Eosinophilic esophagitis is a newly recognized disease first described about 50 years ago.The definition,diagnosis,and management have evolved with new published consensus guidelines and newly approved treatment available to pediatricians,enabling a better understanding of this disease and more targeted treatment for patients.We describe the definition,presentation,and diagnosis of eosinophilic esophagitis including management,challenges,and future directions in children.The definition,diagnosis,and management of eosinophilic esophagitis have evolved over the last 50 years.Consensus guidelines and newly approved biologic treatment have enabled pediatricians to better understand this disease and allow for more targeted treatment for patients.We describe the definition,presentation,diagnosis,management,and treatment in addition to the challenges and future directions of eosinophilic esophagitis management in children.

Plants have to cope with several abiotic stresses,including salinity and heavymetals(HMs).Under these stresses,several extracts have been used as effective natural biostimulants,however,the use of Spirulina platensis(SP)extract(SPE)remains elusive.The effects of SPE were evaluated as soil addition(SA)and/or foliar spraying(FS)on antioxidant defenses and HMs content of common bean grown in saline soil contaminated with HMs.Individual(40 or 80 mg SPE/hill added as SA or 20 or 40 mg SPE/plant added as FS)or integrative(SA+FS)applications of SPE showed significant improvements in the following order:SA-80+FS-40>SA-80+FS-20>SA-40+FS-40>SA-40+FS-20>SA-80>SA-40>FS-40>FS-20>control.Therefore,the integrative SA+FS with 40 mg SP/plant was the most effective treatment in increasing plant growth and production,overcoming stress effects and minimizing contamination of the edible part.It significantly increased plant growth(74%–185%)and yield(107%–227%)by enhancing net photosynthetic rate(78.5%),stomatal conductance(104%),transpiration rate(124%),and contents of carotenoids(60.0%),chlorophylls(49%–51%),and NPK(271%–366%).These results were concurrent with the marked reductions in malondialdehyde(61.6%),hydrogen peroxide(42.2%),nickel(91%–94%),lead(80%–9%),and cadmium(74%–91%)contents due to the improved contents of glutathione(87.1%),ascorbate(37.0%),andα-tocopherol(77.2%),and the activities of catalase(18.1%),ascorbate peroxidase(18.3%),superoxide dismutase(192%),and glutathione reductase(52.2%)as reinforcing mechanisms.Therefore,this most effective treatment is recommended to mitigate the stress effects of salinity and HMs on common bean production while minimizing HMs in the edible part.

Objective:Over 50% of the world populations are infected with Helicobacter pylori (H. pylori). Most subjects are asymptomatic;however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. Methods:Thirty gastric cancer patients (GCs) and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results:H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients (100%) while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients (by at least one of its alleles) while it was only found in 33.3% of gastritis patients. The vacA s1m1 combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients (10%). The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple H. pylori strains was proved, both phenotypically and genotypically. Conclusion:The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

Aim: To investigate depression, anxiety, PTSD, and insomnia levels among Sudanese citizens after a year has passed since the start of Armed Forces conflict in Sudan. Methods: An online survey was distributed. It is composed of five parts, covering the following areas: 1) sociodemographic data;2) depression assessment;3) generalized anxiety disorder assessment;4) post-traumatic stress disorder assessment;and 5) insomnia assessment. Statistical Package for Social Sciences version 27 was used for data analysis;frequency and percentage were used to describe the qualitative variables. Spearman’s correlation analysis and Chi-square test were used for correlation and association analysis;a P-value equal to or less than 0.05 was considered statistically significant. Results: The study included 283 participants, primarily female (76.0%), with a mean age of 35.26 ± 6.96 years. The majority of participants were married (50.5%) and had a university level of education (50.2%). At the start of the war, 70.3% of participants were inside the war zones, and at the time of data collection, 73.5% were outside Sudan. The length of stay in the war zone ranged from 1 to 300 days, with a mean of 48.59 ± 70.284 days. Additionally, 63.3% of participants stated that they did not receive any form of mental health intervention, and the remaining did not provide an answer. There was a high prevalence of moderate-severe depression (50.5%), moderate anxiety (35.3%), PTSD (56.5%), and sub-threshold insomnia (53.4%) among the participants. Correlation analyses revealed associations between demographic factors such as sex, age, marital status, and education and mental health issues. Notably, female participants showed a higher prevalence of PTSD (53.0%) compared to male participants (67.6%). Conclusions: There is a substantial impact of war-related trauma on mental health, revealing a high prevalence of moderate-severe depression, moderate anxiety, PTSD, and sub-threshold insomnia among the participants. Populus alba is a large woody deciduous plant.The plant has been introduced to shooting,then multiplication of rooting on Murashige and Skoog(MS)medium.This work was designed to estimate the effect of two factors(low levels of 1-Naphthaleneacetic acid NAA and sucrose)on P.alba response resulting in 6 treatments compared to the control,with twelve measured responses.There was a significant difference in some measurements in morphology,like plantlets fresh-weight,shoot-,root-length,and leaf number.In the physiological measurements,there were significant differences in all the measured parameters.The low concentrations of sucrose and media composition/power(MS grams/L)led to starvation in plants;however,these conditions led to enhancement in some morphological and physiological parameters to overcome the starvation effect,compared to the control.The RAPD-PCR molecular marker(four decamers)was used to evaluate the new individuals’genetic variation(instability),resulting in a total polymorphism percentage of 50.83%.It was formerly known that the plantlets were identical to each other and to the mother plant.In this study,however,the use of distinct media power,hormonal and sucrose levels resulted in molecular variation reflected in P.alba’s morphological and physiological responses.

BACKGROUND Helicobacter pylori(H.pylori)is a significant human pathogen that is responsible for a variety of illnesses,including mucosa-associated lymphoid tissue lymphoma,gastric cancer,peptic ulcers,and gastritis.AIM To investigate the frequency of H.pylori infection and its resistance patterns among Egyptian patients and to determine the influence of H.pylori virulence genetic determinants on the eradication success of 14-d triple therapy regimen.METHODS H.pylori infections were investigated in 72 patients with gastroduodenal complications suggestive of H.pylori infection.The cagA and vacA genotypes of cultured strains were studied using polymerase chain reaction.The patients underwent 14 d of triple-therapy treatment.The treatment response was examined using histology and a rapid urease test 6 wk after therapy discontinuation.RESULTS The intention-to-treat eradication rate was 59.2%(95%CI:48.2%-70.3%).Rates of H.pylori resistance to clarithromycin,amoxicillin,and metronidazole were 52.8%,81.9%,and 100%,respectively.Successful eradication of H.pylori was more significantly associated with vacA s1-positive strains[adjusted odds ratio(aOR)=0.507,95%CI:0.175-0.822].A significant association was found between failed eradication rate and H.pylori strains resistant to clarithromycin(aOR=0.204,95%CI:-0.005 to 0.412)and amoxicillin(aOR=0.223,95%CI:0.026-0.537).CONCLUSION This study’s low H.pylori eradication rate following 14-d triple therapy is concerning and worrying.H.pylori pan-resistance to metronidazole followed by the high resistance to ciprofloxacin,amoxicillin,and clarithromycin in this research is challenging and of great concern.

Evaluation of children and adults with post-COVID-19 persistent smell,taste and trigeminal chemosensory disorders:A hospital based study

BACKGROUND Smell disorders are the most frequent persistent coronavirus disease 2019(COVID-19)complications.AIM To describe the patterns and characteristics of persistent smell and taste disorders in Egyptian patients.METHODS Assessment was done to 185 patients(adults=150,age:31.41±8.63 years;children=35;age:15.66±1.63 years).Otolaryngology and neuropsychiatric evaluations were done.Measurements included:A clinical questionnaire(for smell and taste);sniffin'odor,taste and flavor identification tests and the Questionnaire of Olfactory Disorders-Negative Statements(sQOD-NS).RESULTS Duration of disorders was 11.53±3.97 ms(6-24 ms).Parosmia(n=119;64.32%)was developed months after anosmia(3.05±1.87 ms).Objective testing showed anosmia in all,ageusia and flavor loss in 20%(n=37)and loss of nasal and oral trigeminal sensations in 18%(n=33)and 20%(n=37),respectively.Patients had low scoring of sQOD-NS(11.41±3.66).There were no specific differences in other demographics and clinical variables which could distinguish post-COVID-19 smell and taste disorders in children from adults.CONCLUSION The course of small and taste disorders are supportive of the nasal and oral neuronal compromises.Post-COVID-19 taste and trigeminal disorders were less frequent compared to smell disorders.Post-COVID-19 flavor disorders were solely dependent on taste and not smell disorders.There were no demographics,clinical variables at onset or specific profile of these disorders in children compared to adults.

Diabetes is a complex condition,and the causes are still not fully understood.However,a growing body of evidence suggests that exposure to air pollution could be linked to an increased risk of diabetes.Specifically,exposure to certain pollutants,such as particulate Matter and Ozone,has been associated with higher rates of diabetes.At the same time,air pollution has also been linked to an increased risk of thyroid cancer.While there is less evidence linking air pollution to thyroid cancer than to diabetes,it is clear that air pollution could have severe implications for thyroid health.Air pollution could increase the risk of diabetes and thyroid cancer through several mechanisms.For example,air pollution could increase inflammation in the body,which is linked to an increased risk of diabetes and thyroid cancer.Air pollution could also increase oxidative stress,which is linked to an increased risk of diabetes and thyroid cancer.Additionally,air pollution could increase the risk of diabetes and thyroid cancer by affecting the endocrine system.This review explores the link between diabetes and air pollution on thyroid cancer.We will discuss the evidence for an association between air pollution exposure and diabetes and thyroid cancer,as well as the potential implications of air pollution for thyroid health.Given the connections between diabetes,air pollution,and thyroid cancer,it is essential to take preventive measures to reduce the risk of developing the condition.

Handling Big Data in Relational Database Management Systems

Currently, relational database management systems (RDBMSs)face different challenges in application development due to the massive growthof unstructured and semi-structured data. This introduced new DBMS categories, known as not only structured query language (NoSQL) DBMSs, whichdo not adhere to the relational model. The migration from relational databasesto NoSQL databases is challenging due to the data complexity. This study aimsto enhance the storage performance of RDBMSs in handling a variety of data.The paper presents two approaches. The first approach proposes a convenientrepresentation of unstructured data storage. Several extensive experimentswere implemented to assess the efficiency of this approach that could resultin substantial improvements in the RDBMSs storage. The second approachproposes using the JavaScript Object Notation (JSON) format to representmultivalued attributes and many to many (M:N) relationships in relationaldatabases to create a flexible schema and store semi-structured data. Theresults indicate that the proposed approaches outperform similar approachesand improve data storage performance, which helps preserve software stabilityin huge organizations by improving existing software packages whose replacement may be highly costly.
