Background Acute bilirubin encephalopathy (ABE) still represents a significant cause of morbidity and mortality throughout the world,especially in developing *** aimed to determine the prevalence of ABE based on the Johnson bilirubininduced neurologic dysfunction (BIND) score and to describe the discharge *** We prospectively studied all newborns ≥ 35 weeks with ABE by evaluating signs of mental sensorium,muscle tone,and cry patterns over 1 *** 12% (81 out of 674) of the newborns admitted for neonatal hyperbilirubinemia had a BIND score > *** admission age was 6 ± 4.1 days;total serum bilirubin (TSB) was 31.2± 10 mg/dL (range 17.5-75.2).Of these newborns,40.7% and 21% had evidence of haemolysis and sepsis,*** mortality was 9.9%;58% of the newborns showed signs of mild-to-moderate BIND at discharge,while 32.1% survived with an apparently normal *** BIND score was significantly correlated with admission TSB (r=0.476,P < 0.001).Similarly,BIND score at discharge was correlated with admission TSB (r=0.442,P < 0.001) and admission BIND score (r =0.888,P < 0.001).The regression model showed that admission TSB (P < 0.001) and maternal illiteracy (P =0.034) were predictors of the BIND score at admission,while admission BIND score was the best indicator of the discharge score (P < 0.001).Conclusions ABE is still a major problem in our *** TSB and maternal illiteracy are good predictors of bilirubin encephalopathy at admission and discharge.

Background To determine the incidence and risk factors of post-phototherapy rebound hyperbilirubinemia because data about bilirubin rebound in neonates are lacking and few studies have concerned this condition. Methods A prospective observational study was conducted on 500 neonates with indirect hyperbilirubinemia who were treated according to standard guidelines. Total serum bilirubin (TSB) was measured at 24–36 h after phototherapy;significant bilirubin rebound (SBR) is considered as increasing TSB that needs reinstitution of phototherapy. Results A total of 124 (24.9%) neonates developed SBR with TSB increased by 3.4 (2.4–11.2) mg/dL after stopping pho-totherapy. Multiple logistic regression model revealed the following significant risk factors for rebound: low birth weight (B = 1.3,P < 0.001, OR 3.5), suspected sepsis (B = 2.5,P < 0.001, OR 12.6), exposure to intensive phototherapy (B = 0.83, P= 0.03, OR 2.3), hemolysis (B = 1.2,P < 0.001, OR 3.1), high discharge bilirubin level (B = 0.3,P = 0.001, OR 1.3), and short duration of conventional phototherapy (B = ? 1.2,P < 0.001, OR 0.3). Conclusions SBR should be considered in neonates with hemolysis, low birth weight, suspected sepsis, short duration of conventional phototherapy, exposure to intensive phototherapy, and relatively high discharge TSB. These risk factors should be taken into account when planning post-phototherapy follow-up.

Integrating large-scale wind plants with the electricity grids has many challenges for grid *** the variability and uncertainty of wind power,coordinating between different technologies of generation in the same grid can be considered the main problem,specifically for short-term frequency ***,a large penetration of wind power generation in modern power grids has a risky influence on the power-system ***-generation plants have contradictory behaviour compared to classic thermal plants,especially in active generated power-shortage events due to the variable nature of wind *** experience in wind plants keeps part of the available wind power unloaded,using what are known as deloading *** deloading techniques are usually applied to emulate the thermal-plant-governor function and confirm a proper spinning reserve for any active-power *** techniques decrease the generated power from wind plants continuously from maximum point tracking ***,the practical capacity,annual generated energy and economical income of wind plants are *** addition,grid-protection and control sub-schemes are set and designed according to the well-known conventional responses of thermal plants,which increase the need for thermal-plant-behaviour *** this paper,instead of the usual deloading methods,a supercapacitors scheme is proposed with wind turbines to emulate the response of conventional power *** study discusses the technical and economic benefits of the proposed addition of supercapacitors in the wind-plant-planning *** frequency grid-code indices are selected to evaluate studied *** results of the IEEE four-generation two-area system determines the effectiveness of suggested schemes *** System Advisor Model(SAM)program estimates the economic benefits of a typical US study case compared with the existing wind-deloading technique.

The neonatal hypoxic-ischemic encephalopathy(HIE)is an important cause of neurological morbidity and mortality in *** therapy is considered a promising method for treating severe neurological disorders such as this *** cells have the capacity for self-renewal and differentiation into certain cell *** present study was aimed to find out the most beneficial route of bone marrow-derived mesenchymal stem cells(BMSCs)administration for the attenuation of experimentally induced HIE in neonatal *** neonatal rats were divided randomly into four *** 1:control *** 2:rats were exposed to bilateral ligation of cephalic *** 3:rats were exposed to bilateral ligation of cephalic arteries and then underwent intravenous(IV)BMSC *** 4:rats were exposed to bilateral ligation of cephalic arteries and then underwent intracerebroventricular(ICV)BMSC *** animals were evaluated by(a)neurobehavioral tests;(b)histopathology,i.e.,histological and immuno-histochemical studies;and(3)gene expression *** BMSC treated groups(3 and 4)showed improvement in neurobehavioral tests,histopathological studies,and gene expression,as compared to non-injected lesioned rats(Group 2)with better improvement in Group 4(ICV injections)than in Group 3(IV injections).

AIM: To assess the relation between central retinal artery(CRA) resistive index(RI) and retinal nerve fiber thickness measured by optical coherence tomography(OCT) in assessment of disease progress in cases of open angle ***: Twenty-three patients with diagnosed open angle glaucoma were included in this study. They were examined by colored duplex ultrasonography of CRA with estimation of RI of CRA and ophthalmic artery(OA)with estimation of CRA/OA RI ratio as well as OCT measurement of the average retinal nerve fiber layer(RNFL) thickness in order to assess the disease ***: There was strong inverse relation between the increased RI in CRA as well as the increased CRA/OA RI ratio and the decrease in average RNFL thickness in cases of open angle ***: Assessment of CRA RI can indirectly assess the vascular changes associated with glaucoma and can assess the degree of retinal atrophy helping in evaluating prognosis thus guiding the choice of treatment.

The Spurge family （Euphorbiaceae） is a large family of flowering plants. Two plant species of this family were collected from the Egyptian desert, extracted with methanol subjected to silica gel column chromatography to give six compounds, three compounds from each plant. The compounds isolated from the methanol extract of Euphorbia retusa Forssk were identified as kaempferol-3-beta-D glucopyranosyl （1）, 3-O-alpha-L-xylopyranosyl （1→3）-alpha-L xylopyranosyl （1→2）, beta-D glucopyranosyllup 1,12-diene-3,21-diol （2） and 5,7,8,3＇,4＇-pentahydroxy-3-methoxyflavone （3） and the compounds isolated from Euphorbia paralais L. identified as 3,5,3＇-trihydroxy-6,7-d-methoxy-4＇（7＂-hydroxygeranyl-1＂-ether） flavones （4）, 3-O-syringic acid （1→4）-alpha-L- xylopyranosyl （1→3）, beta-D-rahminopyranosyl hederagenin （5） and 3-O-syringic acid （1→4）, beta-D-rahminopyranosyl （1→3）,beta-D-rahminopyranosyl hederagenin （6）. The structure of the isolated compounds was elucidated by chemical and spectrometric analysis such as Infera red （IR）, proton and carbon nuclear magnetic resonance （1H-, 13C-NMR） and chemical ionization mass spectrometry （CI-MS）.

Objective： The proper time to commence adjuvant chemotherapy after primary surgery for breast cancer is unknown. It is usually prescribed within 2-3 months after definitive surgery. The aim of this retrospective study was to assess the impact of adjuvant chemotherapy （CT） delay beyond 3 weeks （ 21 days） in premenopausal patients with ER-absent tumors being treated for early stages breast cancer on overall survival （OS） and disease-free survival （DFS）. Methods： This retrospective study was conducted through revision of medical records of premenopausal patients diagnosed with early stage ｜-｜IIA breast cancer and ER-absent tumors who received adjuvant CT after definitive surgery at the Department of Clinical Oncology, Ain-Shams University Hospitals. Results： Between 2005 and 2008, 105 patients were retrospectively analyzed and included. Patients were divided into 2 groups： Group A including 48 patients who started adjuvant CT 〈 21 days of surgery and group B which included 57 patients who had CT delay 〉 21 days. Both groups were matched demographically. Comparisons of overall survival, and disease-free survival between group A and group B patients all favored group A. At 5-year the OS rates were 87% and 73% for groups A and B respectively （P = 0.001）, while DFS rates were 85% and 64% in groups A and B respectively （P = 0.001）. Analysis of other prognostic factors （age, T, N, grade, HER2 status, surgery type, CT type, local radiotherapy received） were analyzed. Only nodal status predicted for worse DFS （P = 0.05） and OS （P = 0.006）. Conclusion： Delay in initiating adjuvant chemotherapy for early stage breast cancer patients with ER-absent tumors was associated with a decrease in both OS and DFS rates.

For the sake of improving patient compliance and sustainability of chemotherapy healthcare system, both TC and CoQ10 were formulated as solid lipid nanoparticles (SLNs). The study was focused on establishing and validating a simple and reproducible spectrophotometric method for simultaneous determination of TC and CoQ10 in their binary mixture or pharmaceutical dosage forms. A new method based on simultaneous estimation of drug mixture without prior separation was developed. Validation parameters were checked with International Conference on Harmonization (ICH) guidelines. The accuracy and reproducibility of proposed method was statistically compared to HPLC. The TC and CoQ10 were quantified at absorptivity wavelengths of 236 nm and 275 nm, respectively. Calibration curves obeyed Beer’s law in range of 2–14 μg/ml with a correlation coefficient (R^2) of 0.999 in both methanol and simplified simulated intestinal fluid (SSIF). The %means recovery of TC and Co Q10 in pure state or binary mixture at various concentration levels were all around 100%.The low values of SD and %RSD (<2%) confirm high precision and accuracy of the proposed method. Formulated SLNs showed different %means recovery in range 81–92% for TC and 32–59% for CoQ10. The data obtained by applying simultaneous Vierordt’s equations showed no statistical significance in comparison to HPLC. Vierordt’s method was successfully applied as a simple, accurate, precise, and economical analysis method for estimating TC and CoQ10 concentrations in pure state, binary mixture and pharmaceutical dosage forms.

The aim of the present work is to illustrate the application of mixed H2/H∞ control theory with Pole-Placement in de- signing controller for semi-active suspension system. It is well known that the ride comfort is improved by reducing vehicle body acceleration generated by road disturbance. In order to study this phenomenon, Two Degrees of Freedom (DOF) in state space vehicle model was built in. However, the role of H is to minimize the disturbance effect on the output while H2 is used to improve the input of controller. Linear Matrix Inequality (LMI) technique is used to calculate the dynamic controller parameters. The simulation results show that the H2 and H techniques can effectively control the vibration of vehicle system where the reduction of suspension working space, dynamic tire load and body acceleration. Moreover, the simulation results show that the (RMS) of suspension working space was reduced by 44.5%, body acceleration and dynamic tire load are reduced by 18.5% and 20% respectively.
