Iris manipulation during phacoemulsification:intraoperative and postoperative complications

AIM:To quantify intraoperative and postoperative complications in complex phacoemulsification cataract extraction(phacoemulsification)with iris manipulation compared to non-complex and complex phacoemulsification without iris ***:All phacoemulsification cases at the University of Colorado between January 1,2014,and June 30,2017 were *** criteria for the primary outcome of intraoperative complications were planned combination surgery and eyes with less than 28 d *** criteria for the secondary outcomes of postoperative complications were unplanned additional surgery,and chronic steroid eye drop use prior to *** including sex,race/ethnicity,surgery length,visual acuity,intraoperative and postoperative complications,and intraocular pressures(IOP)were collected and analyzed utilizing general linear and Logistic regression ***:The medical records of 5772 eyes were reviewed(500 complex without iris manipulation,367 with iris manipulation).The number of any intraoperative complication in the complex with iris manipulation and complex without iris manipulation groups was 15(4.1%)and 26(5.2%),respectively,compared to 41(0.8%)in the non-complex *** inflammation was found in 135(2.8%)non-complex cases,20(4.1%)complex cases without iris manipulation,and 20(5.6%)complex cases with iris *** adjusted odds ratio of postoperative inflammation in phacoemulsification with iris manipulation compared to non-complex was 2.3(95%CI:1.3-4.0,P=0.005).The rate of IOP spikes>10 mm Hg was significantly greater in cases with iris manipulation(P=0.001).CONCLUSION:Complex cases have more intraoperative ***,only complex cases with iris manipulation led to increase rates of postoperative inflammation and IOP spikes>10 mm Hg.

Evaluation of Ear and Facial Indices of Ibibio and Efik Female Children of Akwa Ibom and Cross river States, Nigeria

Background: The ear and face are indispensable and distinctive features for hearing and identification. Objectives: This study was designed to generate anthropometric data of the ear and facial indices of females of Efik and Ibibio children in Cross river and Akwa Ibom States, show morphological and aesthetic differences and ethnicity. Methods: A total of 600 female children (300 Efiks and 300 Ibibios) aged 2 to 10 years that met the inclusion criteria were chosen from selected primary schools in Calabar Municipality, Calabar South of Cross river State and from Uyo, Itu of Akwa Ibom State, Nigeria. Standardized measurements of face length, face width, ear length, and ear width were taken with a spreading caliper;the facial (proscopic) and ear (auricular) indices were determined. results: Efik subjects presented a mean face length of 8.36 ± 0.06 cm, face width of 11.04 ± 0.04 cm, ear length of 4.92 ± 0.02 cm, and ear width of 3.06 ± 0.01 cm. Ibibio subjects had mean values for face length, face width, ear length, and ear width as 8.17 ± 0.05 cm, 10.75 ± 0.05 cm, 4.77 ± 0.03 cm, and 2.94 ± 0.02 cm respectively. The mean facial index and ear index for Efik subjects were 75.68 ± 0.31 and 62.16 ± 0.27 respectively;while the mean facial and ear indices for Ibibio subjects were 74.79 ± 0.36 and 61.80 ± 0.34 respectively. Statistical analysis demonstrated significant differences in face length, ear length, ear width and facial index, with the Efik subjects having higher values than Ibibio subjects (p Conclusion: The results showed hypereuryproscopic face as the prevalent face type among females of both ethnic groups, therefore can be of importance in sex, ethnic, and racial differentiation, and in clinical practice, aesthetics and forensic medicine.

Protocol for electrophysiological monitoring of carotid endarterectomies

Near zero stroke rates can be achieved in carotid endarterectomy （CEA） surgery with selective shunting and electrophysiological *** false negative rates as high as 40% have been *** sought to determine if improved training for interpretation of the monitoring signals can advance the efficacy of selective shunting with electrophysiological monitoring across multiple centers,and determine if other factors could contribute to the differences in *** and raw beta band （12.5-30 Hz） electroencephalogram （EEG） and median and tibial nerve somatosensory evoked potentials （SSEP） were monitored in 668 CEA cases at six surgical centers.A decrease in amplitude of 50% or more in any EEG or SSEP channel was the criteria for shunting or initiating a neuroprotective protocol.A reduction of 50% or greater in the beta band of the EEG or amplitude of the SSEP was observed in 150 *** patient showed signs of a cerebral infarct after *** shunting based on EEG and SSEP monitoring can reduce CEA intraoperative stroke rate to a near zero level if trained personnel adopted standardized *** also found that the rapid administration of a protective stroke protocol by attending anesthesiologists was an important aspect of this success rate.

The reverse supply chain for electronics (and other goods) faces great challenges in the coming years. Volumes of waste are increasing rapidly and environmental impacts at the end-of-life are receiving more and more attention. Given the high cost of recycling waste equipment in rich countries, there is an increasing practice of exporting to developing countries, where it is processed in an informal recycling sector. The environmental impacts arising from primitive recycling processes are considerable and media coverage of the issue is increasing. Also, legislation in the EU (WEEE and roHS Directives) and elsewhere in the world is inducing changes in how the industry designs, recovers and handles end-of-life machines. In addition, there is much untapped potential to bridge the digital division (and develop markets in industrializing countries) via international markets in used IT equipment. redesigning the reverse supply chain to ensure environmentally acceptable processing and maximizing equipment utilization from an international perspective is a challenging technology management problem bridging product design, logistics, and process design.

Malaria and Anaemia in Pregnant and Non-Pregnant Women of Child-Bearing Age at the University Hospital, Kumasi, Ghana

Background: Malaria infection during pregnancy is a major public health problem in tropical and subtropical regions globally. Anaemia is often an adverse outcome of severe parasitic infections during pregnancy in developing countries. Pregnant women in malaria-endemic communities are more susceptible to Plasmodium falciparum infections than non-pregnant women of child-bearing age. Objective: To comparatively investigate malaria and anaemia in pregnant and non-pregnant women of child-bearing age. Design: A cross-sectional comparative study. Three hundred and eighty pregnant women and 380 non-pregnant women were screened for the study. Setting: The study was conducted at the University Hospital, Kwame Nkrumah University of Science and Technology, Kumasi, Ghana. Measurements: Participants’ demographic data were collected via the administration of questionnaires. In addition their blood samples were analyzed for haemoglobin level and malaria parasites, while stool samples from the pregnant women were examined for intestinal parasites. results: The study revealed that pregnant women have higher malaria parasitaemia (12.6%) and anaemia (62.6%). The species of Plasmodium isolated from the pregnant women were P. falciparum (85.4%), P. malariae (4.2%) and P. ovale (10.4%). Malaria parasitaemia was higher in the primigravidae (14%). However multigravidae recorded the highest anaemia prevalence (67.1%). Age of pregnant women was a factor affecting malaria parasitaemia with a significant P-value and Or (P value = 0.0041, 0r = 7.61). Conclusions: Pregnant women were more susceptible to malaria and anaemia than non-pregnant women of child-bearing age. Most of the pregnant women reported at antenatal clinic during the second trimester. Primigravidae however recorded the highest malaria parasitaemia. The main species of Plasmodium observed in the blood samples was falciparum.

Global Dynamics of an SEIrS Compartmental Measles Model with Interrupted Vaccination

Measles is a reemerging disease that has a devastating impact, especially among children under 5. In this paper, an SEIrS model is developed to investigate a possible outbreak among the population of children under 5 in the Sunyani Municipality. We consider waning immunity or loss of immunity among those who were vaccinated, which leads to secondary attacks among some in the population. Using routh-Hurwitz criterion, Matrix Theoretic and Goh-Volterra Lyapunov functions, the stability of the model was investigated around the equilibria. We have computed the threshold parameter, r0, using the Next Generation Matrix method. The disease-free equilibrium is globally stable whenever r0 ≤1 and unstable otherwise. The endemic equilibrium is globally stable when r0?>1.

Bile acids and sphingosine-1-phosphate receptor 2 in hepatic lipid metabolism

The liver is the central organ involved in lipid metabolism. Dyslipidemia and its related disorders, including non-alcoholic fatty liver disease(NAFLD), obesity and other metabolic diseases, are of increasing public health concern due to their increasing prevalence in the population. Besides their well-characterized functions in cholesterol homoeostasis and nutrient absorption, bile acids are also important metabolic regulators and function as signaling hormones by activating specific nuclear receptors, G-protein coupled receptors, and multiple signaling pathways. recent studies identified a new signaling pathway by which conjugated bile acids(CBA) activate the extracellular regulated protein kinases(ErK1/2) and protein kinase B(AKT) signaling pathway via sphingosine-1-phosphate receptor 2(S1Pr2). CBA-induced activation of S1Pr2 is a key regulator of sphingosine kinase 2(Sph K2) andhepatic gene expression. This review focuses on recent findings related to the role of bile acids/S1Pr2-mediated signaling pathways in regulating hepatic lipid metabolism.

Identification and Antimicrobial Susceptibility of <i>Yersinia enterocolitica</i>Found in Chitterlings, raw Milk and Swine Fecal Samples

Foodborne illness is an escalating concern upon public health. The prevalence of Yersinia enterocolitica was assessed in chitterlings, raw milk and swine fecal from North Carolina. Uncleaned thirty chitterling samples procured from a local grocery store, forty-five swine fecal samples, and forty unpasteurized cow milk samples supplied by the University farm were evaluated for the presence of Y. enterocolitica. Isolates identified as presumptive positive were characterized as colonies with a pink or deep-red center on MacConkey and CIN agar, and verified further through polymerase chain reaction (PCr) for the presence of 16S rrNA gene for the Yersinia genera. results showed that 4.4% swine fecal samples, 7.5% milk samples and 11.3% chitterling samples were presumptive positive for Y. enterocolitica by the direct plating method on selective agars. Of the thirty-chitterling samples examined by PCr for the 16S rrNA gene, 26% samples contained the identification gene for the bacteria of interest. After conducting virulence tests, the fecal samples were revealed as non-pathogenic. Only one of the milk samples were considered pathogenic and consisted of the following virulent genes: Yersinia heat-stable toxin (yst), invasion (inv), attachment invasion locus (ail), virulence regulon transcriptional activator (virF), Yersinia adehesin A (yadA), and the O:3 antigen gene (rfbC). Seven out of the eight (87.5%) chitterling samples were shown to be pathogenic. Disc diffusion was conducted to determine the antimicrobial susceptibility of the isolates. Over half (55.5%) of the antimicrobial agents were found effective, with isolates being completely susceptible to ciprofloxacin, kanamycin, trimethoprim, cefotaxime, and gentamycin. Papular mycosis fungoides: Six new cases and association with chronic lymphocytic leukemia

Papular mycosis fungoides(MF) is a rare presentation of MF. Six illustrative cases of papular MF were retrospectively reviewed. Five of the cases studied by immunohistochemistry had variable numbers(range: 1%-20%) of CD30+ cells in the dermal infiltrate, a finding that is characteristic of lymphomatoid papulosis but may occasionally occur in typical early MF. Although none of our papular MF patients had progressive disease, lesions with relatively high numbers of CD30+ cells in 3 patients did not respond well to skin-directed treatments used for MF. Interestingly, these patients had evidence of coexisting clonal B cell populations in the blood(one with clonal B cell lymphocytosis and two with B-cell chronic lymphocytic leukemia). We conclude that:(1) papular MF may contain CD30+ cells, thereby causing confusion with lymphomatoid papulosis; and(2) papular MF, like more typical MF, may be associated with clonal B-cell proliferations including chronic lymphocytic leukemia.
