Some children with previously undiagnosed Hirschprung’s disease present severely ill at an advanced age with markedly dilated bowel and enterocolitis. In this manuscript, the authors propose the following treatment algorithm: 1) a diverting transverse loop colostomy, 2) a transanal Soave endorectal pull-through, and 3) colostomy closure. The authors report their experience with five patients.

Three genera,Cochliobolus,Bipolaris and Curvularia form a complex that contains many plant pathogens,mostly on grasses(Poaceae)with a worldwide *** taxonomy of this complex is confusing as frequent nomenclatural changes and refinements have *** is no clear morphological boundary between the asexual genera Bipolaris and Curvularia,and some species show intermediate *** investigated this complex based on a set of ex-type cultures and collections from northern *** gene analysis of rdNA ITS(internal transcribed spacer),GPdH(glyceraldehyde 3-phosphate dehydrogenase),LSU(large subunit)and EF1-α(translation elongation factor 1-α)shows that this generic complex divides into two *** and Cochliobolus species clustered in Group 1 along with their type species,whereas Curvularia species(including species named as Bipolaris,Cochliobolus and Curvularia)clustered in Group 2,with its generic *** nomenclatural conflict in this complex is resolved giving priority to the more commonly used established generic names Bipolaris and *** descriptions of the genera Bipolaris and Curvularia are provided and species resolved in this study are transferred to one of these genera based on their phylogeny.

Background:Infants undergoing cardiac surgery with cardiopulmonary bypass(CPB)frequently receive intraoperative methylprednisolone(MP)to suppress CPB-related inflammation;however,the optimal dosing strategy and efficacy of MP remain ***:We retrospectively analyzed all infants under 90 days-old who received intra-operative MP for cardiac surgery with CPB from 2014–2017 at our *** combined real-world dosing data from the electronic health record(EHR)and two previously developed population pharmacokinetic/pharmacodynamic models to simulate peak concentration(Cmax)and area under the concentration-time curve for 24 h(AUC24)for MP and the inflammatory cytokines interleukin-6(IL-6)and interleukin-10(IL-10).We evaluated the relationships between post-operative,safety,and other clinical outcomes obtained from the EHR with each predicted exposure using non-parametric ***:A total of 142 infants with median post-natal age 8(interquartile range[IQR]:5,37)days received a total dose of 30(19,49)mg/kg of ***(8%)died,37(26%)met the composite post-operative outcome,114(80%)met the composite safety outcome,and 23(16%)had a major *** median Cmax and AUC24 IL-6 exposure was significantly higher for infants meeting the composite post-operative outcome and those with major *** median Cmax and AUC24 MP exposure was significantly higher for infants requiring *** exposure was associated with death or other safety ***:Pro-inflammatory IL-6,but not MP exposure,was associated with post-operative organ dysfunction,suggesting current MP dosing may not adequately suppress IL-6 or increase IL-10 to impact clinical *** study will be required to define the optimal exposure-efficacy and exposure-safety profiles in these infants.

Analyzing Anomalous Topographic Map drainage System and Landform Evidence as a Glacial History Paradigm Problem: A Literature Review

While not usually stated, detailed topographic maps show well-mapped anomalous drainage system and other erosional landform evidence the accepted North American Cenozoic geologic and glacial history paradigm (accepted paradigm) does not permit geomorphologists to satisfactorily explain. A new and fundamentally different paradigm able to explain the drainage system and other erosional landform evidence has recently emerged, but requires what the accepted paradigm considers to be the preglacial (and probably mid-Cenozoic) Bell River drainage system to have formed on a melting continental ice sheet’s floor. The new paradigm’s melting ice sheet had previously eroded bedrock underneath it and caused crustal warping that raised continental regions and mountain ranges so as to create and occupy a deep “hole” while massive and prolonged meltwater floods flowed across rising continental regions and mountain ranges to the south. The new paradigm leads to a completely different middle Cenozoic geologic and glacial history than the accepted paradigm describes and the two paradigms are analyzed according to good science expectations such as using evidence anyone can see, applying common sense logic during each research step, producing consistent results, and simplicity of paradigm generated explanations. The new paradigm uses topographic map evidence anyone can see, appears to use common sense logic during each research step, and produces remarkably consistent results leading to a simpler Cenozoic northern Missouri River drainage basin region geologic and glacial history than what the accepted paradigm describes. Further work is needed to test the new paradigm’s ability to explain drainage system and erosional landform evidence in other geographic regions such as in the Ohio River drainage basin.

Species and generic recognition in the order Xylariales has been uncertain due to lack of molecular data from authentic cultures,as well as overlapping morphological *** this study,we revise the families Graphostromataceae,Hypoxylaceae,Lopadostomataceae and Xylariaceae in *** study is based on dNA sequence data derived from living cultures of fresh isolates,data from GenBank and morphological observation of type and worldwide herbarium *** also collected new specimens from Germany,Italy and ***bined analyses of ITS,LSU,RPB2 and b-tubulin sequence data were used to reconstruct the molecular phylogeny of the above *** and familiar boundaries between these families are revised and presented in an updated combined phylogenetic *** accept six genera in Graphostromataceae,19 genera in Hypoxylaceae,four in Lopadostomataceae and 37 genera in *** genera previously treated in Xylariaceae are placed in Amphisphaeriales genera incertae sedis and seven genera are placed in Xylariales genera incertae *** genera are placed in Sordariomycetes genera incertae sedis,while four genera are placed as Xylariomycetidae genera incertae *** genera are considered as *** and Cannonia,presently included in Xylariaceae are transferred to diatrypaceae and Coniochaetales respectively,based on their morphology and ***olospora and Myconeesia are excluded from Xylariaceae and synonymized with Phaeosporis and Anthostomella *** descriptions and illustrations are provided for all taxa with notes provided on each *** and doubtful genera are listed with notes on their taxonomy and *** keys are provided for all revised families with morphological details for genera within the families.
