Dear editor, Out-of-hospital cardiac arrest survival rates vary between 7% and 46% and are lower than those for inhospital cardiac arrests (IHCA).[1,2] Therefore, efforts are being made to increase survival rates for out-ofhospital cardiac arrests (OHCA). According to advanced cardiac life support (ACLS) guidelines, out-of-hospital cardiac arrest survival rates may be increased by performing cardiopulmonary resuscitation (CPR) with minimal interruptions.[3] According to the latest ACLS, does the patient need an advanced airway.

BACKGROUND Liver tissue situated outside the liver with a hepatic connection is usually calledan accessory liver, and that without a connection to the mother liver, is calledectopic liver *** To identify studies in the literature on ectopic liver tissue located on thegallbladder surface or *** We present two patients and review published articles on ectopic liver tissuelocated on the gallbladder surface accessed via PubMed, MEDLINE, GoogleScholar, and Google databases. Keywords used included accessory liver lobe,aberrant liver tissue, ectopic liver tissue, ectopic liver nodule, heterotopic livertissue, hepatic choristoma, heterotopic liver tissue on the gallbladder, and ectopicliver tissue on the gallbladder. The search included articles published before June2020 with no language restriction. Letters to the editor, case reports, reviewarticles, original articles, and meeting presentations were included in the *** or abstracts containing adequate information on age, sex, history of liverdisease, preliminary diagnosis, radiologic tools, lesion size, surgical indication,surgical procedure, and histopathological features of ectopic liver tissue wereincluded in the *** A total of 72 articles involving 91 cases of ectopic liver tissue located on the gallbladder surface or mesentery were analyzed. Of these 91 patients, 62 werefemale and 25 were male (no gender available for 4 patients), and the age rangewas 5 d to 91 years. Forty-nine patients underwent surgery for chroniccholecystitis or cholelithiasis, and 14 patients underwent surgery for acutecholecystitis. Background： In the early stages of atrial remodeling, aortic stiffness might be an indication of an atrial myopathy, in particular, atrial fibrosis. This study aimed to investigate the association between left atrial （LA） mechanical function, assessed by two-dimensional speckle tracking echocardiography, and aortic stiffness in middle-aged patients with tile first episode of nonvalvular atrial fibrillation （AF）. Methods： This prospective study included 34 consecutive patients with the first episode of AF, who were admitted to Kartal Kos.;uyolu Research and Training Hospital between May 2013 and October 2015, and 31 age- and gender-matched healthy controls. During the 1^st month （mostly in the first 2 weeks） following their first admission, 34 patients underwent the first pulse wave measurements. Then, 21 patients were recalled for their second pulse wave measurement at 11.8 ± 6.0 months following their initial admission. Echocardiographic and pulse wave findings were compared between these 34 patients and 31 healthy controls. We also compared the pulse wave and echocardiographic findings between the first and second measurements in 21 patients. Results： Pulse wave analysis showed no significant differences between tile AF patients and healthy controls with respect to PWV （ 10.2 ± 2.5 m/s vs. 9.7 ± 2.1 m/s： P = 0.370）, augmentation pressure （9.6 ± 7.4 mmHg vs. 9. 1± 5.7 mmHg; P = 0.740）, and aortic pulse pressure （AoPP; 40.4 ± 14.0 mmHg vs. 42. 1 ±7.6 mmHg, P = 0.550）. The first LA positive peak of strain was inversely related to the augmentation pressure （r 0.30; P = 0.02） and aortic systolic pressure （r - 0.26, P - 0.04）. Comparison between the two consecutive pulse wave measurements in 21 patients showed similar results, except for AoPP. In 21 patients, the AoPP at the second measurement （45.1± 14.1 mmHg） showed a significant increase compared with AoPP at the first measurement （39.0 ±10.6 mmHg, P = 0.028）, which was also higher than that of healthy controls （42.1

Integration of Digital Technologies into Underground Utility Asset Management

Process monitoring, early fault detection and diagnosis, is important in chemical and manufacturing processes for safety, controlling product quality and minimizing waste The multivariate statistical methods, which systematically reduce the number of related variables and transform the system to a simpler form, are highly developed within the field of chemometrics, but they deserve to be explored and applied to process data in other areas. In this study, Principle Component Analysis by NIPALS algorithm and Partial Least Square by NIPALS and Kernel algorithms are coded in MATLAB 6.0 with a graphical user interface for off-line process monitoring and fault diagnosis in continuous and batch processes. The concept of 'high leverage point plot' was introduced to identify out-of-control variables in continuous processes, and 'dissimilarity index', which is easier to implement than the classical score plots, was introduced to differentiate the 'bad' batches from the good ones in batch operations. Data from the literature (simulated PVC plant data) as well as from real industrial plants (textile glue production plant and bakers' yeast plant) were used to assess the effectiveness of the illustrated methods. Comparative studies revealed that score plots detected out-of-control observations more accurately than the other control plots like SPE, Hotelling T2 and high leverage point plots. Hotelling T2 and SPE graphs predicted almost the same out-of-control observations with the leverage graphs. The results of dissimilarity index were similar to those of score plot for the identification of 'good' or 'bad' batches. This work, thus, provides a thorough analysis of multivariate statistical methods for process monitoring and fault diagnosis in chemical processing industries, and highlights the relative merits of individual techniques.

Installation, detection, maintenance, mapping, and management of underground utility assets present challenges to owners, engineers and contractors. Industry-wide practices include the use of geophysical and similar technologies to determine depth and location, and 2D as-built plans integrated with GIS databases for information management. The feasibility of incorporating 3D BIM models of the subsurface to replace the 2D plans to improve visualization and data management is examined in this paper. Obtaining an accurate image of the underground infrastructure would help minimize excavation accidents due to equipment-utility collisions and prevent property damage. Further, the inclusion of automated data collection and sharing features realized through BIM technology can enhance operations of smart cities. The research methodology consists of a state-of-the-art review of the current underground utility management systems, combined with statistical analysis of survey responses received from utility providers and one-call centers in the U.S. Three categories of utility practices are identified based on the level of digital technology integration. It is found that a vast majority of utility firms have adopted GIS databases with 2D plans, depth and other asset information, while a smaller percentage of providers have achieved full GIS-BIM integration, incorporating a wide range of asset data. Future progress on broader implementation appears to be constrained by the digital literacy of personnel and high costs of technology acquisition and application. A three-step framework for converting 2D plans to 3D BIM models is also presented and discussed. The process model proposed for this purpose allows the utilization of commercially available software with minimal need for additional coding.

Comparison of the clinical efficacy of two different types of post systems which were restored with composite restorations

AIM To compare the efficacy of resin composite restorations, retained with either polyethylene or zirconia-rich glass fiber posts. METHODS Sixty-two single rooted maxillary and mandibularcentral incisor teeth in forty-four patients (15 males and 29 females; age range 15-32 years) were restored either with an ultrahigh molecular weight polyethylene(UHMWP) fiber post (Bondable Reinforcement Ribbon, DENSE, Ribbond, Seattle, WA, United States) or a zircon-rich glass fiber post (Snowpost, Lot H 040; Carbotech, Ganges, France). Then, direct resin composite restoration (Clearfil AP-X, Kuraray) was performed for both post systems in tooth color suitable. Patients were recalled for routine inspections at 6 mo, 1, 2 and 3 *** The restorations were assessed during each recall evaluation according to predetermined clinical and radiographic criteria (periapical lesion; marginal leakage and integrity; color stability; surface stain and loss of retention of the post or the composite buildup material). The follow-up data showed no significant difference in these criteria between polyethylene fibre posts and zirconia-rich glass fibre *** The efficacy of resin composite restorations, retained with either polyethylene or zirconia-rich glass fiber posts were similar, suggesting that both types of fiber post can be used successfully to help retain resin composite restorations.

We examined the lipid values of obese and normal-weight children, to look if there is an association between Body Mass Index (BMI) and lipid profiles. Our study group included 100 volunteers (50 children with normal BMI and 50 with high BMI) who were admitted to Kagithane State Hospital Pediatrics Clinic for various reasons between July 2010 and May 2011. The inclusion criteria were as follows: age between 2 and 14 years, no chronic disease and no developmental defects. The high BMI group had significantly higher Low Density Lipoprotein (LDL) levels in comparison to the normal BMI group (p?< 0.05). We also found that the high BMI group had higher cholesterol levels close to upper limit than the normal BMI group. We found high triglyceride levels not only in the high BMI group but also in the normal BMI group with a rate of 26% and 24% respectively. The difference of 2% was not significant. We advise that identification and treatment of elevated LDL cholesterol levels is of tremendous significance for obese children or adolescents for future cardiovascular disease risk in adulthood. In the light of these findings, we must not overlook cardiovascular risk in the normal BMI group children. We believe that necessary precautions must be taken for preventing overweight and dyslipidemia in early childhood. Accordingly, it is of paramount importance not only to reduce obesity in children and adolescents but also to monitor dyslipidemia in normal BMI children to avoid the subsequent risk for cardiovascular disease.

Objective: To determine the antileishmanial vaccine effectiveness of lipophosphoglycan(LPG) and polyacrylic acids(PAA) conjugates on in vivo mice ***: LPG molecule was isolated and purified from large-scale Leishmania donovani parasite culture. Protection efficacies of LPG alone, in combination with Freund's adjuvant, in a physical mixture and in conjugate(consisting of various LPG concentrations) with PAA, were comparatively determined by various techniques, such as cultivation with the micro-culture method, assessment of in vitro infection rates of peritoneal macrophages, determination of parasite load in liver with Leishman-Donovan Units, and detection of cytokine ***: Obtained results demonstrated that the highest vaccine-mediated immune protection was provided by LPG-PAA conjugate due to all parameters investigated. According to the Leishman-Donovan Units results, the sharpest decline in parasite load was seen with a ratio of 81.17% when 35 mg LPG containing conjugate was applied. This value was 44.93% for the control group immunized only with LPG. Moreover, decreases in parasite load were 53.37%, 55.2% and 65.8% for the groups immunized with 10 mg LPG containing LPG-PAA conjugate, a physical mixture of the LPG–PAA, and a mixture of LPG + Freund's adjuvant, respectively. Furthermore, cytokine results supported that Th1 mediated protection occurred when mice were immunized with LPG-PAA ***: It has been demonstrated in this study that conjugate of LPG and PAA has an antileishmanial vaccine effect against visceral leishmaniasis. In this respect, the present study may lead to new vaccine approaches based on high immunogenic LPG molecule and adjuvant polymers in fighting against Leishmania infection.

This study reports a 69-year-old, obese, female patientpresenting with a biliary leakage after laparoscopiccholecystectomy for cholelithiasis. Closure of the um-bilical trocar site had been neglected during the lapa-roscopic cholecystectomy. Early, on postoperative dayfive, endoscopic retrograde cholangiopancreatography(ERCP) requirement after laparoscopic cholecystectomyresolved the biliary leakage problem but resulted with amore complicated clinical picture with an intestinal ob-struction and severe abdominal pain. Computed tomog-raphy revealed a strangulated hernia from the umbilicaltrocar site. Increased abdominal pressure during ERCPhad strained the weak umbilical trocar site. Emergencysurgical intervention through the umbilicus revealed anischemic small bowel segment which was treated withresection and anastomosis. This report demonstratesthat negligence of trocar site closure can result in veryearly herniation, particularly if an endoscopic interven-tion is required in the early postoperative period.
