Protective effect of low dose of melatonin against cholestatic oxidative stress after common bile duct ligation in rats

AIM: to investigate the role of oxidative injury and the effect of exogenous melatonin administration on liver damage induced by bile duct ligation (BDL), and second, to evaluate the role of nitric oxide (NO), a free oxygen radical, in oxidative injury. MEtHODS: thirty-two Sprague-Dawley rats were assigned to four groups: sham operation (SO), BDL, BDL+melatonin, and BDL+vehicle. Cholestasis was achieved by double ligature of the common bile duct. Melatonin was injected intraperitoneally 500 μg/(kg·d) for 8 d. Hepatic oxidative stress markers were evaluated by changes in the amount of lipid peroxides, measured as malondialdehyde (MDA), and reduced GSH. total nitrite (NOx) concentrations were determined in hepatic homogenates. Histopathological examination was performed using a histological scoring system. RESULtS: the histopathological changes including portal inflammation, necrosis,apoptosis, focal inflammation and fibrosis were severe in the BDL and BDL+vehicle groups. there were numerous large areas of coagulation necrosis. Histological Activity Index scores of these groups were significantly higher than that of the SO group. treatment with melatonin reduced these alterations significantly. the degree of necro-inflammation and fibrosis showed significant difference between the BDL and BDL+melatonin groups. BDL was accompanied by a significant increase in MDA and NOx, and a significant decrease in GSH levels. Mean±SE values of MDA, GSH and NOx levels of SO group were 147.47±6.69, 0.88±0.33 μmol/g and 180.70±6.58 nm/g, respectively. the values of BDL group were 200.14±21.30, 0.65±0.02 μmol/g, and 400.46±48.89 nm/g, respectively, whereas the values of BDL+melatonin group were 115.93±6.8,0.74±0.02 μmol/g, and 290.38±32.32 nm/g, respectively. Melatonin treatment was associated with a significant recovery of MDA, GSH and NOx levels. Ammonia utilisation in internal combustion engines has attracted wide interest due to the current trend toward decarbonisation,as ammonia is a zero-carbon fuel with different combustion properties to *** laminar burning velocity(LBV)is a fundamental property of fuels with a significant effect on the combustion processes and accurate calculations and measurements of the LBV over a wide range of fuel blends,pressures and flow conditions is a time-consuming,complicated *** main goal of the current study is to predict the LBV of NH_(3)/H_(2)/air mixtures using a hybrid machine learning(ML)approach based on a training dataset consisting of both the experimental LBV values and additional data obtained from numerical simulations with a detailed kinetic *** ML model training data is collected from existing experimental LBV in the literature for NH_(3)/H_(2)/air ***,synthetic data is generated using one-dimensional(1D)simulations to reduce data inhomogeneity and increase accuracy of the ML *** total,24 different ML algorithms are tested to find the best model both for the experimental and the hybrid *** results suggest that both Gaussian Process Regression(GPR)and Neural Networks(NNs)can be utilised to predict LBV of NH_(3)/H_(2)/air mixtures with reasonable *** hybrid ML model achieved a coefficient of determination of R^(2)=***,hybrid ML model hyperparameters are optimised to achieve a coefficient of determination of R^(2)=*** was also found that ML can speed up LBV computation from 9500 to 27000 times compared to 1D simulations with a reduced mechanism.

AIM:to compare the effects of yellow(577 nm)subthreshold micropulse laser(SML)and intravitreal(IV)anti-vascular endothelial growth factor(VEGF)treatment in patients with diabetic macular edema(DME)with relatively better visual acuity[best corrected visual acuity(BCVA)≤0.15 log MAR].MEtHODS:the medical records of 76 eyes of 47 patients underwent IV(0.5 mg)anti-VEGF injection or SML for the DME with relatively better BCVA were *** IV group received three consecutive monthly IV anti-VEGF injections,then were retreated as *** laser treatment group was treated at baseline and 3 mo,and then retreated at 6 and 9 mo if *** participants were followed up for one *** mean BCVA and mean central macular thickness(CMt)values changes over the follow-up were ***:twenty-four and 23 patients were assigned to the SML and IV subgroups,*** mean number of treatments was 3.64±0.76 in SML group and 5.85±1.38 in IV group(Pthe subgroups were similar with regard to the mean BCVA score at baseline and at the 1st and 3rd months,but the score of SML group was better than that of IV group at the 6th,9th,and 12th months(Pthe decrease in the mean CMt values from baseline values was higher in SML group at the 6th,9th,and 12th months(Ptreatment is superior to IV anti-VEGF injection in DME patients with relatively better BCVA for increasing visual acuity and decreasing CMt at 6,9,and 12 *** can be a good alternative first-line therapy for DME with BCVA≤0.15 log MAR.

Association of rare SPINK1 gene mutation with another base substitution in chronic pancreatitis patients

AIM: to verify and expand the known spectrum of serine protease inhibitor Kazal type 1 (SPINK1) gene mutations in chronic pancreatitis. MEtHODS: DNA extracted from 172 chronic pancreatitis patients was assayed for SPINK1 gene mutations by PCR and DNA sequencing. A control cohort of 90 unrelated healthy individuals was analysed by the same methods for presence of common populational polymorphisms, and frequency of five-loci haplotypes was calculated. Linkages of gene aberrations in single SPINK1 gene copies were analysed by long-distance PCR followed by allele-specifi c PCR and DNA sequencing. RESULtS: the most frequent SPINK1 gene mutation N34S was found at a frequency of 6%. Furthermore, we detected the heterozygous intervening sequence (IVS) 3 + 2 t > C mutated gene in 2 German patients and 1 Macedonian chronic pancreatitis patient. In all three SPINK1 gene copies an additional rare base substitution was found: 5’untranslated region (UtR)-215 G > A. Poly-morphism analysis revealed that all three affected genes carried the same fi ve-loci haplotype. DNA sequencing of another chronic pancreatitis-related gene PRSS1 (cationic trypsinogen) did not reveal any mutations in these 3 ***: We found in 3 (2%) of 172 chronic pancreatitis patients an IVS3 + 2 t > C SPINK1 gene mutation and a base substitution 5’UtR-215 G > A inthe same gene copy. Most probably the 5’UtR-215 G >A represents a rare polymorphism and not a mutationas previously concluded. Haplotype analysis suggests acommon origin of the IVS3 + 2 t > C mutation in thesepatients.

AIM： to determine the expression of L1 in pancreatic neuroendocrine tumor and to correlate it with WHO classification of this tumor. MEtHODS： We retrospectively analyzed L1 expression in 63 cases of pancreatic neuroendocrine tumor by immunohistochemistry on paraffin sections of primary tumors or metastases. Staining was performed by peroxidase technique with monoclonal antibody U3127.11 against human L1. All tumors were classified according to WHO classification as well-differentiated neuroendo- crine tumors and carcinomas or poorly-differentiated neuroendocrine carcinomas. RESULtS： LI was detected in 5 （7.9%） of 63 pancreatic neuroendocrine tumors. Four （44.4%） of 9 poorlydifferentiated carcinomas expressed L1. In contrast, only 1 （1.9%） of 54 well-differentiated tumors or carcinomas was positive for LI. No expression was found in Langerhans islet cells of normal pancreatic tissue. Cross table analysis showed a significant association between L1 expression and classification of neuroendocrine tumors of the pancreas （P〈0.01）. CONCLUSION： L1 is specifically expressed in poorlydifferentiated pancreatic neuroendocrine carcinomas that are known to have the worst prognosis. L1 might be a marker for risk prediction of patients diagnosed with pancreatic neuroendocrine carcinomas.

the crystal structure, magnetic and magnetocaloric characteristics of the pseduo ternary compounds of tbsGe2-xSi2-xMn2x （0 ≤ 2x ≤ 0.1） were investigated by x-ray powder diffraction and magnetization measurements. the x-ray powder diffraction results show that all compounds preserve the monoclinic phase as the majority phase and all the synthesized compounds were observed to be ferromagnetic from magnetization measurements. Magnetic phase transitions were interpreted in terms of Landau theory. Maximum isothermal magnetic entropy change value （20.84 J-kg-1 -K-1） was found for tb5Ge1.95Si1.95Mn0.1 at around 123 K in the magnetic field change of 5t.

BACKGROUND total talar dislocation (ttD) is very uncommon for many orthopedic surgeons and emergency/trauma specialists. Scarce cases of ttD have been reported, mainly in the form of open fracture-dislocation injury. CASE SUMMARY We report a very rare injury of closed ttD with a follow-up period of 36 mo. Initial closed reduction was not successful because of a fractured highly unstable medial malleolus displaced into the ankle mortise, blocking the relocation of the talus. the patient was able to walk pain-free after the 3rd month of surgery. At the 36-mo follow-up, there were 10 degrees of flexion loss and 10 degrees of extension loss in the tibiotalar joint. Furthermore, 5 degrees of subtalar joint inversion-eversion loss was present. CONCLUSION Open reduction should be performed for closed ttDs unless closed reduction is successful.
