Integrating titanium-based implants with the surrounding bone tissue remains challenging. This study aims to explore the impact of different anodization voltages （20-80V） on the surface topography of two-phase （α/β） Ti alloys and to produce TiO2films with enhanced bone formation abilities. Scanning electron microscopy coupled with energy dispersive spectroscopy （SEm-EDS） and atomic force microscopy （AFm） were applied to investigate the morphological， chemical， and surface topography of the prepared alloys and to confirm the growth of hydroxyapatite （HA） on their surfaces. Results disclosed that the surface roughness of TiO2films formed on Ti-6Al-7Nb alloys was superior to that of Ti–6Al–4V alloys. Ti-6Al-7Nb alloy anodized at 80 V had the highest yields of HA after immersion in SBF with enhanced HA surface coverage. The developed HA layer had a mean thickness of 128.38 ± 18.13 μm， suggesting its potential use as an orthopedic implantable material due to its promising bone integration and， hence， remarkable stability inside the human body.

Titanium and its alloys have numerous biomedical applications thanks to the composition and morphology of their oxide *** this study,the colorful oxide films were formed by anodizing cast Ti-6Al-4V and Ti-6Al-7Nb alloys in a 10%oxalic acid solution for 30 s at different voltages(20–80 V)of a direct current power *** force microscopy was used as an accurate tool to measure the surface roughness of thin films on the nanometer *** electron microscopy and X-ray diffraction were performed to analyze surface morphology and phase *** to the results,the produced titanium oxide layer showed high surface roughness,which increased with increasing anodizing *** impact of anodizing voltages on the color and roughness of anodized layers was *** corrosion resistance of the anodized samples was studied in simulated body fluid at pH 7.4 and a temperature of 37℃ utilizing electrochemical impedance spectroscopy and the potentiodynamic polarization *** anodized samples for both alloys at 40 V were at the optimal voltage,leading to a TiO_(2) layer formation with the best compromise between oxide thickness and corrosion ***,findings showed that TiO_(2) films produced on Ti-6Al-7Nb alloys had superior surface roughness properties compared to those of Ti-6Al-4V alloys,making them more appropriate for orthopedic *** the obtained data and the fruitful discussion,it was found that the utilized procedure is simple,low-cost,and ***,anodization in 10%oxalic acid proved a viable alternative for the surface finishing of titanium alloys for biomedical applications.

Structural and Tectonic Evolution of El-Faiyum Depression,North Western Desert,Egypt Based on Analysis of Landsat ETm+,and SRTm Data

False color Landsat ETm＋（bands 7, 4, 2 in RGB） and SRTm images of the EI-Faiyum depression, Egypt, highlights major NE-SW faults and other lineaments trending NW-SE and N-S. Airborne magnetic data reveal some E-W subsurface faults which are not recognized on Landsat ETm＋ and SRTm images. Ratio images （5/7, 3/1, 4/3） and （5/7, 5/1, 4） were used for lithological discrimination of different rock types. The EI-Faiyum area is occupied by sedimentary rocks of middle Eocene, Upper Eocene, Oligocene, Lower miocene, Pliocene, Pleistocene and Holocene. Understanding the structural regime of EI-Faiyum depression in the Western Desert of Egypt is significant in relation to the neotectonics of the Nile Delta and to the development of new communities west of the delta. The area is covered by Late Cretaceous, middle to Late Eocene deposits, Oligocene deposits （comprising fluviatile sediments and basalt sheets） and Pliocene and Quaternary alluvium and sand deposits. New geological and structural maps were prepared at a scale of 1 ： 100 000 using Landsat ETm＋ and SRTm images coupled with extensive field work. The major structural trends are E-W, ENE, NS and NW--the same trends observed elsewhere in North Africa from NW Sudan to NE Egypt including north and central Sinai. EI-Faiyum depression is a structurally-controlled tectonic basin, marked by northeast-striking faults along its northern margin. These faults are parallel to similar faults of the Syrian arc fold belt, which includes a group of related faults, folds, push-up structures, and basins. Qarun Lake is located in the northern part of EI-Faiyum depression, at a right-step between two strands of the NE striking fault system. We have suggested elsewhere that EI-Faiyum depression is a pull-apart basin related to extension between these mismatched strands of strike-slip faults. Background: maternal obesity is reported to be associated with increased incidence of gestational diabetes mellitus and hypertension. These cause failure of labour induction, leading to higher incidence of cesarean section (CS). The aim of this study was to assess which reduces CS rate, labor induction at 39 weeks or leaving women for spontaneous labor onset till 41 weeks. methodology: A randomized controlled trial was conducted in Ain Shams maternity Hospital in Egypt from 2016 to 2018. Study population consisted of 200 term primigravida pregnant obese women delivered in Ain Shams maternity Hospital. They were divided into two groups: Group A: induction of labor at 39 + 0 weeks (n = 100) by vaginal administration of 25 μg misoprostol (PGE1) every 6 hours for 5 doses;Group B: waiting spontaneous labor onset till 41 + 0 weeks (n = 100), and if no spontaneous labor occurred at 41 weeks, induction was performed in the same way. Results: Induction (Group A) vs. waiting spontaneous labor (Group B) showed the followings, which were significant: CS: 22% vs 39%, p = 0.009;maternal birth injury: 4% vs 12%, p = 0.037;non-assisted vaginal deliveries: 93.6% vs 78.7%, p = 0.034;APGAR scores at 1 & 5 min: 7.6 ± 0.8 vs 7.3 ± 1.1 p = 0.038, 8.4 ± 1.0 vs 8.1 ± 1.3 p = 0.040, respectively;birth weight;3.3 ± 0.1 vs 3.5 ± 0.2 kg, p m hemorrhage: 3% vs 5%, blood transfusion: 1% vs 3%. Conclusion: CS rate was significantly lower in women with induction of labor at 39 weeks than those waiting for spontaneous labor onset till 41 weeks in obese Egyptian pregnant women.

Idiopathic intracranial hypertension (IIH) is a disorder characterized by increased intracranial pressure without any identifiable etiology with normal brain imaging and normal cerebrospinal fluid (CSF) content. CSF diversion procedures are commonly used for treatment if medical treatment failed. The aim of this study is to report our experience in treatment of IIH with lumboperitoneal (LP) and stereotactic guided ventriculoperitoneal (VP) shunts. The clinical data of 43 consecutive patients with IIH refractory to medical treatment and underwent CSF diversion procedures between 2009 and 2014 were analyzed. 29 patients underwent LP shunts and the remaining 14 patients underwent stereotactic guided Ventricular shunts. All patients underwent clinical, imaging and CSF manometry evaluation. 38 (88.4%) patients were female and the remaining 5 (11.6%) patients were male. The mean age was 27.2 years. The opening pressure was above 300 mm H2O in 26 (69.8%) patients. Headache (100%) and blurring and/or diminution of vision (81.4%) were the commonest clinical presentation. 36 (83.7%) patients reported recovery of their headache and 30 (69.7%) patients showed complete resolution of papilledema. The clinical outcome between both procedures was not significant. The incidence of perioperative complications (20.7% vs. 0%) and shunt revisions (27.6% vs. 7.1%) were higher in patients with LP shunt than patients with stereotactic Ventricular shunts. The results of this study demonstrate that both LP and Ventricular shunts are valid diversion procedures for treatment of IIH. Stereotactic guided Ventricular shunt has lower incidence of complications and revisions and seems to be safe, effective and feasible alternative procedure for treatment of IIH.

Background: Delayed pregnancy in women and marked increase in the numbers of older women who fail to respond to ovarian stimulation had been a significant issue. This study aims to assess the value of basal serum testosterone level as a predictor of ovarian response for induction of ovulation in women with unexplained infertility undergoing IVF (in vitro fertilization) cycle. Patients and methods: A prospective study was conducted in Ain Shams University maternity hospital Infertility Center during a period of time from October 2016 to June 2017. This study recruited 89 women. On day 2 or 3 of a spontaneous menstrual cycle of the included women within 3 months before fresh IVF cycle, basal hormonal (FSH, LH, estradiol, total testosterone) concentrations, AFC (antral follicle count) were performed. Using the Long-protocol for induction of ovulation, serial monitoring of ovarian response was assessed by transvaginal ultrasound. When the expected ovarian response was reached (at least three oocytes ≥ 17 mm), we gave trigger dose of HCG. Ultrasound guided oocyte aspiration was performed 34 - 36 hours later. Two to three days after oocyte aspiration, we transferred the embryos according to the patient’s age and the condition of embryos available. Biochemical pregnancy was considered if serum B-hCG test was positive at day 14 from embryo transfer, where all the data were correlated with serum testosterone level and ovarian response as 1 ry outcome. Results: There were significant positive correlations between testosterone and LH, Prolactin, AFC, Number of oocytes & Number of Embryos (0.014, 0.032, 0.023, 0.004, 0.033, p 0.001 respectively). Poor responders versus good responders as regards testosterone level (0.81 ± 0.47 versus 1.08 ± 0.45) Fertilized & pregnant cases had significantly higher testosterone than non-fertilized & non pregnant had (1.20 ± 0.45, 0.92 ± 0.47 p value 0.035, 0.021 respectively). The common causes of stridor in adults are abscesses or swelling of the upper airway, tumors, paralysis, or malfunction of vocal cords. Tracheitis due to immunoglobulin deficiency may be a rare explanation for stridor in adults, although occasionally reported in children. We report an adult man having stridor secondary to isolated immunoglobulin A deficiency. We did an in-depth review of the literature to seek out no reported cases of stridor thanks to immunoglobulin deficiency in adults. This case underlines the very fact that a standard symptom like stridor rarely occurs thanks to uncommon causes. This case is exclusive because it reports the presence of stridor associated with isolated immunoglobulin A deficiency in an adult patient. The explanation for the stridor was an intraluminal narrowing of the upper part of the extrathoracic trachea.

Background: Cesarean section (CS), one of the most common major operative procedures, performed all over the world. Incisional infiltration with local anesthetics is a simple, cheap and effective mean of providing good analgesia for surgical operations without any major side effects & allowing early patients’ mobilization & postoperative recovery, so the purpose of study is to compare between the effect of wound infiltration with bupivacaine versus pethidine for post cesarean section pain relief. Patients and methods: A randomized controlled trial (RCT) was conducted in Ain Shams University maternity hospital in the period from August 2016 and January 2017. 100 full term pregnant females randomized into two groups: Group A (50 patients) Bupivacaine group: Subcutaneous and subrectal infiltration with 10 ml 0.25% Bupivacaine (2.5 mg/ml) diluted in 10 ml normal saline before closure of the wound was done;Group B (50 patients) Pethidine group: Subcutaneous and subrectal infiltration with 1 ml pethidine (50 mg/ml) diluted in 19 ml normal saline before closure of the wound was done. All patients had cesarean section under spinal anesthesia. Study outcome measures post-operative pain scores using visual analogue scale, post-operative analgesia requirement time to first rescue analgesia, onset of mobilization, side effect of local anesthetic, wound infection (after one week). It was registered on clinical *** with ID: NCT03652116. Results: Visual analogue scale values differ significantly between pethidine group and that of bupivacaine at rest and on coughing at 4, 8, 12, 24 hours & analgesic consumption (P value st time request analgesia per minute comparing group A to group B (P value 0.001). There is no significant difference between bupivacaine and pethidine regarding time of ambulation, side effects or complications. Conclusion: Infiltration of the wound of cesarean section with pethidine gives effective analgesia for several hours as compared to Bupivacaine.

The Effect of Oral Nifedipine versus Parenteral magnesium Sulfate and Ritodrine for Tocolysis in Patients with Threatened Preterm Labor: A Randomized Controlled Trial

Background: Preterm labor is a serious cause of neonatal morbidity and mortality. This study aims to compare the effects of nifedipine, magnesium sulfate and ritodrine as tocolytic drugs in patients presented with threatened preterm labor. Patients and methods: The current study was randomized controlled trial conducted in Sohag Teaching Hospital between November 2015 and September 2016. Patients were divided into: Group A: 101 patients received intravenous ritodrine infusion;Group B: 101 patients received intravenous magnesium sulfate;Group C: 101 patients received oral nifedipine. Different maternal and neonatal outcomes were assessed. Results: The baseline criteria were homogenous among the study groups with no statistically significant differences. There is no difference between each other group regarding the need for additional tocolysis or the rate of recurrence of labour pains. Nifedipine was associated with the least length of hospital stay. There is no difference between all groups regarding the rate of preterm delivery before full steroid dose (p > 0.05). However, nifedipine group was the least one in the rate of occurrence of preterm delivery within 7 days from initiation of tocolytic therapy. Similarly, nifedipine group was associated with higher gestational age at delivery and significant prolongation of pregnancy than the other groups. Conclusion: Oral nifedipine use was associated with less recurrence of labor pains, less need for additional tocolysis, less duration of hospital stay, and more patient satisfaction in patients with threatened preterm labour.
