Dissemination of Resistance Integrons and Genes Coding for Blse and Cabapenemases in the Urban Drainage Network in Cote d’Ivoire

Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting for the acquisition and dissemination of antibiotic resistance, as they are frequently affected by anthropogenic activities. The objective of this study was to establish a diffusion map of resistance integrons used as genetic markers of resistance associated with antibiotic resistance conferring genes (ARGs). Total DNA extracts from non-cultivable bacterial communities were used for the analyses. These communities were obtained from wastewater samples from 14 sites upstream and downstream of drainage channels or effluents in the cities of Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of positives among the treated samples (n = 39). Among the genetic markers of dissemination, class 1 integrons were the most evident in 94.8% of samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were found respectively in 41% and 51% of all samples. Genes coding for β-lactamases and blaTEm was identified in almost all samples at a rate of 97.4%. A co-presence of the three genes blaTEm, blaSHV and blaCTX-m is also remarkable in the sites of the city of Yamoussoukro. Among the genes coding for carbapenemases, only blaKPC 17.94%, blaNDm 30.76% and blaOXA48 38.46% were detected in the samples.

A new methodology for energy-based seismic design of steel moment frames

A procedure is proposed whereby input and hysteretic energy spectra developed for single-degree-of-freedom （SDOf） systems are applied to multi-degree-of-freedom （mDOf） steel moment resisting frames. The proposed procedure is verified using four frames, viz., frame with three-, five-, seven- and nine-stories, each of which is subjected to the fault- normal and fault-parallel components of three actual earthquakes. A very good estimate for the three- and five-story frames, and a reasonably acceptable estimate for the seven-, and nine-story frames, have been obtained. A method for distributing the hysteretic energy over the frame height is also proposed. This distribution scheme allows for the determination of the energy demand component of a proposed energy-based seismic design （EBSD） procedure for each story. To address the capacity component of EBSD, a story-wise optimization design procedure is developed by utilizing the energy dissipating capacity from plastic hinge formation/rotation for these moment frames. The proposed EBSD procedure is demonstrated in the design of a three-story one-bay steel moment frame.

AIm: To evaluate the efficacy and safety of the fOLfIRI regimen in patients with metastatic pancreatic adenocarcinoma (PAC) after the failure of gemcitabine and platinum salts. mETHODS: All consecutive patients with histologically confirmed, metastatic PAC and World Health Organiza-tion performance status (PS) ≤ 2 received fOLfIRI-1 [irinotecan 180 mg/m2 on day 1 and leucovorin 400 mg/m2 followed by 5-fluorouracil (5-fU) 400 mg/m2 bolus, then 5-fU 2400 mg/m2 as a 46-h infusion, biweekly] or fOLfIRI-3 (irinotecan 100 mg/m2 on day 1 and leucovorin 400 mg/m2, then 5-fU 2400 mg/m2 as a 46-h infusion and irinotecan 100 mg/m2 repeated on day 3, biweekly) after failure of gemcitabine and platinum-based chemotherapies as a systematic policy in two institutions between January 2005 and may 2010. Tumor response, time to progression (TTP), overall survival rate (OS) and grade 3-4 toxicities were retrospectively studied. Subgroup analyses were performed to search for prognostic factors. RESULTS: Sixty-three patients (52.4% male, median age 59 years) were analyzed. Among them, 42.9% were PS 0, 38.1% were PS 1 and 19.0% were PS 2. fifty one patients (81.0%) had liver metastases. Before the fOLfIRI regimen, patients had received 1 line (n = 19), 2 lines (n = 39) or 3 lines (n = 5) of chemotherapy. median TTP obtained with the line before fOLfIRI was 3.9 mo (95% CI: 3.4-5.3 mo). A total of 480 cycles was completed (median: 6 cycles, range: 1-51 cycles). The main reason for discontinuing fOLfIRI was tumor progression (90.3%). Tumor control was achieved in 25 patients (39.7%) (partial response: n = 5, stable disease: n = 20) with fOLfIRI. median TTP was 3.0 mo (95% CI: 2.1-3.9 mo) and median OS was 6.6 mo (95% CI: 5.3-8.1 mo). Dose adaptation was required in 36 patients (57.1%). fifteen patients (23.8%) had grade 3-4 toxicities, mainly hematological (n = 11) or digestive (n = 4). febrile neutropenia occurred in 3 patients. Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, Italy) as a treatment for benign prostatic hyperplasia (BPH) and prostate cancer (PCa) using the Delphi consensus ***: Italian and international experts on BPH and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPH and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. If the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and ***: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPH in patients with ample range of prostate volume, from mL (80%) to >80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). A RANS-Vof Numerical model to Analyze the Output Power of An OWC-WEC Equipped with Wells and Impulse Turbines in A Hypothetical Sea-State

Wave energy is a renewable source with significant amount in relation to the global demand. A good concept of a device applied to extract this type of energy is the onshore oscillating water column wave energy converter(OWC-WEC). This study shows a numerical analysis of the diameter determination of two types of turbines, Wells and Impulse, installed in an onshore OWC device subjected to a hypothetical sea state. Commercial software fLUENT?,which is based on RANS-Vof(Reynolds-Averaged Navier-Stokes equations and Volume of fluid technique), is employed. A methodology that imposes air pressure on the chamber, considering the air compressibility effect, is used. The mathematical domain consists of a 10 m deep flume with a 10 m long and 10 m wide OWC chamber at its end(geometry is similar to that of the Pico's plant installed in Azores islands, Portugal). On the top of the chamber, a turbine works with air exhalation and inhalation induced by the water free surface which oscillates due to the incident wave. The hypothetical sea state, represented by a group of regular waves with periods from 6 to 12 s and heights from 1.00 to 2.00 m(each wave with an occurrence frequency), is considered to show the potential of the presented methodology. maximum efficiency(relation between the average output and incident wave powers) of46% was obtained by using a Wells turbine with the diameter of 2.25 m, whereas the efficiency was 44% by an Impulse turbine with the diameter of 1.70 m.

Tight focusing with very small f-numbers is necessary to achieve the highest at-focus ***,tight focusing imposes strong demands on precise target positioning in-focus to achieve the highest on-target irradiance We describe several near-infrared,visible,ultraviolet and soft and hard X-ray diagnostics employed in a~10^(22)W/cm^(2)laser±plasma *** used nearly 10 J total energy femtosecond laser pulses focused into an approximately1.3-μm focal spot on 5±20μm thick stainless-steel *** discuss the applicability of these diagnostics to determine the best in-focus target position with approximately 5μm accuracy(i.e.,around half of the short Rayleigh length)and show that several diagnostics(in particular,3ωreflection and on-axis hard X-rays)can ensure this *** demonstrated target positioning within several micrometers from the focus,ensuring over 80%of the ideal peak laser intensity *** approach is relatively fast(it requires 10±20 laser shots)and does not rely on the coincidence of low-power and high-power focal planes.

Collaboration Exercises——The Lack of Collaborative Benefits

The purpose of this article is to analyze what professional emergency personnel learn during collaboration exercises and the benefits of what they have ***(n = 19) and semistructured interviews(n = 32) were carried out in conjunction with major exercises held in Sweden(2007–2012). The results show that exercises tend to be based on their own logic, which differs from actual events. Exercise participants believe that they mainly learn single-track, parallel, and pathdependent behavior. The exercises do not facilitate the use of cross-boundary activities. This means that learning, as well as benefits from the exercises for actual events, is limited. The exercises would be more appropriate if those participating had the opportunity to identify weaknesses,try alternative ways, and engage in comprehensive organizational analyses at the conclusion of the *** on the results of the study, alternative models for collaboration exercises are suggested, with elements that would better develop collaboration and contribute to learning.

Infection with tapeworms is a major problem in many parts of the world. Patients may be asymptomatic or have a significant morbidity depending on the species. Infection with Taenia species is sometimes found by expulsion of eggs or proglottids in stool. Species specific diagnosis of Taenia is difficult,but possible. We present a case of Taenia saginata incidentally discovered,and risk factors for transmission,diagnosis,symptoms,and treatment.
