Relevance and Method: The purpose of this project was to evaluate the effectiveness of the Emergency Physiotherapy Practitioner (EPP) service against quality care indicators identified as part of the “gold standard” for emergency care in England. The study was prospective and evaluated time to initial assessment, total time in the emergency department and un-planned re-attendance rate within a seven-day period for all patients seen by the EPP’s over a period of one year. Outcomes: One thousand and seven patients were seen by EPPs in the emergency department. The median wait time for treatment by an EPP was 34.5 minutes (95th percentile = 122). Regional median wait time was 45 minutes (95th percentile = 138). National median wait time was 55 minutes (95th percentile = 192). Median total time spent in Ed for patients seen by EPPs was 99 minutes (95th percentile = 224). Regional median total time in Ed was 223 (95th percentile = 239). Nationally median total time in Ed was 136 minutes (95th percentile = 336). Three percent of patients seen by an EPP returned to the Ed, compared to 6% regionally and 7.5% nationally. Conclusions: EPPs excelled in all three indicators and exceeded regional and national figures. The re-return rate met the current standard of being less than 5%. It could be justified that the addition of the EPPs to the emergency department was an efficient and effective service development.

This paper describes the development of a monitoring system capable of detecting the concentration of magnesium ions(Mg^(2+)) released during the degradation of magnesium implants. The system consists of a microdialysis probe that samples fluid adjacent to the implant and a catalytic biosensor specific to Mg^(2+)ions. The biosensor was fabricated on a cotton fabric platform, in which a mixture of glycerol kinase and glycerol-3-phosphate oxidase enzymes was immobilized on the fabric device via a simple matrix entrapment technique of the cotton fibers. Pure magnesium was used as the implant material. Subsequently, the concentration of ions released from the degradation of the magnesium specimen in Ringer's solution was evaluated using cyclic voltammetry technique. The device demonstrated a pseudo-linear response from 0.005 to 0.1 mmol L^(-1) with a slope of 67.48 μA mmol^(-1) L. detectable interfering species were lesser than 1%indicating a high selectivity of the fabric device. Furthermore,the device requires only 3 μL of fluid sample to complete the measurement compared to spectroscopic method(±50 μL),hence providing a higher temporal resolution and reduced sampling time. The system could potentially provide a real time assessment of the degradation behavior, a new studied aspect in biodegradable metals research.

Why rehabilitation must have priority during and after the COVId-19-pandemic: a position statement of the Global Rehabilitation Alliance

COVId-19 has become a pandemic with strong influence on health systems. In many cases it leads to a disruption of rehabilitation service provision. On the other hand, rehabilitation must be an integral part of COVId-19 management. Rehabilitation for COVId-19 should start from acute and early post-acute care and needs to be continued in the post-acute and long-term rehabilitation phase. Of course, it should follow specific safety protocol. Additionally, rehabilitation must be kept available for all other people who are in need. From the perspective of health system, the Global Rehabilitation Alliance urges decision makers to ensure that rehabilitation services will be available for all patients with COVId-19 in the acute, post-acute and long-term phase. Additionally, it must be ensured that all other persons with rehabilitation need have access to rehabilitation services. Rehabilitation services must be equipped with personal protection equipment and follow strict hygiene measures. In particular, rehabilitation must be accessible for vulnerable populations. For that reason, rehabilitation must be kept a health priority during the COVId-19 pandemic and given adequate financial resources. Last but not least, scientific studies should be performed to clarify the impact of the pandemic on rehabilitation services as well as on the needs for rehabilitation of COVId-19 patients.
