The Erasmus plus programme’Innovative Education and Training in high power laser plasmas’,otherwise known as powerLaps,is described.The powerLaps programme employs an innovative paradigm in that it is a multi-centre programme where teaching takes place in five separate institutes with a range of different aims and styles of delivery.The ’in class’ time is limited to four weeks a year,and the programme spans two years.powerLaps aims to train students from across Europe in theoretical,applied and laboratory skills relevant to the pursuit of research in laserplasma interaction physics and inertial confinement fusion(ICF).Lectures are intermingled with laboratory sessions and continuous assessment activities.The programme,which is led by workers from the Technological Educational Institute(TEI)of Crete,and supported by co-workers from the Queen’s University Belfast,the University of Bordeaux,the Czech Technical-University in prague,Ecole polytechnique,the University of Ioannina,the University of Salamanca and the University of York,has just completed its first year.Thus far three Learning Teaching Training(LTT)activities have been held,at the Queen’s University Belfast,the University of Bordeaux and the Centre for plasma physics and Lasers(CppL)of TEI Crete.The last of these was a two-week long Intensive programme(Ip),while the activities at the other two universities were each five days in length.Thus far work has concentrated upon training in both theoretical and experimental work in plasma physics,high power laser-matter interactions and high energy density physics.The nature of the programme will be described in detail and some metrics relating to the activities carried out to date will be presented.

A combined risk-based and condition monitoring approach:developing a dynamic model for the case of marine engine lubrication

This paper focuses on the creation of a dynamic probabilistic model which simulates deterioration trends of a marine engine lubricationsystem. The approach is based on risk and the implementation is achieved through a dynamic Bayesian network (dBN).Risk can be useful for decision making, while dBNs are a powerful tool for risk modelling and prediction models. The model takesinto account deterioration of engine components, oil degradation and the off-line condition monitoring technique of oil analysis, inthe context of predictive maintenance. The paper aims to efficiently predict probability evolution for main engine lubrication failureand to decide upon the most beneficial schemes from a variety of lubrication oil analysis interval schemes by introducing monetarycosts and producing the risk model. Real data and respective analysis, along with expert elicitation, are utilized for achieving modelquantification, while themodel is materialized through a code in the Matlab environment. Results from the probabilistic model showa realistic simulation for the system and indicate the obvious, that with more frequent oil analyses and respective maintenance orrepairs, the probability of failure drops significantly. However, the results from the risk model highlight that the costs can redefinescheme suggestions, as they can correspond to low probabilities of failure but also to higher costs. A two-month interval scheme issuggested, in contrast to the most preferred practice among shipping companies of a three-month interval. The developed model isin general identified as a failure prediction tool focusing on marine engine lubrication failure.

Fixation of complex proximal humeral fractures in elderly patients with a locking plate： A retrospective analysis of radiographic and clinical outcome and complications

purpose: The optimal surgical treatment of displaced type B and C fractures of the proximal humerus in the elderly remains controversial. Good clinical results have been reported by plating these fractures as well as a high rate of complications. Our retrospective study aims to evaluate clinical recovery and complications using the S3 locking plate in elderly patients. Methods: Fifty-one patients older than 65 years of age, with a complex proximal humeral fracture type B or C (AO classification system), were included. patients have been followed up for a minimum of 12 months. We assessed callus formation, radiological results, clinical outcome (according to the Constant Shoulder Score System) and complications. Any difference in the clinical recovery among the 2 types of fracture pattern (B and C) was investigated. Results: The mean time of fracture healing was 12.4 weeks. The mean Constant score at 3, 6 and 12 months was 68, 73 and 75 respectively. No statistically significant difference in the clinical outcome was observed between the B and C fracture patterns ( p > 0.05). We noticed an overall of 5 complications (9.8%). There was no need to revision any of the implants. Conclusion: Anatomic reduction and proper plate positioning are essential for minimizing implantrelated complications. In our experience the S3 angular stability system offers a proper osteosyntesis and a good clinical recovery with a low rate of complications.

Semaglutide for nonalcoholic steatohepatitis: closer to a solution?

In the issue of March 25,2021 of the New England Journal of Medicine,Newsome et al.published the results of a phase II clinical trial of semaglutide,a glucagon-like peptide-1 receptor agonist(GLp-1RA),given once-daily,subcutaneously for 72 weeks for the treatment of non-alcoholic steatohepatitis(NASH)(1).The authors found that among 230 NASH patients with fibrosis stage of F2 or F3 the primary endpoint of the study(NASH resolution with no worsening of fibrosis)was achieved in 59%of patients with semaglutide at a dose of 0.4 mg compared to 17%in the placebo group(OR 6.87;95%CI:2.60-17.63;p<0.001).

AIM: To investigate the technical success and clinical complication rate of a cannulated pancreatic duct with guidewire for biliary access. METHODS: During a five-year study period, a total of 2843 patients were included in this retrospective analysis. Initial biliary cannulation method consisted of single-guidewire technique (SGT) for up to 5 attempts, followed by double-guidewire technique (DGT) when repeated unintentional pancreatic duct cannulation had taken place. pre-cut papillotomy technique was reserved for when DGT had failed or no pancreatic duct cannulation had been previously achieved. Main outcome measurements were defined as biliary cannu-lation success and post-endoscopic retrograde cholangiopancreatography (ERCp) complication rate. RESULTS: SGT (92.3% success rate) was characterized by statistically significant enhanced patient outcome compared to either the DGT (43.8%, p pre-cut failed DGT (73%, p pre-cut as first step method (80.6%, p = 0.002). pre-cut as first step method offered a statistically significantly more favorable outcome compared to the DGT (p post-ERCp pancreatitis did not differ in a statistically significant manner between either method (SGT: 5.3%, DGT: 6.1%, pre-cut failed DGT: 7.9%, pre-cut as first step: 7.5%) or with patients' gender. CONCLUSION: Although DGT success rate proved not to be superior to SGT or pre-cut papillotomy, it is considered highly satisfactory in terms of safety in order to avoid the risk of a pre-cut when biliary therapy is necessary in difficult-to-cannulate cases.

Highly sensitive ozone and hydrogen sensors based on perovskite microcrystals directly grown on electrodes

Highly-sensitive and stable ozone and hydrogen sensing elements were fabricated based on well-crystalline rounded cube-shaped CspbBr 3 microcrystals,synthesized by a facile solution process per-formed under ambient conditions.It is shown that such elements demonstrate enhanced room tem-perature gas sensing ability compared to the previously reported metal halide and oxide-based ones.Electrical measurements performed on these sensing components revealed high sensitivity to ultra-low ozone and hydrogen concentrations,namely 4 ppb and 1 ppm respectively,as well as a remarkable repeatability,even after a few months of storage in ambient conditions.Both ozone and hydrogen sensors were self-activated,as they did not require the use of UV or heating external stimuli to operate,and exhibited fast detection and short restoration times.All such attractive properties along with the simple fabrication process could provide an easy,efficient and low-cost technology for the realization of future gas sensing devices.

BACKGROUND Ewing sarcoma(ES)is a malignant neoplasm of neuroectodermal origin and is commonly observed in children and young adults.The musculoskeletal system is the main body system impacted and ES is rarely seen in the visceral organs particularly the adrenal gland.AIM To present a comprehensive review of primary adrenal ES,with emphasis on diagnosis,therapy and oncological outcomes.METHODS A systematic review of the literature was performed according to the preferred Reporting Items for Systematic reviews and Meta-Analyses 2020.pubMed/MEDLINE,EMBASE and Google Scholar bibliographic databases were searched to identify articles from 1989 to 2022 and included patients with ES/primitive neuroectodermal tumor(pNET)of the adrenal gland.pubMed,Google Scholar and EMBASE medical databases were searched,combining the terms“adrenal”,“ES”and“pNET”.Demographic,clinical,pathological and oncological data of patients were analyzed by SpSS version 29.0.RESULTS A total of 52 studies were included for review(47 case reports and 5 case series)with 66 patients reported to have primary adrenal ES.Mean age at diagnosis was 26.4±15.4 years(37.9%males,57.6%females,sex not reported in 3 cases).The most frequent complaint was abdominal/flank pain or discomfort(46.4%)followed by a palpable mass(25.0%),and the average duration of symptoms was 2.6±3.1 mo.The imaging modality of choice was computed tomography scan(81.5%),followed by magnetic resonance imaging(20.4%).preoperative staging revealed that 17 tumors(27.9%)were metastatic and 14 patients had inferior vena cava or renal vein neoplastic thrombus at initial diagnosis.Open adrenalectomy was performed in the majority of cases(80.0%),of which 27.9%required more extensive resection.Minimally invasive surgery was attempted in 8.2%of tumors.Complete surgical resection was achieved in 89.4%of the patients.Adjuvant therapy was administered to 32 patients,in the form of chemotherapy(62.5%),radiotherapy(3.1%)or combination(34.4%).Median overall survival was 15 mo and 24-mo overall survival was

Background:As of 2015 thousands of refugees are being hosted in temporary refugee camps in Greece.Displaced populations,travelling and living under poor conditions with limited access to healthcare are at a high risk of exposure to vector borne disease(VBD).This study sought to evaluate the risk for VBD transmission within refugee camps in Greece by analyzing the mosquito and sand fly populations present,in light of designing effective and efficient context specific vector and disease control programs.Methods:A vector/pathogen surveillance network targeting mosquitoes and sand flies was deployed in four temporary refugee camps in Greece.Sample collections were conducted bi-weekly during June-September 2017 with the use of Centers for Disease Control(CDC)light traps and oviposition traps.Using conventional and molecular diagnostic tools we investigated the mosquito/sand fly species composition,population dynamics,pathogen infection rates,and insecticide resistance status in the major vector species.Results:Important disease vectors including Anopheles sacharovi,Culex pipiens,Aedes albopictus and the Leishmania vectors phlebotomus neglectus,p.perfiliewi and p.tobbi were recorded in the study refugee camps.No mosquito pathogens(plasmodium parasites,flaviviruses)were detected in the analysed samples yet high sand fly Leishmania infection rates are reported.Culex pipiens mosquitoes displayed relatively high knock down resistance(kdr)mutation allelic frequencies(ranging from 41.0 to 63.3%)while kdr mutations were also detected in Ae.albopictus populations,but not in Anopheles and sand fly specimens.No diflubenzuron(DFB)mutations were detected in any of the mosquito species analysed.Conclusions:Important disease vectors and pathogens in vectors(Leishmania spp.)were recorded in the refugee camps indicating a situational risk factor for disease transmission.The Cx.pipiens and Ae.albopictus kdr mutation frequencies recorded pose a potential threat against the effectiveness of pyrethroid insecticides in these settings.In co

Insecticide resistance in Bemisia tabaci from Cyprus

A comprehensive study on the Bemisia tabaci （biotype B） resistance to neonicotinoid insecticides imidacloprid, acetamiprid and thiamethoxam, and pyrethroid bifenthrin was conducted in Cyprus. The resistance level to eight field-collected B. tabaci populations was investigated. The activities of enzymes involved in metabolic detoxification and the frequencies ofpyrethroid and organophosphates target site resistance mutations were determined. Moderate to high levels of resistance were detected for imidacloprid （re- sistance factor [RF] 77-392） and thiamethoxam （RF 50-164） while low resistance levels were observed for acetamiprid （RF 7-12）. Uniform responses by the Cypriot whiteflies could be observed against all neonicotinoid insecticides. No cross-resistance between the neonicotinoids was detected as well as no association with the activity of the p450 microsomal oxidases. Only imidacloprid resistance correlated with carboxylesterase activity. Low to extremely high resistance was observed for insecticide bifenthrin （RF 49 1 243） which was associated with the frequency of the resistant allele in the sodium channel gene but not with the activity of the detoxification enzymes. Finally, the F331W mutation in the acetylcholinesterase enzyme acel gene was fixed in all B. tabaci populations from Cyprus.

BACKGROUND:The frequency and pattern of hyperbili-rubinemia after open-heart surgery and its severe perioperative complications are not well clarified. The purpose of this study was to investigate the incidence and nature of postoperative jaundice in patients undergoing cardiac operation,to analyze the determinants,and to identify the clinical significance of this complication with regard to the associated morbidity and mortality. METHODS:A prospective observational study was made during the period of 2003-2004 in a Surgical Intensive Care Unit of a Cardiac Surgery Center,Athens. One hundred twenty-eight adult patients for open heart surgery were divided into three groups. Group A included 50 patients who underwent coronary artery bypass crafting(CABG),group B 31 patients who were subjected to aortic valve replacement(AVR)+CABG and group C 47 patients who underwent mitral valve replacement(MVR)+CABG. Aminotransferases,alkaline phosphatase,gamma-glutamyl-transpeptidase and both types of bilirubin were determined at admission,24 hours after the operation and thereafter according to clinical evolution. The presence of jaundice was associated with elevated serum bilirubin above 3 mg/dl. RESULTS:Hyperbilirubinemia developed in 34 patients(26.5%). The incidence of postoperative jaundice was higher in patients who were subjected to MVR+CABG than to CABG and AVR+CABG. Hyperbilirubinemia was correlated with prolonged cardiopulmonary by-passtime(pping time(ppumping(pps and the number of blood and plasma transfusions. postoperative jaundice resulted mainly from an increase in conjugated bilirubin. CONCLUSIONS:Although hyperbilirubinemia seems to be multifactorial,the type of operation,the preoperative hepatic dysfunction due to advanced heart failure(NYHA Ⅱ-Ⅲ) and the decreased hepatic flow during the operation seem to determine the incidence of jaundice.
