Parent-Reported Reasons for Loss-to-Follow Up in Patients Hospitalized at the Neonatology Unit at Laquintinie

Introduction: Despite the known benefits of newborn follow-up clinics, attendance has historically been difficult. Infants with reported follow-up difficulty have a greater incidence of severe sensorimotor and cognitive deficits and poorer access to early intervention programs. Our objective was to determine the parent-reported reasons for loss-to-follow up in patients hospitalised at the neonatology unit of Laquintinie Hospital. Methodology: We carried out a cross-sectional study through phone interviews with parents/caregivers of patients hospitalised at Laquintinie over a 2-year period from 1st January 2021 to 31st December 2022. A non-standardised structured interview guide was used for data collection. Loss-to-follow up referred to absence to at least one visit as recorded in the neonatal follow-up chart. All necessary administrative and ethical considerations were duly respected. Results: Most neonates were born through vaginal delivery (n = 313, 69.45%). The neonates were admitted at a median gestational age of 33 weeks (Q1-Q3;32 - 35) and the median duration of hospitalisation was 12 days (Q1 - Q3;8 - 18). A total of 23 neonates had died at the time of interview giving a mortality rate of 5.1%. The three most reported reasons for loss-to-follow-up was lack of money (n = 310, 68.13%), assumption that follow-up had ended (n = 37, 8.13%), and newborn that died (n = 23, 5.1%). Conclusion: This study highlights the significant impact of financial constraints and absence of a robust follow-up system on poor uptake of neonatal follow-up post-discharge in resource limited settings like Cameroon. Our results serve as advocacy for national health insurance especially in neonates.

Risk Factors for Geo-Helminthiasis in Children Aged 6 - 36 Months in a Rural Health District in Cameroon

Introduction and Objectives: Soil-Transmitted-Helminthiasis (STH) is a public health problem in Cameroon. The control strategies currently in place, particularly chemoprevention, has shortcomings linked to the target population, which are school-age children. The objective was to determine the prevalence and the risk factors associated with geo-helminthiasis in children aged 0 to 3 years in a rural health district. Method: From December 2020 to May 2021, a descriptive and analytical cross-sectional study of 376 children between 6 and 36 months was carried out in the Akonolinga health district. This was a cluster sampling in 4 health areas. Stool samples were collected and analysed using the mini-FLOTAC method. The results expressed as the number of eggs per gram of stool. A questionnaire on socio-demographic and lifestyle data was administered to the parents. The Chi-squared test was used to measure the association between geo-helminth infection and the data collected. A multivariate analysis using logistic regression was performed (p 0.05). Results: The prevalence of STH was 19.4% (Ascaris lumbricoides: 16% and Trichuris trichiura: 8%). Risk factors were: consumption of contaminated water (AOR = 1.93 [1.03 - 3.6];p = 0.040), early contact of the child with the ground (before age of 4 months) (AOR = 4.9 [2.1 - 11.37];p .001), habit of walking barefoot (AOR = 2.91 [1.1 - 7.97];p = 0.038), and living in a habitat with unpaved ground (AOR = 7.4 [1.55 - 35.7];p = 0.012). Conclusion: The prevalence of STHs in infants was high. Preventive chemotherapy should be extended to this age-group, and other measures intensified.

A short climatology of black and brown carbon and their sources at a suburban site impacted by smoke in Brazil

emissions from biomass burning challenge efforts to curb air pollution in cities downwind of fire-prone regions,as they contribute large amounts of brown carbon(Br C)and black carbon(BC)*** investigated the patterns of Br C and BC concentrations using Aethalometer data(atλ=370 and 880 nm,respectively)spanning four years at a site impacted by the outflow of *** data required to be post processed for the shadowing effect since,without correction,concentrations would be between 29%and 35%*** Br C concentrations were consistently higher than the BC concentrations,indicating the prevalence of aerosols from biomass *** results were supported by the Angstrom coefficient(A_(370/880)),with values predominantly larger than 1(mean±standard deviation:1.25±0.31).A_(370/880)values below 1 were more prevalent during the wet season,which suggests a contribution from fossil fuel *** observed sharp Br C and BC seasonal signals,with mean minimum concentrations of 0.40μg/m^(3)and 0.36μg/m^(3),respectively,in the wet season,and mean maximum concentrations of 2.05μg/m^(3)and 1.53μg/m^(3)in the dry *** largest concentrations were observed when northerly air masses moved over regions with a high density of fire *** burning of residential solid waste and industrial combustion caused extreme Br C and BC concentrations under favourable wind *** neither pollutant is included in any ambient air quality standards,our results suggest that transboundary smoke may hamper efforts to meet the World Health Organization guidelines for fine particles.

Alkali and Plasma-Treated Guadua angustifolia Bamboo Fibers:A Study on Reinforcement Potential for Polymeric Matrices

This study focuses on treating Guadua angustifolia bamboo fibers to enhance their properties for reinforcement applications in composite ***(alkali)and physical(dry etching plasma)treatments were used separately to augment compatibility of Guadua angustifolia fibers with various composite *** influence of these treatments on the fibers’performance,chemical composition,and surface morphology were *** analysis indicated that alkali treatments reduced the tensile modulus of elasticity and strength of fibers by up to 40%and 20%,respectively,whereas plasma treatments maintain the fibers’mechanical *** spectroscopy revealed significant alterations in chemical composition due to alkali treatments,while plasma-treated fibers showed minimal *** examination through Scanning electron Microscopy(SeM)revealed post-treatment modifications in both cases;alkali treatments served as a cleanser,eliminating lignin and hemicellulose from the fiber surface,whereas plasma treatments also produce rough *** results validate the impact of the treatments on the fiber mechanical performance,which opens up possibilities for using Guadua angustifolia fibers as an alternative reinforcement in composite manufacturing.

Background: Pancreaticoduodenectomy is the standard treatment for resectable periampullary cancer. Surgical site infections(SSI) are common complications with increased morbidity. The study aimed to describe the prevalence, risk factors, microbiology, and outcomes of SSI among patients undergoing pancreaticoduodenectomy. Methods: We conducted a retrospective study in a referral cancer center between January 2015 and June 2021. We analyzed baseline patient characteristics and SSI occurrence. Culture results and susceptibility patterns were described. Multivariate logistic regression was used to determine risk factors, proportional hazards model to evaluate mortality, and Kaplan-Meier analysis to assess long-term survival. Results: A total of 219 patients were enrolled in the study;101(46%) developed SSI. Independent factors for SSI were diabetes mellitus, preoperative albumin level, biliary drainage, biliary prostheses, and clinically relevant postoperative pancreatic fistula. The main pathogens were enterobacteria and enterococci. Multidrug-resistance rate in SSI was high but not associated with increased mortality. Infected patients had higher odds of sepsis, longer hospital stay and intensive care unit stay, and readmission rate. Neither 30-day mortality nor long-term survival was significantly different between infected and non-infected patients. Conclusions: SSI prevalence among patients undergoing pancreaticoduodenectomy was high and largely caused by resistant microorganisms. Most risk factors were related to preoperative instrumentation of the biliary tree. SSI was associated with greater risk of unfavorable outcomes;however, survival was unaffected.

Adverse Pregnancy Outcomes Following Cryotherapy, Thermal Ablation and Loop electrosurgical excision Procedure for Cervical Intraepithelial Neoplasia Treatment: A Pilot Study among Zambian Women

Background: Cervical Intraepithelial neoplasia treatments have become essential interventions to manage cervical lesions. Majority of the recipients of these treatments are women within the reproductive age group, who according to literature may be at risk of adverse pregnancy outcomes. This pilot study is part of a study investigating adverse pregnancy outcomes among women who received Cryotherapy, Thermal ablation and Loop electrosurgical excision Procedure compared to the untreated women in Zambia. Materials and Methods: This descriptive study analyzed records of 886 (n = 443 treated and n = 443 untreated) women aged 15 - 49 years. The women were either screened with Visual Inspection with Acetic Acid or treated for Cervical Intraepithelial neoplasia at the Adult Infectious Disease Centre between January 2010 and December 2020. Women meeting the criteria were identified using the Visual Inspection with Acetic Acid screening records and telephone interviews to obtain the adverse pregnancy outcome experienced. Data were analysed using STATA version 16 to determine the prevalence and obtain frequency distribution of outcomes of interest. Univariate and multivariable binary logistic regression estimated odds of adverse pregnancy outcomes across the three treatments. Results: The respondents were aged 15 to 49 years. Adverse pregnancy outcomes were observed to be more prevalent in the treatment group (18.5%) compared to the untreated group (5.4%). Normal pregnancy outcomes were lower in the treated (46.3%;n = 443) than the untreated (53.7%;n = 443). The treated group accounted for the majority of abortions (85.2%), prolonged labour (85.7%) and low birth weight (80%), whereas, the untreated accounted for the majority of still births (72.7%). Women treated with cryotherapy (aOR = 2.43, 95% CI = 1.32 - 4.49, p = 0.004), thermal ablation (aOR = 6.37, 95% CI = 0.99 - 41.2, p = 0.052) and Loop electrosurgical excision Procedure (aOR = 9.67, 95% CI = 2.17 - 43.1, p = 0.003)
