BACKGROUND The impact of age on outcomes in cardiogenic shock(CS)is poorly described in the pre-hospital *** assessed the impact of age on outcomes of patients treated by emergency medical services(EmS).mETHODS This population-based cohort study included consecutive adult patients with CS transported to hospital by *** linked patients were stratified into tertiles by age(18-63,64-77,and>77 years).Predictors of 30-day mortality were assessed through regression *** primary outcome was 30-day all-cause *** A total of 3523 patients with CS were successfully linked to state health *** average age was 68±16 years and 1398(40%)were *** patients were more likely to have comorbidities including pre-existing coronary artery disease,hypertension,dyslipidemia,diabetes mellitus,and cerebrovascular *** incidence of CS was significantly greater with increasing age(incidence rate per 100,000 person years 6.47[95%CI:6.1-6.8]in age 18-63 years,34.34[32.4-36.4]in age 64-77 years,74.87[70.6-79.3]in age>77 years,Pmortality with increasing age *** adjustment,compared to the lowest age tertile,patients aged>77 years had increased risk of 30-day mortality(adjusted hazard ratio=2.26[95%CI:1.96-2.60]).Older patients were less likely to receive inpatient coronary *** Older patients with EmS-treated CS have significantly higher rates of short-term *** reduced rates of invasive interventions in older patients underscore the need for further development of systems of care to improve outcomes for this patient group.

中国是世界上鸟类多样性最丰富的国家之一。根据201 5年环境保护部和中国科学院发布的《中国生物多样性红色名录》评估报告,在中国现有的1372种鸟类中,列为极危、濒危和易危等级的受威胁鸟类有146种,区域灭绝的3种。本论文首次系统地研究了中国鸟类的灭绝模式和受威胁过程。我们分析和探讨了以下4个科学问题。(1)受威胁物种在各个科中的分布是否是随机的?(2)如果不是,哪些科含有比随机分布更多的受威胁物种?(3)哪些物种特征与灭绝危险有关?(4)物种特征之间的作用方式是累加的(additive)还是协同作用的(synergistic)?研究发现受威胁物种在各个科中的分布不是随机的。犀鸟科(Bucerotidae)、鹤科(Gruida)、八色鸫科(Pittidae)、雉科(Phasianidae)和鹰科(Accipitridae)含有比随机分布更多的受威胁物种。通过查阅文献获得了11个与灭绝相关的物种特征:身体大小、窝卵数、营养级、扩散能力、生境专属性、地理分布范围、巢址、巢的类型、集群状态、迁徙状态和捕猎敏感度(hunting vulnerability)。在控制系统发育的影响后,发现身体大小和捕猎敏感度的协同作用是预测中国鸟类灭绝危险的最佳模型。本研究表明,为了有效保护中国鸟类,我们应优先保护那些含有比随机分布更多的受威胁物种的科以及那些体型大和捕猎敏感度高的物种。

Purpose: The primary aim of this study was to develop an assessment of the fundamental, combined, and complex movement skills required to support childhood physical literacy. The secondary aim was to establish the feasibility, objectivity, and reliability evidence for the ***: An expert advisory group recommended a course format for the assessment that would require children to complete a series of dynamic movement skills. Criterion-referenced skill performance and completion time were the recommended forms of evaluation. Children, 8–12 years of age, self-reported their age and gender and then completed the study assessments while attending local schools or day camps. Face validity was previously established through a Delphi expert(n = 19, 21% female) review process. Convergent validity was evaluated by age and gender associations with assessment performance. Inter-and intra-rater(n = 53, 34% female) objectivity and test–retest(n = 60, 47% female) reliability were assessed through repeated test ***: median total score was 21 of 28 points(range 5–28). median completion time was 17 s. Total scores were feasible for all 995 children who self-reported age and gender. Total score did not differ between inside and outside environments(95% confidence interval(CI) of difference:-0.7 to 0.6;p = 0.91) or with/without footwear(95%CI of difference:-2.5 to 1.9; p = 0.77). Older age(p male gender(p mpletion time and substantial for skill score(ICC = 0.69) for 104 attempts by 53 children(34% female). Intra-rater objectivity was moderate(ICC = 0.52) for skill score and excellent for completion time(ICC = 0.99). Investigating the evolutionary history of widespread higher taxa, subjected to multiple tectonic events, can provide evidence for or against various palaeogeographical models of early Earth history [1,2]. Contemporary biotic distributions have been strongly influenced by events associated with the breakup of Gondwana into present-day Africa,Antarctica, Australia, South America,madagascar and India, during the Late mesozoic and Early Palaeogene [2]. The fragmentation of Gondwana and subsequent tectonic drift ultimately allowed biotic exchanges between Laurasia and Gondwana [2,3], influencing the global distributions of many taxa.

Dear Editor Previous research found that childhood cancer survivors of African ancestry have significantly higher morbidity and mortality than those of European ancestry[1].However,after adjusting for socio-economic factors,the magnitudes of racial health disparities are either substantially decreased or become statistically non-significant[1],suggesting that social and economic determinants may contribute to racial health ***,we conducted epigenome-wide association studies(EWAS)for three key social determinants of health(SDOHs),namely,personal educational attainment,personal income,and neighborhood deprivation among survivors of childhood cancer,where 130 epigenome-wide significant SDOH-CpG associationswere identified among European ancestry survivors,and 25 of which were also validated in African ancestry survivors[2].Notably,many SDOH-associated CpG sites are also associated with tobacco use.

Objective The purpose of this study was to assess knowledge of and barriers to osteopathic medicine in Chinese immigrant communities in New York City(NYC).Design A cross-sectional study was designed in which a culturally appropriate survey in Chinese and English versions was administered anonymously to measure immigrant perceptions and knowledge of osteopathic *** Data collection occurred in the municipal delineations for the Chinatown neighbourhood within the New York,New York borough of *** Community members were selected using convenience sampling from high-density areas to *** gathered from the survey included demographics,education level,healthcare habits and knowledge of the osteopathic *** 120 surveys were conducted with 68 males and 52 females,with an average age=*** in the age range of 18-29 years,those with fluent English-language proficiency,and participants with graduate-level education status demonstrated a higher proportion of knowledge of osteopathic manipulative medicine and osteopathic physicians(doctors of osteopathic medicine)among the study *** Compared with research on the general US population,a general lack of knowledge of osteopathic medicine exists within NYC’s Chinese immigrant *** this difference may be ascribed to linguistics and ethnosociological factors,greater outreach and education is needed in urban minority communities to make immigrants aware of all healthcare resources available during the current shortage of US primary care physicians.

Hyperhidrosis of Central Hypogonadism Leading to Diagnosis of Shapiro Syndrome: Temperature Dysregulation, Hypothermia and Agenesis of the Corpus Callosum

Background: Central or hypothalamic hypogonadism as an initial manifestation of Shapiro Syndrome has not been described in the literature. Herein, we report first case in which initial presentation of central hypogonadism led to a confirmed diagnosis of Shapiro Syndrome during a casual evaluation of hypothalamic pituitary anatomy with mRI of brain. Case presentation: 53 year old Caucasian man was documented to manifest Central or hypogonadotropic hypogonadism following evaluation of excessive sweating episodes, lack of libido and erectile dysfunction for a duration of several years. Brain mRI performed for assessment of the etiology documented no pituitary abnormality. Instead agenesis of Corpus Callosum was noted. The subject had been hospitalized on many occasions at this and several other medical centers with hypothermia or hyperthermia without a documentation of a definite cause. Therefore, the diagnosis of Shapiro Syndrome was made. Conclusion: This report is the first documentation of subject manifesting central, more likely to be hypothalamic rather than hypogonadotropic hypogonadism in conjunction with Shapiro Syndrome.

Predominantly antibody deficiency(PAD)is the most prevalent form of primary immunodeficiency,and is characterized by broad clinical,immunological and genetic *** the current gold standard of whole exome sequencing for diagnosis,pathogenic gene variants are only identified in less than 20% of *** elucidation of the causal genes underlying PAD has provided many insights into the cellular and molecular mechanisms underpinning disease pathogenesis,many other genes may remain as yet undefined to enable definitive diagnosis,prognostic monitoring and targeted therapy of *** that many patients display a relatively late onset of disease presentation in their 2^(nd) or 3^(rd) decade of life,it is questionable whether a single genetic lesion underlies disease in all ***,combined effects of other gene variants and/or non-genetic factors,including specific infections can drive disease *** this review,we define(1)the clinical and immunological variability of PAD,(2)consider how genetic defects identified in PAD have given insight into B-cell immunobiology,(3)address recent technological advances in genomics and the challenges associated with identifying causal variants,and(4)discuss how functional validation of variants of unknown significance could potentially be translated into increased diagnostic rates,improved prognostic monitoring and personalized medicine for PAD patients.A multidisciplinary approach will be the key to curtailing the early mortality and high morbidity rates in this immune disorder.
