A 67-year-old woman underwent an orthotopic liver transplantation for end stage liver disease secondary to chronic autoimmune hepatitis. She developed sudden massive hematochezia on post-operative day 23 with hemodynamic compromise. The source of hemorrhage was found at colonoscopy after careful irrigation and inspection to be a dieulafoy lesion situated j.st proximal to the anorectal j.nction. Hemostasis was achieved with epinephrine inj.ction and thermal coagulation.

Over the past several years, more so recently, treatment options for hepatitis C virus(HCV) have seemed to exponentially grow. Up until recently, the regimen of pegylated interferon(peg-IFN) and ribavirin(RBV) stood as the standard of care. Direct acting antivirals, which target nonstructural proteins involved in replication and infection of HCV were first approved in 2011 as an addition to the peg-IFN and RBV regimen and with them have come increased sustained virological response rates(SVR). The previously reported 50%-70% SVR rates using the combination of peg-IFN and RBV are no longer the standard of care with direct acting antiviral(DAA) based regimens now achieving SVR of 70%-90%. PegIFN free as well as "all oral" regimens are also available. The current randomized controlled trials available show favorable SVRs in patients who are naive to treatment, non-cirrhotic, and not human immunodeficiency virus(HIV)-co-infected. What about patients who do not fit into these categories? In this review, we aim to discuss the currently approved and soon to be approved DAAs while focusing on their roles in patients that are treatment experienced, cirrhotic, or co-infected with HIV. In this discussion, review of the clinical trials leading to recent consensus guidelines as well as discussion of barriers to treatment will occur. A case will attempt will be made that social services, including financial support and drug/alcohol treatment, should be provided to all HCV infected patients to improve chances of cure and thus prevention of late stage sequela.

The development of strong-scaling computational tools for high-throughput methods with an open-source code and transparent metadata standards has successfully transformed many computational materials science *** such tools are mature already in the condensed-matter physics community,the situation is still very different for many *** probe tomography(APT)is one *** microscopy and microanalysis technique has matured into a versatile nano-analytical characterization tool with applications that range from materials science to geology and possibly ***,data science tools are required for extracting chemo-structural spatial correlations from the reconstructed point *** APT and other high-end analysis techniques,post-processing is mostly executed with proprietary software tools,which are opaque in their execution and have often limited *** development by members of the scientific community has improved the situation but compared to the sophistication in the field of computational materials science several gaps *** is particularly the case for open-source tools that support scientific computing hardware,tools which enable high-throughput workflows,and open welldocumented metadata standards to align experimental research better with the fair data stewardship *** this end,we introduce paraprobe,an open-source tool for scientific computing and high-throughput studying of point cloud data,here exemplified with *** show how to quantify uncertainties while applying several computational geometry,spatial statistics,and clustering tasks for post-processing APT datasets as large as two billion *** tools work well in concert with Python and HDF5 to enable several orders of magnitude performance gain,automation,and reproducibility.

Tropical cyclones(TC)pose a persistent natural hazard to Costa *** to natural hazards,such as mass movements and floods,is compounded by a growing urban population and inadequate land use *** study conducted a comprehensive analysis of the economic impacts of TC of Costa Rica from Hurricane j.an in 1988 to Hurricane Eta in 2020,assessing the impact by municipality and economic sector using baseline information of the Ministry of National Planning and Economic *** to the study,road infrastructure(933.8 US million),agriculture(280.5 US million),river rehabilitation(153.96 US million),housing 98.26(US million),and health(81.74 US million)were among the sectors most severely affected by TC over the past 30 *** Pacific basin municipalities in Costa Rica were found to be the most vulnerable,primarily due to the indirect impacts of *** study's results offer useful information on the economic sectors and municipalities that are most exposed from TC in Costa Rica and provide a replicable methodology for other regions and countries facing similar tropical phenomena.

To test the performance and optimize operational parameters of various frost protection wind machines,a control system of performance test-bed was designed and developed based on Programmable Logic Controller(PLC)and touch *** system realized automatic control of operational parameters including hydraulic platform height,impeller rotation speed,depression angle,rotation range and *** case study results shows that the control accuracy of all the parameters was achieved with the average errors of 2.11 cm for height,9.5 r/min for impeller rotation speed,0.83°for pan-tilt rotation range and 0.061 min for rotation *** the control of the test-bed,the performances of various wind machines,such as the distribution of airflow and temperature,and frost protection effect,could be tested under various working conditions,which provide experiment supports for developing wind machines adapted to different conditions with higher frost protection effect.

Flavonoids are by far the most dominant class of phenolic compounds isolated from Morus alba leaves(MAL). Other classes of compounds are benzofurans, phenolic acids, alkaloids, coumarins, chalcones and stilbenes. Maj.r flavonoids are kuwanons, moracinflavans, moragrols and morkotins. Other maj.r compounds include moracins(benzofurans), caffeoylquinic acids(phenolic acids) and morachalcones(chalcones). Research on the anticancer properties of MAL entailed in vitro and in vivo cytotoxicity of extracts or isolated compounds. Flavonoids, benzofurans, chalcones and alkaloids are classes of compounds from MAL that have been found to be cytotoxic towards human cancer cell lines. Further studies on the phytochemistry and anticancer of MAL are suggested.
