffness value regardle

AIM： To evaluate the efficacy of CT-maximum intensity projection （CT-MIP） in the detection of gastric varices and their infiowing and outflowing vessels in patients with gastric varices scheduled to undergo balloonoccluded retrograde transvenous obliteration （B-RTO）. METHODS： Sixteen patients with endoscopically confirmed gastric varices were included in this study. All patients were evaluated with CT-MIP using threedimensional reconstructions, before and after B-RTO. RESULTS： CT-MIP clearly depicted gastric varices in 16 patients （100%）, the left gastric vein in 6 （32.5%）, the posterior gastric vein in 12 （75.0%）, the short gastric veins in 13 （81.3%）, gastrorenal shunts in 16 （100%）, the hemiazygos vein （HAZV） in 4 （25.0%）, the pericardiophrenic vein （PCPV） in 9 （56.3%）, and the left inferior phrenic vein in 9 patients （56.3%）. Although flow direction itself cannot be determined from CT-MIP, this modality provided clear images of the inflowing and the outflowing vessels. Moreover, in one patient, short gastric veins were not seen on conventional angiographic portography images of the spleen, but were clearly revealed on CT-MIP. CONCLUSION： We suggest that CT-MIP should be considered as a routine method for detecting and diagnosing collateral veins in patients with gastric varices scheduled for B-RTO. Furthermore, CT-MIP is more useful than endoscopy in verifying the early therapeutic effects of B-RTO.
