Modulation of oxidative damage and genotoxicity of biosynthesized zinc oxide nanoparticles via supplementation with modified cinnamaldehyde-loaded chitosan nanoparticles in mice

Synthesis of zinc oxide nanoparticles (Zn-NPs) via green method is an outstanding alternative to conventional/regular methods; however， the safety or toxicity of the biosynthesized ZnO-NPs in vivo is not fully explored. This study was conducted to evaluate the protective efficiency of cinnamaldehyde-loaded chitosan nanoparticles (Cin@CSNPs) against oxidative damage and genotoxicity of zinc oxide nanoparticles (ZnO-NPs) in mice. ZnO-NPs were biosynthesized using the extract of fresh leaves of Mentha pulegium L. Cin was extracted from cinnamon essential oil， and was loaded into chitosan nanoparticle (Cin@CSNPs). Both ZnO-NPs， Cin@CSNPs and CSNPs were characterized. The in vitro release of Cin@CSNPs was determined. In the biological study， 6 groups of male Balb/c mice were treated by gavage for 3 weeks as follows， control group， the group received ZnO-NPs (25 mg/kg b.w)， the groups received Cin@CSNPs at low dose (50 mg/kg b.w) or high dose (100 mg/kg b.w)， and the groups received ZnO-NPs plus Cin@CSNPs at the two tested doses. Blood and tissue samples were collected for different biochemical， genetical and histological studies. The particle size of ZnO-NPs， CSNPs， and Cin@CSNPs were (20.78 ± 2.6)， (170.0 ± 3.7)， and (218.23 ± 2.9) nm， and ξ-potential were (32.7 ± 4.6)， (8.32 ± 0.27) and (4.8 ± 0.21)， respectively. ZnO-NPs disturbed the biochemical and oxidative stress indices， AFP， CEA， TNF-α， chromosomal aberrations in somatic and germ cells， and sperm abnormality along with severe pathological changes in the hepatic， renal， and testicular tissues. Cin@CSNPs improved significantly all the parameters tested and the histological picture in a dose-dependent. Therefore， the biosynthesized ZnO-NPs exhibit oxidative damage and genotoxicity， and Cin@CSNPs have potential protective effects against the risks of ZnO-NPs and may be a promising tool to overcome the challenges of using Cin in food and pharmaceuticals applications.

Difference between papillary and follicular thyroid carcinoma outcomes:an experience from Egyptian institution

Objective： Differentiated thyroid carcinomas （DTCs） are classified into papillary thyroid carcinoma （PTC） and follicular thyroid carcinoma （FTC）. DTCs are analyzed as a single group in clinical studies that investigated the prognostic factors and prognosis of these malignancies. However, the biological behaviors of these carcinomas significantly differ. In the present study, we aimed to detect differences in the outcomes between PTC and FTC in Mansoura University Hospital in Egypt. Methods： A total of SS8 patients with histologically proven thyroid carcinomas from January 2003 to December 2012 were retrospectively enrolled. The clinical and pathological data of patients were reviewed. Results： Large primary tumor size, lymph node involvement, extrathyroid extension, and distant metastasis were significant poor prognostic factors for overall survival （OS） in old PTC patients. Cox hazard analysis showed that the patient＇s age, extra thyroid extension, and distant metastasis were the only independent prognostic factors. In FTC patients, only the distant metastasis and degree of tumor invasion were significant poor prognostic factors in OS univariate analysis. However, these factors were nonsignificant in multivariate analysis. The 10-year OS rates were 97% and 89% for PTC and FTC, respectively （P=0.003）. The 10-year disease-free survival （DFS） rates were 77.2% in PTC vs. 65% in FTC （P=0.179）. Conclusion： The significant prognostic factors vary between the two types of DTCs. Therefore, PTC and FTC patients need to be analyzed and reported independently. PTC survival is widely and significantly affected by age, extrathyroid extension, and distant metastasis. By contrast, these factors were nonsignificant in FTC, which showed poorer survival than PTC.

Male Breast Cancer:10-Year Experience at Mansoura University Hospital in Egypt

Objective Male breast cancer （MBC） is a rare disease representing less than 1% of all malignancies. The objective of the study is to report clinicopathological characteristics, treatment patterns, and outcomes of MBC in Mansoura University Hospital, Egypt. Methods This retrospective study focused on male breast cancer patients during 10 years （2000-2009）. The studied variables were data regarding general characteristics of patients, treatment modalities and survival. Results The series included 37 patients （0.8% of all breast cancer）. The median age was 57.7 years （range： 26-86 years）. The main clinical complaint was a mass beneath the areola ill 94.5% of the cases. Most patients had a locally advanced disease. 94.5% of tumors were invasive duct carcinomas. The treatment was essentially surgery in 91.8%, followed by adjuvant radiotherapy （in 89.2%）, hormonal therapy （in 56.7%） and chemotherapy （in 91.8%）. Follow-up period ranged from 6-115 months. Local recurrence occurred in 4 cases and metastasis in 11 cases. The 2-year and 5-year overall survival （OS） rates were 81.6% and 60.5%, respectively. The 2-year and 5-year disease-free survival （DFS） rates were 68.4%, and 52.6%, respectively. OS was not significantly affected by any of the studied parameters. Factors influencing DFS were： T stage （P=0.05）, positive lymph nodes （P=0.043）, metastasis （P=0.004）, and chemotherapy （1＇=0.046）. Conclusions MBC is a rare disease and often diagnosed at a locally advanced stage. The management of male and female breast carcinoma is identical. Future research for better understanding of this disease is needed to improve the management and prognosis of male breast cancer patients.

Second-line panitumumab as a triweekly dose for patients with wild-type KRAS exon 2 metastatic colorectal cancer:a single-institution experience

Objective: Panitumumab administered as monotherapy in colorectal cancer(CRC) has shown response and disease stabilization rates of approximately 30%. The current study aimed to evaluate the progression-free survival(PFS) and overall survival(OS) of patients with metastatic colorectal cancer(mCRC) treated with panitumumab every 3 weeks as a second line ***: This study is a retrospective analysis of 18 patients, aged more than 18 years, with wild-type KRAS exon 2 mCRC treated with panitumumab as a second-line single agent after progression on first-line ***: The median number of courses received was 10(range, 4-29), and the median duration of treatment was 30 weeks(range,12-96 weeks). After a median follow-up period of 13 months, the median PFS was 6 months(range, 4.3-7.7 months) and the median OS was 11 months(range, 7.4-14.5 months). The median PFS was 4 months for patients with sh(P=0.05). The median OS was 9 months(range, 6.4-11.5months) and 14 months(range, 11.6-16.3 months) for the two groups of patients(P=0.002).Conclusions: Panitumumab given every 3 weeks is effective and well tolerated in patients with advanced CRC that progressed after standard chemotherapy.

Currently,COVID-19 is spreading all over the world and profoundly impacting people’s lives and economic *** this paper,a novel approach called the COVID-19 Quantum Neural Network(CQNN)for predicting the severity of COVID-19 in patients is *** consists of two phases:In the first,the most distinct subset of features in a dataset is identified using a Quick Reduct Feature Selection(QRFS)method to improve its classification performance;and,in the second,machine learning is used to train the quantum neural network to classify the *** is found that patients’serial blood counts(their numbers of lymphocytes from days 1 to 15 after admission to hospital)are associated with relapse rates and evaluations of COVID-19 ***,the severity of COVID-19 is classified in two categories,serious and *** experimental results indicate that the proposed CQNN’s prediction approach outperforms those of other classification algorithms and its high accuracy confirms its effectiveness.

People who are deaf or have difficulty speaking use sign language,which consists of hand gestures with particular motions that symbolize the“language”they are communicating.A gesture in a sign language is a particular movement of the hands with a specific shape from the fingers and whole *** this paper,we present an Intelligent for Deaf/Dumb People approach in real time based on Deep Learning using Gloves(IDLG).The approach IDLG offers scientific contributions based deep-learning,a multimode command techniques,real-time,and effective use,and high accuracy *** this purpose,smart gloves working in real time were *** data obtained from the gloves was processed using deep-learning-based approaches and classified multi-mode commands that allow dumb people to speak with regular people via their smart ***,the glove has five flex sensors and an accelerometer using to achieve Low-Cost Control *** flex sensor generates a proportional change in resistance for each individual *** processing of these hand gestures is in Atmega32A Microcontroller which is an advance version of the microcontroller and the lab view *** compares the input signal to memory-stored specified voltage *** performance of the IDLG approach was verified on a dataset created using different hand gestures from 20 different *** the test using the IDLG approach on 10,000 data points,process time performance of milliseconds was achieved with 97%accuracy.

Surgical outcomes of complicated cataract with pediatric trematode granulomatous uveitis

AIM:To describe the clinical characteristics and surgical outcomes of complicated cataract with pediatric trematodal granulomatous uveitis(TGU).METHODS:Patients of cataract with TGU in the membranous(inactive) stage underwent cataract surgery with intraocular lens(IOL) *** history and ophthalmic examination were conducted for all cases,whereas Schimphlug imaging and corneal topography were done for some *** follow up was done on the 1st,2nd,and 5th postoperative days after ***,it was done at least at one,three,and six months *** and postoperative complications and the methods of their management were ***:Twelve eyes of 12 male children were included in this study ranging from 8 to *** mean best corrected Snellen visual acuity(BCVA) was significantly improved from 0.09±0.06 preoperatively to 0.37±0.11 at the final visit 6mo postoperatively(Pshowed flattening of the anterior surface of the inferior *** difficulties and complications included the poor dilatability of the drown down pupil,strong posterior synechia between the anterior lens capsule and the iris at the site of the inferior retrocorneal vascularized membrane and *** cases underwent primary hydrophobic IOL ***:Surgery for this type of cataract is relatively safe and *** is associated with some specific difficulties and complications that should be considered during surgery and follow up.

Efficacy and Safety of Ombitasvir plus Paritaprevir,Ritonavir and Ribavirin in Non-cirrhotic Treatment-naive and Treatment-experienced Egyptians with Chronic HCV Genotype-4 Infection

Hepatitis C virus genotype 4(HCV-GT4)is a risk factor for cirrhosis,hepatocellular carcinoma and liver failure.A combination of three new direct-acting antivirals ombitasvir,paritaprevir,and ritonavir has been recommended for treatment of HCV-GT4 *** current study was aimed to assess the efficacy and safety of this combination plus ribavirin in non-cirrhotic,treatment-naive and-experienced Egyptians with HCV-GT4 infection in a real-world setting.A total of 255 Egyptians with HCV-GT4 infection were enrolled,including 82 treatment-experienced and 173 treatment-naive *** of them completed 12-week treatment protocol of ombitasvir,paritaprevir and ritonavir as an oral dose combination with *** response(VR)was measured,as well as the biochemical parameters related to treatment efficacy and adverse events at baseline and after treatment,at 4(VR4)and 12(VR12)weeks *** results showed that the VR4 rates were 98.8%in both groups,and VR12 rates were 97.7%and 96.3%in treatment-naive and-experienced patients,respectively,with no significant differences found between the groups concerning VR4(P=0.9)and VR 12(P=0.3).The most common adverse events were headache and fatigue,which were significantly more common(P=0.001 and 0.003,respectively)in treatment-experienced than in treatment-naive *** quadruple regimen was well-tolerated,and the reported adverse events were generally mild to *** real-world setting study confirms that the combination of ombitasvir,paritaprevir,ritonavir,and ribavirin is highly effective in the treatment of HCV-GT4 infection with a good safety and tolerability profile.

Management of tear trough deformity with and without tear trough-orbicularis retaining ligament complex release in transconjunctival blepharoplasty: a comparative study

AIM: To evaluate the effect of releasing the orbicularis retaining ligament(ORL) complex of the tear trough combined with the transconjunctival blepharoplasty in the surgical management of tear trough deformity compared with the effect of blepharoplasty alone. METHODS: A prospective, randomized(by closed envelope technique), controlled surgical trial which included 50 patients(100 eyes) with bilateral visible tear trough deformity and lower eyelid fat bulging, was divided into two groups where 25 patients(50 eyes) had tear trough-ORL release with blepharoplasty and 25 patients(50 eyes) didn't. Qualitative and quantitative assessments of tear trough deformity were done at 6 mo postoperatively, achieving grade 0 or 1 of Barton's classification was considered a success. Standardized photographic documentation of each patient was done pre and postoperatively, also assessment of the patients' satisfaction postoperatively was done and ranked as excellent, very good, good or fair. RESULTS: There was statistically significant difference between the two groups in the overall aesthetic results postoperatively regarding the qualitative and quantitative analysis of the tear trough deformity, where patients who had tear trough-ORL complex release had more successful outcomes than those of the second group. CONCLUSION: ORL release should be done in patients with tear trough deformity in order to release the tethering effect of this ligament which causes the prominence of the naso jugal groove.
