Ipsilateral proximal and shaft femoral fractures

OBJECTIVE:To study the management and evaluate anatomical and functional outcomes of patients with ipsilateral proximal and shaft femoral fractures.;METHODS:A retrospective, descriptive and analytic study lasted for ten years and a half ranging from January 1, 2005 to June 30, 2015. The following parameters were studied: epidemiology, fracture characteristics, therapeutic, anatomical and functional outcomes. The correlation between different parameters was analyzed with Fischer test. The significant threshold was defined for p value eric fracture in 3 cases, type VI in 2 cases and type VII in 1 case. According to AO classification, there was type A shaft fracture in 6 cases (A2 in 4 cases and A3 in 2 cases), type B in 2 cases (B1 in 1 case and B2 in 1 case) and type C in 2 cases (C1 in 1 case and C2 in 1 case). Average surgical delay was 28.7 days (range: 11-61 days). For proximal femoral fracture, Moore prosthesis was used in 1 case, blade plate 130° in 2 cases, long Gamma nail in 4 cases, double screwing in 2 cases and dynamic hip screw in 1 case. For shaft femoral fracture, blade plate 95° was used in 3 cases, low compressive plate in 2 cases. Osseous contention was achieved in 4 cases with long Gamma nail and in 1 case with long blade plate 130°. Nonunion of cervical fracture was achieved in 2 cases. Assessment of Variability in the Quality of an Acrisol under Different Land Use Systems in Ghana

Three land use types (natural fallow, Leucaena leucocephala woodlot and cultivated plots) on a Ferric Acrisol in a semi-arid tropical zone of Ghana were compared to assess their effects on variability in selected soil properties and plant biomass accumulation. Organic carbon accumulation in the representative natural fallow profile was 22.7 g/kg, followed by 16.5 g/kg for the Leucaena woodlot and lastly 11.8 g/kg for the cultivated site. The mean bulk density of the natural fallow, Leucaena woodlot and cultivated sites were from 1.36 Mg/m3, 0.92 Mg/m3 and 1.33 Mg/m3 with corresponding range in mean weight diameter of 0.5 mm - 1.2 mm, 0.6 mm - 1.2 mm and 1.0 mm - 1.2 mm, respectively. The lower bulk density observed for the woodlot corresponds to increased total porosity, aeration, and root proliferation due to the stronger and extensive rooting system. Significant differences (P >36%). For the surface soils, exchangeable Na was very variable in the natural fallow. Exchangeable Na, Ca and K and total nitrogen were very variable in the Leucaena woodlot and the cultivated sites. Variability in clay content was very high in the cultivated soils only. The order cultivated land > Leucaena woodlot > natural fallow was noted for properties with high variability (CV > 36%). Plant biomass accumulation was 1834 kg/ha (natural fallow) and 830 kg/ha (Leucaena woodlot) indicating that natural fallows do not only maintain soil quality but they also decrease variability in soil properties which is desirable for soil productivity and quality.

Risk factors for fracture in adult kidney transplant recipients

AIM: To determine the general and transplant-specific risk factors for fractures in kidney transplant recipients.METHODS: We conducted a cohort study of all adults who received a kidney-only transplant(n = 2723) in Ontario, Canada between 2002 and 2009. We used multivariable Cox proportional hazards regression to determine general and transplant-specific risk factors for major fractures(proximal humerus, forearm, hip, and clinical vertebral). The final model was established using the backward elimination strategy, selecting risk factors with a P-value ≤ 0.2 and forcing recipient age and sex into the model. We also assessed risk factors for other fracture locations(excluding major fractures, and fractures involving the skull, hands or feet). RESULTS: There were 132 major fractures in the follow-up(8.1 fractures per 1000 person-years). General risk factors associated with a greater risk of major fracture were older recipient age [adjusted hazard ratio(a HR) per 5-year increase 1.11, 95%CI: 1.03-1.19] and female sex(a HR = 1.81, 95%CI: 1.28-2.57). Transplant-specific risk factors associated with a greater risk of fracture included older donor age(5-year increase)(a HR = 1.09, 95%CI: 1.02-1.17) and end-stage renal disease(ESRD) caused by diabetes(a HR = 1.72, 95%CI: 1.09-2.72) or cystic kidney disease(a HR = 1.73, 95%CI: 1.08-2.78)(compared to glomerulonephritis as the reference cause). Risk factors across the two fracture locations were not consistent(major fracture locations vs other). Specifically, general risk factors associated with an increased risk of other fractures were diabetes and a fall with hospitalization prior to transplantation, while length of time on dialysis, and renal vascular disease and other causes of ESRD were the transplant-specific risk factors associated with a greater risk of other fractures.CONCLUSION: Both general and transplant-specific risk factors were associated with a higher risk of fractures in kidney transplant recipients. Results can be used for clinical prognostication.
