Surgical management of facial dog bite trauma:A contemporary perspective and review

Objective:To perform a review evaluating management of and complications stemming from dog bite trauma sustained to the head and neck over the past decade.Data Sources:PubMed and cochrane Library.Methods:The authors searched the PubMed and cochrane Library databases for relevant published literature.A total of 12 peer-reviewed canine-exclusive series inclusive of 1384 patient cases describing facial dog bite trauma met inclusion criteria.Wounds including fractures,lacerations,contusions,and other soft-tissue injuries were evaluated.Demographics related to clinical course and management,operating room requirements,and antibiotic usage were compiled and analyzed.Initial trauma and surgical management complications were also assessed.Results:75.5%of patients sustaining dog bites required surgical intervention.Of these patients,7.8%suffered from postsurgical complications,including hypertrophic scarring(4.3%),postoperative infection(0.8%),or nerve deficits and persistent paresthesias(0.8%).Prophylactic antibiotics were administered to 44.3%of patients treated for facial dog bites and the overall infection rate was 5.6%.concomitant fracture was present in 1.0%of patients.conclusion:Primary closure,often in the OR may be necessary,with few cases requiring grafts or flaps.Surgeons should be aware that the most common complication is hypertrophic scarring.Further research is needed to elucidate the role of prophylactic antibiotics.

Unlike European species,the potential of Nearctic syrphids as biological control agents is still poorly studied.However,the American hoverfly,Eupeodes americanus(Wiedemann),has recently demonstrated promising traits as a biocontrol agent,notably against the foxglove aphid,Aulacorthum solani Kaltenbach,on pepper.The present study aims to extend our knowledge of the American hoverfly by evaluating its potential as a biocontrol agent in a banker plant system against the melon aphid,Aphis gossypi Glover,in a greenhouse cucumber crop.The preimaginal development and voracity of E.americanus were compared when preying upon the focal prey/pest(A.gossypi)or the banker prey(bird cherry-oat aphid,Rhopalosiphum padi L.)by daily observations of larvae from egg to adult.Preimaginal development time,survival rate,and occurrence of deformation were similar on both prey species.The weight of third instar and pupae,however,was higher for larvae that fed on the banker prey.The ad libitum voracity of the syrphid larvae was generally very high and did not significantly differ between prey species,except for the third-instar larvae which consumed more focal prey.Results suggest that a banker plant system involving the bird cherry-oat aphid may be a promising tactic for utilizing E.americanus for melon aphid biocontrol.

Prognosis value of heat-shock proteins in esophageal and esophagogastric cancer:A systematic review and meta-analysis

BAcKGROUND Heat shock proteins(HSPs)are molecular chaperones that play an important role in cellular protection against stress events and have been reported to be overex-pressed in many cancers.The prognostic significance of HSPs and their regulatory factors,such as heat shock factor 1(HSF1)and cHIP,are poorly understood.AIM To investigate the relationship between HSP expression and prognosis in esophageal and esophagogastric cancer.METHODS A systematic review was conducted in accordance with PRISMA recommend-ations(PROSPERO:cRD42022370653),on Embase,PubMed,cochrane,and LILAcS.cohort,case-control,and cross-sectional studies of patients with eso-phagus or esophagogastric cancer were included.HSP-positive patients were compared with HSP-negative,and the endpoints analyzed were lymph node metastasis,tumor depth,distant metastasis,and overall survival(OS).HSPs were stratified according to the HSP family,and the summary risk difference(RD)was calculated using a random-effect model.RESULTS The final selection comprised 27 studies,including esophageal squamous cell carcinoma(21),esophagogastric adenocarcinoma(5),and mixed neoplasms(1).The pooled sample size was 3465 patients.HSP40 and 60 were associated with a higher 3-year OS[HSP40:RD=0.22;95%confidence interval(cI):0.09-0.35;HSP60:RD=0.33;95%cI:0.17-0.50],while HSF1 was associated with a poor 3-year OS(RD=-0.22;95%cI:-0.32 to-0.12).The other HSP families were not associated with long-term survival.HSF1 was associated with a higher probability of lymph node metastasis(RD=-0.16;95%cI:-0.29 to-0.04).HSP40 was associated with a lower probability of lymph node dissemination(RD=0.18;95%cI:0.03-0.33).The expression of other HSP families was not significantly related to tumor depth and lymph node or distant metastasis.cONcLUSION The expression levels of certain families of HSP,such as HSP40 and 60 and HSF1,are associated with long-term survival and lymph node dissemination in patients with esophageal and esophagogastric cancer.

Foam rolling and stretching do not provide superior acute flexibility and stiffness improvements compared to any other warm-up intervention: A systematic review with meta-analysis

Background:Acute improvement in range of motion(ROM)is a widely reported effect of stretching and foam rolling,which is commonly explained by changes in pain threshold and/or musculotendinous stiffness.Interestingly,these effects were also reported in response to various other active and passive interventions that induce responses such as enhanced muscle temperature.Therefore,we hypothesized that acute ROM enhancements could be induced by a wide variety of interventions other than stretching or foam rolling that promote an increase in muscle temperature.Methods:After a systematic search in PubMed,Web of Science,and SPORTDiscus databases,38 studies comparing the effects of stretching and foam rolling with several other interventions on ROM and passive properties were included.These studies had 1134 participants in total,and the data analysis resulted in 140 effect sizes(ESs).ES calculations were performed using robust variance estimation model with R-package.Results:Study quality of the included studies was classified as fair(PEDro score=4.58)with low to moderate certainty of evidence.Results showed no significant differences in ROM(ES=0.01,p=0.88),stiffness(ES=0.09,p=0.67),or passive peak torque(ES=-0.30,p=0.14)between stretching or foam rolling and the other identified activities.Funnel plots revealed no publication bias.conclusion:Based on current literature,our results challenge the established view on stretching and foam rolling as a recommended component of warm-up programs.The lack of significant difference between interventions suggests there is no need to emphasize stretching or foam rolling to induce acute ROM,passive peak torque increases,or stiffness reductions.

Reactive oxygen species are essential in photocatalytic water treatment.In this paper,Gd doped carbon nitride(cN)photocatalysts were prepared by simple thermal polymerization for the photocatalytic degradation of tetracycline under visible light irradiation.The photodegradation rate of *** is as high as 95%in 18 min,and the photocatalytic performance is much higher than that of cN.The improvement of photocatalytic performance is mainly attributed to the fact that Gd ion implantation directly provides active sites for oxygen activation and induces the formation of N vacancies.The results of characterizations show that the introduction of Gd in cN can improve the conversion ability of activated oxygen,carrier separation and energy band structure adjustment.Therefore,1.0GdcN photocatalyst can be employed for efficient photocatalytic synthesis of tetracycline.Furthermore,three ways of photocatalytic degradation of tetracycline were revealed by high performance liquid chromatographymass spectrometry.This work provides insights into the doping strategy of cN to improve the production of reactive oxygen species for environmental remediation.

Background: Scrotal trauma is a rare injury, and can compromise reproductive function. If in closed trauma, a morphological assessment of the lesion is usually necessary before treatment, in open trauma otherwise, namely in the case of testicular evisceration, surgical exploration is the rule. Objective: To present the management of open scrotal trauma. case Presentation: These were two adults aged 42 and 63, admitted for post-traumatic testicular evisceration. One of the patients presented with signs of infection due to a diagnosis delay. Management consisted on surgical exploration, with scrotal debridement and testicular reintegration. The evolution was favourable. conclusion: Open scrotal trauma is a rare lesion that can cause testicular evisceration. It requires surgical exploration, which should be systematic.

The availability of groundwater of acceptable quality has become a difficult challenge to meet in many countries, particularly côte d’Ivoire. The State of côte d’Ivoire and rural populations are building hydraulic infrastructure for this purpose. However, the quality of water coming from these infrastructures remains to be determined. This study therefore aims to analyze certain physicochemical parameters of the groundwater of Gloazra, Kouezra and Trahonfla. These are temperature, pH, electrical conductivity, TDS, salinity level, hardness, alkalinity, chloride content and bicarbonate. To determine the pH, electrical conductivity, temperature, TDS, and salinity rate, an electrochemical analysis was carried out. The titrimetric analysis made it possible to determine the chloride and bicarbonate content, alkalinity, and hardness. The results indicate that the studied groundwater is soft and acidic with good mineralization. This water does not present a health hazard to consumers but can affect hydraulic equipment and household containers.

Background: Perforations of the terminal ileum are a frequent clinical situation and a therapeutic challenge. Surgical treatment is controversial. Several surgical modalities have been proposed and postoperative morbidity and mortality remain high. The aim of this study was to review our experience in the surgical management of perforations of the terminal ileum. Patients and methods: This is a descriptive cross-sectional study with retrospective data collection from January 1, 2017 to December 31, 2021, in five hospitals in cameroon. Patients’ demographic, clinical presentation, surgical findings and 30-days postoperative outcomes data were collected. Results: We collected 34 files. The sex ratio was 1.4 and the average age was 20.28 years. The average consultation time was 9.1 days. Abdominal pain was present in all our patients. Peritoneal irritation was present in 46 cases (88.5%). The mean time to treatment after admission was 17.5 hours. The perforation was unique in 40 cases (76.9%) and located between 6 and 10 cm from the ileocecal valve in 29 cases (55.8%). Typhoid was the most suspected etiology intraoperatively in 38 cases (73.1%). A simple suture was performed in 18 patients (34.6%), resection with anastomosis in 11 patients (21.2%) and an ileostomy in 5 patients (9.6%). The average length of hospitalization was 21.74 days. Postoperative morbidity and mortality were 32.7% and 17.3% respectively. conclusion: Perforations of the terminal ileum are common and affect young people. Surgical procedures are varied. Reducing consultation and care times and respecting treatment principles could improve postoperative morbidity and mortality, which remain high.

Adjuvant Rectal Diclofenac for Post Operative Analgesia after caesarean Section—A Randomized controlled Study

BAcKGROUND: Pain management following caesarean section still remains a challenge in our environment. Most potent analgesics are either not readily available or expensive. Diclofenac suppository is an NSAID that can be used for postoperative analgesia. It is available and affordable. OBJEcTIVE: To compare the efficacy and safety of combined rectal diclofenac and intramuscular pentazocine with intramuscular pentazocine alone for post operative pain control following lower segment caesarean section. METHODOLOGY: A total of 120 women who met the selection criteria scheduled for caesarean section under spinal anaesthesia with bupivacaine were randomized into two equal groups to receive either 75 mg diclofenac suppository 12 hourly for 24 hours or one anusol suppository (the placebo) 12 hourly for 24 hours. Both groups received pentazocine as primary analgesia. RESULT: The primary outcome measure is the proportion of patients with severe pain at 24 hours using the visual analogue rating scale. Secondary outcome measures are the time from surgery to ambulation, Passage of flatus, maternal satisfaction and presence of complications. Statistical analysis was done using spss version 22 and graph pad statistical package. Student T-test was used for continuous variables whereas chi square was used for categorical variables P cONcLUSION: Adjuvant rectal diclofenac is superior to pentazocine alone in the management of pain after caesarean section. Less number of patients had moderate to severe pain at 24 hours post operation. Maternal satisfaction in relation to pain management is better with diclofenac suppository. The levels of complications were comparable in both groups.

Results of Surgical Management of Malignant Obstruction of the common Bile Duct in Yaoundé

Introduction: In cameroon, surgery remains the only approach in malignant obstructions of the common bile duct (MOcBD) even in palliative situations. The aim of this work was to describe the modalities of surgical treatment of MOcBD, evaluate the results and detect the factors associated with postoperative morbidity and mortality. Patients and Methods: We conducted an analytical observational study, with retrospective data collection from the files of patients operated upon for MOcBD. This was done in four referral hospitals in the city of Yaoundé for a 42-month period spanning from January 1, 2020 to June 30, 2023. Demographic data, clinical presentation, surgical data, and 30-day postoperative outcomes were collected. Results: We collected 71 files. The sex ratio was 1.4 and the mean age was 56 ± 11 years. Fifty-three (53) patients (74.6%) were overweight or obese and 10 patients (14.1%) were hypertensive. A clinical cholestasis syndrome was present in 69 patients (97.2%). Fifty-five (55) patients (77.5%) had a cancer of the head of the pancreas, 8 patients (11.3%) had an extra-hepatic cholangiocarcinoma and 8 patients (11.3%) had an ampullary adenocarcinoma. Eight (8) resections (11.3%) with curative intent had been carried out and in 63 cases (88.7%), surgery was palliative. Postoperative morbidity was 55.7%, influenced by advanced WHO stage (p = 0.02). Postoperative mortality was 25.7%, associated with a high ASA score (p = 0.01). conclusion: Pancreatic head cancer is the main etiology of malignant obstructions of the common bile duct in cameroon. Surgical treatment is most often palliative. Postoperative morbidity and mortality are high, influenced by high WHO and ASA scores.
