AIM:To study the incidence and survival rate of stomach cancer(sC)and its associated factors in a high risk population in Chile. METHODs:The population-based cancer registry of Valdivia,included in the International Agency for Research on Cancer system,covers 356 396 residents of Valdivia Province,southern *** studied all sC cases entered in this Registry during 1998-2002 (529 cases).Population data came from the Chilean census(2002).standardized incidence rates per 100 000 inhabitants(sIR)using the world population, cumulative risk of developing cancer before age 75, and rate ratios by sex,age,ethnicity and social factors were *** survival(EdererⅡmethod) and age-standardized estimates(Brenner method) were *** survival rates(Kaplan-Meier) were measured at 3 and 5 years and survival curves were analyzed with the Logrank and Breslow tests. survival was studied in relation to demographics, clinical presentation,laboratory results and medical management of the *** variables significantly associated with survival were later included in a Cox multivariate model. REsULTs:Between 1998 and 2002,529 primary gastric cancers occurred in Valdivia(crude incidence rate 29.2 per 100000 inhabitants).Most cases were male(69.0%), residents of urban areas(57.5%)and Hispanic(83.2%), with a low education level(84.5%school years). sC sIR was higher in men than women(40.8 and 14.8 respectively,Psk factors were low education RR 4.4(95%CI:2.9-6.8)and 1.6,(95%CI:1.1-2.1) for women and men respectively and Mapuche ethnicity only significant for women(RR 2.2,95%CI:1.2-3.7).Of all cases,76.4%were histologically confirmed,11.5% had a death certificate only(DCO),56.1%were TNM stageⅣ;445 cases(84.1%)were eligible for survival analysis,all completed five years follow-up;42 remained alive,392 died of sC and 11 died from other causes. specific 5-year survival,excluding cases with DCO,was 10.6%(95%CI:7.7-13.5);5-year relative survival rate was 12

早期非小细胞肺癌(NsCLC)患者完全手术切除后需要新的辅助治疗策略来优化其预后。该研究的目的是评估患者在以铂类药物为基础的辅助化疗后辅助阿替利珠单抗治疗与最佳支持治疗之间的效果。IMpower010是一项随机、全球多中心、开放标签的Ⅲ期研究, 在22个国家和地区的227个地点完成。根据国际癌症联合会和美国癌症联合委员会的分期系统(第7版), 符合条件的患者为年龄18岁或以上, 且可完全切除的ⅠB期(肿瘤≥4 cm)至ⅢA NsCLC。2015年10月7日至2018年9月19日期间, 共有1 280例患者接受肿瘤完全性切除术后入组。1 269例接受辅助化疗, 其中1 005例患者适合接受随机阿替利珠单抗(n=507)或最佳支持治疗(n=498);每组495例接受治疗。患者通过排列区组法(4个区块)随机分配(1∶1)在铂类药物化疗辅助(1至4个周期)后接受阿替利珠单抗辅助(每21天 1 200 mg;16个周期或1年)或进行最佳支持治疗(观察和定期扫描疾病复发情况)。主要终点为研究人员评估无病生存期, 按次序首先在Ⅱ~ⅢA期人群亚组[≥1%肿瘤细胞表达PD-L1(sP263)]进行分级检测, 然后是Ⅱ~ⅢA期所有患者, 最后是意向治疗(ITT)人群(ⅠB~ⅢA期)。对所有随机分配并接受阿替利珠单抗或最佳支持治疗的患者进行安全性评估。Ⅱ~ⅢA期患者经中位随访32.2个月(四分位区间:27.4~38.3)后, 与最佳支持治疗相比, 阿替利珠单抗治疗提高了有≥1%肿瘤细胞表达PD-L1的Ⅱ~ⅢA期患者(HR=0.66;95%CI:0.50~0.88;P=0.003 9), 以及所有Ⅱ~ⅢA期患者(0.79;0.64~0.96;P=0.020)的无病生存期。在ITT人群中, 无病生存期的HR为0.81(0.67~0.99;P=0.040)。不良事件方面, 495患者中有53例(11%)发生阿替利珠单抗相关的3级和4级不良事件, 4例(1%)发生阿替利珠单抗相关的5级不良事件。

Adjuvant atezolizumab after adjuvant chemotherapy in resected stage IB-IIIA non-small-cell lung cancer (IMpower010): a randomised, multicentre, open-label, phase 3 trial

Methods:IMpower010 was a randomised,multicentre,open-label,phase 3 study done at 227 sites in 22 countries and *** patients were 18 years or older with completely resected stage IB(tumours>4 cm)to IIIA NsCLC per the Union Internationale Contre le Cancer and American Joint Committee on Cancer staging system(7th edition).Patients were randomly assigned(1:1)by a permuted-block method(block size of four)to receive adjuvant atezolizumab(1200 mg every 21 days;for 16 cycles or 1 year)or best supportive care(observation and regular scans for disease recurrence)after adjuvant platinum-based chemotherapy(one to four cycles).The primary endpoint,investigator-assessed disease-free survival,was tested hierarchically first in the stage II-IIIA population subgroup whose tumours expressed PD-L1 on 1%or more of tumour cells(sP263),then all patients in the stage II-IIIA population,and finally the intention-to-treat(ITT)population(stage IB-IIIA).safety was evaluated in all patients who were randomly assigned and received atezolizumab or best supportive ***010 is registered with ***,NCT02486718(active,not recruiting).

The objective of this article is to analyze the new cooperation of the European Union,in particular with middle and upper-middle-income countries,in order to understand the development policy of the Union in view of the new global scenario,taking into account the new status that these countries have acquired and the implications of this entails in international cooperation *** leads us to delve into how the EU cooperates with the middle-income countries and more advanced developing *** contribution of this work addresses some of the challenges and perspectives that are currently being planned about the EU’s development policy,providing different elements of analysis and reflection on its future evolution.
