Prophylactic endovascular management of peripheral artery disease in elderly candidates prior to cardiac surgery

Background and objectives Peripheral vascular disease (PVD) is a major risk factor in candidates for cardiac surgery and can impact morbidity and mortality in the perioperative and follow-up period. Elderly patients with PVD may benefit from endovascular treatment prior to cardiac surgery. We sought to assess the common clinical settings requiring prophylactic endovascular treatment before coronary surgery in elderly patients, the results, and the mid-term impact on subsequent revascularization. Methods Between November 2002 and June 2006, 37 patients (25 males, mean age 79.9±8.3 years, mean serum creatinine 1.9±0.6 mg/dl) underwent endovascular repair of PVD before cardiac surgery. For each patient, diagnostic methods, indications for intervention, types of interventions, procedural success, and complications were recorded. Results Four clinical settings were identified: renal artery stenting prior to coronary surgery (7 patients), iliac artery angioplasty and stenting (10 patients) in order to facilitate aortic balloon pump insertion after surgery, subclavian artery angioplasty and stenting prior to utilization of ipsilateral arterial conduits bypass surgery (5patients), and carotid artery stenting before coronary surgery (15 patients). Technical success was achieved in all patients (100%);complications included brachial artery occlusion (1 patient), minor stroke (2 patients), contrast nephropathy (1 patient), and minor bleeding at the puncture site (3 patients). All patients underwent successful coronary or valvular surgery; no patients died in the perioperative period. After a mean follow-up of 26.6±3.1 months, all patients are alive and free from anginal symptoms or valvular dysfunction without clinical or Doppler ultrasonography evidence of restenosis of the implanted peripheral vascular stents. The amount of photosynthetic radiation inter- cepted by a crop is a function of the incident solar radiation on the plants, the leaf area index （LAI）, and the light extinction coefficient （k）. We quantified LAI and k in stands of black wattle （Acacia mearnsii De Wild.） over a 7-year growth cycle at two locations in the state of Rio Grande do Sul, Brazil. Our study was conducted in commercial stands in agroecological regions with high densities of black wattle plantations. LAI was calculated as the ratio between the leaf area of a tree and its planting space, and k was derived from Beer＇s law. LAI depends on the planting site and stand age. Between the two sites, the LAI was similar over time, the amount of variation differed. Values of k depended only on stand age, with the highest average observed for stands up to 5 years old. The trend of k during the plantation cycle was inversely proportional to LAI and was correlated with LAI, leaf area, leaf dry mass, canopy volume, height, branches dry mass, total dry mass, and crown diameter.

Games often provide a good introduction to interesting phenomena in mathematics. In this note, we examine three variations of an iterative sharing game played around a circular (or not so circular) table. More precisely, for each variation, we study the tendency toward equal distribution among the players. In the first variation, the players have discrete amounts at each step. The second variation removes this restriction, and the third one considers an infinitely long table with an infinite number of players.

Dermatitis herpetiformis(DH)is a gluten-sensitive autoimmune blistering disorder with a chronic-relapsing *** recently,several Authors reported atypical cases of patients with DH,suggesting that different clinical subsets may exist at least among different ethnicities and that the classical picture of DH probably need a significant ***,different pathogenetic aspects of the disease are currently under investigation,including the role of epidermal transglutaminase,apoptosis and inflammatory cells in the occurrence of skin lesions,in order to explain why only a subgroup of celiac patients will develop ***,although glutenfree diet is still regarded as the only curative approach to the disease,it is very hard to comply with and even small amounts of gluten can re-activate the ***,different therapeutical approaches for the spectrum DH/celiac disease are still under *** the present paper,the most recent advances in DH will be discussed,and a novel interpretation of the disease based on the data emerging from the Literature will be proposed.

5-Fluorouracil(5-FU)is an anticancer drug extensively used for different *** metabolic activation leads to several nucleoside and nucleotide metabolites essential to exert its cytotoxic activity on multiple cellular targets such as enzymes,DNA and *** this paper,we describe the development of a method based on liquid chromatography coupled with high resolution mass spectrometry suitable for the simultaneous determination of the ten anabolic metabolites(nucleoside,nucleotide and sugar nucleotide)of *** chromatographic separationwas optimized on a porous graphitic carbon column allowing the analysis of the metabolites of 5-FU as well as endogenous *** detection was performed on an Orbitrap■tandem mass *** of the method was verified in intracellular content and in RNA *** limit of detection was equal to 12 pg injected on column for nucleoside metabolites of 5-FU and 150 pg injected on column for mono-and tri-phosphate nucleotide *** effect was evaluated in cellular contents,DNA and RNA extracts for nucleoside and nucleotides *** method was successfully applied to i)measure the proportion of each anabolic metabolite of 5-FU in cellular contents,ii)follow the consequence of inhibition of enzymes on the endogenous nucleotide pools,iii)study the incorporation of metabolites of 5-FU into RNA and DNA,and iv)to determine the incorporation rate of 5-FUrd into 18 S and 28 S sub-units of rRNA.

Simulating Microbial Functional Diversity Dynamics in Agricultural Soils: An Individual Based Modeling Approach

New agricultural soil model approaches based on the microbiome dynamics must be considered as they can contribute to understand microbiological soil processes directly linked to substrate metabolism and the influence of these processes on plant growth. The present work presents an approach to the modelling of the interactions of the soil microbial functional diversity with the plant in terms of functions associated to specific processes of organic Carbon and Nitrogen metabolism. The substrates transformations arising in the organic matter that enters as a part of an agricultural scheme are the base for define this metabolism. As result, it has been possible to simulate a rhizospheric soil based on the concept of complex system dynamics and Individual Based Modeling known too as Agent-Based Modeling in an agricultural management context. The explicit definition of the microbiome functional diversity and the processing of the structural elements Carbon and Nitrogen, allowed representing the functional dynamics of this complex system composed by microorganisms, Carbon, Nitrogen and the plant. The variables that reflect the biology and the adaptation to the rhizospheric environment characterized the microorganisms and the assemblage community patterns in time. The main simulations output are system glucose and nitrate levels and an approach to plant growth, all resultant from the metabolic process of the considered Carbon and Nitrogen consortia. The results indicate that the microorganism’s diversity assemblages and its functional expression have a fundamental role in terms of plant growth.
