Spirometric Reference Equations for Semi-Urban and Urban Bantu Cameroonians

Background: Spirometric reference values vary substantially across ethnic groups, and remain largely poorly characterized among Africans. We derived spirometric reference equations for adult Cameroonians and compared their performance with those derived from other ethnic groups. Methods: Spirometric variables according to the American Thoracic Society/European Respiratory Society 2005 guidelines were acquired in voluntary healthy non-smoker subjects in Yaounde (Capital City) and Foumbot (semi-urban area in West Region), in Cameroon during November 2011 to January 2012 (Yaounde) and August 2012 (Foumbot). Reference equations were derived separately for men and women from multiple linear regressions. Results: A total of 411 subjects (206 men) met the inclusion criteria. The mean age was 39.5 ± 16.1 years (min - max: 18 - 85 years) for men and 39.2 ± 14.1 years (18 - 90 years) for women. Age and height were the only variables significantly associated with spirometric values in the final linear regression models. Derived reference values were lower than those derived from Global Lung Initiative 2012 equations for different ethnic groups, except for the forced expiratory volume in 1 second/forced vital capacity ratio (FEV1/FVC ratio). The mean FEV1/FVC ratio was 0.88 ± 0.07 for Cameroonian men and 0.89 ± 0.07 for Cameroonian women. Variations in the performance of derived models in bootstrap internal validation were marginal. Conclusion: This study highlights the importance of deriving specific predictive equations for each ethnic group. The use of adjustment factors applied to Caucasian equations when compared with the values derived in our study leads to an overestimation of the values for FEV1 and FVC.

Inter-Observer Variability in the Detection and Interpretation of Chest X-Ray Anomalies in Adults in an Endemic Tuberculosis Area

Purpose: To assess the inter-observer agreement in reading adults chest radiographs (CXR) and determine the effectiveness of observers in radiographic diagnosis of pulmonary tuberculosis (PTB) in a tuberculosis endemic area. Methods: A quasi-observational study was conducted in the Pneumology Department of Yaounde Jamot Hospital (Cameroon) from January to March 2014. This included six observers (two chest physicians, two radiologists, two end-training residents in medical imaging) and 47 frontal CXRs (4 of diffuse interstitial lung disease, 6 normal, 7 of lung cancers, 7 of bacterial pneumonia, 23 of PTB). The sample size was calculated on the basis of an expected 0.47 Kappa with a spread of 0.13 (α = 5%, CI = 95%) for six observers and five diagnostic items. The analysis of concordance was focused on the detection of nodules, cavitary lesions, pleural effusion, adenomegaly and diagnosis of PTB and lung cancer. These intervals of kappa coefficient were considered: discordance (0.81). Results: The average score for the detection of caverns was the highest (58.3%) followed by that of the correct diagnosis of tuberculosis (49.3%). Pneumologists had the highest proportions of correct diagnosis of tuberculosis (69.6% and 73.9%) and better inter-observer agreement (k = 0.71) for PTB diagnosis. Observers were more in agreement for the detection of nodules (0.32 - 0.74), adenomegalies (0.43 - 0.69), and for the diagnosis of cancer (0.22 - 1) than for the diagnosis of tuberculosis (0.19 - 0.71). Disagreements were more frequent for the detection of pleural effusions (-0.08 - 0.73). Conclusion: The inter-observer agreement varies with the type of lesions and diagnosis. Pneumologists were most effective for the diagnosis of pulmonary tuberculosis. Observers were more in agreement for the detection of nodules and the diagnosis of cancer than for the diagnosis of pulmonary tuberculosis.

Purpose: The complexity of chest radiography (CXR) is a source of variability in its interpretation. We assessed the effect of an interpretation grid on the detection of CXR anomalies and radio- graphic diagnosis of tuberculosis in an endemic area for tuberculosis. Methods: The study was conducted in Yaounde (Cameroon). Six observers (2 pulmonologists, 2 radiologists and 2 senior residents in medical imaging) interpreted 47 frontal CXR twice two months apart without (R1) and with (R2) the aid of an interpretation grid. We focused on the detection of micro nodules (n = 16), cavitations (n = 12), pleural effusion (n = 6), adenomegaly (n = 6), and diagnosis of tuberculosis (n = 23) and cancer (n = 7). Results: The average score for accurate detection of elementary lesions was 40.4% [95%CI: 25% - 58.3%] in R1 and 52.1% [36.9% - 65.3%] in R2. The highest im- provement was observed for micro nodules (19.8%). Cavitations had the highest proportions of accurate detections (58.3% in R1 and 65.3% in R2). The average score of accurate diagnosis was 46.1% in R1 and 57.4% in R2. Accurate diagnosis improved by 3.6% for tuberculosis and 19% for cancer between R1 and R2. Intra-observer agreement was higher for the diagnosis of cancers (0.22 ≤ k ≤ 1) than for diagnosing tuberculosis (0.21 ≤ k ≤ 0.68). Inter-observer agreement was highly variable with a modest improvement for the diagnosis of tuberculosis in R2. Conclusion: Standardized interpretation scheme improved the detection of CXR anomalies and diagnosis of tuberculosis. It significantly improved inter-observer’s agreement in diagnosing tuberculosis but not in detecting most lesions.

Objective: To determine the early electrocardiographic changes in a cohort of ambulatory cameroonian COVID-19 patients treated with hydroxychlo- roquine and Azithromycin. Design: Prospective study. Setting: Treatment centres of the city of Yaounde, Cameroon, from May 7th to 24th 2020. Participants: We enrolled 51 consecutive confirmed COVID-19 on RT-PCR who having mild forms of COVID-19 and treated by hydroxychloroquine 200 mg twice daily during seven #days plus Azithromycin 500 mg the first day and 250 mg the remaining 4 days as per national standard. Main Outcomes Measures: The primary end-point was the change in QTc interval between day 0 (D0), day 3 (D3) and day 7 (D7). Secondary endpoints were changes in all other cardiac electrical conductivity patterns and the occurrence of clinical arrhythmic events during the course of treatment. Background: Tuberculosis (TB) is one of the top lethal infectious diseases worldwide. In recent years, interferon-γ (INF-γ) release assays (IGRAs) have been established as routine tests for diagnosing TB infection. However, produced INF-γ assessment cannot permit to distinguish active ATB from latent TB infection (LTBI), especially in TB epidemic areas. In addition to IFN-γ, interleukin-2 (IL-2), secreted by activated T cells, is involved in immune response against Mycobacterium tuberculosis. This could be involved in the follow up of treatment response. The aim of our study was to determine IFN-γ and IL2 cytokines profiles of patients under antituberculosis treatment. Materials and Methods: A six months’ cross-sectional study was conducted at the Jamot Hospital of Yaoundé, from May to August 2021. Sociodemographic and clinical data as well as 5 mL of blood were collected from each participant. INF-γ and IL-2 were determined using indirect Enzyme linked Immuno-Sorbent Assay (ELISA) according to the manufacturer’s recommendations and spectrum exam in combination with radiography and GeneXpert were used as standard. P-values Results: The results showed that men were more infected 14/61 (31.8%) with a high presence in active and resistant TB groups. The mean age was 41.3 ± 13.1 years with a 95% CI = [38.2 - 44.7], the age group with the highest infection rate was ranged between 31 and 40 years. The IL-2 and INF-γ means were respectively 327.6 ± 160.6 pg/mL and 26.6 ± 13.0 pg/mL in ATB patients, 251.1 ± 30.9 pg/mL and 21.4 ± 9.2 pg/mL in patients with resistant tuberculosis, while it was 149.3 ± 93.3 pg/mL and 17.9 ± 9.4 pg/mL in cured patients, 15.1 ± 8.4 pg/mL and 5.3 ± 2.6 pg/mL in participants presumed healthy (p γ and IL-2rates were observed between the different groups. Conclusion: Monitoring the serum levels of INF-γ and IL-2 would be useful for the follow-up of anti-tuberculosis patients, particularly in the both cytokines association case.

The S100 proteins are a unique class of EF-hand Ca^(2+) binding proteins distributed in a cell-specific, tissue-specific, and cell cycle-specific manner in humans and other vertebrates. These proteins are distinguished by their distinctive homodimeric structure, both intracellular and extracellular functions, and the ability to bind transition metals at the dimer interface. Here we summarize current knowledge of S100 protein binding of Zn^(2+), Cu^(2+) and Mn^(2+) ions, focusing on binding affinities, conformational changes that arise from metal binding, and the roles of transition metal binding in S100 protein function.

Wheat yield prediction by zero sink and equilibrium-type soil phosphorus tests

Diffusive gradients in thin films(DGT)measurements have been shown to outperform other phosphorus(P)tests in soils with strong P sorption,but this has not been confirmed for moderately weathered European *** compared the performance of DGT in predicting wheat grain yield in Swedish long-term fertility experiments with those of standard intensity(water-extractable P(P-H2O))and quantity(ammonium lactate-extractable P(P-AL))tests.A Mitscherlich-type model was used to fit wheat yield response to P application rates(0,15,30 or 35,and 45 kg P ha-1 year-1)in each individual trial replicate to estimate the maximum *** trials with clear plateau-type yield responses and the goodness of fit(R2)>0.75,relative yields(RYs)were calculated for each P treatment and plotted against the soil P test results(n=143).The goodness of the Mitscherlich-type fits decreased in the following order:DGT-measured P(P-DGT)(R2=0.35)>P-H2O(R2=0.18)>P-AL(R2=0.13).When excluding soils with P-AL:P-DGT≥0.1 L g-1,R2 was considerably improved to 0.55 for P-AL,0.46 for P-H2O,and 0.65 for P-DGT(n=61).At 95%of maximum yield,the upper limit of P deficiency for P-DGT was 44.8(the soils with P-AL:P-DGT<0.1 L g-1)and 61.9μg L-1(all soils),falling within the range reported for other European and Australian soils(6.0–142μg L-1).We show that in the investigated Swedish soils,DGT performed better than the quantity and intensity tests,which is attributed to its ability to capture P diffusion and resupply from the soil solid phase,similar to plant roots in the rhizosphere.

A Synthesizing Land-cover Classification Method Based on Google Earth Engine: A Case Study in Nzhelele and Levhuvu Catchments, South Africa

This study designed an approach to derive land-cover in the South Africa with insufficient ground samples, and made a case demonstration in Nzhelele and Levhuvu catchments, South Africa. The method was developed based on an integration of Landsat 8, Sentinel-1, and Shuttle Radar Topography Mission(SRTM) Digital Elevation Model(DEM), and the Google Earth Engine(GEE) platform. Random forest classifier with 300 trees is employed as land-cover classification model. In order to overcome the defect of insufficient ground data, the stratified sampling method was used to generate the training and validation samples from the existing land-cover product. Likewise, in order to recognize different land-cover categories, the percentile and monthly median composites were employed to expand input metrics of random forest classifier. Results showed that the overall accuracy of the land-cover of Nzhelele and Levhuvu catchments, South Africa in 2017–2018 reached to 76.43%. Three important results can be drawn from our research. 1) The participation of Sentinel-1 data can slightly improve overall accuracy of land-cover while its contribution on land-cover classification varied with land types. 2) Under-fitting problem was observed in the training of non-dominant land-cover categories using the random sampling, the stratified sampling method is recommended to make sure the classification accuracy of non-dominant classes. 3) When related reflectance bands participated in the training process, individual Normalized Difference Vegetation index(NDVI), Enhanced Vegetation Index(EVI), Soil Adjusted Vegetation Index(SAVI), Normalized Difference Built-up Index(NDBI) have little effect on final land-cover classification result.

One of the primary aims of the actinide community within nanoscience is to develop a good understanding similar to what is currently the case for stable elements. As a consequence, efficient, reliable and versatile synthesis techniques dedicated to the formation of new actinide-based nano-objects （e.g., nanocrystals） are necessary. Hence, a ＂library＂ dedicated to the preparation of various actinidebased nanoscale building blocks is currently being developed. Nanoscale building blocks with tunable sizes, shapes and compositions are of prime importance. So far, the non-aqueous synthesis method in highly coordinating organic media is the only approach which has demonstrated the capability to provide size and shape control of actinide-based nanocrystals （both for thorium and uranium, and recently extended to neptunium and plutonium）. In this paper, we demonstrate that the non-aqueous approach is also well adapted to control the chemical composition of the nanocrystals obtained when mixing two different actinides. Indeed, the controlled hot co-injection of thorium acetylacetonate and uranyl acetate （together with additional capping agents） into benzyl ether can be used to synthesize thorium/uranium mixed oxide nanocrystals covering the full compositional spectrum. Additionally, we found that both size and shape are modified as a function of the thorium：uranium ratio. Finally, the magnetic properties of the different thorium/uranium mixed oxide nanocrystals were investigated. Contrary to several reports, we did not observe any ferromagnetic behavior. As a consequence, ferromagnetism cannot be described as a universal feature of nanocrystals of non-magnetic oxides as recently claimed in the literature.

Enterprise stent-within-stent embolization of a basilar artery perforator aneurysm

Objective and Importance—Vertebrobasilar artery aneurysms can be very challenging to treat and are preferentially dealt with using endovascular tech-niques, since they are associated with lower risk than surgical clipping. Small aneurysms located on perfo-rating arteries can pose problems though, as their dimensions may not be favorable for coiling and leave the endovascular surgeon without many options. We present a basilar perforating artery aneurysm that was successfully embolized using a stent-within-stent technique. Clinical Presentation—A 47 year old fe-male presented with a Hunt-Hess 3, Fisher Grade-3 subarachnoid hemorrhage with blood in the pre-pontine cistern. Initial imaging (CT angiogram and digital subtraction angiography) did not reveal an aneurysm. Follow-up angiography on post-bleed day eight demonstrated a three-millimeter basilar perforating artery aneurysm. After an unsuccessful coiling attempt a closed-cell stent-within-stent tech-nique was used to divert flow away from the aneu-rysm neck to induce aneurysm thrombosis. Interven-tion (or Technique)—Multiple attempts were made to access and stabilize a microcatheter in the small basilar perforator artery aneurysm in order to de-liver coils for endovascular embolization;this could not be done safely. Therefore a closed-cell 4.5 × 22 mm Enterprise stent (Cordis Neurovascular, Inc., Miami Lakes, Florida) was deployed in the basilar artery across the origin of the perforator aneurysm in order for the stent tines to divert flow away from the aneurysm neck and induce thrombosis. Persistant brisk flow within the aneurysm continued however, and a second closedcell 4.5 × 22 mm Enterprise stent was placed within the first one to increase the stent metal surface area across the aneurysm neck to further reduce flow into the aneurysm. Subsequently, angiography demonstrated stagnant blood flow in the aneurysm dome and the aneurysm spontaneously thrombosed, sparing all associated vessels. Conclu-sion—Stent-within
