Is V-Y plasty necessary for penile lengthening？ Girth enhancement and increased length solely through circumcision： description of a novel technique

Our objective is to describe a novel ligamentolysis approach using a subcoronal incision technique and to determine its safety and efficacy. During the last 7 years, 82 consecutive patients had penile augmentation surgery. Ligamentolysis, through a lower abdominal incision （V-Y plasty） in the first 35 males, was performed （Group A）, followed by circumcision ligamentolysis in the next 47 males （Group B）. The operation time, complications, and the preoperative and postoperative values of penile length and girth along with the self-esteem and relations questionnaire score as well as satisfaction score was calculated before and after the surgery, and a comparison was conducted between the groups. The mean age at presentation was 32 years （range： 18-56 years）. Seventy-nine patients suffered from penile dysmorphophobia, and three patients had micropenises （length 〈7.5 cm）. The mean surgical times were 150.7 and 125.2 rain for Groups A and B, respectively （P=0.005）. Postoperatively, four Group A patients and three Group B patients （11% versus 6%, respectively） experienced penile retraction （P=0.453）. Hypertrophic scars were observed in 18 men （51%） in the former group. In the circumcision group, no major wound complications were recorded. The length and girth improvements between the groups were similar. In terms of satisfaction and SEAR improvement, the resulting difference for both variables favored the circumcision group （P=0.007 and 〈0.001, respectively）. With strict selection criteria, the circumcision ligamentolysis procedure compared to the V-Y plasty demonstrated improved results in terms of safety, operation time, retraction rate and cosmetic appearance without any compromise in the gained penile size.

Thermal performance of lightweight concrete applications in building envelopes in Lebanon

Innovative building materials are being used in building envelopes for reducing their heating and cooling *** paper aims to assess the thermal impact of using lightweight concrete in Lebanese building constructions by pouring an 8 cm thickness of lightweight concrete on the roof and the slab and replacing traditional hollow concrete block by lightweight concrete *** properties of two different samples were experimentally determined:the first one(558 kg/m^(3))used for the roof and the slab and the second one(1074 kg/m^(3))used for the *** numerical simulations were carried out for a Lebanese traditional detached house using the characteristics of these two *** thermally improved Light Weight Concrete building(LWC)was compared to a traditional Lebanese house base case(BC)using a dynamic building energy simulation tool in the four different Lebanese climate zones:coastal,mid-mountain,mountain,and inland *** results highlight the effectiveness of integrating LWC to building envelopes by reducing energy consumption and improving thermal comfort in both winter and summer climate conditions and in the different Lebanese climatic *** paper demonstrates that the use of LWC in the vertical walls replacing the traditional hollow blocks can reduce the heating needs by up to 9%and by up to 13%for cooling *** the other hand,adding a LWC roof screed has a very high impact on cooling and heating energy consumption,which can reach up to 74%in cooling energy savings and up to 24%in heating energy savings.

Role of Kupffer cells in the pathogenesis of liver disease

Kupffer cells, the resident liver macrophages have long been considered as mostly scavenger cells responsible for removing particulate material from the portal circulation. However, evidence derived mostly from animal models, indicates that Kupffer cells may be implicated in the pathogenesis of various liver diseases including viral hepatitis, steatohepatitis, alcoholic liver disease, intrahepatic cholostasis, activation or rejection of the liver during liver transplantation and liver fibrosis. There is accumulating evidence, reviewed in this paper, suggesting that Kupffer cells may act both as effector cells in the destruction of hepatocytes by produdng harmful soluble mediators as well as antigen presenting cells during viral infections of the liver. Moreover they may represent a significant source of chemoattractant molecules for cytotoxic CD8 and regulatory T cells. Their role in fibrosis is well established as they are one of the main sources of TGFβ1 production, which leads to the transformation of stellate cells into myofibroblasts. Whether all these variable functions in the liver are mediated by different Kupffer cell subpopulations remains to be evaluated. In this review we propose a model that demonstrates the role of Kupffer cells in the pathogenesis of liver disease.

Nonalcoholic fatty liver disease(NAFLD)has becomeone of the most common causes of liver disease worldwide and has been recognized as a major health *** prevalence of NAFLD has grown proportionallywith the rise in obesity,sedentary lifestyle,unhealthydietary pattern,and metabolic ***,there is no drug therapy that can be formulated fortreating NAFLD.A combination of dietary modificationsand increased physical activity remains the mainstayof NAFLD *** is hard to maintain thismode of management;however,it seems to havesignificant long-term ***,NAFLDpatients,whether obese or not,should be educatedthat a healthy diet and physical activity have benefitsbeyond weight *** large controlled randomized trials are needed in order to identify the bestdietary regimen and physical activity in the management of NAFLD *** review highlights the roleof diet and lifestyle modifications in the managementof NAFLD,and focuses on human studies regarding dietary modifications and physical activity.

Patients with primary biliary cirrhosis have increased serum total antioxidant capacity measured with the crocin bleaching assay

AIM： The balance between oxidants and antioxidants can play an important role in the initiation and development of liver diseases. Recently, we have described a new automated method for the determination of total antioxidant capacity （TAC） in human serum and ***： We measured TAC and corrected TAC （CTACabstraction of interactions due to endogenous uric acid,bilirubin and albumin） in 52 patients with chronic liver diseases （41 patients with primary biliary cirrhosis （PBC）,10 patients with chronic hepatitis C and 13 patients with viral HCV cirrhosis） as well as in 10 healthy controls. In 23 PBC patients measurement were also done 6 mo after treatment with ursodeoxycholic acid （UDCA）. The TAC assay was based on a modification of the crocin bleaching assay. The results were correlated with routine laboratory measurements and the histological stage of ***： There were no significant differences in TAC between the various groups. However, CTAC was considerably increased in the PBC group compared to controls and cirrhotics. Analysis of these patients according to disease stages showed that this increase was an early phenomenon observed only in stages I and II compared to controls, cirrhotics and patients with chronic hepatitis C）.After 6 mo of treatment with UDCA, levels of CTAC decreased to those similar to that of ***： Patients in the early stages of PBC present with high levels of corrected total antioxidant capacity and this maybe related to the pathophysiology of the disease. UDCA treatment restores the levels of CTAC to control levels.

3D Characterization of corneal deformation using ultrasound speckle tracking

The three-dimensional(3D)mechanical response of the cornea to intraocular pressure(IOP)elevation has not been previously *** this study,we use an ultrasound speckle tracking technique to measure the 3D displacements and strains within the central 5.5 mm of porcine corneas during the whole globe *** tests were performed on dextran-treated corneas(treated with a 10%dextran solution)and untreated *** dextran-treated corneas showed an inflation response expectod of a thin spherical shell,with through-thickness thinning and in-plane stretch,although the strain magnitudes exhibited a heterogeneous spatial distribution from the central to more peripheral *** untreated eyes demon-strated a response consistent with swelling during experimentation,w ith through-thickness expansion overriding the infation *** average volume ratios obtained in both groups was near 1 confirming general incompresibility,but local regions of volume loss or expansion were *** results suggest that biomechanical measurements in 3D provide important new insight to understand the mechanical response of ocular tissues such as the cornea.

Mathematical analysis of corneal remodelling after intracorneal ring surgery in keratoconus

AIM：To represent mathematically the intersection between the ectatic corneal geometry and the plane of intracorneal ring implants（ICRS）in order to determine the corneal response to ICRS surgery in keratoconus（KC）.Thereafter,to present the concept and early results of a newly derived topography-guided nomogram for ICRS surgery for the treatment of keratectasia. METHODS：The corneal rings plane intersection was modelled to a conic section. Ring effect was the result of：the ring size,position（steep vs flat）,location（distance from the geometric centre of the cornea）,and the discrepancy between the ring＇s curvature and the tunnel＇s *** laser was used to create the tunnels and the incision sites were chosen according to the nomogram in order to place the thickest ring in the steepest portion of the cornea regardless of the astigmatism axis of ***：The conic section had a more prolate shape in the steep area of the cornea than in the flat area,depending on the corneal sagittal curvature. Equal ring size had more flattening effect in the steep area than in the flat area. Thick segment should be implanted under the steep portion of the cornea regardless of the cylinder axis of refraction. Single segment in the steep area was sufficient in early and moderate cases of KC. The new nomogram provided more topographic regularity with significant reduction of astigmatism and better improvement in uncorrected visual acuity（UCVA）and best-corrected visual acuity（BCVA）than the conventional nomogram. CONCLUSION：The newly derived nomogram can produce better results than the conventional nomogram. Moreover,based on this concept,a new nomogram can be integrated into the femtosecond laser software to create topography-guided,customized,elliptical tunnels with modified focal asphericity that allows for customized focal flattening of the irregularly steepened ectatic cornea.

Water Repellency of Cellulosic Fibrous Mats Impregnated with Organic Solutions Based on Recycled Polystyrene

Recycled polystyrene in combination with paraffin wax,alkyd resin,and gum rosin were used as components in formulations to investigate their water repellency when applied to cellulosic filter paper *** was used in concentration of 5,10,15 and 20%,alkyd resin and gum rosin of 5%each and paraffin wax of 0.5%.Totally,twenty four water repellent solutions were *** repellency was evaluated in terms of water absorption of the cellulosic fibrous *** relations between retention of solid substances of the formulations and grammage and water absorption of filter paper samples were also *** results showed that all the water repellent formulations exhibited a degree of water *** absorption decreased by increasing the polystyrene concentration in the solution and polystyrene retention by the impregnated filter paper *** incorporation of 0.5%paraffin wax improved the hydrophobicity of treated *** best of the three water repellent formulations including paraffin wax was found to be the“polystyrene+gum rosin+paraffin wax”solution followed by the“polystyrene+alkyd resin+paraffin wax”and“polystyrene+paraffin wax”*** inclusion of 5%gum rosin in polystyrene solutions compared to that of 0.5%paraffin wax was found more effective in almost all cases.

Nitrogen-doped titanium dioxide(N/TiO2)nanophotocatalysts were successfully synthesized in the presence of environmentally benign nitrogen dopant source,guanidinium chloride,by the sol-gel *** effect of calcination temperature(300–600℃)on their physicochemical properties was investigated by means XRD,XPS,FESEM,HRTEM,Raman spectroscopy,UV-vis DRS,PL and ***,their photocatalytic activities were evaluated against rhodamine B(RhB)degradation under direct sun *** showed that the crystal phase of spheroidal N/TiO2 nanoparticles was changed from anatase(300℃)to rutile(600℃)via an intermediate anatase/rutile(A/R)mixed phase(400–500℃),and the RhB photodegradation performance was increased with the decrease of the calcination ***,N/TiO2 prepared at 400℃demonstrated the best degradation performance(99%)after 5 h *** enhanced performance with high photostability was mainly attributed to its higher surface area and pore volume,stronger light absorption,and lower recombination *** nanomaterials have practical applications for environmental remediation.
