Dear editor,We read with interest the commentary by Mark Tremblay and colleagues,1 which outlines the new 24-Hour Canadian sedentary behavior *** guidelines take the perspective of a continuum across the 24-h day,providing a vehicle for discussing physically active time,sitting and other sedentary time,as well as healthy sleep *** commentary 1 focuses on the sedentary behavior(SB)recommendations in these guidelines:(1)limiting sedentary time to8 h per day,(2)limiting recreational screen time to3 h,and(3)breaking up long periods of sitting as often as possible throughout the *** for this perspective was evidenced by a survey of a sample(n=126,6% of total)of Sedentary Behavior Research Network(SBRN)members2 indicating broad acceptance of these SB recommendations.1 In this response,we put this survey in context and discuss the evidence base underpinning these 3 Canadian sedentary-behavior recommendations.

Background:Little is known about the association between different types of physical activity(PA)and chronic back conditions(CBCs)at the population *** investigated the association between levels of total and type-specific PA participation and ***'.The sample comprised 60,134 adults aged>16 years who participated in the Health Survey for England and Scottish Health Survey from 1994 to *** logistic regression models,adjusted for potential confbunders,were used to examine the association between total and type-specific PA volume(walking,domestic activity,sport/exercise,cycling,football/rugby,running/jogging,manual work,and housework)and the prevalence of ***'.We found an inverse association between total PA volume and prevalence of *** with inactive participants,the fully adjusted odds ratio(OR)for very active participants(≥15 metabolic equivalent h/week)was 0.77(95%confidence interval(CI):0.69—0.85).Participants reporting≥300 min/week of moderate-intensity activity and≥75 min/week of vigorous-intensity activity had 24%(95%CI:6%—39%)and 21%(95%C1:11%—30%)lower odds of CBCs,*** odds of CBCs were observed for participation in high-level manual domestic activity(OR=1.22;95%CI:1.00-1.48).Sport/exercise was associated with CBCs in a less consistent manner(e.g.,OR=1.18(95%CI:1.06—1.32)for low levels and OR=0.82(95%CI:0.72—0.93)for high levels of sport/exercise).Conclusion'.PA volume is inversely associated with the prevalence of CBCs.

Background:This study examined the joint associations of sleep patterns and physical activity(PA) with all-cause,cardiovascular disease(CVD),and cancer ***:A total of 341,248 adults(mean age=39.7 years;men:48.3%) were included in the study,with a 15-year *** reported sleep duration and disturbances(difficulty falling asleep,easily awakened,or use of sleeping medication).PA was classified into 4 levels:30.0 metabolic equivalent hours per week(MET-h/week).To understand the joint associations of sleep patterns and PA with mortality,Cox proportional hazard models were conducted,with exposure variables combining sleep duration/disturbances and ***:Compared with the reference group(sleeping 6-8 h/day),individuals who slept>8 h/day had higher risk for all-cause mortality(hazard ratio(HR)=1.307,95% confidence interval(95%CI):1.248-1.369),CVD mortality(HR=1.298,95%CI:1.165-1.445),and cancer mortality(HR=1.128,95%CI:1.042-1.220).Short sleep duration was not associated with mortality *** risk of all-cause and CVD mortality was found in participants who had difficulty falling asleep(HR=1.120,95%CI:1.068-1.175;HR=1.163,95%CI:1.038-1.304,respectively),and used sleeping medication(HR=1.261,95%CI:1.159-1.372;HR=1.335,95%CI:1.102-1.618,respectively) compared with those who slept *** sleep duration and sleep disturbances were not associated with risk of all-cause and CVD mortality among individuals achieving a PA level of>15 MET-h/week,and in particular among those achieving> 30 MET-h/***:Long sleep duration,difficulty falling asleep,and use of sleeping medication were related to a higher risk of *** physically active at a moderate intensity for 25-65 min/day eliminated these detrimental associations.

BACKGROUND: Hepatobiliary cystadenocarcinoma represents a rare epithelial malignant tumor derived from the intrahepatic bile duct. METHODS: A 71-year-old woman, who had undergone laparoscopic drainage of a cystic lesion of the right hepatic lobe, was misdiagnosed as having hepatic echinococcal disease, and received intracystic infusion of 95% ethanol four years ago. She was admitted to our hospital for further treatment. RESULTS: Physical examination revealed dilated superficial veins across the right abdominal wall. After mapping the direction of blood flow in these vessels, we assumed that this was a sign of inferior vena cava obstruction. Abdominal ultrasound, computed tomography, magnetic resonance imaging combined with magnetic resonance angiography showed a large cystic mass in the right upper quadrant and epigastrium, displacing the adjacent structures, adherent to the inferior vena cava, which was not patent, resulting in dilation of superficial epigastric veins. The patient underwent an exploratory laparotomy. Total excision of the huge mass measuring 16×15 cm was possible under selective vascular exclusion of the liver. Removal of the tumor resulted in immediate restoration of flow in the inferior vena cava. On the basis of the pathology and findings of immunohistochemical analysis, a hepatobiliary cystadenocarcinoma was ***: In the present case, hepatobiliary cystadenocar-cinoma was accompanied by dilated superficial venous collaterals due to inferior vena cava obstruction. Selective vascular exclusion of the liver allowed a safe oncological resection of the tumor.

Apple(Malus domestica Borkh)is an important fruit crop cultivated in a broad range of environmental *** fruit ripening is a physiological process,whose molecular regulatory network response to different environments is still not sufficiently investigated and this is particularly true of the peel *** this study,the influence of environmental conditions associated with low(20 m)and high(750 m)altitude on peel tissue ripening was assessed by physiological measurements combined with metabolomic and proteomic analyses during apple fruit development and *** apple fruit ripening was itself not affected by the different environmental conditions,several key color parameters,such as redness and color index,were notably induced by high *** with this observation,increased levels of anthocyanin and other phenolic compounds,including cyanidin-3-O-galactoside,quercetin-3-O-rhamnoside,quercetin-3-O-rutinoside,and chlorogenic acid were identified in the peel of apple grown at high ***,the high-altitude environment was characterized by elevated abundance of various carbohydrates(e.g.,arabinose,xylose,and sucrose)but decreased levels of glutamic acid and several related proteins,such as glycine hydroxymethyltransferase and glutamate–glyoxylate *** processes affected by high altitude were the TCA cycle,the synthesis of oxidative/defense enzymes,and the accumulation of photosynthetic *** the obtained data we were able to construct a metabolite-protein network depicting the impact of altitude on peel *** combined analyses presented here provide new insights into physiological processes linking apple peel ripening with the prevailing environmental conditions.

Aims: To examine the childhood overweight and obesity prevalence trends between 1974 and 2003 and to assess whether these trends relate to parental social class and household income. Methods: A school based and a general population health survey: the National Study of Health and Growth in 1974, 1984, and 1994, and the Health Survey for England, yearly from 1996 to 2003. Participants were 14 587 white boys and 14 014 white girls aged 5-10 years. Overweight and obesity prevalence were calculated using UK specific as well as international body mass index (kg/m2) cut-offs. Socioeconomic status was measured using the Registrar General’ s social class; household income (1997 onwards only) was adjusted for household size. Results: The prevalence of obesity (UK specific definition) in boys increased from 1.2% in 1984 to 3.4% in 1996-97 and 6.0% in 2002-03. In girls, obesity increased from 1.8% in 1984 to 4.5% in 1996-97 and 6.6% in 2002-03. Obesity prevalence has been increasing at accelerating rates in the more recent years. Children from manual social classes had marginally higher odds (OR 1.14, 95% CI 0.98 to 1.33) and children from higher income households had lower odds (OR 0.74, 95% CI 0.61 to 0.89) to be obese than their peers from non-manual class, and lower income households, respectively. Conclusion: Childhood obesity is increasing rapidly into the 2000s in England and these increases are more marked among children from lower socioeconomic strata. There is an urgent need for action to prevent further increase in obesity among children.
