Thoracic endovascular aortic repair for thoracic aortic aneurysms is an accepted alternative to open surgery, especially in patients with significant comorbidities. The procedure itself has a low risk of complications and fistulas to surrounding organs are rarely reported. An 86-year-old patient was admitted to our hospital with gastro intestinal(GI) bleeding and a suspected aortoesophageal fistula. Eight months prior, the patient had undergone a stent graft repair of a mycotic thoracic aneurysm. Computerized tomography angiography and upper GI endoscopy confirmed an aortoesophageal fistula, which was treated by esophageal stenting. With early recognition, esophageal stenting may have a role in the initial emergency control of bleeding from and palliation of aortoesophageal fistula.

AIM: To evaluate the positive predictive value of abdominal non-prepared computed tomography (CT) for diagnosing intestinal lumen or wall lesions in patients presenting to the emergency room (ER) with abdominal ***: For 1-year we prospectively evaluated all ER patients hospitalized after abdominal CT scan detected either intraluminal or intestinal wall lesions. These patients underwent colonoscopy serving as gold standard. Patients with prior abdominal pathology or CT findings of appendicitis or diverticulitis were ***: Five hundred and sixty-eight abdominopelvic CT scans were performed in the ER, 96 had positive colonic findings. Sixty-two patients were excluded, 46 because of diverticulitis or appendicitis, 16 because of prior abdominal pathology. Of the remaining 34 patients, 14 did not undergo colonoscopy during *** eligible patients were included in the study. The positive predictive value of the CT scans performed in the ER was calculated to be 45% (95% CI 25-67).CONCLUSION: CT findings correlated with colonoscopic findings only in approximately half of the cases. Relying on non-prepared CT scan findings in planning patient management and colonoscopy may lead to unnecessary diagnostic work-ups.

Objective:Contemporary prostate cancer(PCa)screening modalities such as prostate specific antigen(PSA)and digital rectal examination(DRE)are limited in their ability to predict the detection of clinically significant ***-parametric magnetic resonance imaging(mpMRI)of the prostate has been explored as a staging modality for *** is known regarding its utility as a primary screening *** examined our experience with mpMRI as both a screening and staging ***:mpMRI studies performed between 2012 and 2014 in patients without PCa were cross-referenced with transrectal ultrasonography(TRUS)biopsy *** analyses were performed to determine association of mpMRI findings with overall cancer diagnoses and clinically significant(Gleason score≥7)*** analyses were then performed on patients with a history of prior negative biopsy and those without a history of TRUS *** studies were also cross-referenced with RP *** analyses determined predictive ability of extracapsular extension(ECE),seminal vesicle involvement(SVI),and pathologic evidence of clinically significant disease(Gleason score7).Results:Four hundred biopsy naive or prior negative biopsy patients had positive mpMRI *** sensitivity,specificity,positive and negative predictive values were 94%,37%,58%,and 87%,respectively and 95%,31%,42%,and 93%,respectively for overall cancer detection and Gleason score≥7 *** patients with no prior biopsy history,mpMRI sensitivity,specificity,positive and negative predictive values were 94%,36%,65%,and 82%,for all cancers,and 95%,30%,50%,and 89%for Gleason score7 lesions,*** those with prior negative biopsy sensitivity,specificity,positive and negative predictive values were 94%,37%,52%,and 90% for all cancers,and 96%,32%,36%,and 96% for Gleason score7 lesions,***-four patients underwent radical prostatectomy(RP)after *** size on mpMRI corr

We have previously shown that fibroblasts resistant to SCD1 inhibition acquire the metabolic sensitivity when reprogrammed into induced PSCs, but this sensitivity is quickly lost upon differentiation (Ben-David et al., 2013). Here we demonstrate that these fibroblasts acquire the same metabolic vulnerability when they are transformed into undifferentiated stem cell-like cancer cells, but the resistance to SCD1 inhibition is conferred again once cancer cells are induced to differentiate (Figure 1I). These findings link, for the first time, the differentiation state of cancer cells with their response to inhibition of oleate synthesis, suggesting that SCD1 inhibition might be an especially effective cancer therapy for undifferentiated tumor types. This work also demonstrates that research on cellular reprogramming can yield insights into cancer biology, and validates that in vitro generated CSCs can be successfully used for screening anticancer drugs. AIM:To investigate the nature and significance of unexpected positron emission tomography with fluorodeoxyglucose(FDG-PET)uptake within the gastrointestinal tract(GIT).METHODS:Patients with unexpected FDG-PET findings in the GIT were *** patients had a previous confirmed malignancy,either solid or *** radiologic reports were performed by experienced radiologists with an exclusive PET ***,i.e.,esophagogastroduodenoscopy(EGD)and colonoscopy,and histopathological evaluation of all findings was performed in all patients in accordance to the FDG-PET *** findings from each of these modalities were compared to each *** clinically significant and insignificant findings were ***:Seventy-two patients were endoscopically ***-seven patients(37.5%)had primarily a lymphoproliferative tumor and 45(62.5%)had solid *** 50 patients(69.4%)the endoscopic examination revealed lesions in the same anatomical areas as the FDG-PET *** these 50 patients,malignant and premalignant lesions i.e.,adenomatous polyps were found in 16(32%)and 9(18%)patients,*** was noted in an additional20 patients(40%).Compared to primary solid tumors,a background of primary lymphoproliferative malignancy was more likely to reveal an additional primary malignancy(15.6%vs 33.3%,respectively,P<0.01).EGD compared to colonoscopy,revealed altogether 11(25.6%)new malignancies compared to 5(17.2%),respectively,P=*** GIT clinically significant findings were overseen by the ***:Unexpected FDG uptake in the GIT is commonly encountered and may contain significant *** evaluation is justified in order to detect these additional findings.
