Septic cardiomyopathy phenotype in the critically ill may depend on antimicrobial resistance

Background Sepsis is a life-threatening organ dysfunction,and septic cardiomyopathy(SCM)may complicate the course of the *** with multidrug-resistant(MDR)pathogens has been linked with worse *** study aims to evaluate SCM in patients with infections caused by different antimicrobial-resistant *** This retrospective study included patients with sepsis/septic shock,hospitalized,and intubated in the intensive care unit of the University Hospital of Larissa between January 2022 and September 2023 with echocardiographic data during the first two days after infection *** patients were divided into two groups:non-MDR-SCM group and MDR-SCM *** cardiac function was compared between the two *** A total of 62 patients were included in the ***-four patients comprised the MDR-SCM and 18 the non-MDR-SCM ***-six patients(41.9%)presented with left ventricular(LV)systolic dysfunction,and≤35%right ventricular fractional area change(RVFAC)was present in 56.4%.LV systolic function was more severely impaired in the non-MDR-SCM group(left ventricular ejection fraction,35.8%±4.9%vs.45.6%±2.4%,P=0.049;LV outflow tract velocity time integral,[10.1±1.4]cm vs.[15.3±0.74]cm,P=0.001;LV-Strain,–9.02%±0.9%vs.–14.02%±0.7%,P=0.001).The MDR-SCM group presented with more severe right ventricular(RV)dilatation(right ventricular end-diastolic area/left ventricular end-diastolic area,0.81±0.03 vs.0.7±0.05,P=0.042)and worse RV systolic function(RVFAC,32.3%±1.9%vs.39.6%±2.7%,P=0.035;tricuspid annular plane systolic excursion,[15.9±0.9]mm vs.[18.1±0.9]mm,P=0.165;systolic tissue Doppler velocity measured at the lateral tricuspid annulus,[9.9±0.5]cm/s vs.[13.1±0.8]cm/s,P=0.002;RV-strain,–11.1%±0.7%vs.–15.1%±0.9%,P=0.002).Conclusion SCM related to MDR infection presents with RV systolic dysfunction predominance,while non-MDR-SCM is mainly depicted with LV systolic dysfunction impairment.

RNA binding proteins:a common denominator of neuronal function and dysfunction

In eukaryotic cells, gene activity is not directly reflected by protein levels because mRNA processing, transport, stability, and translation are co- and post-transcriptionally regulated. These processes, collectively known as the ribonome, are tightly controlled and carried out by a plethora of trans-acting RNA-binding proteins （RBPs） that bind to specific cis elements throughout the RNA sequence. Within the nervous system, the role of RBPs in brain function turns out to be essential due to the architectural complexity of neurons exemplified by a relatively small somal size and an extensive network of projections and connections, Thus far, RBPs have been shown to be indispensable for several aspects of neurogenesis, neurite outgrowth, synapse formation, and plasticity. Consequently, perturbation of their function is central in the etiology of an ever-growing spectrum of neurological diseases, including fragile X syndrome and the neurodegenerative disorders frontotemporal lobar degeneration and amyotrophic lateral sclerosis.

Spatial resource allocation via extremal optimization enhanced by cell-based local search

A new treatment is presented for land use planning problems by means of extremal optimization(EO)in conjunction to cell-based neighborhood local ***,inspired by self-organized critical models of evolution has been applied mainly to the solution of classical combinatorial optimization ***-based local search has been employed by the author elsewhere in problems of spatial resource allocation in combination with genetic algorithms and simulated *** this paper,it complements EO in order to enhance its capacity for a spatial optimization *** hybrid method thus formed is compared to methods of the literature on a specific resource allocation problem by taking into account both the development and the transportation *** yields better results both in terms of objective function values and in terms of *** latter is an important quantity for spatial planning and its meaning is *** appearance of significant compactness values as emergent results is investigated.

Learning and change are key elements of clinical governance and are responsible for the progression of our *** orthopaedics has been slow to embrace quality improvement,recent years have seen global developments in surgical education,quality improvement,and patient outcome *** review covers recent advances in the evaluation of learning and change and identifies the most important research questions that remain *** into proxies of learning is improving but more work is required to identify the best proxy for a given *** curves are becoming commonplace but are poorly integrated into postgraduate training curricula and there is little agreement over the most appropriate method to analyse learning curve *** various organisations promoting centralisation of care,learning curve analysis is more important than ever *** use of simulation in orthopaedics is developing but is yet to be formally mapped to resident training *** outcome research is rapidly changing,with an increased focus on quality of life *** are key to patients and their *** analysis is increasingly seen in orthopaedic manuscripts and this needs to continue to improve evidence-based ***-scale international,multi-centre randomised trials are gaining popularity and updated guidance on sample size estimation needs to become widespread.A global lack of surgeon equipoise will need to be *** improvement projects frequently employ interrupted time-series analysis to evaluate *** technique’s limitations must be acknowledged,and more work is required to improve the evaluation of change in a dynamic healthcare environment where multiple interventions frequently *** in the evaluation of learning and change are needed to drive improved international surgical education and increase the reliability,validity,and importance of the conclusions drawn from orthopaedic research.

Harmony Search and Cellular Automata in Spatial Optimization

The combined optimization problem of resource production and allocation is considered. The spatial character of the problem is emphasized and cellular modeling is introduced. First a new enhanced harmony search algorithm is applied combined with cellular concepts. Then another new approach is presented involving a cellular automaton combined with harmony search. This second approach renders solutions with greater compactness, a desirable characteristic in spatial optimization. The two algorithms are compared and discussed.
