Hospitalization for variceal hemorrhage in an era with more prevalent cirrhosis

AIM: To examine hospitalization rates for variceal hemorrhage and relation to cause of cirrhosis during an era of increased cirrhosis ***: We performed a retrospective review of patients with cirrhosis and gastroesophageal variceal hemorrhage who were admitted to a tertiary care referral center from 1998 to 2009. Subjects were classified according to the etiology of their liver disease: alcoholic cirrhosis and non-alcoholic cirrhosis. Rates of hospitalization for variceal bleeding were determined. Data were also collected on total hospital admissions per year and cirrhosis-related admissions per year over the same time period. These data were then compared and analyzed for trends in admission ***: Hospitalizations for cirrhosis significantly increased from 611 per 100000 admissions in 1998-2001 to 1232 per 100000 admissions in 2006-9(P value for trend < 0.0001). This increase was seen in admissions for both alcoholic and non-alcoholic cirrhosis(P values for trend < 0.001 and < 0.0001 respectively). During the same time period, there were 243 admissions for gastroesophageal variceal bleeding(68% male, mean age 54.3 years, 62% alcoholic cirrhosis). Hospitalizations for gastroesophageal variceal bleeding significantly decreased from 96.6 per 100000 admissions for the time period 1998-2001 to 70.6 per 100000 admissions for the time period 2006-2009(P value for trend = 0.01). There were significant reductions in variceal hemorrhage from non-alcoholic cirrhosis(41.6 per 100000 admissions in 1998-2001 to 19.7 per 100000 admissions in 2006-2009, P value for trend = 0.007). CONCLUSION: Hospitalizations for variceal hemorrhage have decreased, most notably in patients with non-alcoholic cirrhosis, and this may reflect broader use of strategies to prevent bleeding.

A family study of endophenotypes for psychosis within an early intervention programme in Hong Kong: Rationale and preliminary findings

The study of endophenotypes may be a viable strategy to tackle the genetic complexity and phenotypic heterogeneity of psychosis, but this research direction is relatively under-developed in China as compared to Western countries. We have recently initiated one of the first family studies of endophenotypes for psychosis in China. Patients entering an established early psychosis intervention service are recruited into this research project for phenotyping, endophenotyping and genotyping. At the endophenotypic level, four domains (neurological soft signs, neurocognition of prospective memory, social cognition of facial emotion recognition, and affective cognition of anticipatory and consummatory pleasure) are studied in the sample of patients with psychosis and their unaffected siblings. This article illustrates the benefit of a research-oriented clinical programme and its findings based on the data collected as of early 2011.

HⅠ emission from the red giant Y CVn with the VLA and FAST

Imaging studies with the Very Large Array（VLA） have revealed H I emission associated with the extended circumstellar shells of red giants. We analyze the spectral map obtained on Y CVn, a j-type carbon star on the Asymptotic Giant Branch. The H I line profiles can be interpreted with a model of a detached shell resulting from the interaction of a stellar outflow with the local interstellar *** reproduce the spectral map by introducing a distortion along a direction corresponding to the star＇s motion in space. We then use this fitting to simulate observations expected from the Five-hundred-meter Aperture Spherical radio Telescope（FAST）, and discuss its potential for improving our description of the outer regions of circumstellar shells.

Mode selection and dispersion engineering in Bragg-like slot photonic crystal waveguides for hybrid light-matter interactions

We introduce a family of slot photonic crystal waveguides(SPh CWs) for the hybrid integration of low-index active materials in silicon photonics with energy-confinement factors of ~30% in low-index regions. The proposed approach, which is based on a periodic indentation of the etched slot in the middle of the SPh CW, makes it possible to reconcile a simultaneously narrow and wide slot for exploiting the two modes of even symmetry of a SPh CW. The resulting mode-selection mechanism allows a flexible choice of the modes to be used. Furthermore,the proposed structure offers tremendous flexibility for adjusting the dispersive properties of the slot-confined modes, in particular of their slow-light effects. Flat band slow light in a bandwidth of about 60 nm with a group velocity dispersion factor jβ_2 j below 1 ps^2∕mm is numerically demonstrated by this approach, corresponding to a normalized delay bandwidth product of around 0.4. These results, obtained from hollow-core periodic waveguides that are directly designed in view of hybrid integration of active materials in mechanically robust structures(not based on free-standing membranes) could pave the way for the realization of on-chip slow-light bio-sensing,active hybrid-silicon optoelectronic devices, or all-optical hybrid-silicon nonlinear functionalities.

Cell-free protein synthesis(CFPS)systems from crude lysates have benefitted from modifications to their enzyme *** example,functionally deleting enzymes in the source strain that are deleterious to CFPS can improve protein synthesis ***,making such modifications can take substantial *** a proof-of-concept to accelerate prototyping capabilities,we assessed the feasibility of using the yeast knockout collection to identify negative effectors in a Saccharomyces cerevisiae CFPS *** analyzed extracts made from six deletion strains that targeted the single deletion of potentially negative effectors(e.g.,nucleases).We found a statistically significant increase in luciferase yields upon loss of function of GCN3,PEP4,PPT1,NGL3,and XRN1 with a maximum increase of over 6-fold as compared to the wild *** work has implications for yeast CFPS and for rapidly prototyping strains to enable cell-free synthetic biology applications.

High-Throughput UAV Image-Based Method Is More Precise Than Manual Rating of Herbicide Tolerance

Background:Patients with von Hippel-Lindau(VHL)disease have a high incidence of renal cell carcinoma owing to VHL gene inactivation and constitutive activation of the transcription factor hypoxia-inducible factor 2α(HIF-2α).Methods:In this phase 2,open-label,single-group trial,we investigated the efficacy and safety of the HIF-2αinhibitor belzutifan(MK-6482,previously called PT2977),administered orally at a dose of 120 mg daily,in patients with renal cell carcinoma associated with VHL *** primary end point was objective response(complete or partial response)as measured according to the Response Evaluation Criteria in Solid Tumors,version 1.1,by an independent central radiology review *** also assessed responses to belzutifan in patients with non-renal cell carcinoma neoplasms and the safety of belzutifan.
