Objective: To compare the diagnostic accuracy of contrast enema (CE), anorectal manometry (ArM), and rectal suction biopsy (rSB) for the detection of Hirschsprung’ s disease (HD). Study design: Following a prospective pro-tocol, infants suspected of HD underwent all 3 index tests. Children with positive results on 2 or more index tests or who continued to have severe bowel problems underwent a full thickness biopsy as reference standard. Clinical follow-up was the reference standard in all other children. results: Between 2000 and 2003, 111 consecutive patients (67 boys; median age, 5.3 months) in whom HD was suspected were enrolled. HD was found in 28 patients. rSB had the highest sensitivity (93% ) and specificity (100% ) rates, but values were not significantly different from CE (sensitivity, 76% ; specificity, 97% ) or from ArM (sensitivity, 83% ; specificity, 93% ). Inconclusive test results occurred in 8 infants with CE, in 15 infants with ArM because of agitation, and in 2 infants with rSB. Conclusion: rSB is the most accurate test for diagnosing HD, and it has the lowest rate of inconclusive test results.

Ileal atresia is considered to be an acquired anomaly. It is thought to be caused by a mesenteric vascular accident during fetal life which leads to necrosis of downstream tissues. We present a case in which ileal atresia occurred after intrauterine laser coagulation therapy for a twin-to-twin transfusion syndrome. The laser treatment might have been the etiological factor of the atresia. A second case in which thrombo-embolic calcifications were found in the liver is presented as well.

星形胶质细胞膜蛋白MLC1的功能丧失是伴皮质下囊肿的巨脑性白质脑病(MLC)的主要遗传原因,这一罕见病的特征是脑内离子和水稳态失衡。MLC1蛋白主要分布于大脑中的液体屏障周围,例如在星形胶质细胞接触血管的终足和接触脑膜的突起中。该蛋白是否在其他星形胶质细胞结构域中发挥作用尚不清楚。本研究表明MLC1存在于星形胶质细胞远端突起中,也称为突触周围星形胶质细胞突起(PAP)或星形胶质细胞小叶;在海马CA1区域它们与兴奋性突触密切相互作用。我们发现在Mlc1缺失小鼠中,向兴奋性突触延伸的PAP末端缩短。这一改变会影响谷氨酸能突触传递,导致自发释放事件的速率降低,并在具有挑战性的条件下减慢谷氨酸的再摄取。此外,虽然野生型小鼠中的PAP在恐惧调节过程从突触回缩,但我们发现在Mlc1缺失小鼠中,这种结构可塑性在本已更短的PAP出现障碍。最后,Mlc1缺失小鼠显示情境恐惧记忆减少。总之,本研究揭示了星形胶质细胞蛋白MLC1在调节PAP结构方面的意想不到的作用。MLC1的缺失会改变兴奋性突触传递,阻止恐惧调节诱导的正常PAP重塑,并破坏情境恐惧记忆表达。因此,MLC1是调节星形胶质细胞-突触相互作用的新参与者。

背景：癌症患儿存活改善与多种治疗相关并发症相伴。然而，绝大多数有关癌症儿童生存率的研究仅局限于单一的远期效应。目的：在一个有完整的长期随访资料的大型儿童癌症生存队列中对癌症长期存活的不良预后（临床或亚临床病症）的总负荷进行评估。设计、地点及人群：于1362例已生存5年的儿童癌症患者中进行回顾性队列研究。患儿1966至1996年在荷兰一家医疗机构接受治疗。所有生存者均被邀请到晚期效应诊所进行不良事件医学评估。2004年1月以前发生的不良事件依据标准方式进行严重度分级。主要观测指标：随访结束时治疗别不良事件（依据严重性）的发生率以及各种治疗相关疾病高度或严重负荷（危险度92或危及生命或致残的不良事件≥1个）的相对危险度。结果：94．3％的生存者完成了随访（中位随访时间17年）。随访结束时中位年龄为24、4岁。大约75％的生存者有1个或更多不良事件，24．6％发生5个或以上不良事件。而且，40％的生存者至少有1个严重、危及生命或致残的不良事件。在接受单纯放疗的生存者中，55％的患者观察到高度或严重不良事件负荷。接受单纯化疗的生存者发生率为15％，而单纯手术治疗的存活者相应发生率则为25％（校正后相对危险分别为2．18[95％可信区间，1．62—2．95]和0．65[95％可信区间，0．46—0．90]）。不良事件高度或严重负荷最常见于骨肿瘤生存者（64％），较少见于白血病或Wilms瘤生存者（12％）。结论：在成年时，很大一部分儿童癌症生存者都存在高度或严重疾病负荷，尤其是在放疗后。这强调了对儿童癌症生存者根据危险分层进行终生医学监视的必要性。
