Systemic inflammation and immune response after laparotomy vs laparoscopy in patients with acute cholecystitis, complicated by peritonitis

AIM: To evaluate acute cholecystitis, complicated by peritonitis, acute phase response and immunological status in patients treated by laparoscopic or open approach. METHODS: From January 2002 to May 2012, we conducted a prospective randomized study on 45 consecutive patients (27 women, 18 men; mean age 58 years). These subjects were taken from a total of 681 patients who were hospitalised presenting similar preoperative findings: acute upper abdominal pain with tenderness, involuntary guarding under the right hypochondrium and/or in the flank; fever higher than 38 ℃, leukocytosis greater than 10 × 10 9 /L or both, and ultrasonographic evidence of calculous cholecystitis possibly complicated by peritonitis. These patients had undergone cholecystectomy for acute calculous cholecystitis,complicated by bile peritonitis. Randomly, 23 patients were assigned to laparoscopic cholecystectomy (LC), and 22 patients to open cholecystectomy (OC). Blood samples were collected from all patients before operation and at days 1, 3 and 6 after surgery. Serum bacteraemia, endotoxaemia, white blood cells (WBCs), WBC subpopulations, human leukocyte antigen-DR (HLA-DR), neutrophil elastase, interleukin-1 (IL-1) and IL-6, and C-reactive protein (CRP) were measured at 0, 30, 60, 90, 120 and 180 min, at 4, 6, 12, 24 h, and then daily (8 A.M.) until post-operative day ***: The two groups were comparable in the severity of peritoneal contamination as indicated by the viable bacterial count (open group = 90% of positive cultures vs laparoscopic group = 87%) and endotoxin level (open group = 33.21 ± 6.32 pg/mL vs laparoscopic group = 35.02 ± 7.23 pg/mL). Four subjects in the OC group (18.1%) and 1 subject (4.3%) in the LC group (P < 0.05) developed intra-abdominal abscess. Severe leukocytosis (range 15.8-19.6/mL) was observed only after OC but not after LC, mostly due to an increase in neutrophils (days 1 and 3, P < 0.05). The discussion about the use of the bicycle as a means of transport has been gaining importance on a global scale,although countries like Brazil still have urban mobility difficulties caused by how urban densification occurs,the growing number of motor vehicles and the lack of space for the construction of cycle paths in *** general objective of this manuscript is to analyze the result of public policies aimed at the implementation of bicycle paths,with the“Passo Fundo Vai de Bici”Program created to investigate the variations in intensity in relation to the areas served by the current incentive for active cycling mobility in the city of Passo Fundo(South of Brazil).Methodologically,Google Earth Pro satellite images were used to map the existing bike paths in the ***,geographic coordinate points and altitudes of specific locations on the bike paths and bike sharing stations were collected,generating a database,transferred to the Qgis 3.10 software for the preparation of heat *** results showed the lack of integration between the cycling systems analyzed by the distance factor,needing to encourage discussions about its expansion and functionality,consolidating it as an urban public policy of local functionality to foster sustainable transport.

Antioxidant Activity of Cosmetic Formulations Based on Novel Extracts from Seeds of Brazilian <i>Araucaria angustifolia</i>(Bertoll) Kuntze

The process of oxidation, due to free radicals, is the cause of major concern for human health. In particular damages related to the skin have great relevance;therefore, many antioxidants based products are developed and marketed with the intention to counteract the action of free radicals. The seed of Araucaria angustifolia is a rich source of antioxidants due to the presence of bioflavonoids to counteract free radicals damages. In this study, two extracts, one from the seed external teguments (shell) and the other from the inner seed pulp (endosperm and embryo) were obtained in order to evaluate possible applications to the dermo-cosmetic field. In parallel the following different methods were employed to characterize both the extracts and to determine their antioxidant capacity: HPTLC, ABTS and DPPH, ORAC and PLC. The qualitative analysis showed that both extracts have the antioxidant activity, but the quantitative evaluation revealed a more promising bioactivity from the shell than from the pulp. Therefore, it was evaluated the potential skin application of different cosmetic formulations, based on the presence of seed shell extract (W/O emulsion, O/W emulsion and gel). The best result was achieved with the W/O emulsion.

目的,通过分析欧洲设计硕士交换教育项目(全文后简称MEDes项目)的运行现状和相关数据,反思其利弊与前车之鉴。方法展示提取包括工作机制、合作学院在内的MEDes项目的基本情况,从项目学校基本情况、生源学生专业分布、学生交换去向学校三个方向去分析所得数据,最后从"要肯定的"、"要改进的"和"要反思的"三个反面入手总结其经验得失。结论 MEDes项目作为欧盟内部交换项目成功运行十余年,这对和欧盟体量相当,但设计高校和学生数量巨大的中国有重要的借鉴意义。但总体上看,中国需要类似MEDes项目的出现来平衡教育资源、增长学生知识以及传承本土文化。

Bifidobacteria as Potential Functional Starter Cultures: A Case Study by MSc Students in Food Science and Technology (University of Foggia, Southern Italy)

This research paper was the results of activity of MSc students of Food Science and Technology, attending the class “Biotechnology of Functional Starter”. Five strains of bifidobacteria (Bifidobacterium animalis subsp. lactis;B. longum subsp. infantis;B. breve;B. animalis subsp. animalis;B. bifidum) were evaluated in order to assess their suitability as functional starter cultures, by studying the following technological and probiotic traits: growth at different temperatures, NaCl amounts and pH values;acidifying ability;metabolism (arginin deamination, esculin hydrolysis, acetoin production);survival at low pH and in presence of bile salts;hydrophobic properties;antibiotic resistance. After laboratory assays and strain selection through a multivariate analyses, it was highlighted that B. longum subsp. infantis and B. animalis subsp. lactis represent a good compromise as potential functional starter cultures, as B. animalis subp. lactis showed a high growth index at pH 5 and good values at 25?C and 30?C, as well as the minimal viability loss at pH 2.5. B. longum subsp. infantis DSMZ 20088 was the best microorganism for its growth index in presence of 6.5% of salt added and at 25?C and 30?C.

Rituximab has provided a revolutionary contribution to the treatment of B-cell non-Hodgkin’s lymphomas (NHL). A high prevalence of hepatitis C virus (HCV) infection has been described in B-cell NHL patients. Cases of liver dysfunction in HCV-positive patients have been reported with Rituximab-containing regimens. In this paper we review the recent data regarding the effects of Rituximab in NHL patients with HCV infection. We also added a section devoted to improving communication between oncohaematologists and hepatologists. Furthermore, we propose a common methodological ground to study hepatic toxicity emerging during chemotherapy.
