Difference between papillary and follicular thyroid carcinoma outcomes:an experience from Egyptian institution

Objective: Differentiated thyroid carcinomas(DTCs) are classified into papillary thyroid carcinoma(PTC) and follicular thyroid carcinoma(FTC). DTCs are analyzed as a single group in clinical studies that investigated the prognostic factors and prognosis of these malignancies. However, the biological behaviors of these carcinomas significantly differ. In the present study, we aimed to detect differences in the outcomes between PTC and FTC in mansoura University Hospital in Egypt. methods: A total of 558 patients with histologically proven thyroid carcinomas from January 2003 to December 2012 were retrospectively enrolled. The clinical and pathological data of patients were reviewed. Results: Large primary tumor size, lymph node involvement, extrathyroid extension, and distant metastasis were significant poor prognostic factors for overall survival(OS) in old PTC patients. Cox hazard analysis showed that the patient's age, extra thyroid extension, and distant metastasis were the only independent prognostic factors. In FTC patients, only the distant metastasis and degree of tumor invasion were significant poor prognostic factors in OS univariate analysis. However, these factors were nonsignificant in multivariate analysis. The 10-year OS rates were 97% and 89% for PTC and FTC, respectively(P=0.003). The 10-year disease-free survival(DFS) rates were 77.2% in PTC vs. 65% in FTC(P=0.179). Conclusion: The significant prognostic factors vary between the two types of DTCs. Therefore, PTC and FTC patients need to be analyzed and reported independently. PTC survival is widely and significantly affected by age, extrathyroid extension, and distant metastasis. By contrast, these factors were nonsignificant in FTC, which showed poorer survival than PTC.

Second-line panitumumab as a triweekly dose for patients with wild-type KRAS exon 2 metastatic colorectal cancer:a single-institution experience

Objective: Panitumumab administered as monotherapy in colorectal cancer(CRC) has shown response and disease stabilization rates of approximately 30%. The current study aimed to evaluate the progression-free survival(PFS) and overall survival(OS) of patients with metastatic colorectal cancer(mCRC) treated with panitumumab every 3 weeks as a second line ***: This study is a retrospective analysis of 18 patients, aged more than 18 years, with wild-type KRAS exon 2 mCRC treated with panitumumab as a second-line single agent after progression on first-line ***: The median number of courses received was 10(range, 4-29), and the median duration of treatment was 30 weeks(range,12-96 weeks). After a median follow-up period of 13 months, the median PFS was 6 months(range, 4.3-7.7 months) and the median OS was 11 months(range, 7.4-14.5 months). The median PFS was 4 months for patients with months(range, 4.5-7.5 months) for patients with ≥ grade 2 skin rash(P=0.05). The median OS was 9 months(range, 6.4-11.5months) and 14 months(range, 11.6-16.3 months) for the two groups of patients(P=0.002).Conclusions: Panitumumab given every 3 weeks is effective and well tolerated in patients with advanced CRC that progressed after standard chemotherapy.

Surgical outcomes of complicated cataract with pediatric trematode granulomatous uveitis

male Breast Cancer:10-Year Experience at mansoura University Hospital in Egypt

Objective male breast cancer(mBC) is a rare disease representing less than 1%of all *** objective of the study is to report clinicopathological characteristics,treatment patterns,and outcomes of mBC in mansoura University Hospital,Egypt. methods This retrospective study focused on male breast cancer patients during 10 years(2000-2009).The studied variables were data regarding general characteristics of patients,treatment modalities and survival. Results The series included 37 patients(0.8%of all breast cancer).The median age was 57.7 years(range:26-86 years).The main clinical complaint was a mass beneath the areola in 94.5%of the *** patients had a locally advanced disease.94.5%of tumors were invasive duct *** treatment was essentially surgery in 91.8%,followed by adjuvant radiotherapy(in 89.2%), hormonal therapy(in 56.7%) and chemotherapy(in 91.8%).Follow-up period ranged from 6-115 *** recurrence occurred in 4 cases and metastasis in 11 *** 2-year and 5-year overall survival(OS) rates were 81.6%and 60.5%,*** 2-year and 5-year disease-free survival(DFS) rates were 68.4%,and 52.6%,*** was not significantly affected by any of the studied *** influencing DFS were:T stage(P=0.05),positive lymph nodes(P=0.043),metastasis(P=0.004),and chemotherapy (P=0.046). Conclusions mBC is a rare disease and often diagnosed at a locally advanced *** management of male and female breast carcinoma is *** research for better understanding of this disease is needed to improve the management and prognosis of male breast cancer patients.

Currently,COVID-19 is spreading all over the world and profoundly impacting people’s lives and economic *** this paper,a novel approach called the COVID-19 Quantum Neural Network(CQNN)for predicting the severity of COVID-19 in patients is *** consists of two phases:In the first,the most distinct subset of features in a dataset is identified using a Quick Reduct Feature Selection(QRFS)method to improve its classification performance;and,in the second,machine learning is used to train the quantum neural network to classify the *** is found that patients’serial blood counts(their numbers of lymphocytes from days 1 to 15 after admission to hospital)are associated with relapse rates and evaluations of COVID-19 ***,the severity of COVID-19 is classified in two categories,serious and *** experimental results indicate that the proposed CQNN’s prediction approach outperforms those of other classification algorithms and its high accuracy confirms its effectiveness.

Efficacy and Safety of Ombitasvir plus Paritaprevir,Ritonavir and Ribavirin in Non-cirrhotic Treatment-naive and Treatment-experienced Egyptians with Chronic HCV Genotype-4 Infection

Hepatitis C virus genotype 4(HCV-GT4)is a risk factor for cirrhosis,hepatocellular carcinoma and liver failure.A combination of three new direct-acting antivirals ombitasvir,paritaprevir,and ritonavir has been recommended for treatment of HCV-GT4 *** current study was aimed to assess the efficacy and safety of this combination plus ribavirin in non-cirrhotic,treatment-naive and-experienced Egyptians with HCV-GT4 infection in a real-world setting.A total of 255 Egyptians with HCV-GT4 infection were enrolled,including 82 treatment-experienced and 173 treatment-naive *** of them completed 12-week treatment protocol of ombitasvir,paritaprevir and ritonavir as an oral dose combination with *** response(VR)was measured,as well as the biochemical parameters related to treatment efficacy and adverse events at baseline and after treatment,at 4(VR4)and 12(VR12)weeks *** results showed that the VR4 rates were 98.8%in both groups,and VR12 rates were 97.7%and 96.3%in treatment-naive and-experienced patients,respectively,with no significant differences found between the groups concerning VR4(P=0.9)and VR 12(P=0.3).The most common adverse events were headache and fatigue,which were significantly more common(P=0.001 and 0.003,respectively)in treatment-experienced than in treatment-naive *** quadruple regimen was well-tolerated,and the reported adverse events were generally mild to *** real-world setting study confirms that the combination of ombitasvir,paritaprevir,ritonavir,and ribavirin is highly effective in the treatment of HCV-GT4 infection with a good safety and tolerability profile.

Effect of Laser Cladding Processing Parameters on Nitinol’s Clad Dimensions, microstructure, and Hardness

Nickel Titanium alloy (Nitinol) is characterized by its good mechanical properties, good damping properties in addition to its distinctive shape-memory effect and superelasticity effect besides its great bio-mechanical compatibility and corrosion resistance. These properties have empowered its applications, particularly within the bio-medical and aerospace industry. Despite these exceptional properties, the manufacturing of Nitinol by conventional methods is exceptionally troublesome and costly and consequently must be inspected. Therefore, additive manufacturing specifically laser-based ones were used recently. In this research, the effect of processing parameters of laser cladding/laser direct deposition on Nitinol’s microstructure, Hardness and Clad Dimensions was evaluated. Systematic characterization of Nitinol samples was done utilizing Optical microscopy and Vickers hardness tester. Samples of Nitinol were synthesized with different processing parameters using laser cladding and its properties were investigated and compared to one another to get the optimum processing parameters to synthesize a near net shape, fully dense Nitinol component with reliable properties. The results showed that there’s a processing parameter window at which the alloy possesses its best mechanical and functional properties which were of Laser power of value 1.25 Kw, Scan speed of 1.5 m/min and powder deposition rate of 1.5/1.5 RPm, these conditions resulted in the formation of martensite phase which is responsible for its functional properties with 40% volume fraction and a hardness value of 598 HV.

Background: Until recently, the experience on implementing community based management of acute malnutrition (CMAM) among children has been largely based in African settings. While the government in Bangladesh is yet to scale up CMAM approach, there is still paucity of knowledge on the experience of CMAM within the complex milieu of an urban slum context. In Kamrangirchar slum, Dhaka, Bangladesh, this paper describes a CMAM programme performance and outcomes run by Médecins Sans Frontières (MSF)/Doctors without Borders, in light to performance indicators set by MSF and the Sphere minimum standards. Methods: This was a descriptive retrospective study using routinely collected programme data of children admitted with severe acute malnutrition between May 2010 and November 2011. Kamrangirchar is an urban slum of a large migrant population in Dhaka, Bangladesh. Results: There was a total of 640 new admissions, of whom 333 (52%) were males. The median age was 18 months (Inter-quartile range (IQR) 12 - 41). 599 children had a reported nutritional outcome at discharge from ambulatory therapeutic feeding centre (ATFC), this included: cure rate of 69% with an average length of stay of 68.8 (SD ± 46.0) days and average weight gain of 3.8 g/kg/day (SD ± 2.7). The lost-to-follow-up rate was 18% and 5% reported to the programme that they will leave the slum and go back to their villages. These performance indicators did not meet the threshold level indicators set by MSF and Sphere standards. Role of Endotoxin and TNF in Developing NAFLD in Non-Obese Egyptian Patients

Background and aim of the work: Non-alcoholic fatty liver disease (NAFLD) was considered the hepatic presentation of insulin resistance and obesity for a long time. Studies on lean weighted Asian subjects with NAFLD revealed that NAFLD pathogenesis may be multifactorial. NAFLD is associated with disturbances in gut flora and excess expression of inflammatory mediators. This study aims to find out the relation of endotoxins absorbed from gut and the tumor necrosis factor alpha with NAFLD in non-obese Egyptian patient in comparison to obese patients and healthy control subjects. Patients and methods: This study was performed on three groups group I: Patients with NAFLD and body mass index 2. Group II: Patients with NAFLD and body mass index >25 kg/m2. Group III: healthy control subjects. Results: Group I had significantly higher endotoxin, tumor necrosis factor(TNF) alpha and ALT than group II (endotoxin 11.7 ± 1.7 ug/L vs 9.5 ± 1.4) (TNF 14.8 ± 5.3 vs 11.3 ± 3.3) (ALT 67.8 ± 5.3 IU/L vs 51.8 ± 4.2). There was a highly significant correlation between TNF, endotoxin levels and level of liver enzymes in group I and II. Conclusion: Endotoxemia and TNF alpha may contribute in the pathogenesis of NAFLD especially in non-obese patients.
