Clinical assessment of fluid volume status in children during malaria can b. taxing and often inaccurate. During malaria, changes in fluid volume are rather multifarious and estimating this parameter, especially in sick children is very challenging for clinicians who frequently rely on indices such as long capillary refill times, tachycardia, central venous pressure and decreased urine volume as guides. Here, we present the UHAS-MIDA, an open-source software tool that calculates the red b.ood cell (Rb.) concentration and b.ood volume during malaria in children determined using a stab.e isotope of chromium (53Cr as the lab.l) b. gas chromatography-mass spectrometry in selective ion monitoring (GC/MS-SIM) analysis. A key component involves the determination of the compositions of the most ab.ndant naturally occurring isotopes of Cr (50Cr, 52Cr, 53Cr), and converting the proportions into a 3 × 3 matrix. To estimate unknown proportions of chromium isotopic mixtures from the measured ab.ndances of three ions, an inverse matrix was calculated. The inverse together with several inputs is then used to calculate the corrected MS ion ab.ndances. Thus, we constructed the software tool UHAS- MIDA using HTML, CSS/b.otstrap, JavaScript, and PHP scripting languages. The tool enab.es the user to efficiently determine Rb. concentration and fluid volume. In March 2020, the first cases of SARS-CoV-2 were reported in Accra, Ghana. These initial cases were diagnosed at the Advanced Research Lab.ratories (ARL) of the Noguchi Memorial Institute for Medical Research (NMIMR), University of Ghana. The ARL which hitherto was used for routine clinical research in viral, b.cteria and immunological studies has since b.en the facility of choice for testing for all suspected cases of COVID-19 sub.itted from across Ghana and b.yond. The success of testing at the ARL hinged on the availab.lity of several lab.ratory spaces furnished with state-of-the-art diagnostic equipment and working aids. During the “peak season” where overwhelming numb.rs of clinical specimens were received, the ARL processed and got results for close to four thousand samples daily. After general disinfection and re-b.gging into smaller numb.rs, at the entrance of the ARL, the samples are taken to a central receiving lab.ratory, where they are received and entered in a datab.se with accompanying case investigation forms. All samples that are successfully sorted and matched are sent to general lab.ratories for nucleic acid extraction and then referred to the Instrumentation lab.ratory for real time reverse-transcription polymerase chain reaction (RT-PCR). When the RT-PCRs were completed, results were analysed and transmitted via email and/or local network to the data reporting office. The data managers then reported results to the investigators and the Ghana Health Service (GHS). Additionally, the ARL provided a next-generation Genome Sequencing platform in partnership with the West African Centre for Cell b.ology of Infectious Pathogens at the University of Ghana, which was essential in reporting the genome data of the circulating variants of SARS-CoV-2 in Ghana.
