Orthostatic hypotension (OH)is a common condition in older *** to the 2011criteria,OH is defined by a sustained reduction of systolic blood pressure of 20 mmHg or a 10mmHg-fall of diastolic blood pressure,[1] Its prevalence increases with age and according to a recent systematic review the pooled prevalence in community dwelling older people is 22.2%.[2]Although OH is largely explained by concurrent clinical conditions (in particular, hypertension,diabetes,malnutrition,vascular encephalopa- thy,anemia,Parkinson's disease)[3],aging per se determines a series of physiological dysfunctions potentially representing its biological substratum (e.g.,reduction in barore- flex-mediated cardiovascular function,altered salt and water balance at renal level,impaired cardiac diastolic filling).It is also noteworthy that age-related changes in pharmacokinetic and pharmacodynamic mechanisms of drugs (especially in the context of polypharmacy)also increase the risk of OH.

This study aimed to identify the key physical abilities (aerobic endurance, gait speed, balance, strength) and psychological variables associated with the level of basic (BADL) and instrumental (IADL) activities of daily living in an autonomous community-dwelling elderly population in Italy. 135 elderly people (63% women;mean age = 73.3, SD = 5.5) were included in the study. Stepwise regression was performed to verify the association between these variables and the level of BADL and IADL in the elderly participants. Results showed that balance (β = -0.21, p of physical functioning (β = 0.32, p L scores, and IADL score was associated with perception of physical functioning (β = 0.30, p 001) and upper limb strength (β = 0.21, p < 0.05). The results demonstrate a relationship between physical functioning and ADL, both real physical functioning and perceived physical functioning.

Replacement Meal: Effectiveness of a Soluble Powder Rich in Fibers and at Low Glycemic Index in Overweight but Not in Obese Patients

Active lifestyle and qualitative and quantitative diets are the best way to lose weight. In our study we have assessed the effectiveness and tolerability of a soluble powder rich in fibers and at low glycemic index and used, with a portion of fresh fruit, as replacement meal (for a total calorie intake ranging from a minimum of about 130 kcal to a maximum of 260 kcal) in 52 overweight/obese subjects of the outpatient clinic of the Italian National Research Council (CNR) of Rome already in treatment with a low-fat low-carbohydrates qualitative diet. The study was a parallel, prospective trial of two randomized sample groups, “intervention” (26 subjects) and “control” (26 subjects). After 8 weeks, the treatment based on the replacement meal has improved the results obtained through the qualitative diet alone above all in the overweight subjects: 11/16, versus only 2/16 in the control group, became ranged within normal weight. On the contrary, the majority of obese subjects didn’t change their BMI classification suggesting that, in case of obesity, a controlled quantitative correction with weighing food portions could be a better strategy than low-calorie replacement meal.

The use of calcium hydroxide nanoparticles (nanolime) in hydro-alcoholic suspensions represents a good enhancement in particular in the conservation of stones and surfaces of interest in Cultural Heritage. In this work we have produced nanolime by using our alternative method of synthesis (based on the use of a surfactant agent in the original solution) and we have studied, for the first time, structural and morphological properties of the synthesized nanoparticles directly in hydro-alcoholic suspension by synchrotron X-ray diffraction technique and transmission electron micro-scope observations. The particles characterization was performed on suspensions synthesized by varying surfactant content in the synthesis procedure and the water/alcohol ratio, too. We have found that only pure and crystalline Ca(OH)2 particles produced during the synthesis procedure and they maintained stably in suspension. These particles were hexagonally plated and regularly shaped with side dimension ranging from 300 to 40 nm or less;they had a platy habit and they presented, particularly with high alcohol content in suspension, a preferred orientation along z-axis.

Contribution to the Knowledge of Biodeteriogenic Flora on Three Historical Calabrian (Southern Italy) Churches

The effect of biodeteriogenic living organisms on historical and archeological sites is a well-known problem affecting the world’s cultural heritage. Indeed, the growth inside the bricks of shrubs and herbs roots can cause breakage, collapse and, detachment of materials. The knowledge of the spontaneous vascular flora present on monuments is essential for protection and safeguarding of cultural heritage. In this paper, we identify and describe biodeteriogenic vascular flora of three historical Calabrian (Southern Italy) churches, showing the relationship between plant biodiversity and exposure and building material of churches. The species present on the wall and in perimeter for each church were sampled noting substrate type, surface inclination (vertical or horizontal) and exposure. The total number of plant species recorded was 27. Hemicryptophytes (41%) prevail over the other biological forms and the most represented families are that of the Asteraceae. Parietaria judaica is the only species present in all three sites. Substrate type, climatic condition and surface inclination affect the floristic composition. Sixteen out of 27 species grow on vertical surfaces, all Geophythes and Chamaephytes grow on horizontal surfaces, while Therophytes were rather on vertical surfaces. A significant difference was found in the Hazard Index values in the three churches. Only one site, presents a high average value of Hazard Index, due to the simultaneous presence of Ailanthus altissima, Ficus carica, Rubus ulmifolius and Sambucus nigra. The data collected indicate that the flora growing on wall of three Calabrian churches partially reflects the floristic context in which they are located. The diversity of the vegetational spectrum is remarkable, most of the plants are ruderal herbaceous, many weeds and invasive, and for the most part typical of the Mediterranean environment. 1. Introduction The tegmental wall of the tympanic cavity is a thin plate of the temporal bone that separates the middle cranial fossa(MCF) from the ear. This anatomical region consists of two areas: an anterior one, comprised of the tegmen tympani(To′th et al., 2007), and a posterior one, formed by the tegmen antri and the tegmen mastoideum(Makki et al., 2011). In some patients, the tegmental region of the temporal bone can be interrupted, causing a tegmen defect(TD). A TD is sometimes associated with a meningoencephalic herniation(MEH), in which brain tissue herniates through a TD.

Multi-phase post-mortem CT-angiography （MPMCTA） has the great potential to increase the quality of the postmortem investigation, especially in the area of sudden death; however, its role as routine complement to the pathology toolbox is still questioned as it needs to be further standardized. The aim of this study is to investigate the contribution of MPMCTA in cases of sudden unexplained death in adults and in particular in sudden cardiovascular death. Sixty-eight sudden unexpected deaths of adults were investigated at our institution between 2012 and 2013. Ten cases underwent MPMCTA and autopsy and were included in the study. Before the angiographic step by complete filling of the vascular system, prior to any manipulation of the body, a non-contrast CT-scan was carried out. Image reconstructions were performed on a CT workstation （Vitrea） and two radiologists experienced with post mortem imaging interpreted the MPMCTA findings. In all 10 cases, we could state a good correlation between combination of post-mortem CT and MPMCTA and autopsy procedures, confirming a high diagnostic sensitivity. With this case series we want to illustrate the advantages offered by performing MPMCTA when facing a sudden death, regardless of specific suspicion for acute coronary syndrome or other vascular or ischemic disease.

作为一名罗马尼亚留学生,我在学习的过程中曾一度困扰于汉语定语规则的严格与复杂,如多项定语语序规律、汉语定语标记“的”的隐现规律以及汉罗定语不同的使用规则等,这些都是罗马尼亚学生学习汉语定语的难点,因此本文要就罗马尼亚汉语学习者习得定语的情况展开研究,以提高罗马尼亚汉语学者的学习效果。本文共分为四章,具体内容如下:第一章为绪论,介绍了本文的研究目的、研究意义及研究方法,并从汉语定语本体研究及汉语定语习得研究两个方面对相关研究做了述评。第二章从定语类型、定语与中心语的语义关系、多项定语语序及定语标记隐现规律四个方面对汉罗定语进行了比较与分析,发现了汉罗定语一一对应、不完全对应及不对应的部分。第三章首先以问卷调查的形式对罗马尼亚汉语学习者学习和使用定语的难点进行了调查,然后以调查数据为依据,归纳、总结、描写了偏误类型,发现偏误率最高的是多项定语语序的使用。第四章根据前文发现的偏误类型深入挖掘偏误原因,并对此进行了较为全面的分析,主要归纳出母语负迁移、目的语知识泛化及汉语语法的复杂性三种偏误原因。之后,根据实际情况提出了具有针对性及可行性的解决建议,分别为:加强汉罗定语对比、培养目的语语感以及课堂上精讲多练、增加有意义的练习。

The present study aims at verifying whether participation in a physical activity programme has positive effects on the daily life autonomy and vitality of elderly people living in residential care facilities by the mediation of their physical well- being. Fifty-one institutionalised individuals took part in the study. The control group included 11 people (84.26, ± 7.4 years), whereas the experimental group was made up of 40 people (85 ± 6.6 years). The experimental group was involved in a physical activity programme twice a week. The 36-Item Short Form Health Survey Questionnaire, the Activities of Daily Living Scale, and the Tinetti Test were administered to the participants. The linear regression method as well as Sobel’s formula were used for the analysis. The results show that participation in a physical activity programme has positive effects on autonomy in bathing and on the participants’ sense of vitality due to the mediation of physical functioning. These results confirm the importance of physical activity for the elderly populations living in residential care facilities.

Chronic heart failure （CHF） is a highly prevalent condition among the elderly and is associated with considerable morbidity, institution-alization and mortality. In its advanced stages, CHF is often accompanied by the loss of muscle mass and strength. Sarcopenia is a geriatric syndrome that has been actively studied in recent years due to its association with a wide range of adverse health outcomes. The goal of this review is to discuss the relationship between CHF and sarcopenia, with a focus on shared pathophysiological pathways and treatments. Mal- nutrition, systemic inflammation, endocrine imbalances, and oxidative stress appear to connect sarcopenia and CHF. At the muscular level, alterations of the ubiquitin proteasome system, myostatin signaling, and apoptosis have been described in both sarcopenia and CHF and could play a role in the loss of muscle mass and function. Possible therapeutic strategies to impede the progression of muscle wasting in CHF patients include protein and vitamin D supplementation, structured physical exercise, and the administration of angiotensin-converting enzyme inhibitors and β-blockers. Hormonal supplementation with growth hormone, testosterone, and ghrelin is also discussed as a potential treatment.
