BACKGROUND: The effects of gangrenous cholecystitis (GC) and consequent surgical interventions on the clinical outcomes and prognosis of patients with severe acute pancreatitis are not clear. The present study was to characterize the clinical outcomes of patients with severe acute pancreatitis complicated with GC. METHODS: We retrospectively analyzed 253 consecutive patients hospitalized for acute pancreatitis in intensive care unit. Among them, 68 were diagnosed as having severe acute pancreatitis; 10 out of the 68 patients had GC. We compared these 10 patients with GC and 58 patients without GC. The indices analyzed included sepsis/septic shock, pancreatic encephalopathy, acute respiratory distress syndrome, acute renal failure, multiple organ dysfunction syndrome, and death. RESULTS: Specific CT images of GC in patients with severe acute pancreatitis included enlarged and high-tensioned gallbladder, wall thickening, lumenal emphysema, discontinuous and/or irregular enhancement of mucosa, and pericholecystic effusion. The rates of severe sepsis/septic shock (70.0% vs 24.1%, Pencephalopathy (50.0% vs 17.2%, Penal failure (40.0% vs 27.6%, Pents with GC than in those without ***: CT scans can help to identify early GC in patients with severe acute pancreatitis; early diagnosis and intervention for patients with GC can reduce morbidity and mortality.

为提高风功率预测精度，减轻输出风能波动性对风电并网不利影响，提出了基于WT-IAGA-BP神经网络的短期风电功率预测方法。利用风速分区、3σ准则及拉格朗日插值法清洗风电场历史数据；其次，依据小波重构误差，选择db4小波分别提取风速、风向、历史风功率的不同频率特征信号，并引入改进自适应遗传算法(IAGA)对各序列BP神经网络的初始权值与阈值寻优，使用Sigmiod函数通过适应度值自适应改变交叉概率与变异概率；构建各序列的WT-IAGA-BP模型对短期风功率组合预测。通过仿真分析，并与ELM、IAGA-BP、WT-ELM及WT-LSSVM方法对比，验证该方法具有更高的预测精度和更好的预测性能。

A simple nanobody-based competitive ELISA to detect antibodies against African swine fever virus

African swine fever virus(ASFV) infection is a big threat to the global pig industry. Because there is no effective vaccine, rapid, low-cost, and simple diagnosis methods are necessary to detect the ASFV infection in pig ***, with advantages of small molecular weight and easy genetic engineering, have been universally used as reagents for developing diagnostic kits. In this study, the recombinant ASFV-p30 was expressed and served as an antigen to immunize the Bactrian camel. Then, seven nanobodies against ASFV-p30 were screened using phage display technique. Subsequently, the seven nanobodies fused horseradish peroxidase(nanobody-HRP) were secretory expressed and one fusion protein ASFV-p30-Nb75-HRP was selected with the highest sensitivity in blocking ELISA. Using the ASFV-p30-Nb75-HRP fusion protein as a probe, a competitive ELISA(cELISA) was developed for detecting anti-ASFV antibodies in pig sera. The cut-off value of cELISA was determined to be 22.7%by testing 360 negative pig sera. The detection limit of the cELISA for positive pig sera was 1:320, and there was no cross-reaction with anti-other swine virus antibodies. The comparative assay showed that the agreement of the cELISA with a commercial ELISA kit was 100%. More importantly, the developed cELISA showed low cost and easy production as a commercial kit candidate. Collectively, a simple nanobody-based cELISA for detecting antibodies against ASFV is developed and it provides a new method for monitoring ASFV infection in the pig herds.

目的分析咪达唑仑与丙泊酚用于老年体外循环(CPB)心脏术后机械通气患者对镇静效果及血气指标的影响。方法将老年CPB心脏术后机械通气患者按队列法分为咪达唑仑组(M组)、丙泊酚组(P组)和咪达唑仑与丙泊酚联合给药组(M-P组)。M组静脉注射0.05~0.10mg·kg^(-1)咪达唑仑进行镇静诱导,再通过微量泵持续静脉泵入0.05~0.15mg·kg^(-1)·h^(-1)咪达唑仑;P组静脉注射0.5~1.0mg·kg^(-l)丙泊酚进行镇静诱导,再通过微量泵持续静脉泵入0.5~2.0mg·kg^(-1)h^(-1)丙泊酚;M-P组首先静脉注射0.02~0.05mg·kg^(-l)咪达唑仑及0.2~0.5mg·kg^(-l)丙泊酚进行镇静诱导,再继续持续静脉泵入0.05~0.1 mg·kg^(-l)咪达唑仑及0.5~0.8mg·kg^(-l)·h^(-l)丙泊酚。比较3组患者的镇静效果、血气指标、血流动力学指标、拔管时间、重症监护病房(ICU)滞留时间及治疗费用,并记录镇静治疗期间的药物不良反应发生情况。结果M组、P组和M-P组分别有43例、44例和39例。M组、P组和M-P组的药物起效时间分别为(77.94±12.05)、(18.18±5.20)和(21.25±9.36)s,达到满意镇静效果时间分别为(42.57±11.41)、(22.63±8.17)和(23.98±10.25)min,停药后苏醒时间分别为(59.30±14.86)、(19.83±5.44)和(22.16±6.29)min,拔管时间分别为(1.61±0.20)、(1.45±0.22)和(1.37±0.15)d,ICU滞留时间分别为(2.17±0.29)、(1.91±0.36)和(1.84±0.25)d,治疗费用分别为(186.59±60.83)、(922.97±164.34)和(375.03±71.16)千元,药物不良反应总发生率分别为34.88%、4.55%和7.69%,在统计学上差异均有统计学意义(均P0.05)。结论咪达唑仑与丙泊酚联合给药用于老年CPB心脏术后机械通气患者具有显著的镇静效果,有利于减少镇静药物用量,且对患者血气指标及血流动力学指标的影响较小,较单一使用咪达唑仑有利于缩短拔管时间和ICU滞留时间,并降低药物不良反应总发生率,而较单一使用丙泊酚有利于减少治疗费用,是一种更加理想的镇静给药模式。

Regenerating gene IV （RegIV）, a member of the regenerating gene family discovered in 2001, has been found to be involved in malignancy in several different organs including the stomach, colorectum, pancreas and prostate, but the overall expression profile of RegIV has not been reported. To learn more about RegIV, we evaluated its distribution by immunohistochemistry （IHC） in a total of 360 samples including 24 types of normal tissue, 40 benign and malignant lesions, and 18 neuroendocrine tumors. We found that in normal tissues, in addition to its relative specificity for the gastrointestinal tract, RegIV was detected in the adrenal gland and mammary gland. Among all the malignancies of various histological types under evaluation, RegIV was found mostly in adenocarcinomas. Studies on additional sets of colorectal tumor samples showed that RegIV expression was predominant in colorectal adenoma （87.5%） and peritumoral tissue （100%） but not in cancer tissue （30.8%）. Among neuroendocrine tumors, ReglV had a relatively restricted expression to those of digestive system.
