Introduction: Hydatidosis is caused by the larvae of the cestode?Echinococcus granulosus. Liver and lung are the most frequent locations. We report a rare case of a primary hydatid disease location in pre-sacral space. Clinic case: A 64-year-old male patient who consulted due to right gluteal area pain radiating to outer side of the right leg. Physical examination showed a positive Lasègue with hypoesthesia in right L4 L5 S1 territories, with loss of strength in S1. The x-ray showed L3 vertebral wedging and a lumbar lordosis rectification. Laboratory tests and tumor markers were negative. Endorectal ultrasonography described a multilobulatedpresacral cystic tumor. Magnetic resonance imaging (MRI) showed a 16 × 10 × 11 cm retro rectal mass. Due to uncertain diagnosis, surgery was performed, with previous albendazol administration and surgical field protection with 20% hypertonic saline solution. Combined approach (laparotomy + Kraske incision) was realized. Postoperative course was uneventful. The patient remained asymptomatic 36 months after surgery. Summary: Presacralhydatid cysts are rare entities, but must be considered in differential diagnosis of space-occupying lesions in presacral region. Surgery is treatment of choice, and medical treatment may be useful to prevent recurrences.

This study assessed the quality parameters and consumer perception and behavior of butter processed with different ratios of cow and sheep milk *** milk cream butter(CMCB)showed lower moisture content and higher fat content,smaller fat globules,and higher concentrations of long-chain saturated fatty acids and monounsaturated fatty acids,resulting in improved potential health *** addition,CMCB showed yellow color,little salt,and butter taste as the main sensory characteristics,resulting in grateful and good digestibility *** milk cream addition improved the rheological properties of the products(lower viscosity,consistency index,and rigidity(J0),and higher fluidity and viscoelastic behavior(J1)),mainly due to the increased concentrations of short-chain and medium-chain saturated fatty acids and polyunsaturated fatty ***,sheep milk cream butter(SMCB)was characterized by a greasy and striking aroma,with a lower perception of good digestibility and good appearance,and a higher bitter *** association of cow and sheep milk cream could be an exciting option to obtain products with improved fatty acid profiles,health indices,and yet maintain desired rheological and sensory *** formulation with 60%sheep milk cream and 40%cow milk cream would be the suggested combination.

Substituted imidazoles are of interest because of their useful biological activities. While several methods have been developed for the synthesis of such compounds, some of the reported methods utilize corrosive or toxic catalysts. We report a bismuth (III) triflate catalyzed multicomponent synthesis of 2,4,5-trisubstituted imidazoles. Bismuth (III) compounds are attractive from a green chemistry perspective because they are remarkably non-toxic and non-corrosive. Multicomponent syntheses save time and generate less waste.

Introduction: Sleep is critical to human physiological function, cognitive performance, and emotional regulation. Healthcare personnel, especially physicians, are chronically exposed to long working hours, which are often accompanied by decreased sleep time. Clinical evidence indicates that these conditions affect their cognitive function and professional practice, but researchers in the field have not sufficiently explored the possible effects of reductions in sleep time on social cognition. Objective: The aim of this is to further explore the effects of sleep restriction among medical residents, the specific impairments in social cognition that it produces, and the effects of a sleep hygiene orientation on those impairments. Method: There were 124 medical residents (50 males, 74 females) who completed a daily sleep/work log, a battery of sleep tests/questionnaires, and neuropsychological evaluations. The participants then received a short course (8 hours) on basic concepts of sleep hygiene and sleep psychoeducation. Once the course was completed, participants filled out the questionnaires again. Results: The results indicate that sleep disturbances in medical residents were associated with severe neurocognitive and social cognition impairments. After the sleep hygiene intervention, neuropsychological function and sleep parameters improved, awareness increased, and social cognition performance significantly improved. Conclusion: Using sleep psychoeducation and sleep hygiene intervention in medical residents is a simple strategy to mitigate the effects of sleep restriction.

Contexte:?The fight against HIV/AIDS epidemics is one of the greatest challenges of this century. The epidemic affects generally under-developed countries, and Sub-Saharan Africa are the most concerned. The combined marketed form known as Triomune was used as first-line treatment in several sub-Saharan African Countries (60% of VIH infected people), including Chad. However, no evaluation has been done for that treatment in the country. Objective: To evaluate the efficacy and safety immuno-virological of Triomune at the General Hospital in N’Djamena/Chad. Methods: 48 HIV-1 positive patients eligible for ARV treatment were enrolled in our study, and they have been then followed for 8 months. We have measured in these patients the CD4 cell count before treatment and at the 8th month of treatment. After 8 months of treatment, we have also evaluated the Lymphocyte T CD4 and the plasma viral load (VL). Comparisons of means of CD4 lymphocytes and plasma CV (≥1000 copies/ml) were used to define treatment failure.?Results:?48 patients were under Triomune regime. The average CD4 count was decreased from 462 ± 179.22 [56 - 981] cells/mm3?before treatment to 327.23 ± 153.77 [10 - 1008] cells/mm3?at the 8th month of treatment. The mean plasma viral load for patients was 66008.62 copies/ml. The failure rate to Triomune was 43.75% (21/48).?Conclusion:?Aside from the side effects already described for Triomune, our study reveals a high treatment failure rate. Hence, there is the need of regular revisions of therapeutic regime administer in the first intention.

Context: Two years after the transition to Dolutegravir (DTG), at the national level, data on compliance with the execution of this transition and the rational use of the new molecule are not available. Objective: The objective of this study was to determine compliance with first-line ART prescriptions among People Living with HIV starting AntiRetroViral treatment in Kinshasa during the Dolutegravir era. Methods: This study is a descriptive cross-sectional study to determine compliance with first-line ART prescriptions among people living with HIV during the Dolutegravir era in Kinshasa. Sixteen Outpatients Treatment Centers (OTCs) were included in the study for their expertise in the care of PLHIV, their technical collection platform and their accessibility. The parameters of interest were: age, sex, outpatient treatment centers, and medical prescription. Results: One hundred and nineteen (119) patients were included in this work in accordance with the inclusion criteria;67 (56.3%) are female, giving a sex ratio of 1.29 in favor of women. The average age of the patients included is 39.87 ± 12.36 years with extremities of 18 to 69 years. The most represented age group is that of 36 to 45 years with 37 patients (31.9%). One hundred and nineteen (119) patients were received from 16 centers in Kinshasa, according to 6 OTC for the district of Funa, 4 OTC for Tshangu and 3 OTC respectively for Lukunga and Mont-Amba. All the centers respect the new recommendations of the National Program;all patients (100%) are on the Tenofovir (TDF) + Lamivudine (3TC) + Dolutegravir (DTG) combination. Conclusion: Compliance with the prescription of DTG as a first-line ARV in the DRC is effective in the city province of Kinshasa.

Context: The evaluation of plasma Viral Load constitutes an indicator of the progression of the infection, the effectiveness and the tolerance of the treatment. Objective: The objective of this study is to present the virological profile of Patients Living with HIV (PLHIV) after 12 months of AntiRetro Viral Treatment (ART) based on Dolutegravir (DTG) in Kinshasa. Method: The present study is a cross-sectional view of the virological profile of the twelfth month of a prospective cohort of PLHIV at M12 of DTG-based ART in Kinshasa. During the M12 appointment, a blood sample was taken for Molecular Biology analyses from all PLHIV included. Result: During the M12 appointment, 28 patients were registered, including 16 (57.1%) women. Nine (9) patients (45.0%) had an undetectable Viral Load (VL). The median VL value was 3.18 log10 RNA copies/mL (1530 RNA copies/mL). The mutations K65R, T69P/N, K70R and M184V have been listed as mutations conferring resistance to Nucleotide Reverse Transcriptase Inhibitors. No mutations associated with Dolutegravir were observed at M12. Conclusion: After 12 months of AntiRetroViral Treatment based on Dolutegravir, half of the Patients on first-line ART are in a state of virological failure.
