Effect of Optimization of Hemodialysis on Ophthalmological Variables in Patients with End Stage Renal Disease

Background: Conventional hemodialysis (HD) used in treating end stage renal disease (ESRD) can result in multiorgan insult including the eye-owing to the resultant reperfusion after the ischemic event. Optimization of HD can be done using a process called remote ischemia which includes applying cycles of brief, nonlethal ischemia followed by reperfusion to one limb. This method sends signal to the end organs to prepare themselves for the upcoming ischemia thus preventing their injury. Aim: To evaluate the effect of remote ischemia preconditioning in HD on ophthalmological variables. Patients and Methods: A pre-post interventional analytical study conducted on patients receiving hemodialysis. Remote ischemia preconditioning was done before each HD session incorporating 3 cycles of alternating ischemia and reperfusion (5 minutes each) performed in the upper limb using sphygmomanometer cuff. Ophthalmic examination was done at baseline and 3 months after HD optimization. Assessment was done half to one hour pre- and post-session for visual acuity, corneal, conjunctival deposits, tear break up time (TBUT), anterior chamber depth and central macular thickness. Results: The study included 50 eyes of 25 patients with almost equal gender distribution and mean age of 37.52 ± 9.824 years. They were maintained on hemodialysis for median 10 years (range 3-25 years). The commonest cause of ESRD was hypertension. The studied ocular parameters showed insignificant change after pre-conditioned HD except for TBUT that was statistically longer (p = 0.018). Conclusion: Optimization of hemodialysis using remote ischemia does not seem to have significant ocular effect apart from prolonged TBUT. .

Currently, relational database management systems (RDBMSs)face different challenges in application development due to the massive growthof unstructured and semi-structured data. This introduced new DBMS categories, known as not only structured query language (NoSQL) DBMSs, whichdo not adhere to the relational model. The migration from relational databasesto NoSQL databases is challenging due to the data complexity. This study aimsto enhance the storage performance of RDBMSs in handling a variety of *** paper presents two approaches. The first approach proposes a convenientrepresentation of unstructured data storage. Several extensive experimentswere implemented to assess the efficiency of this approach that could resultin substantial improvements in the RDBMSs storage. The second approachproposes using the JavaScript Object Notation (JSON) format to representmultivalued attributes and many to many (M:N) relationships in relationaldatabases to create a flexible schema and store semi-structured data. Theresults indicate that the proposed approaches outperform similar approachesand improve data storage performance, which helps preserve software stabilityin huge organizations by improving existing software packages whose replacement may be highly costly.

Incidence and risk factors of post-phototherapy neonatal rebound hyperbilirubinemia

Background To determine the incidence and risk factors of post-phototherapy rebound hyperbilirubinemia because data about bilirubin rebound in neonates are lacking and few studies have concerned this condition. Methods A prospective observational study was conducted on 500 neonates with indirect hyperbilirubinemia who were treated according to standard guidelines. Total serum bilirubin (TSB) was measured at 24–36 h after phototherapy;significant bilirubin rebound (SBR) is considered as increasing TSB that needs reinstitution of phototherapy. Results A total of 124 (24.9%) neonates developed SBR with TSB increased by 3.4 (2.4–11.2) mg/dL after stopping pho-totherapy. Multiple logistic regression model revealed the following significant risk factors for rebound: low birth weight (B = 1.3,P < 0.001, OR 3.5), suspected sepsis (B = 2.5,P < 0.001, OR 12.6), exposure to intensive phototherapy (B = 0.83, P= 0.03, OR 2.3), hemolysis (B = 1.2,P < 0.001, OR 3.1), high discharge bilirubin level (B = 0.3,P = 0.001, OR 1.3), and short duration of conventional phototherapy (B = ? 1.2,P < 0.001, OR 0.3). Conclusions SBR should be considered in neonates with hemolysis, low birth weight, suspected sepsis, short duration of conventional phototherapy, exposure to intensive phototherapy, and relatively high discharge TSB. These risk factors should be taken into account when planning post-phototherapy follow-up.

Ameliorative effect of Morus alba leaves extract against developmental retinopathy in pups of diabetic and aluminum intoxicated pregnant albino rats

Objective: To investigate the possible ameliorative effect of crude water extract of Morus alba(M. alba) leaves on retinopathy of rat pups maternally subjected to diabetes and/or Al ***: Both control and experimental groups were subjected to certain integrated approaches, namely, biochemical assessments, light microscopic investigation, transmission electron microscopic investigation, single cell gel electrophoresis(comet assay) and determination of DNA ***: The retina of pups of diabetic and/or Al-intoxicated mothers exhibited abnormal alterations in retinal cell layers including retinal pigmented epithelium, photoreceptor inner segment and ganglion cells. Increased incidence of DNA fragmentation and apoptosis were evident in pups of diabetic and/or Al-intoxicated mothers. However, retina of pups maternally received M. alba extract plus diabetes or Al-intoxicated alone or in combination showed marked amelioration. Less degree of ameliorations was seen in retina of pups maternally subjected to combined treatment. Furthermore, application of crude water extract of *** resulted in amelioration of the alterations of maternal serum glucose as well as Al ***: Based on the results of the present study, M. alba extract is effective against experimentally diabetic and Al-induced developmental retinopathy.

Soybean lecithin-based extender improves Damascus goat sperm cryopreservationand fertilizing potential following artificial insemination

Objective: To study the effect of adding different concentrations of soybean lecithin in Tris based extender and to compare the results of Tris soybean lecithin extender with two commercial diluents (egg yolk-based: BullXcell and plant-based: OptiXcell) for Damascus goat sperm cryopreservation. Methods: The ejaculates from 4 mature male Damascus goats were obtained by using an artificial vagina. Semen samples were pooled and diluted in Tris extender supplemented with soybean lecithin at different concentrations of 1.5%, 3.0%, 6.0% and 10.0% to get better concentration to be used for further experiments, with a final concentration of 240×106 spermatozoa/mL. Semen samples were packed in straws (0.25 mL), frozen by using an automated system and stored in liquid nitrogen at -196 ℃ for 48 h. After thawing (37 ℃/30 s), the samples were evaluated for sperm quality parameters, including sperm motility, membrane integrity and acrosome integrity. Malondialdehyde concentration was estimated as a marker for lipid peroxidation. Based on the previous investigations, only Tris extender supplemented with 3.0% soybean lecithin (based on its positive results) was used versus BullXcell and OptiXcell for sperm ultrastructure evaluation and artificial insemination by using electron microscope and artificial insemination of the synchronized does. Results: There was no significant difference between Tris-soybean lecithin at 3.0% and BullXcell/OptiXcell diluents in post-thaw sperm parameters and fertility following artificial insemination;meanwhile, the other concentrations of soybean lecithin (1.5%, 6.0% and 10%) showed lower sperm parameters following cryopreservation. Conclusions: Using of Tris-soybean lecithin based extender at the level of 3.0% can be an appropriate alternative to either BullXcell or OptiXcell for Damascus goat sperm cryopreservation.

A Reconfigurable UWB Bandpass Filters with Embedded Multi-Mode Resonators

The two proposed filters described here satisfy the Federal Communications Commission Ultra-wideband (FCC-UWB) specifications and also control the center frequency and bandwidth of the filters passband. These filters consist of two distinguishing parts, Electromagnetic bandgap (EBG)-embedded multiple- mode resonator (MMR) and interdigital coupled lines to realize high performance in the operation band with a compact size of 14.0 mm × 10.1 mm. The main advantage of the two proposed filters is that three different bands are tuned. The 1st tuned band is from 3.5 GHz to 11.4 GHz for the first filter and from 3.1 GHz to 11.6 GHz for the second proposed filter, respectively. The 2nd tuned band is from 3.5 GHz to 7.5 GHz for the first filter and from 3.1 GHz to 7.8 GHz for the second proposed filter, respectively. While the 3rd tuned band of the first proposed filter is from 3.5 GHz to 5.9 GHz and from 3.1 GHz to 5.8 GHz for the second proposed filter. The bandwidth of the filters can be changed by increasing the length of the outer open circuited stubs which are controlled by using switching matrix equipment (mini circuit, replacement of PIN diodes). To validate the design theory, a reconfigurable UWB bandpass filters (BPFs) with EBG Embedded MMR are designed, fabricated and measured. Good agreement is found between simulated and measured results.

Diagnostic and Prognostic Value of Serum Tissue Polypeptide Antigen in Advanced Lung Cancer

Lung cancer is a lethal malignancy, however, no serum marker is routinely recommended till now. Prospectively two groups of patients included: Group I: Patients with advanced lung cancer. Group II: patients with benign lung disease as control. Serum Tissue Polypeptide Antigen (TPA) levels were measured by ELISA technique before the first line chemotherapy. The TPA cutoff taken was 1800 pg./ml. End points were comparison of high TPA in cases and controls and correlation between high TPA and disease progression (PD), progression free survival (PFS) and overall survival (OS). 30 patients with advanced lung cancer (16 non-small and 14 small cell lung cancer) and 15 patients with benign lung disease were included and followed up during the period from October 2008 to October 2011 with median follow-up of 1.5 years. High TPA was found in 50% of lung cancer cases compared to 26% in controls (p = 0.014). High TPA was found in 64% of cases showing PD versus 36% normal TPA (p = 0.08). 1 year PFS in high TPA was 32% versus 39% in normal TPA, (p = 0.2). 1 year OS in high TPA was 46% versus 73% in normal TPA (p = 0.6). Serum TPA is a potential marker for advanced lung cancer.

A Validated High-Performance Thin-Layer Chromatography (Hptlc) Method for the Quantitative Determination of Tricin in Two <i>Spergularia</i>Species

A simple, sensitive, precise and robust high-performance thin-layer chromatographic (HPTLC) method for analysis of tricin has been developed and validated for the determination of tricin in genus Spergularia. Analysis of tricin was performed on pre-coated TLC aluminum plates with silica gel as the stationary phase. Linear ascending development was carried out in double twin trough glass chamber saturated with a mobile phase consisting of chloroform: methanol (37:3 v/v). Spectro-densitometric scanning was performed by TLC scanner III (CAMAG) in absorbance mode at the wavelength of 270 nm. The system was found to give compact spots for tricin (Rf value of 0.34 ± 0.04). The linear regression analysis data for the calibration plots showed a good linear relationship with r2 = 0.9981 in the concentration range 2-15 μl with respect to peak area. According to the International Conference on Harmonization (ICH) guidelines, the method was validated for precision, recovery, and robustness. The tricin content quantified from Spergularia marina and S. daiandra was found to be 0.398 mg/100g and 1.642 mg/100g fresh weight, respectively. Statistical analysis of the data showed that the method is reproducible and selective for the estimation of tricin.

Field Assessment of the Level of Protection Conferred by a Newly Prepared Combined Inactivated Vaccine against E. coli and P. multocida in Rabbit in Egypt

Pasteurellosis is the most prevalent, extremely contagious bacterial disease among domestic rabbits and is considered the leading cause of deaths in rabbits, resulting in enormous economic losses to the rabbit industry. Screening for bacterial agents causing mortalities in rabbits revealed the presence of Enterobacteriacae species in approximately 42% of studied cases, with E. coli the most commonly isolated organism. The present study was designed to evaluate the immune response of rabbits vaccinated with a locally prepared, combined inactivated vaccine of Pasteurella multocida and E. coli, adjuvanated with Montanide ISA70. A total of 370 rabbits, aged 2 - 3 weeks, were divided into four groups: (G1) vaccinated with a polyvalent P. multocida vaccine, (G2) vaccinated with a polyvalent E. coli vaccine, (G3) vaccinated with a combined inactivated Montanide ISA70 vaccine of P. multocida and E. coli, and (G4) kept as a non-vaccinated control group. All rabbits received two doses of 0.5 ml of the prepared vaccines, administered one month apart, and were then challenged with virulent strains of P. multocida and E. coli three weeks after the second vaccination. The prepared vaccines were evaluated by determining humoral immunity using indirect haemagglutination (IHA) test and ELISA. The potency of the vaccines was assessed through challenge and determination of LD50. Experimental findings on the prepared polyvalent combined inactivated P. multocida and E. coli vaccine indicated that it is a potent vaccine, producing the highest antibody titers and a 90% protection rate against challenges with virulent strains of P. multocida type A, D2, and E. coli types O157, O151 and O125. Thus, this vaccine is promising in addressing both P. multocida and E. coli problems in rabbits, farms, providing significant protection, and we recommend its commercial production to help rabbit producers control these two major bacterial infections.
