Helicobacter pylori eradication lowers serum homocysteine level in patients without gastric atrophy

AIM: To determine whether Helicobacter pylori (H pylori) infection caused hyperhomocysteinemia by altering serum vitamin B_(12), serum folate and erythrocyte folate levels and whether eradication of this organism decreased serum homocysteine level. METHODS: The study involved 73 dyspeptic H pylork positive patients, none of them had gastric mucosal atrophy based on rapid urease test and histology. Out of 73 patients, 41 (56.2%) showed a successful eradication of H pylori 4 wk after the end of treatment. In these 41 patients, fasting serum vitamin B_(12) folate and homocysteine levels, and erythrocyte folate levels before and 4 wk after H pylori eradication therapy were compared. RESULTS: The group with a successful eradication of H pylori had significantly higher serum vitamin B_(12) and erythrocyte folate levels in the post-treatment period compared to those in pre-treatment period (210±97 pg/mL vs 237±94 pg/mL,P＜0.001 and 442±212 ng/mL vs 539±304 ng/mL, P=0.024, respectively), but showed no significant change in serum folate levels (5.6±2.6 ng/mL vs 6.0+2.4 ng/mL, P=0.341). Also, the serum homocysteine levels in this group were significantly lower after therapy (13.1±5.2 μmol/L vs 11.9±6.2 μmol/L, P=0.002). Regression analysis showed that serum homocysteine level was positively correlated with age (P=0.01) and negatively with serum folate level before therapy (P=0.003). CONCLUSION: Eradication of H pylori decreases serum homocysteine even in patients who do not exhibit gastric mucosal atrophy. It appears that the level of homocysteine in serum is related to a complex interaction among serum vitamin B_(12), serum folate and erythrocyte folate levels.

AIM:The eradication rate of Helicobacter pylori (H pylori) shows variation among countries and regimens of treatment. We aimed to study the eradication rates of different regimens in our region and some factors affecting the rate of eradication. METHODS:One hundred and sixty-four H pylori positive patients (68 males,96 females;mean age:48±12 years) with duodenal or gastric ulcer without a smoking history were included in the *** patients were divided into three groups according to the treatment *** 20mg,clarithromycin 500mg,amoxicillin 1g were given twice daily for 1 week (Group Ⅰ) and 2 weeks (Group Ⅱ). Patients in Group Ⅲ received bismuth subsitrate 300mg, tetracyline 500mg and metronidazole 500mg four times daily in addition to Omeprazole 20mg twice *** biopsies each before and after treatment were obtained from antrum and corpus,and histopathologically evaluated. Eradication was assumed to be successful if no H pylorus was detected from four biopsy specimens taken after *** effects of factors like age,sex,H pylori density on antrum and corpus before treatment,the total H pylori density,and the inflammation scores on the rate of H pylori eradication were evaluated. RESULTS:The overall eradication rate was 42%.The rates in groups Ⅱ and Ⅲ were statistically higher than that in group Ⅰ (P<0.05).The rates of eradication were 24.5%, 40.7% and 61.5% in groups Ⅰ,Ⅱ and Ⅲ,*** eradication rate was negatively related to either corpus H pylori density or total H pylori density (P<0.05).The median age was older in the group in which the eradication failed in comparison to that with successful eradication (55 yr vs 39 yr,P<0.001).No correlation between sex and H pylori eradication was found. CONCLUSION:Our rates of eradication were significantly lower when compared to those reported in *** believe that advanced age and high H pylori density are negative predictive factors for the rate of H pylori eradication.

AIM: To investigate and compare frequencies of serum positive cagA in patients from two separate regions of Turkey who were grouped according to the presence of peptic ulcer disease or non-ulcer ***: One hundred and eighty Helicobacter pyloripositive patients with peptic ulcer disease or non-ulcer dyspepsia were included in the study. One hundred and fourteen patients had non-ulcer dyspepsia and 66 had peptic ulcer disease (32 with gastric ulcers and/or erosions and 34with duodenal ulcers). Each patient was tested for serum antibody to H. pylori cagA protein by enzyme ***: The total frequency of serum positive cagA in the study group was 97.2 %. The rates in the patients with peptic ulcers and in those with non-ulcer dyspepsia were 100% and 95.6%, respectively. These results were similar to those reported in Asian studies, but higher than those that have been noted in other studies from Turkey and Western ***: The high rates of serum positive cagA in these patients with peptic ulcer disease and non-ulcer dyspepsia were similar to results reported in Asia. The fact that there was high seroum prevalence regardless of ulcer status suggests that factors other than cagA might be responsible for ulceration or other types of severe pathology in H. pylori-positive individuals.

Thinning effects on biomass and element concentrations of roots in adjacent hornbeam and oak stands in Istanbul,Turkey

Litter decomposition in pure and mixed Quercus and Fagus stands as influenced by arthropods in Belgrad Forest,Turkey

Arthropod assemblages and decomposition rates were compared in the litter of pure and mixed Quercus(Quercus petraea L.)and Fagus(Fagus orientalis L.)*** was sampled on a monthly basis for 1 year and stored in litterbags of different mesh *** experimental objective was to test the effect of mesh size on litter decomposition,decomposition rates of litter,and diversity of the invertebrate fauna between the two types of *** rates were measured by filling three fiber litterbags of different mesh sizes with pure Quercus litter(3 g)left in the pure Quercus stand,and litterbags with Fagus litter(3 g)were left in the pure Fagus *** litter samples were prepared by mixing of equal amounts of each litter in the same litterbag and leaving them in the mixed *** residual mass of litter from the pure Quercus stand was significantly lower in fine-and coarse-mesh bags than in the medium-mesh bags in pure the Fagus *** and nitrogen levels in the pure Quercus litter were significantly different among the mesh sizes at the end of the incubation *** from 27 taxa were collected from pitfall traps every *** relative numbers differed significantly between the pure and mixed-stand litter ***-dwelling Isotomidae(Collembola)and Mesostigmata(Acarina),and soil-dwelling Mesostigmata were the most numerous in the mixed *** is significant that the abundance of macroarthropods contributed to the mass loss of litter in both the medium and coarse mesh sizes in the mixed stand,but did not significantly affect the mass of litter in the pure *** the mixed stand,there was a negative correlation between litter mass loss and total number of microarthropods in all mesh ***-stand litter decomposed more slowly than pure-stand litter.

Spirulina is a popular herbal food that has a preventive effect on *** the study,it aimed to evaluate the effects of different doses of Spirulina platensis(Arthrospira platensis)on the morphological properties of the duodenum of rats,and to assign the effect of Spirulina on the expression of proliferating cell nuclear antigen(PCNA)in the rat duodenum,and thereby to observe the effects on intestinal health.30 male Wistar albino rats were divided into 3 groups for 45 *** first group was received the basal diet;the second group was given 500 mg/kg Spirulina daily by oral gavage;the third group was given 1000 mg/kg Spirulina *** duodenum segments were taken at the end of the trial and processed for histological *** the total mucosa,villus height and villus/cript ratio were found higher in high dose Spirulina,the lower cript depth was detected in same group compared to control and low dose group.A significant increase was observed at high dose compared to the control and low dose group in terms of PCNA expression intensity and proliferation *** suggest that high dose of Spirulina may support the duodenal growth,and thereby intestinal health.

Impacts of low-intensity prescribed fire on microbial and chemical soil properties in a Quercus frainetto forest

Prescribed fire is a common economical and effective forestry practice, and therefore it is important to understand the effects of fire on soil properties for better soil management. We investigated the impacts of low-intensity prescribed fire on the microbial and chemical properties of the top soil in a Hungarian oak(Quercus frainetto Ten.) forest. The research focused on microbial soil parameters(microbial soil respiration(RSM), soil microbial biomass carbon(Cmic) and metabolic quotient(qCO2) and chemical topsoil properties(soil acidity(pH),electrical conductivity(EC), carbon(C), nitrogen(N), C/N ratio and exchangeable cations). Mean annual comparisons show significant differences in four parameters(C/N ratio,soil pH, Cmic and qCO2) while monthly comparisons do not reveal any significant differences. Soil pH increased slightly in the burned plots and had a significantly positive correlation with exchangeable cations Mg, Ca, Mn and *** mean annual C/N ratio was significantly higher in the burned plots(28.5:1) than in the control plots(27.0:1). The mean annual Cmic(0.6 mg g-1) was significantly lower although qCO2(2.5 lg CO2–C mg Cmic h-1) was significantly higher, likely resulting from the microbial response to fire-induced environmental stress. Low-intensity prescribed fire caused very short-lived changes. The annual mean values of C/N ratio, pH, Cmic and qCO2showed significant differences.

Partial virological response to three different nucleotide analogues in naive patients with chronic hepatitis B

BACKGROUND： The definition of partial virological response （PVR） was proposed because of its clinical relevance. PVR relates to subsequent therapeutic failure which results in the modification of the regimen. Whether this rationale can be applied to all nucleotide analogues （NA） is not clear. This study was undertaken to analyze PVR influence on therapeutic outcomes during lamivudine, entecavir or tenofovir mono- therapy in NA-naive patients with chronic hepatitis B in routine clinical practice. METHODS： We retrospectively analyzed 150 NA-naive patients with chronic hepatitis B. These subjects received lamivudine, entecavir or tenofovir monotherapy between February 2001 and July2013. RESULTS： Sixty-nine patients were treated with lamivudine, 35 with entecavir, and 46 with tenofovir. The median therapeutic duration was 19.5 （6-147） months. PVR rates at 24 weeks were similar among three NAs （lamivudine 33.3%, entecavir 35.0%, tenofovir 32.4%, P--0.981）. For all three NAs, patients with a higher baseline viral load or HBeAg-positive status had a higher serum viral positive rate tested by polymerase chain reaction at week 24 and 48. Cumulative probability of virological breakthrough （VBR） for patients treated with lamivudine was 67% at 5 years, and PVR at 24 weeks was the independent risk factor for VBR （HR： 3.09; 95% CI： 1.09-8.74; P=0.034）; also lamivudine treated patients older than 50 years seemed to have a tendency for VBR （HR： 2.80; 95% CI： 0.99-8.18; P=0.052）. A majority of entecavir and tenofovir partial responders achieved and maintained virological response with prolonged monotherapy, except one entecavir treated patient who experienced VBR due to resistance ***： Management strategy for lamivudine treatment should include adaptation of regimen according to PVR as an on-treatment response parameter due to its relation with unacceptably high VBR probability. Similar conclusion should not be directly related to entecavir or tenofovir treatment.
