Percutaneous coronary intervention for the treatment of coronary artery disease is most commonly performed in the UK through the radial artery,as this is considered to be safer than the femoral approach.However,despite improvements in technology and techniques,complications can occur.The most common complication,arterial spasm,can cause intense pain and,in some cases,procedural failure.The incidence of spasm is dependent on several variables,including operator experience,artery size,and equipment used.An antispasmolytic cocktail can be applied to reduce spasm,which usually includes an exogenous nitric oxide(NO)donor(glyceryl trinitrate).NO is an endogenous local vasodilator and therefore is a potential target for anti-spasm intervention.However,systemic administration can result in unwanted side-effects,such as hypotension.A method that adopts local delivery of NO might be advantageous.This review article describes the mechanisms involved in radial artery spasm,discusses the advantages and disadvantages of current strategies to reduce spasm,and highlight the potential of NO-loaded nanoporous materials for use in this setting.

While most mammals show birth hour peaks at times of the 24-h cycle when they are less active,there are exceptions to this general pattern.Such exceptions have been little explored,but may clarify evolutionary reasons for the diel timing of births.We investigated intraspecific variation in birth hour in wild blue monkeys Cercopithecus mitis stuhlmanni,a diurnal primate,to identify factors that differentiated daytime versus nighttime births.Behavioral and life history data from 14 groups over 14 years revealed that 4%of 484 births occurred during the day.Probability of daytime birth varied with mother’s age,peaking at 15.7 years.Births whose annual timing deviated most from the population’s peak birth months were 5 times more likely to occur during daytime than those that deviated less.There was no evidence that mother’s rank or infant sex influenced birth hour,and mixed evidence that daytime births were more probable in larger groups.Survivorship did not differ significantly for infants born during the day versus night.Prime-aged mothers may be able to handle the consequences of an unusual birth hour more successfully than mothers with less experience or those weakened by age.Daytime birth may be more advantageous in the off-season because nights are colder at that time of year.These findings are consistent with hypotheses relating birth hour to the risk of losing social protection in group-living animals,but are not consistent with those emphasizing risk of conspecific harassment.Patterns of within-species variation can help in evaluating evolutionary hypotheses for non-random birth hour.

Objective:Although standard of care for primary nasopharyngeal carcinoma(NPC)is chemoradiotherapy,there remains no consensus on management of recurrent or metastatic disease.We characterized recent clinical trials on NPC to assess trends in NPC treatment and establish promising areas for future research.Study Design:Retrospective database study.Setting:ClinicalTrials.gov database.methods:Retrospective review of all NPC trials from November 1999 to June 2021.For each study,the following variables were extracted:study characteristics,intervention,outcome measures,and inclusion criteria.Secondary searches via Pubmed and Google scholar determined trial publication status.Results:A total of 448 clinical trials were identified:72(16%)observational and 376(84%)interventional,of which there were 30(8%)Phase I,183(49%)Phase II,86 Phase III(23%),and 5(1%)Phase IV trials.Fifty-four percent of trials included only primary NPC while 111(25%)exclusively studied recurrent cancers.The most common interventions were cisplatin(n=64)and intensity modulated radiation therapy(n=54);there were 38 trials involving PD-1 monoclonal antibodies.Thirty-four studies examined quality of life measures,including xerostomia and mucositis.Of the completed studies,53.2%have published manuscripts.Poor patient accrual was the most common reason for premature study termination.Conclusions:Novel immunotherapies have been increasingly incorporated into NPC studies in recent years,however,chemotherapy and radiation,despite their numerous side effects,are still widely used due to their clinical effectiveness.Future trials are warranted to determine the optimal therapeutic regimens to decrease relapse rates and side effects.

mucopolysaccharidoses(mPS)are a group of rare inborn errors of metabolism caused by defective lysosomal enzymes which prevent cells from degrading and recycling certain carbohydrates and fats,resulting in the storage of glycosaminoglycans in cells throughout the body.This leads to multisystem abnormalities involving bone,connective tissues,brain,blood,spinal cord,skin,and other tissues.

Introduction:Auditory symptoms in individuals with Autistic Spectrum Disorder(ASD)are well described within the neurodevelopmental literature,yet there is minimal mention of ASD in Otolaryngology literature.This is surprising considering the potential clinical and diagnostic implications of this link,and the potential for ASD to present to Otolaryngologists in the form of unexplained auditory symptoms.The aims of this literature review were to explore the intersection of auditory symptoms and ASD from the perspective of clinical Otolaryngology,and to outline a clinically focused research agenda based on emerging themes relevant to Otolaryngology.methods:We searched Pubmed,Embase,Ovid and Cochrane library for studies until November 2021.Four authors independently reviewed 227 publications identified.39 were filtered into the final analysis.The PRISmA 2020 guidelines were followed.The heterogeneity of literature meant that a Systematic Review was not feasible.Included studies were therefore classified thematically,forming the basis of the scoping review.Results:Diagnostic theories for auditory symptoms in ASD include the entire auditory pathway and brain.There is a growing body of literature on auditory symptoms in ASD,suggesting that a primary diagnosis of ASD should be considered in patients presenting with otherwise unexplained auditory symptoms,and indicating a learning need for Otolaryngologists and audiologists,to whom these patients may present.Conclusion:We recommend a research agenda focusing on multidisciplinary collaboration,stakeholder engagement,responsible clinical screening,and clarification of pathophysiological mechanisms and terminology.

Thermal processes on the Tibetan Plateau(TP)influence atmospheric conditions on regional and global scales.Given this,previous work has shown that soil moisture−driven surface flux variations feed back onto the atmosphere.Whilst soil moisture is a source of atmospheric predictability,no study has evaluated soil moisture−atmosphere coupling on the TP in general circulation models(GCms).In this study,we use several analysis techniques to assess soil moisture−atmosphere coupling in CmIP6 simulations including:instantaneous coupling indices;analysis of flux and atmospheric behaviour during dry spells;and a quantification of the preference for convection over drier soils.Through these metrics we partition feedbacks into their atmospheric and terrestrial components.Consistent with previous global studies,we conclude substantial inter-model differences in the representation of soil moisture−atmosphere coupling,and that most models underestimate such feedbacks.Focusing on dry spell analysis,most models underestimate increased sensible heat during periods of rainfall deficiency.For example,the model-mean bias in anomalous sensible heat flux is 10 W m−2(≈25%)smaller compared to observations.Deficient dry-spell sensible heat fluxes lead to a weaker atmospheric response.We also find that most GCms fail to capture the negative feedback between soil moisture and deep convection.The poor simulation of feedbacks in CmIP6 experiments suggests that forecast models also struggle to exploit soil moisture−driven predictability.To improve the representation of land−atmosphere feedbacks requires developments in not only atmospheric modelling,but also surface processes,as we find weak relationships between rainfall biases and coupling indexes.

Associations between maternal vitamin D status and porcine litter characteristics throughout gestation

Emerging evidence suggests an important role of vitamin D in the establishment and maintenance of pregnancy,and the regulation of foetal growth across mammalian species.However,the temporal changes in maternal vitamin D sta-tus throughout gestation in the pig and the relationship between maternal vitamin D status and litter characteristics of interest across gestation remain poorly understood and under-investigated.The abundance of 25(OH)D in maternal plasma was quantified by HPLC–mS/mS at gestational days(GD)18,30,45,60 and 90(n=5–11 gilts/GD).maternal plasma 25(OH)D concentrations significantly increased between GD18 and GD30(Pmaternal vitamin D metabolite concentrations and litter characteristics of interest including gilt weight,ovulation rate,mean litter weight,number of live foetuses,percentage prenatal survival,and sex ratio of the litter was assessed.maternal 25(OH)D(P=0.059)concentrations tended to be positively associated with percentage prenatal survival on GD60.On GD90,maternal 25(OH)D(Pmaternal plasma 25(OH)D concentrations were inversely associated with the percentage of male foetuses in the litter on GD90(Pmporal changes in maternal vitamin D status throughout ges-tation and the relationship between maternal vitamin D status and the economically important litter characteristics of gilt weight,percentage prenatal survival and percentage of male foetuses in the litter.Improving the understanding of the role of vitamin D across important developmental timepoints in relation to foetal growth is essential to improve reproductive success in livestock species.

This study uses the dynamic conditional correlation to investigate how technology subsector stocks interact with financial assets in the face of economic and financial uncertainty.Our results suggest that structural breaks have diverse effects on financial asset connectedness and that the level of bond linkage increases when the trend breaks.We see a growing co-movement between the technology sector and major financial assets when uncertainty is considered.Overall,our findings indicate that the connectedness response varies depending on the type of uncertainty shock.

BACKGROUND The diagnosis of type 2 diabetes(T2D)in younger adults,an increasingly common public health issue,is associated with a higher risk of cardiovascular complications and mortality,which may be due to a more adverse cardiovascular risk profile in individuals diagnosed at a younger age.AIm To investigate the association between age at diagnosis and the cardiovascular risk profile in adults with T2D.mETHODS A pooled dataset was used,comprised of data from five previous studies of adults with T2D,including 1409 participants of whom 196 were diagnosed with T2D under the age of 40 years.Anthropometric and blood biomarker measurements included body weight,body mass index(BmI),waist circumference,body fat percentage,glycaemic control(HbA1c),lipid profile and blood pressure.Univariable and multivariable linear regression models,adjusted for diabetes duration,sex,ethnicity and smoking status,were used to investigate the association between age at diagnosis and each cardiovascular risk factor.RESULTS A higher proportion of participants diagnosed with T2D under the age of 40 were female,current smokers and treated with glucose-lowering medications,compared to participants diagnosed later in life.Participants diagnosed with T2D under the age of 40 also had higher body weight,BmI,waist circumference and body fat percentage,in addition to a more adverse lipid profile,compared to participants diagnosed at an older age.modelling results showed that each one year reduction in age at diagnosis was significantly associated with 0.67 kg higher body weight[95%confidence interval(CI):0.52-0.82 kg],0.18 kg/m^(2) higher BmI(95%CI:0.10-0.25)and 0.32 cm higher waist circumference(95%CI:0.14-0.49),after adjustment for duration of diabetes and other confounders.Younger age at diagnosis was also significantly associated with higher HbA1c,total cholesterol,low-density lipoprotein cholesterol and triglycerides.CONCLUSION The diagnosis of T2D earlier in life is associated with a worse cardiovascular risk factor profile,compared to those

Biopolymers play a critical role as scaffolds used in tendon and ligament(TL)regeneration.Although advanced biopolymer materials have been proposed with optimised mechanical properties,biocompatibility,degradation,and processability,it is still challenging to find the right balance between these properties.Here,we aim to develop novel hybrid biocomposites based on poly(p-dioxanone)(PDO),poly(lactide-co-caprolactone)(LCL)and silk to produce high-performance grafts suitable for TL tissue repair.Biocomposites containing 1-15%of silk were studied through a range of characterisation techniques.We then explored biocompatibility through in vitro and in vivo studies using a mouse model.We found that adding up to 5%silk increases the tensile properties,degradation rate and miscibility between PDO and LCL phases without agglomeration of silk inside the composites.Furthermore,addition of silk increases surface roughness and hydrophilicity.In vitro experiments show that the silk improved attachment of tendon-derived stem cells and proliferation over 72 h,while in vivo studies indicate that the silk can reduce the expression of pro-inflammatory cytokines after six weeks of implantation.Finally,we selected a promising biocomposite and created a prototype TL graft based on extruded fibres.We found that the tensile properties of both individual fibres and braided grafts could be suitable for anterior cruciate ligament(ACL)repair applications.
