An estimated 300 million people worldwide suffer from chronic hepatitis C with a prevalence of 0.8%-1.0% of the general population in Canada. An increasing pool of evidence exists supporting the use of pegylated-interferon （pegIFN） and ribavirin combination therapy for hepatitis C. We report a 49-year old male of North American aboriginal descent with chronic hepatitis C （genotype 2b）. Biopsy confirmed that he had cirrhosis with a 2-wk history of left eye pain and decreased visual acuity. He developed retinal vein thrombosis after 16 of 24 wk of pegIFN-α 2a and ribavirin combination therapy. He was urgently referred to a retinal specialist and diagnosed with non-ischemic central retinal vein occlusion of the left eye. PegIFN and ribavirin combination therapy was discontinued and HCV RNA was undetectable after 16 wk of treatment. Hematologic investigations revealed that the patient was a factor V Leiden heterozygote with mildly decreased protein C activity. Our patient had a number of hypercoagulable risk factors, including factor V Leiden heterozygosity, cirrhosis, and hepatitis C that alone would have most likely remained clinically silent. We speculate that in the setting of pegIFN treatment, these risk factors may coalesce and cause the retinal vein thrombosis.

Peginterferon and ribavirin combination therapy for the treatment of hepatitis C virus （HCV） is well known to be associated with significant adverse effects. Sensorineural hearing loss, that in most cases is unilateral, has been reported as a consequence of therapy with both non-pegylabed and pegylated interferon （pegIFN） but is not a well-known adverse effect. We report a 45-year-old Caucasian woman who developed acute sensorineural hearing loss 2 mo after starting therapy with pegIFN-α 2b and ribavirin for the treatment of chronic HCV, genotype la. She did not report the hearing loss to the hepatitis clinic until 1 mo, later whereupon therapy was promptly discontinued. Although her serum alanine aminotransferase （ALT） normalized and her HCV-RNA became undetectable after 12 wk of pegIFN and dbavirin therapy, after discontinuation, her HCV-RNA became detectable with significant elevations of serum ALT. Four months after initial discontinuation, the patient re-commenced pegIFN and ribavirin combination therapy. After 44 of 48 wk of therapy, the patient＇s liver biochemistry has normalized and the HCV-RNA is undetectable. She has not developed worsening of her hearing loss and hearing on the left-side is unaffected. Both patients and physicians should be aware that sensorineural hearing loss may occur with pegIFN therapy. Our experience suggests that re-institution of therapy is not always associated with further hearing impairment.

Celiac disease(CD)is one of the most common food intolerances in developed *** affects genetically susceptible individuals and has severe consequences if it remains undiagnosed.A disease known for more than a century,it is still the focus for experts from various fields of research and ***,pathologists,immunologists,food engineers and dieticians share their knowledge and expertise to improve the conditions of CD *** new insights in the pathomechanism of gluten processing and antigen presentation in CD,it was possible to improve the diagnostic antigen mimicking the primary epitope in *** celiac neo-epitopes are comprised of a complex of gliadin peptides crosslinked with transglutaminase(tTg).They are an early diagnostic marker for CD which occurs up to 6 months earlier than classical markers known to miss a certain amount of CD patients.

Background:medical literature on the prevalence of genetic liver disease is *** this study,we investigated the inhospital healthcare and economic burden from genetic causes of non-alcoholic chronic liver disease(NACLD)and nonalcoholic liver cirrhosis(NALC)in the ***:Data were abstracted from the National Inpatient Sample database between 2002 and 2014 using ICD9 codes for patients discharged with NACLD and NALC secondary to genetic diseases including alpha-1 antitrypsin deficiency(A1ATd),cystic fibrosis(CF),Wilson disease(WD),hereditary hemochromatosis(HHC),glycogen storage disease,and disorders of aromatic amino-acid metabolism(DAAAm).Results:Throughout the study period,there were 19,332 discharges for NACLD associated with the six genetic diseases including 14,368 for *** were$1.09 billion in hospital charges,790 in-hospital deaths,and 955 liver transplants ***,A1ATd was associated with 8,983(62.52%)hospitalizations for NALC followed by WD,CF,and *** highest in-hospital mortality was seen with *** greatest frequency of liver transplants was seen with ***:The number of hospitalizations for genetic liver diseases continues to *** increased funding and directed research efforts,we can aim to improvemedical treatments and the quality of life for patients at risk for liver deterioration.

Hippopotamus bites are rare in medical consultations, despite several studies exposing their aggressive character. A case of a hippopotamus bite on the shoulder of a fisherman has been reported. In addition to the osteoarticular aspect representing a therapeutic problem, the infectious aspect was marked by sepsis, with a death in less than 48 hours. Based on this observation, a protocol for the initial management of large animal bites was submitted.

This work uses regression models to analyze two characteristics of recurrent congestion： breakdown, the transition from freely flowing conditions to a congested state, and duration, the time between the onset and clearance of recurrent congestion. First, we apply a binary logistic regression model where a continuous measurement for traffic flow and a dichoto- mous categorical variable for time-of-day （Am- or Pm-rush hours） is used to predict the probability of breakdown. Second, we apply an ordinary least squares regression model where categorical variables for time-of-day （Am- or Pm-rush hours） and day-of-the-week （monday-Thursday or Friday） are used to predict recurrent congestion duration. models are fitted to data collected from a bottleneck on 1-93 in Salem, NH, over a period of 9 months. Results from the breakdown model, predict probabilities of recurrent congestion, are consistent with observed traffic and illustrate an upshift in breakdown probabilities between the Am- and Pm-rush periods. Results from the regression model for congestion duration reveal the presences of significant interaction between time-of-day and day-of-the-week. Thus, the effect of time-of-day on congestion duration depends on the day-of-the-week. This work provides a simplification of recurrent congestion and recovery, very noisy processes. Simplification, conveying complex relationships with simple statistical summaries-facts, is a practical and powerful tool for traffic administrators to use in the decision-making process.

Primary biliary cirrhosis(PBC) is an autoimmune disease of the liver characterized by progressive bile duct destruction eventually leading to cirrhosis and liver *** serological hallmark of the disease is the presence of circulating antimitochondrial antibodies(AmA).These reflect the presence of autoreactive T and B cells to the culprit antigens,the E2 subunits of mitochondrial 2-oxo-acid dehydrogenase enzymes,chiefly pyruvate dehydrogenase(PDC-E2).The disease results from a combination of genetic and environmental risk *** predisposition is indicated by the higher familial incidence of the disease particularly among siblings and the high concordance rate among monozygotic *** triggering events appear crucial to disrupt a pre-existing unstable immune tolerance of genetic origin allowing,after a long latency,the emergence of clinical *** mimetopes of the vulnerable epitope of the PDC-E2 autoantigen can be derived from microbes that utilize the PDC enzyme or,alternatively,environmental xenobiotics/chemical compounds that modify the structure of native proteins to make them immunogenic.A further alternative as a source of antigen is PDC-E2 derived from apoptotic *** the effector phase the biliary ductular cell,by reason of itsproclivity to express the antigen PDC-E2 in the course of apoptosis,undergoes a multilineage immune attack comprised of CD4+ and CD8+ T cells and *** this article,we critically review the available evidence on etiopathogenesis of PBC and present interpretations of complex data,new developments and theories,and nominate directions for future research.
