Postoperative morbidity and mortality rates are still very high among patients undergoing pancreaticoduodenectomy(PD).However,mortality rates secondary to morbidities that are detected early and well-managed postoperatively are lower among patients undergoing *** early detection of complications plays a very important role in the management of these patients,many ongoing studies are being conducted on this *** endoscopic retrograde cholangiopancreatography and biliary drainage history of the patient study group is important for comparison of C-reactive protein(CRP),an inflammatory parameter evaluated in the retrospective study by Coppola et al published in the World Journal of Gastrointestinal Surgery and titled“Utility of preoperative systemic inflammatory biomarkers in predicting postoperative complications after pancreaticoduodenectomy:Literature review and single center experience”.Therefore,it may be more appropriate to compare CRP values in randomized patients.

Acute malignant colorectal obstruction is a complication of colorectal cancer that can occur in 7%-29% of patients. Self-expanding metallic stent placement for malignant colorectal obstruction has gained popularity as a safe and effective procedure for relieving obstruction. This technique can be used in the palliation of malignant colorectal obstruction, as a bridge to elective surgery for resectable colorectal cancers, palliation of extracolonic malignant obstruction, and for nonmalignant etiologies such as anastomotic strictures, Crohn's disease, radiation therapy, and diverticular diseases. Self-expanding metallic stent has its own advantages and disadvantages over the surgery in these indications. During the insertion of the self-expanding metallic stent, and in the followup, short term and long term morbidities should be kept in mind. The most important complications of the stents are perforation, stent obstruction, stent migration, and bleeding. Additionally, given the high risk of perforation, if a patient is treated or being considered fortreatmentwith antiangiogenic agents such as bevacizumab, it is not recommended to use self-expanding metallic stent as a palliative treatment for obstruction. Therefore, there is a need for careful clinical evaluation for each patient who is a candidate for this procedure. The purpose of this review was to evaluate self-expanding metallic stent in the management of the obstruction of the colon due to the colorectal and extracolonic obstruction.

Early diagnosis and prediction of severity in acute pancreatitis using the urine trypsinogen-2 dipstick test:A prospective study

AIM： To evaluate the use of the trypsinogen-2 dipstick （Actim Pancreatitis） test for early diagnosis and prediction of severity in acute pancreatitis （AP）. METHODS： Ninety-two patients with AP were included in this study. The control group was 25 patients who had acute abdominal pain from non-pancreatic causes. Urine trypsinogen-2 dipstick test （UTDT） and conventional diagnostic tests were performed in all patients. Patients were divided by the Atlanta classification into two groups as having mild or severe pancreatitis. RESULTS： UTDT was positive in 87 （94.6%） of the AP patients and in two （8%） controls （P 〈 0.05）. Positive UTDT was found in 61 （92.4%） of 66 （71.7%） patients with mild pancreatitis and in all （100%） of the 26 （28.3%） with severe pancreatitis （P 〉 0.05）. UTDT positivity lasted longer in severe pancreatitis compared with that in mild pancreatitis （6.2 ＋ 2.5 d vs 2.0 ＋ 1.43 d, P 〈 0.05）. The sensitivity, specificity, positive predictive value, negative predictive value （NPV）, positive likelihood ratio （PLR） and negative likelihood ratio （NLR） of UTDT were 91%, 72%, 96.6%, 70.4%, 3.4 and 0.1, respectively. CONCLUSION： UTDT is a simple, rapid and reliable method for use on admission. It has high specificity and low NLR for early diagnosis and prediction of severity in AP. However, its relatively low NPV does not allow trypsinogen-2 dipstick test to be a stand-alone tool for diagnosis of acute pancreatitis; the use of other conventional diagnostic tools remains a requirement.

There are limited studies in the literature about machinability of bulk metallic glass （BMG）. As a novel and promising structural material, BMG material machining characteristics need to be verified before its utilization. In this paper, the effects of cutting speed, feed rate, depth of cut, abrasive particle size/type on the BMG grinding in dry conditions were experimentally investigated. The experimental evaluations were carried out using cubic boron nitride （CBN） and A1203 cup wheel grinding tools. The parameters were evaluated along with the results of cutting force, temperature and surface roughness measurements, X-ray, scanning electron microscope （SEM） and surface roughness analyse. The results demonstrated that the grinding forces reduced with the increasing cutting speed as specific grinding energy increased. The effect of feed rate was opposite to the cutting speed effect, and increasing feed rate caused higher grinding forces and substantially lower specific energy. Some voids like cracks parallel to the grinding direction were observed at the edge of the grinding tracks. The present investigations on ground surface and grinding chips morphologies showed that material removal and surface formation of the BMG were mainly due to the ductile chip formation and ploughing as well as brittle fracture of some particles from the edge of the tracks. The roughness values obtained with the CBN wheels were found to be acceptable for the grinding operation of the structural materials and were in the rangeof 0.34-0.58 lam. This study also demonstrates that con- ventional A1203 wheel is not suitable for grinding of the BMG in dry conditions.

Prognostic efficacy of inflammation-based markers in patients with curative colorectal cancer resection

AIM To evaluate the prognostic significance of neutrophilto-lymphocyte ratio(NLR), platelet-to-lymphocyte ratio(PLR), lymphocyte-to-monocyte ratio(LMR), and prognostic nutritional index(PNI) and other clinicopathological factors in patients undergoing curative resection of colon ***183 patients with histologically proven colorectal cancer who had undergone potentially curative resection between 2010 and 2016 at Ankara Numune Training and Research Hospital were retrospectively analyzed and clinicopathological characteristics included age, sex, tumor type, grade, size and localization, the number of metastatic and total number of lymph nodes removed, vascular and perineural invasion of the tumor, TNM stages, tumor marker levels(CEA, CA19-9, AFP, CA-125, CA15-3), complete blood counts, albumin levels, overall survival(months), NLR, PLR, LMR and PNI ratios were retrospectively reviewed and analyzed from the electronic database. The primary outcome measure was overall *** Regarding overall survival, on univariate analysis the following variables were significantly associated with poor outcome following resection: T-stage(P = 0.037), lymph node invasion(P = 0.037), cancer stage(P = 0.034), CEA(P = 0.042), CA19-9(P = 0.004), and PNI(P = 0.001). To evaluate the independent prognostic value, multivariate Cox proportional hazard analysis to control for other prognostic factors was used. Using cancer-specific death as an end point for NLR, PLR, LMR, PNI and CA19-9 the optimal cut off values were calculated by ROC analysis. Regarding overall survival, on multivariate analysis high CA19-9(HR = 1.001, 95%CI: 1.00-1.002, P = 0.012) and low PNI(HR = 0.938, 95%CI: 0.891-0.987, P = 0.014) were the only variables independently associated with shortened overall survival. AIM: To compare the outcomes achieved with external dacryocystorhinostomy(EX-DCR) and transcanalicular dacryocystorhinostomy(TC-DCR) using a multidiode laser in patients with bilateral nasolacrimal duct obstruction(NLDO).METHODS: This prospective study was conducted on38 eyes of 19 patients with bilateral NLDO. Simultaneous bilateral surgery was performed on all patients. TC-DCR(Group 1) with a diode laser was used in the right eye,and EX-DCR(Group 2) was used in the left eye. All patients were placed under general anesthesia. Routine follow-ups were scheduled at 1wk; 1, 3, 6 and 12 mo postoperative intervals. Objective(lacrimal system irrigation) and subjective [tearing, irritation, pain,discharge and visual analogue scale(VAS) score]outcomes were ***: The overall objective success rate at 12 mo was 73.7%(14/19) in Group 1 and 89.5 %(17/19) in Group2. This difference was statistically significant. There were no significant between-group differences in the subjective results, such as tearing, pain and *** the discharge scores were found to be significantly higher in Group 1 compared to Group 2 at the 1y followup. The average VAS score was 6.8 in Group 1 and 8.7 in Group 2, with no statistically significant ***: Although TC-DCR allows surgeons to perform a minimally invasive and safe procedure, EX-DCR offers better objective and subjective outcomes than TC-DCR.

AIM:To examine the possible effects of honey supplementation on hepatic damage due to obstruction of the common bile duct in an experimental rat model. METHODS:The study was performed with 30 male rats divided into three groups:a sham group,an obstructive jaundice group,and an obstructive jaundice plus honey group. At the end of the study period,the animals were sacrificed,and levels of nitric oxide(NO) ,and NO synthase(NOS) activities were measured in liver tissues,and levels of adenosine deaminase(ADA) and alanine transaminase(ALT) activities were measured in serum. RESULTS:Blood ALT and ADA activities were significantly elevated in the jaundice group as compared to those of the sham group. In the obstructive jaundice plus honey group,blood ALT and ADA activities were significantly decreased as compared to those of the jaundice group. In erythrocytes and liver tissues,NO levels were found to be significantly higher in the obstructive jaundice plus honey group compared to those of the sham group. Additionally,NO levels were found to be significantly higher in liver tissues from the animals in the obstructive jaundice plus honey group than those of the jaundice ***:Honey was found to be beneficial in the prevention of hepatic damage due to obstruction of the common bile duct.
