Spatial distribution of small molecules, such as neurotransmitters, alongside lipids, in brain improve our understanding of their biological functions. Mass spectrometry imaging(MSI), such as matrix assisted laser desorption ionization(MALDI) and desorption electrospray ionization(DESI), can be used to map distributions of metabolites on tissue sections. DESI allows for the direct analysis of sample surfaces at atmospheric pressure without tedious sample preparation and matrix deposition. DESI imaging uses a charged jet of solvent to deposit microdroplets onto a surface where analytes are extracted and desorbed into the gas phase at ambient pressure and temperature. Subsequently, they are drawn into the MS inlet where they can be analyzed using a TOF-MS. This technique is compatible with Waters Synapt G2-Si or Xevo G2-XS Mass Spectrometers. Here we show the utility of DESI imaging to detect neurotransmitters, such as serotonin, adenosine, and glutamine directly in brain tissue samples. Coronal tissue sections if rat brain were mounted on microscope slides and imaged without any further sample preparation. DESI imaging platform was coupled with a high definition mass spectrometer(HDMS) with ion mobility separation(Synapt G2-Si), and MS data were collected both positive mode and negative mode. Image creation was performed using High Definition Imaging(HDI) software(Waters Corporation). Interested molecular identification was done using high mass accuracy database searches against curated databases, such as METLIN, also ID confidence was increased by high-fidelity isotopic distribution, collision cross section(CCS). DESI imaging results showed the basic distribution of brain metabolites, such as amino acids(including taurine, aspartic acid, glutamine, and glutamine), neurotransmitters(including GABA, serotonin). Standard parenteral nutrition solutions are mixtures comprising interacting components that may degrade themselves over time. The objective of this study was to investigate the physicochemical and microbiological stability of a hospital preparation for parenteral nutrition in neonatology. The analyses were performed throughout the storage of the preparations at 2–8 °C(up to 4 months). The extent of stability was based on the determination of amino acids dosage, visual and physicochemical properties(glucose and electrolytes concentrations, pH and osmolality measurements, particle counting) and microbiological analysis(sterility test). A thermal degradation of ascorbic acid was conducted to evaluate the antioxidant properties of the parenteral mixture. Physicochemical and microbiological controls were found to comply with the specifications. Amino acids showed a good stability throughout the 4 months storage except for cysteine, which was progressively degraded to cystine, conferring a yellow coloration to parenteral solutions. Parenteral nutrition standards solutions remain stable for 4 months at 2–8 °C,ensuring safe administration in preterm infants.

T-lymphoblastic lymphoma with cutaneous involvement

To study dermatological manifestation of T-lymphoblastic lymphoma and to help clinicians in the diagnosis, we report here the case of a 75-year-old patient who presented with violaceous nodules acquired during the last 4 wk and affecting the scalp and right arm. The diagnosis of systemic lymphoma was suggested upon the appearance of cutaneous tumors, palpable lymph nodes and general symptoms including asthenia and weight-loss. The pathology features: positive immunostaining for CD3 and terminal deoxynucleotidyl transferase(Td T) and staging, led us to the final diagnosis of T-lymphoblastic lymphoma(T-LBL) with cutaneous involvement. He received a CHOP regimen as first-line treatment. Unfortunately, the patient relapsed and died 8 mo after the treatment initiation. T-LBL may be diagnosed by skin lesions. Additional immunostaining including Td T and experienced histopathologists are needed to correctly classify this aggressive disease and discuss the correct management including bone-marrow transplantation where appropriate.

BACKGROUND Primary liver cancer is common in West Africa due to endemic risk ***,epidemiological studies of the global burden and trends of liver cancer are *** report changes in trends of the incidence of liver cancer over a period of 28 years using the population-based cancer registry of Bamako,*** assess the trends and patterns of liver cancer by gender and age groups by analyzing the cancer registration data accumulated over 28 years(1987-2015)of activity of the population-based registry of the Bamako *** Data obtained since the inception of the registry in 1987 through 2015 were stratified into three periods(1987-1996,1997-2006,and 2007-2015).Age-standardized rates were estimated by direct standardization using the world *** rate ratios and the corresponding 95%confidence intervals(CI)were estimated using the early period as the reference(1987-1996).Joinpoint regression models were used to assess the annual percentage change and highlight trends over the entire period(from 1987 to 2015).RESULTS Among males,the age-standardized incidence rates significantly decreased from 19.41(1987-1996)to 13.12(1997-2006)to 8.15(2007-2015)per 105 *** incidence rate ratio over 28 years was 0.42(95%CI:0.34-0.50),and the annual percentage change was-4.59[95%CI:(-6.4)-(-2.7)].Among females,rates dropped continuously from 7.02(1987-1996)to 2.57(2007-2015)per 105 person-years,with an incidence rate ratio of 0.37(95%CI:0.28-0.45)and an annual percentage change of-5.63[95%CI:(-8.9)-(-2.3)].CONCLUSION The population-based registration showed that the incidence of primary liver cancer has steadily decreased in the Bamako district over 28 *** trend does not appear to result from biases or changes in registration *** is the first report of such a decrease in an area of high incidence of liver cancer in *** decrease may be explained by the changes and diversity of diet that could reduce exposure to

背景休克患者一般建议实施动脉置管监测。然而，这项有创操作有一定风险，而且可能延误其他的急救操作。本研究针对袖带无创测压（N1BP）在有创平均动脉血压（MAP）低于65mmHg以及给予心血管干预后MAP升高时的追踪能力进行探讨。方法这项前瞻性研究在3个重症监护室进行，选取循环衰竭的成年患者，依次进行腿抬高45°、补液300ml、再次补液200ml3种处理。每个试验阶段分别测量4个有创和无创MAP值。结果111例患者（50例感染性休克、15例心源性休克和46例其他原因休克）中，当有创MAP小于65iilmHg时，NIBP可以准确反映患者血压：受试者操作特征曲线（ROC）下面积（AUC）为0．90（95％可信区间：0．71—1），阳性似然比（LHR）为7．7（95％可信区间：5．4—11），阴性似然比为0．31（95％可信区间：0．22—0．44）（界点65mmHg）。NIBP可以有效判断患者MAP10％以上的上升：AUCh0．95（95％可信区间：0．92—0．96），阳性LHR为25．7（95％可信区间：10．8—61．4），阴性LHR为0．26（95％可信区间：0．2—0．34）（界点10％）。结论NIBP对于低血压有很好的辨别能力，如果取4次测量平均值的话，甚至可以比有创血压更好地反映MAP的变化。
