The burden and psychosocial impact on immediate caregivers of patients with spinal cord injury in a tertiary health facility,Edo State,Nigeria

Objective:This study aimed to assess the burden and psychosocial impact of spinal cord injury on the immediate caregivers at a tertiary health facility in Benin City,Edo State.Materials and Methods:A descriptive cross-sectional survey design was adopted among 73 family caregivers in the neurological ward of a tertiary health facility using a structured questionnaire as instrument of data collection.Data collected were analyzed using descriptive statistics and multiple logistic regression.Results:Findings revealed that an overall mean of 2.73 indicates that the burden on the caregivers was moderate with a moderate psychological impact(m=2.88)and a high sociological impact(m=3.12).Factors affecting the caregiving provided by immediate caregivers include social and household responsibilities(94.5%),financial situation(89.0%),family support(83.6%),and health status of the caregiver(80.8%).Age(odds ratio[OR]=5.67,95%confidence interval[CI]:1.23-27.17,P=0.027),education(OR=3.75,95%CI:1.05-13.39,P=0.041),and“others”(spouses,siblings,friends,or extended family members)(OR=3.167,95%CI:1.583-6.337,P=0.001)were predictors for high psychological impact while education(OR=0.074;95%CI:0.015-0.370,P=0.001)and caregiving role(OR=3.167;95%CI:0.1.583-6.337,P=0.001)high sociological impact.Conclusion:Majority of the caregivers experience moderate burden,moderate psychological impact,and high sociological impact.Understanding these factors is essential for developing targeted interventions and support services to address the unique needs of caregivers and mitigate the burden of caregiving on their psychosocial health.

BACKGROUND With the emergence of severe acute respiratory syndrome coronavirus 2(SARSCoV-2) in late 2019 in Wuhan, China, liver injury in patients with coronavirus disease 2019(COVID-19) due to SARS-CoV-2 infection has been regularly reported in the literature. There are a growing number of publications describing the occurrence of secondary sclerosing cholangitis(SSC) after SARS-CoV-2 infection in various cases. We present a case of sudden onset SSC in a critically ill patient(SSC-CIP) following COVID-19 infection who was previously healthy.CASE SUMMARY A 33-year old female patient was admitted to our University Hospital due to increasing shortness of breath. A prior rapid antigen test showed a positive result for SARS-CoV-2. The patient had no known preexisting conditions. With rapidly increasing severe hypoxemia she required endotracheal intubation and developed the need for veno-venous extracorporeal membrane oxygenation in a setting of acute respiratory distress syndrome. During the patient’s 154-d stay in the intensive care unit and other hospital wards she underwent hemodialysis and extended polypharmaceutical treatment. With increasing liver enzymes and the development of signs of cholangiopathy on magnetic resonance cholangiopancreatography(MRCP) as well as endoscopic retrograde cholangiopancreatography(ERCP), the clinical setting was suggestive of SSC. At an interdisciplinary meeting, the possibility of orthotopic liver transplantation and additional kidney transplantation was discussed due to the constant need for hemodialysis. Following a deterioration in her general health and impaired respiratory function with a reduced chance of successful surgery and rehabilitation, the plan for transplantation was discarded. The patient passed away due to multiorgan failure.CONCLUSION SSC-CIP seems to be a rare but serious complication in patients with SARS-CoV-2 infection, of which treating physicians should be aware. To the Editor:Undifferentiated carcinoma with osteoclast-like giant cells(UCOGC)is one of eight histological subtypes of pancreatic ductal adenocarcinoma(PDAC)according to the 2019 WHO classification[1].It accounts for<1%of exocrine pancreatic carcinomas.This rare variant occurs in slightly younger patients than conventional PDAC,with a slight female predominance(male:female,7:10).

Human echinococcosis is a zoonosis caused by Echinococcus(E.)tapeworms.There are different species,but E.granulosus and E.multilocularis have clinical relevance causing cystic and alveolar echinococcosis,respectively.Cystic echinococcosis(CE)is defined as presence of one or more hydatid cysts,most frequently in the liver and the lungs,less usual in the kidneys,bones,spleen,pancreas,heart or brain.The location,the number and size of the cysts determine the severity of the disease[1].

Amphibians produce defensive chemicals which provide protection against both predators and infections.Within species,populations can differ considerably in the composition and amount of these chemical defenses.Studying intraspecific variation in toxins and linking it to environmental variables may help us to identify the selective drivers of toxin evolution,such as predation pressure and infection risk.Recently,there has been a renewed interest in the unique toxins produced by salamanders from the genus Salamandra:the samandarines.Despite this attention,intraspecific variation has largely been ignored within Salamandra-species.The aim of this study was to investigate whether geographic variation in profiles of samandarines exists,by sampling 4 populations of Salamandra atra over its range in the Dinaric Alps.In addition,we preliminary explored whether potential variation could be explained by predation(counting the number of snake species)and infection risk(cultivation and genomic analyses of collected soil samples).Salamanders from the 4 populations differed in toxin composition and in the size of their poison glands,although not in overall toxin quantity.Nor predation nor infection risk could explain this variation,as populations barely differed in these variables.Sampling over a much broader geographic range,using better estimators for predation and infection risk,will contribute to an improved understanding of how environment may shape variation in chemical defenses.Nevertheless,as the 4 populations of S.atra did differ in their toxin profiles,we propose that this species provides an interesting opportunity for further ecological and evolutionary studies on amphibian toxins.

Environmental mercury exposure, semen quality and reproductive hormones in Greenlandic Inuit and European men： a cross-sectional study

Several animal studies indicate that mercury is a male reproductive toxicant, but human studies are few and contradictory. We examined semen characteristics and serum levels of reproductive hormones in relation to environmental exposure to mercury. Blood and semen samples were collected from 529 male partners of pregnant women living in Greenland, Poland and Ukraine between May 2002 and February 2004. The median concentration of the total content of mercury in whole blood was 9.2 ng ml-1 in Greenland （0.2- 385.8 ng ml-1）, 1.0 ng m1-1 in Poland （0.2-6.4 ng ml-z） and 1.0 ng ml-I in Ukraine （0.2-4.9 ng ml-1）. We found a significantly positive association between the blood levels of mercury and serum concentration of inhibin B in men from Greenland （p=0.074, 95% confidence interval （CI）=0.021 to 0.126） and in an analysis including men from all three regions （p=0.067, 95% C1=0.024 to O. 110）. The association may be due to beneficial effects of polyunsaturated fatty acids （PUFAs）, which are contained in seafood and fish. No significant association （P〉O.05） was found between blood concentrations of mercury and any of the other measured semen characteristics （semen volume, total sperm count, sperm concentration, morphology and motility） and reproductive hormones （free androgen index {FAI）, follicle-stimulating hormone （FSH）, luteinizing hormone （LH）, testosterone and LH xtestosterone） in any region. In conclusion, the findings do not provide evidence that environmental mercury exposure in Greenlandic and European men with median whole blood concentration up to 10 ng m1^-1 has adverse effects on biomarkers of male reproductive health.

This paper explores approaches concerning complex forest planning challenges, such as restoration after large-scale disturbances and under climate change. It introduces a new framework that integrates qualitative scenario planning with quantitative multi-criteria decision analysis. This framework allows stakeholders without background in forestry to express their preferences as a set of scenarios that are further assessed for specific forest management goals and activities using multi-criteria models. The assessment of the modelled scenarios created a common understanding for the stakeholders and experts to compare trade-offs between several management options and needed policy choices. The framework was applied in the case study of forest restoration following insect disturbance in British Columbia, Canada. The framework enabled structured stakeholder groups’ interactions such as industry, business associations, local and regional governments, and non-governmental organizations to identify potential restoration options. Different community futures were envisioned by two scenarios: one resembling current conditions and standard practices, while another promoting diversification of the forestry sector. The results indicated that each of the scenarios leads to different consequences for the community measured by levels of economic benefits, total harvest volumes and harvest flows over time. The results also show that the developed framework linking scenarios and multi-criteria decision analyses proved crucial to broaden the discussion on relevant species mixes and management practices, and their implications for the community and policy development.

The paper starts from the theoretical model of multiculturalism as a possibility for an individual to develop their own identity, to accept and respect the identity of other members of a community. The aim of this research is to determine how the students of pedagogy define the concept of multiculturalism and in how far they consider it to be current in contemporary social relationships. The survey was carried out using the descriptive method and the analysis of contents. It examined the attitudes of students who are preparing themselves for advisory work with children in elementary school and high school. The results of the research confirm that the pedagogy students define the concept of multiculturalism clearly. They recognize the need for upbringing that fosters multiculturalism in modern education, in order to gain extensive knowledge and use it in a particular situation.

Android is the mobile operating system most frequently targeted by malware in the smartphone ecosystem,with a market share significantly higher than its competitors and a much larger total number of applications.Detection of malware before being published on official or unofficial application markets is critically important due to the typical end users'widespread security inadequacy.In this paper,a novel feature selection method is proposed along with an Android malware detection approach.The feature selection method proposed in this study makes use of permissions,API calls,and strings as features,which are statically extractable from the Android executables(APK files)and it can be used in a machine learning process with different algorithms to detect malware on the Android platform.A novel document frequency-based approach,namely Delta IDF,was designed and implemented for feature selection.Delta IDF was tested upon three universal benchmark datasets that contain Android malware samples and highly promising results were obtained by using several binary classification algorithms.
