Can We Predict De Novo Urge Incontinence by Perineal Ultrasound?

Introduction: Urinary incontinence affects over 200 million people worldwide [1]. Tension free vaginal tape is the standard surgical treatment for stress urinary incontinence. De novo urge urinary incontinence is a long-term complication of this treatment with a significant impact in the quality of life of these patients. Objective: The major aim of this study is to assess the correlation between perineal ultrasonography findings and the incidence of de novo urge urinary incontinence. material and methods: A prospective observational study was designed. Patients with stress urinary incontinence diagnosed by clinical and urodynamic findings submitted to a tension-free vaginal tape surgery were included. International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) and introital perineal ultrasound were performed before surgical intervention (I-STOP©). Clinical and ultrasound re-evaluation were executed six months after surgery. Stress urinary incontinence was defined according to the ICS-IUGA. Data were recorded using a microsoft Access database and statistical analysis using SAS©. Results: Bladder thickness equal to or below 6 mm has a low positive predictive value (PPV, 0.55), but a negative predictive power (NPP) of 0.72. Following surgery, a slight increase in postvoid residue is observed. Considering demographic data, an increase of 5 points in body mass index (BmI) resulted in an odds ratio (OR) of 1.74 of presenting de novo urge urinary in continence. Conclusions: In patients submitted to a tension-free vaginal tape surgery, high BmI seems to be associated with a higher rate of de novo urge urinary incontinence. Preoperative bladder wall thickness below 6 mm seems predict absence of this complication.

Grassland is one of the largest terrestrial biomes,providing critical ecosystem services such as food production,biodiversity conservation,and climate change *** climate change and land-use intensification have been causing grassland degradation and desertification *** one of the primary medium for ecosystem energy flow and biogeochemical cycling,grassland carbon(C)cycling is the most fundamental process for maintaining ecosystem *** this review,we first summarize recent advances in our understanding of the mechanisms underpinning spatial and temporal patterns of the grassland C cycle,discuss the importance of grasslands in regulating inter-and intra-annual variations in global C fluxes,and explore the previously unappreciated complexity in abiotic processes controlling the grassland C balance,including soil inorganic C accumulation,photochemical and thermal degradation,and wind *** also discuss how climate and land-use changes could alter the grassland C balance by modifying the water budget,nutrient cycling and additional plant and soil ***,we examine why and how increasing aridity and improper land use may induce significant losses in grassland C ***,we identify several priorities for future grassland C research,including improving understanding of abiotic processes in the grassland C cycle,strengthening monitoring of grassland C dynamics by integrating ground inventory,flux monitoring,and modern remote sensing techniques,and selecting appropriate plant species combinations with suitable traits and strong resistance to climate fluctuations,which would help design sustainable grassland restoration strategies in a changing climate.

Cysteine Supplementation to Parenteral Nutrition Improves Red Blood Cell Glutathione Concentrations of Critically Ill Preterm Neonates

Premature neonates have immature antioxidant enzyme systems rendering them more susceptible to oxidative injury. One key antioxidant is glutathione (GSH). The rate limiting amino acid (AA) in GSH production is thought to be cysteine. Critically ill premature neonates who are parenterally fed are often supplemented with additional cysteine, yet the need for cysteine and optimal dose is unknown. This was a prospective, un-blinded, three-group, randomized crossover study aimed to evaluate three doses of cysteine by analyzing red blood cell (RBC) GSH, plasma AA, weight, and nitrogen balance. Neonates were randomized to receive 72 hours of each of the following cysteine doses: 10 mg/g AA, 20 mg/g AA, and 40 mg/g AA. GSH, plasma AAs, weight, and nitrogen balance were evaluated at baseline (after 72 hours of 0 mg/g AA), day three, day six, and day nine. Sixteen patients completed all doses of cysteine, which resulted in significantly increased RBC GSH concentrations over baseline. Plasma concentrations of cystine, total and free cysteine/cystine, glycine and serine increased with cysteine dose. All cysteine doses were associated with adequate weight gain, and positive nitrogen balance. These results are contrary to more recent studies of cysteine effect on RBC GSH concentrations in preterm neonates and infants, but may reflect the severity of illness in our study subjects, where cysteine requirements may be increased.

The microsatellite polymorphism of heme oxygenase-1 is associated with baseline plasma IL-6 level but not with restenosis after coronary in-stenting

Background Vascular smooth muscle cells （VSmCs） can express heme-oxygenase （HO）, a rate-limiting enzyme in the degradation of heme to bilirubin, ferritin and carbon monoxide （ CO）. VSmC-derived CO can suppress VSmC proliferation and may serve as an antiproliferation factor. The promoter region of HO-1 shows a polymorphism with different （GT）n repeats that has been reported to differently induce gene expression. The objective of this study was to examine the effect of this variation on the occurrence of restenosis after in-stent treatment in patients with coronary artery disease. methods Candidates who underwent coronary stent implantation were genotyped for the HO-1 promoter polymorphism using polymerase chain reaction （PCR） and automated DNA capillary sequencer. Serum levels of IL-6 and C-reactive protein （CRP） were obtained at baseline, 24 hours and 48 hours after stenting. The primary end point for the study was angiographic evidence of in-stent restenosis at 6 months. All parameters for evaluation of restenosis were analysed by quantitatve computer-assisted angiographic analysis （QCA）. Results One hundred and eighty-seven patients who underwent coronary stent implantation were studied of whom 27.8% showed 〉150% restenosis after 6 months. The distribution of （GT）n repeats of all patients in the promoter region of HO-1 genotype ranged from 22 to 42, with （GT） 25 and （GT） 32 being the two most common alleles. The allelic repeats were divided into the short class （S） with 29 （ GT）., the middle class （m） with 30- 37 （GT）n and the long class （L） with 38 （GT）n. There was no significant difference in the restenosis between the genotype groups or between post operation levels of inflammation markers, but carriers of the S allele （ n = 120） had 33.3% lower baseline IL-6 compared with non-S carriers （n =67, P =0. 0008）. Exploiting a metal wire grating in total internal reflection geometry to achieve achromatic polarization conversion

We demonstrate how a metal wire grating can work as a 45° polarization converter, a quarter-wave retarder, and a half-wave retarder over a broadband terahertz range when set up in total internal reflection geometry. Classical electromagnetic theory is applied to understand the mechanism, and equations to calculate the polarization state of reflected light are derived. We use a metal grating with a period of 20 μm and width of 10 μm on a fused silica surface: linearly polarized terahertz light incident from fused silica with a supercritical incident angle of 52° is totally reflected by the metal grating and air. The polarization of the terahertz light is rotated by 45°, 90°, and circularly polarized by simply rotating the wire grating. The performance is achromatic over the measured range of 0.1–0.7 THz and comparable to commercial visible light wave retarders.

Hydroxylation of phenol with hydrogen peroxide catalyzed by Fe- and AIFe-Bentonite

The paper described pillarisations of natural bentonite from Pacitan East Java of Indonesia by using AI and Fe. Intercalation process by using surfactant molecule has also been carried out. Natural bentonite was intercalated with HDTmA-Br （hexadecyltrimethylammonium-bromide） 1, 5% solution before pillared with AI and Fe metal to give HDTmA-bentonite forms. The ratio of bentonite and intercalating agent or pillaring agent was 1 gr/50 mL. The mixture was agitated, and then the solid phase was washed with distilled water. Then it was dried and calcined at 450℃ for 4 hours. Their catalytic activity and selectivity were studied for phenol hydroxylation using tlzOz （30%）. The reaction condition of this reaction was as follows： ratio of phenol/ H202 = 1：1 （molar ratio）, concentration of phenol = 1 m, reaction temperature was 60℃, and ratio of catalyst/phenol was 1：10. The products were hydroquinone and cathecol.
