Clinical characteristics of intermediate uveitis in adult Turkish patients

·AIM: To describe the clinical characteristics of Turkish patients with intermediate uveitis(IU) and to investigate the effect of clinical findings and complications on final visual acuity(VA).·METHODS: We retrospectively analyzed the medical records of patients with IU who had at least 6mo of follow-up and were older than 16 y.· RESULTS: A total of 78 eyes of 45 patients were included in the study and the mean follow-up period was19.4mo. The mean age at the time of presentation was42.9s. Systemic disease associations were found in17.7% of cases; sarcoidosis(8.8%) and multiple sclerosis(6.6%) were the most common diseases. Recurrence rate(odds ratio=45.53; 95%CI: 2.181-950.58), vitritis equals to or more than 3+ cells(odds ratio =57.456; 95%CI: 4.154-794.79) and presenting with VA less than 20/40(odds ratio =43.81; 95% CI: 2.184-878.71) were also found as high risk factors for poor final VA. At the last follow-up examination, 67.9% of eyes had VA of 20/40 or better.·CONCLUSION: IU is frequently seen at the beginning of the fourth decade of life. The disease is most commonly idiopathic in adult Turkish patients. Patients with severe vitritis at presentation and patients with frequent recurrences are at high risk for poor visual outcome.

·AIM: To determine the prevalence of ophthalmopathy in Hashimoto’s patients and to make a comparison in subgroups of patients·METHODS: The study involved 110 Hashimoto’s thyroiditis patients and 50 control subjects attending to the endocrinology department of the hospital. Subgroup classification of patients was made as euthyroid,subclinic and clinic in Hashimato’s thyroiditis. All patients were evaluated by a single experienced ophthalmologist for the prevalence and characteristics of eye signs.·RESULTS:Theoverallprevalencesofeyechanges were22.7%(25 patients) in patients and 4%(2 persons) in control subjects respectively(P =0.002). In patients the most common symptom was retrobulbar eye pain with or without any eye movement. Thirteen patients had significant upper eyelid retraction(11.8%). Six patients had eye muscle dysfunction as reduced eye movements in up gaze. In control patients one person had proptosis and another had lid retraction. The clinical activity score and classification of the ophthalmopathy did not show any significant differences among subgroups.·CONCLUSION:Theeyesignsweremostlymild(22.7%)and the most common eye sign was the presence of upper eyelid retraction(11.8%). Additionally six patients had eye muscle dysfunction as reduced eye movements in up gaze. Therefore we recommend to make a routine ophthalmic examination in Hashimoto’s thyroiditis patients in order not to omit the associated ophthalmopathy.

The aim is to present a case operated due to one-month ongoing intussusception and diagnosed as lymphoma. A six years old boy was admitted to our emergency department with complaint of colicky abdominal pain and bilious vomiting for the last month. He was operated due to 3 cm intussusception at ileocecal part. A mass which is about 6 cm in diameter, hard, partly mobile and invaginate the intestine at its posterior aspect was found. Pathology was reported as Burkitt lymphoma. He received two cycles of chemotherapy at post-operative period. He is still event free for the last two years. One must remember that lymphoma can be the lead point in intussusception especially in elderly children.

In this study, we propose an incremental learning approach based on a machine-machine interaction via relative attribute feedbacks that exploit comparative relationships among top level image categories. One machine acts as ‘Student (S)’ with initially limited information and it endeavors to capture the task domain gradually by questioning its mentor on a pool of unlabeled data. The other machine is ‘Teacher (T)’ with the implicit knowledge for helping S on learning the class models. T initiates relative attributes as a communication channel by randomly sorting the classes on attribute space in an unsupervised manner. S starts modeling the categories in this intermediate level by using only a limited number of labeled data. Thereafter, it first selects an entropy-based sample from the pool of unlabeled data and triggers the conversation by propagating the selected image with its belief class in a query. Since T already knows the ground truth labels, it not only decides whether the belief is true or false, but it also provides an attribute-based feedback to S in each case without revealing the true label of the query sample if the belief is false. So the number of training data is increased virtually by dropping the falsely predicted sample back into the unlabeled pool. Next, S updates the attribute space which, in fact, has an impact on T's future responses, and then the category models are updated concurrently for the next run. We experience the weakly supervised algorithm on the real world datasets of faces and natural scenes in comparison with direct attribute prediction and semi-supervised learning approaches, and a noteworthy performance increase is achieved.

Objective: To prepare and characterize poly lactic-co-glycolic acid(PLGA) nanoparticles loaded with soluble leishmanial antigen or autoclaved leishmanial antigen and explore in vitro and in vivo immunogenicity of antigen encapsulated nanoparticles. Methods: Water/oil/water double emulsion technique was employed to synthesize PLGA nanoparticles, and scanning electron microscopy, Fourier transform infrared spectroscopy and Zeta-potential measurements were used to identify the characteristics of nanoparticles. Cytotoxicity of synthetized nanoparticles on J774 macrophage were investigated by MTT assays. To determine the in vitro immunostimulatory efficacies of nanoparticles, griess reaction and ELISA was used to measure the amounts of NO and cytokines. During the in vivo analysis, Balb/c mice were immunized with vaccine formulations, and protective properties of nanoparticles were measured by Leishman Donovan unit in the liver following the infection. Cytokine levels in spleens of mice were determined by ELISA. Results: MTT assay showed that neither soluble leishmanial antigen nor autoclaved leishmanial antigen encapsulated nanoparticles showed cytotoxicity against J774 macrophage cells. Contrary to free antigens, both autoclaved leishmanial antigen-nanoparticle and soluble leishmanial antigen-nanoparticle formulations led to a 10 and 16-fold increase in NO amounts by macrophages, respectively. Leishman Donovan unit calculations revealed that soluble leishmanial antigen-nanoparticles and autoclaved leishmanial antigen-nanoparticles yielded 52% and 64% protection against visceral leishmaniasis in mouse models. Besides, in vitro and in vivo tests demonstrated that by increasing IFN-γ and IL-12 levels and inhibiting IL-4 and IL-10 secretions, autoclaved leishmanial antigen-nanoparticles and soluble leishmanial antigennanoparticles triggered Th1 immune response. Voluntary soil protection measures are not sufficient to achieve sustainable soil management at a global ***,binding soil protection legislation at national and international levels has also proved to be insufficient for the effective protection of this almost non-renewable natural *** European Soil Partnership(ESP)and its sub-regional partnerships(Eurasian Sub-Regional Soil Partnership,Alpine Soil Partnership)were established in the context of FAO's Global Soil Partnership(GSP)with the mission to facilitate and contribute to the exchange of knowledge and technologies related to soils,to develop dialogue and to raise awareness for the need to establish a binding global agreement for sustainable soil management The ESP has taken a role of an umbrella network covering countries in Europe and Central *** aims to improve the dialogue in the whole region and has encouraged establishing goals that would promote sustainable soil management taking into account various national and local approaches and priorities,as well as cultural *** ESP first regional implementation plan for the 2017—2020 period was adopted and implemented along the five GSP pillars of *** on the experience of the last four years,this study demonstrates that establishing sub-regional and national partnerships is an additional step in a concrete sustainable soil management implementation *** also suggests that a complementary approach between legal instruments and voluntary initiatives linked to the development of efficient communication and strong commitment is the key to success.

Effect of refractive error on temperament and character properties

AIM: To determine the effect of refractive error on temperament and character properties using Cloninger’s psychobiological model of ***: Using the Temperament and Character Inventory(TCI), the temperament and character profiles of 41 participants with refractive errors(17 with myopia,12 with hyperopia, and 12 with myopic astigmatism) were compared to those of 30 healthy control ***, temperament comprised the traits of novelty seeking, harm-avoidance, and reward dependence, while character comprised traits of self-directedness,cooperativeness, and ***: Participants with refractive error showed significantly lower scores on purposefulness,cooperativeness, empathy, helpfulness, and compassion(P <0.05, P <0.01, P <0.05, P <0.05, and P <0.01,respectively).CONCLUSION: Refractive error might have a negative influence on some character traits, and different types of refractive error might have different temperament and character properties. These personality traits may be implicated in the onset and/or perpetuation of refractive errors and may be a productive focus for psychotherapy.
